ACHIEVE EFFICIENCIES. 2.) EXPAND ACCESS TO, COORDINATE, AND IMPROVE THE QUALITY OF BASIC HEALTHCARE SERVICES. 3.) STRENGTHENING OF THE RURAL HEALTH CARE SYSTEM. FOCUS AREA: OUR FOCUS AREA AREAS ARE CARE COORDINATION, MANAGING CHRONIC CONDITIONS ACROSS CARE CONTINUUMS, PREVENTATIVE AND PRIMARY CARE COMPLIANCE, AND THE SIMPLIFICATION OF REFERRAL AND CARE PATHWAYS FOR OUR RURAL OLDER ADULTS. PROPOSED SERVICE AREA: ALL COUNTIES IN THE PROJECT ARE RURAL; MINNESOTA COUNTIES OF FOCUS INCLUDE DOUGLAS, GRANT, HUBBARD, OTTER TAIL, POPE, STEVENS, TODD, TRAVERSE, AND WADENA. TARGET POPULATION: THIS NETWORK WILL FOCUS ON NINE MINNESOTA RURAL COUNTIES AND TARGET AGING ADULTS, COMMUNITY AND FAMILY CAREGIVERS, AND PROVIDERS. EFFORTS FOR COLLABORATION AND VALUE-BASED APPROACHES TO CARE WILL BE IMPLEMENTED TO ALIGN WITH THE DOMAINS OF THIS NETWORK DEVELOPMENT PROJECT AND IMPACT THE WELL-BEING OF OUR COMMUNITY MEMBERS. ALL AREAS OUTLINED IN THIS PROPOSAL ARE UNDERSERVED RURAL COMMUNITIES WITH LARGE PERCENTAGES OF OLDER ADULTS AND A SEVERE LACK OF COMMUNITY RESOURCES, BEHAVIORAL HEALTH SUPPORT, AND PRIMARY CARE. NETWORK MEMBERS: ALL FOUR NETWORK MEMBERS SERVE RURAL COMMUNITIES AS THEIR FOCUS OF CARE DELIVERY. KNUTE NELSON: EIN: 411263433 S KILLED NURSING; INDEPENDENT SENIOR LIVING; ASSISTED LIVING; MEMORY CARE; HOME AND COMMUNITY; POPULATION HEALTH; WELLNESS ALOMERE HEALTH: EIN: 411410148 HOSPITAL AND CLINIC HEALTH SYSTEM DANCING SKY AREA AGENCY ON AGING: EIN: 411237594 HOME AND COMMUNITY SERVICES; DEVELOPMENT LAKE REGION HEALTH CARE: EIN: 410730602 HOSPITAL AND CLINIC HEALTH SYSTEM EXPERIENCE: THE ORGANIZATIONS REPRESENTED AS PARTNERS IN THIS PROPOSAL PROVIDE THE MAJORITY OF PRIMARY, POST-ACUTE, AND COMMUNITY-BASED CARE IN THE FOCUS COUNTIES. THEY HAVE EXPERIENCE WORKING WITH OTHER ORGANIZATIONS AND HAVE MISSIONS TO DELIVER QUALITY CARE AND COMMUNITY SUPPORT THROUGHOUT THEIR SERVICE AREAS. COLLABORATIVELY, THE JOINT GOAL SETTING AND INITIATIVES OF THIS PROPOSAL WILL SIGNIFICANTLY IMPACT THE ORGANIZATIONS AND POPULATION IN WEST AND CENTRAL MINNESOTA AND DEMONSTRATE TO OTHER REGIONS THAT CARE FOR OUR AGING POPULATION REQUIRES A COORDINATED, HUMBLED, AND LOCAL APPROACH ACROSS THE CONTINUUM TO SEE AND ADDRESS THE WHOLE PERSON NEEDS, PARTICULARLY IN RURAL AREAS. THE ORGANIZATIONS REPRESENTED IN THIS NETWORK PROPOSAL HAVE INVESTED IN THE HEALTH OF THEIR COMMUNITIES AND HAVE DEMONSTRATED FOR DECADES THEIR COMMITMENT TO IMPROVING COMMUNITY HEALTH THROUGH INDIVIDUAL AND JOINT EFFORTS. KNUTE NELSON HAS ALSO SERVED AS A BACKBONE ORGANIZATION FOR A GRANT-FUNDED INITIATIVE THAT HAS BROUGHT TOGETHER COMMUNITY AND HEALTHCARE PARTNERS KNOWN AS THE ALEXANDRIA LAKES AREA CONNECTED COMMUNITIES COLLABORATIVE. THIS INITIATIVE HAS GIVEN OUR ORGANIZATION THE UNIQUE EXPERIENCE OF LEADING A COMMUNITY HUB AND JOINT EFFORTS AROUND CARE MODELS AND GAPS IN CARE SOLUTIONS. IT IS WITH THIS BASE KNOWLEDGE THAT WE WILL DEVELOP OUR NETWORK.
