COMPARED TO CONVENTIONAL SURFACE ELECTRODES, THIS SYSTEM WILL PROVIDE: - MORE CHANNELS FOR PROSTHESIS CONTROL FROM A LARGER NUMBER OF MUSCLES IN THE RESIDUAL LIMB, - IMPROVED SPECIFICITY AND REPEATABILITY FOR RECORDING FROM INDIVIDUAL MUSCLES AND MUSCLE GROUPS, - HIGHER RELIABILITY AND QUALITY FOR THE RECORDED SIGNALS UNDER DIFFERENT SOCKET CONDITIONS, - SELECTIVE, CONSISTENT SIGNALS FROM DEEP MUSCLES, AND - THE ABILITY TO USE GEL, VACUUM, AND OTHER PROSTHESIS SOCKET LINING SYSTEMS THAT DO NOT EASILY ACCOMMODATE SURFACE ELECTRODES. THESE MULTICHANNEL RECORDINGS WILL ENABLE USERS TO GENERATE SIMULTANEOUS MULTI-AXIS MOVEMENTS WITH A MORE NATURAL FEEL OF CONTROL THAN EXISTING MYOCONTROLLERS THAT ONLY ACTUATE A SINGLE JOINT AXIS AT A TIME. IN PHASE I, WE WILL COMPLETE UPGRADES AND TESTING OF THE EXTERNAL TRANSCEIVER TO ELIMINATE THE NEED OF THE BELT-WORN PROCESSOR. CURRENT TREATMENT STRATEGIES ARE EMPIRIC BECAUSE WE HAVE NO MARKERS TO SUGGEST WHICH THERAPY IS BEST FOR AN INDIVIDUAL. TUMOR NECROSIS FACTOR INHIBITORS (TNFI) ARE THE MOST COMMON INITIAL BIOLOGIC TREATMENT IN RA. RESPONSES ARE VARIABLE, WITH APPROXIMATELY 30% NOT RESPONDING AND ANOTHER 30% HAVING ONLY PARTIAL RESPONSE. WE HAVE SHOWN IN TEST AND VALIDATION COHORTS THAT PRE-TREATMENT CIRCULATING TYPE I IFN (T1IFN) PREDICTS NON-RESPONSE TO TNFI. PRE-TREATMENT IFN-SS TO IFN-A ACTIVITY RATIO >1.3 WAS STRONGLY PREDICTIVE OF NON-RESPONSE TO TNFI (SPECIFICITY=77% IN THE VALIDATION COHORT). NO PATIENT WITH A RATIO >1.3 ACHIEVED REMISSION OR LOW DISEASE ACTIVITY. WE USED SINGLE CELL ANALYSIS TO STUDY BLOOD MONOCYTE (MO) GENE EXPRESSION IN RA PATIENTS WITH HIGH VS. LOW IFN-SS-TO-A ACTIVITY RATIO AND FOUND MAJOR DIFFERENCES BETWEEN THE GROUPS, SUPPORTING DOWNSTREAM EFFECTS UPON A CRITICAL EFFECTOR CELL POPULATION. PRESENCE OR ABSENCE OF JAK1 EXPRESSION STRONGLY ALIGNED WITH IFN-SS-TO-A RATIO. IN RA, BLOOD MO INVADE SYNOVIUM, AND LOCAL STIMULI DRIVE EXPANSION OF INFLAMMATORY MACROPHAGE (MF) POPULATIONS. IT IS NOT KNOWN WHETHER THE CIRCULATING IFN RATIO IN RA REFLECTS T1IFN LEVELS AND PATHWAY ACTIVATION IN THE SYNOVIUM, AND THIS WILL BE EXAMINED IN AIM 1. THE RELATIVE CONTRIBUTIONS OF BLOOD VS. SYNOVIAL SIGNALS IN THE DIAPEDESIS AND DIFFERENTIATION OF MO TO INFLAMMATORY MO/MFS IS NOT KNOWN. THIS WILL BE EXPLORED IN AIM 2 USING A NOVEL PERFUSABLE ORGAN-ON-A-CHIP SYSTEM. HYPOTHESIS: RA TNFI NON-RESPONDERS HAVE INCREASED BLOOD AND SYNOVIAL IFN-SS/A RATIO THAT RESULTS IN ALTERED EXPRESSION OF JAK1 AND IFN-STIMULATED GENES, INCREASED DIAPEDESIS AND DIFFERENTIATION OF MO INTO INFLAMMATORY MFS. I WILL EXPLORE THIS OVERARCHING HYPOTHESIS IN 2 SPECIFIC AIMS: (1) DETECT DIFFERENCES IN T1IFN, PATHWAY ACTIVATION, AND HISTOPATHOLOGY IN SYNOVIUM OF RA PATIENTS WHO HAVE A EULAR GOOD RESPONSE