THE ABILITY TO BRING CARE TO THE PATIENTS AND COMMUNITIES WE SERVE WILL IMPROVE ACCESS FOR PEOPLE WHO OTHERWISE MIGHT FACE BARRIERS, SUCH AS TRANSPORTATION AND OFFER SERVICES SUCH AS VACCINATIONS, SCREENING (BLOOD PRESSURE, WEIGHT, LABS), INTERVENTIONS (MENTAL HEALTH/ADDICTION), AND HELP TO IMPROVE HEALTH AND SUPPORT WELLNESS INITIATIVES.