OR NO RESPONSE TO TNFI. (2) DETERMINE THE IMPACT OF IFN-A AND IFN-SS ON RA MO, MO-DERIVED MF, AND FIBROBLAST-LIKE SYNOVIOCYTES. SEVERAL STUDIES HAVE INDEED SHOWN THAT LARYNGEAL MUSCLES WITH SYNKINETIC REINNERVATION CAN BE EFFECTIVELY STIMULATED OR “PACED” TO RESTORE DYNAMIC VF OPENING (ABDUCTION). CLINICAL MALE INFERTILITY IS TYPICALLY ASCERTAINED BASED ON SEMEN-QUALITY REFERENCE VALUES ESTABLISHED BY THE WORLD HEALTH ORGANIZATION (WHO) CRITERIA; HOWEVER, THESE CRITERIA ARE POOR PREDICTORS OF REPRODUCTIVE SUCCESS, RAISING QUESTIONS ABOUT THE SIGNIFICANCE AND RELIABILITY OF CONVENTIONAL METHODS FOR EVALUATING MALE FERTILITY. DESPITE SIGNIFICANT ADVANCES IN UNDERSTANDING THE MOLECULAR CHARACTERISTICS OF SEMEN, MEASURES OF MALE FERTILITY HAVE REMAINED UNCHANGED FOR DECADES. THEREFORE, THE DEVELOPMENT OF NOVEL BIOMARKERS OF MALE REPRODUCTIVE HEALTH IS VITAL TO IMPROVING REPRODUCTIVE CLINICAL CARE. WE PROPOSE THAT THE ROBUST EPIGENETIC REPROGRAMMING DURING SPERMATOGENESIS PROVIDES A FINAL OPPORTUNITY FOR SPERM TO “EPIGENETICALLY MATCH” WITH THEIR INTERNAL FACTORS (E.G., GENETICS) AND EXTERNAL FACTORS (E.G., ENVIRONMENT) PRIOR TO CONCEPTION. IN RECENT YEARS, EPIGENETIC CLOCKS, WHICH ESTIMATE BIOLOGICAL AGE, HAVE BEEN DEVELOPED AS NOVEL BIOMARKERS FOR PHENOTYPES SUCH AS CANCER AND MORTALITY. IMPORTANTLY, OUR GROUP WAS THE FIRST TO DEVELOP A SPERM-SPECIFIC EPIGENETIC CLOCK, WHICH HAS BEEN ASSOCIATED WITH COUPLES’ PREGNANCY OUTCOMES AND VALIDATED IN AN INDEPENDENT INFERTILITY COHORT. THIS APPLICATION WILL LEVERAGE OUR MACHINE LEARNING PIPELINE AND EXTANT SPERM METHYLATION DATA, ALONG WITH REPRODUCTIVE OUTCOME DATA FROM 1,825 COUPLES FROM BOTH THE GENERAL POPULATION AND INFERTILITY COHORTS, TO FURTHER OPTIMIZE AND VALIDATE A NEW SPERM EPIGENETIC CLOCK THAT PREDICTS REPRODUCTIVE OUTCOMES. TYPE I INTERFERON IMPACTS TREATMENT RESPONSE IN RHEUMATOID ARTHRITIS - RHEUMATOID ARTHRITIS (RA) IS A COMMON MULTISYSTEM INFLAMMATORY CONDITION. DELAY IN EFFECTIVE TREATMENT RESULTS IN INCREASED MORBIDITY AND MORTALITY, AND A HEAVY ECONOMIC BURDEN. CURRENT TREATMENT STRATEGIES ARE EMPIRIC BECAUSE WE HAVE NO MARKERS TO SUGGEST WHICH THERAPY IS BEST FOR AN INDIVIDUAL. TUMOR NECROSIS FACTOR INHIBITORS (TNFI) ARE THE MOST COMMON INITIAL BIOLOGIC TREATMENT IN RA. RESPONSES ARE VARIABLE, WITH APPROXIMATELY 30% NOT RESPONDING AND ANOTHER 30% HAVING ONLY PARTIAL RESPONSE. WE HAVE SHOWN IN TEST AND VALIDATION COHORTS THAT PRE-TREATMENT CIRCULATING TYPE I IFN (T1IFN) PREDICTS NON-RESPONSE TO TNFI. PRE-TREATMENT IFN-SS TO IFN-A ACTIVITY RATIO >1.3 WAS STRONGLY PREDICTIVE OF NON-RESPONSE TO TNFI (SPECIFICITY=77% IN THE VALIDATION COHORT). NO PATIENT WITH A RATIO >1.3 ACHIEVED REMISSION OR LOW DISEASE ACTIVITY. WE USED SINGLE CELL ANALYSIS TO STUDY BLOOD MONOCYTE (MO) GENE EXPRESSION IN RA PATIENTS WITH HIGH VS. LOW IFN-SS-TO-A ACTIVITY RATIO AND FOUND MAJOR DIFFERENCES BETWEEN THE GROUPS, SUPPORTING DOWNSTREAM EFFECTS UPON A CRITICAL EFFECTOR CELL POPULATION. PRESENCE OR ABSENCE OF JAK1 EXPRESSION STRONGLY ALIGNED WITH IFN-SS-TO-A RATIO. IN RA, BLOOD MO INVADE SYNOVIUM, AND LOCAL STIMULI DRIVE EXPANSION OF INFLAMMATORY MACROPHAGE (MF) POPULATIONS. IT IS NOT KNOWN WHETHER THE CIRCULATING IFN RATIO IN RA REFLECTS T1IFN LEVELS AND PATHWAY ACTIVATION IN THE SYNOVIUM, AND THIS WILL BE EXAMINED IN AIM 1. THE RELATIVE CONTRIBUTIONS OF BLOOD VS. SYNOVIAL SIGNALS IN THE DIAPEDESIS AND DIFFERENTIATION OF MO TO INFLAMMATORY MO/MFS IS NOT KNOWN. THIS WILL BE EXPLORED IN AIM 2 USING A NOVEL PERFUSABLE ORGAN-ON-A-CHIP SYSTEM. HYPOTHESIS: RA TNFI NON-RESPONDERS HAVE INCREASED BLOOD AND SYNOVIAL IFN-SS/A RATIO THAT RESULTS IN ALTERED EXPRESSION OF JAK1 AND IFN-STIMULATED GENES, INCREASED DIAPEDESIS AND DIFFERENTIATION OF MO INTO INFLAMMATORY MFS. I WILL EXPLORE THIS OVERARCHING HYPOTHESIS IN 2 SPECIFIC AIMS: (1) DETECT DIFFERENCES IN T1IFN, PATHWAY ACTIVATION, AND HISTOPATHOLOGY IN SYNOVIUM OF RA PATIENTS WHO HAVE A EULAR GOOD RESPONSE OR NO RESPONSE TO TNFI. (2) DETERMINE THE IMPACT OF IFN-A AND IFN-SS ON RA MO, MO-DERIVED MF, AND FIBROBLAST-LIKE SYNOVIOCYTES. I WILL GAIN NEW SKILLS BIASED AND UNBIASED ANALYSES, DATA ARCHITECTURE/ARTIFICIAL INTELLIGENCE, AND IN USE OF A BIOENGINEERED MICROPHYSIOLOGICAL SYSTEM TO INTERROGATE HUMAN RA BIOLOGY, WHICH I NEED TO SUCCESSFULLY LAUNCH AND ESTABLISH MYSELF AS AN INDEPENDENT INVESTIGATOR ADVANCING PRECISION MEDICINE IN RA. CURRENT AVAILABLE SURGICAL TREATMENTS OFFER ONLY TEMPORARY IMPROVEMENT IN VOICE THROUGH PLACEMENT OF RESORBABLE MATERIALS OR INVOLVE PERMANENT, INVASIVE ALTERATION OF THE LARYNX THAT INCOMPLETELY RESTORES VOICE AND OFTEN REQUIRES REVISION SURGERIES. THERE IS LIMITED ACCESS TO ADVANCED SURGICAL PROCEDURES WITH MODERATELY BETTER OUTCOMES DUE TO THE NEED FOR SPECIALIZED SURGICAL TECHNIQUES AND AN INCREASED RISK OF INVASIVE PROCEDURES. FUNCTIONAL ELECTRICAL STIMULATION (FES) HAS THE POTENTIAL TO RESTORE DYNAMIC VOCAL FOLD (VF) CLOSURE (ADDUCTION), IMPROVING VOICE AND AIRWAY PROTECTIVE OUTCOMES FOR THE UVFP PATIENT POPULATION. LARYNGEAL ELECTROMYOGRAPHY (LEMG) STUDIES HAVE SHOWN THAT ABOUT 85% OF THOSE WITH CHRONIC UVFP DO NOT HAVE COMPLETE VF DENERVATION, OPENING THE DOOR TO LOW-INTENSITY CLOSED-LOOP FES TO REANIMATE PARALYZED VFS. SEVERAL STUDIES HAVE INDEED SHOWN THAT LARYNGEAL MUSCLES WITH SYNKINETIC REINNERVATION CAN BE EFFECTIVELY STIMULATED OR “PACED” TO RESTORE DYNAMIC VF OPENING (ABDUCTION). HOWEVER, LARYNGEAL PACING DEVICES TO DATE HAVE ONLY STIMULATED VF ABDUCTION TO FACILITATE BREATHING AND HAVE ONLY USED PRESET PATTERNS OF STIMULATION (OPEN-LOOP CONTROL) RATHER THAN SENSING AND RESPONDING TO THE PHYSIOLOGICAL NEEDS OF THE INDIVIDUAL (CLOSED-LOOP CONTROL). HEREIN WE DESCRIBE A NOVEL CLOSED-LOOP LARYNGEAL FES SYSTEM FOR REANIMATION OF VF CLOSURE (ADDUCTION) TO TREAT UVFP. SPERM EPIGENETIC DIAGNOSTIC TO INFORM COUPLES TIME TO PREGNANCY - SUMMARY THIS APPLICATION IS FOR AN STTR PHASE I PROJECT AIMED AT DEVELOPING A SPERM-SPECIFIC TEST TO PREDICT THE TIME IT TAKES FOR COUPLES TO CONCEIVE. INFERTILITY IS ONE OF THE MOST COMMON REPRODUCTIVE HEALTH DISORDERS, AFFECTING 17% OF COUPLES IN THE U.S. NOTABLY, INFERTILITY HAS HISTORICALLY BEEN TREATED AS A FEMALE ISSUE; HOWEVER, AROUND HALF OF ALL INFERTILITY CASES ARE PARTIALLY OR COMPLETELY ATTRIBUTED TO MALE FACTORS. CLINICAL MALE INFERTILITY IS TYPICALLY ASCERTAINED BASED ON SEMEN-QUALITY REFERENCE VALUES ESTABLISHED BY THE WORLD HEALTH ORGANIZATION (WHO) CRITERIA; HOWEVER, THESE CRITERIA ARE POOR PREDICTORS OF REPRODUCTIVE SUCCESS, RAISING QUESTIONS ABOUT THE SIGNIFICANCE AND RELIABILITY OF CONVENTIONAL METHODS FOR EVALUATING MALE FERTILITY. DESPITE SIGNIFICANT ADVANCES IN UNDERSTANDING THE MOLECULAR CHARACTERISTICS OF SEMEN, MEASURES OF MALE FERTILITY HAVE REMAINED UNCHANGED FOR DECADES. THEREFORE, THE DEVELOPMENT OF NOVEL BIOMARKERS OF MALE REPRODUCTIVE HEALTH IS VITAL TO IMPROVING REPRODUCTIVE CLINICAL CARE. SPERMATOGENESIS REQUIRES DYNAMIC REPROGRAMMING OF THE EPIGENOME TO ENABLE THE PROGRESSION FROM DIPLOID SPERMATOGONIA TO HAPLOID MATURE SPERMATOZOA. WE PROPOSE THAT THE ROBUST EPIGENETIC REPROGRAMMING DURING SPERMATOGENESIS PROVIDES A FINAL OPPORTUNITY FOR SPERM TO “EPIGENETICALLY MATCH” WITH THEIR INTERNAL FACTORS (E.G., GENETICS) AND EXTERNAL FACTORS (E.G., ENVIRONMENT) PRIOR TO CONCEPTION. IN RECENT YEARS, EPIGENETIC CLOCKS, WHICH ESTIMATE BIOLOGICAL AGE, HAVE BEEN DEVELOPED AS NOVEL BIOMARKERS FOR PHENOTYPES SUCH AS CANCER AND MORTALITY. IMPORTANTLY, OUR GROUP WAS THE FIRST TO DEVELOP A SPERM-SPECIFIC EPIGENETIC CLOCK, WHICH HAS BEEN ASSOCIATED WITH COUPLES’ PREGNANCY OUTCOMES AND VALIDATED IN AN INDEPENDENT INFERTILITY COHORT. THIS APPLICATION WILL LEVERAGE OUR MACHINE LEARNING PIPELINE AND EXTANT SPERM METHYLATION DATA, ALONG WITH REPRODUCTIVE OUTCOME DATA FROM 1,825 COUPLES FROM BOTH THE GENERAL POPULATION AND INFERTILITY COHORTS, TO FURTHER OPTIMIZE AND VALIDATE A NEW SPERM EPIGENETIC CLOCK THAT PREDICTS REPRODUCTIVE OUTCOMES. THE RESEARCH PROPOSED IN THIS APPLICATION WILL OPTIMIZE AND VALIDATE OUR PREDICTION MODELS ACROSS THREE DISTINCT COHORTS, DEMONSTRATING THE FEASIBILITY OF OUR PRODUCT AS A POTENTIAL MALE FERTILITY DIAGNOSTIC TOOL TO INFORM CLINICAL DECISION-MAKING AND SUPPORT THE MANAGEMENT OF COUPLES SEEKING TO ACHIEVE PREGNANCY IN BOTH CLINICAL AND NON-CLINICAL SAMPLES.
