FOCUS AREA: PRIMARY CARE/MATERNAL FETAL MEDICINE. PROJECT TITLE: PERINATAL OUTREACH AND RURAL TELEMEDICINE (PORT) PROJECT; FUNDS REQUESTED: $300,000 IN YEAR 1, $300,000 IN YEAR 2; 300,000 IN YEAR 3; AND, 300,000 IN YEAR 4. THE PERINATAL OUTREACH AND RURAL TELEMEDICINE (PORT) PROJECT PROVIDES PERINATAL CARE SPECIALIST SERVICES AT THREE RURAL PRIMARY CARE HEALTH CLINICS IN CENTRAL MINNESOTA, EXPANDING ACCESS TO PERINATAL CARE FOR AN ADDITIONAL 1,872 PATIENTS ANNUALLY WITH HIGH RISK PREGNANCIES WHO REQUIRE THESE SERVICES AND WHO WOULD NOT OTHERWISE HAVE ACCESS TO THESE SERVICES. THE APPLICANT ORGANIZATION IS CENTRACARE'S (CC) MATERNAL-FETAL MEDICINE (MFM) UNIT. CC IS ONE OF THE LARGEST HEALTH CARE SYSTEMS IN MINNESOTA AND INCLUDES HOSPITALS AND CLINICS TO SERVE THE HEALTH CARE NEEDS OF A RURAL, LOW-INCOME POPULATION. THE TARGET SERVICE AREA CONSISTS OF THREE RURAL COMMUNITIES THAT INCLUDE THE CITY OF MELROSE WITH A POPULATION OF 3,650 AND THE CITY OF PAYNESVILLE WITH A POPULATION OF 2,388 IN RURAL PORTIONS OF STEARNS COUNTY OUTSIDE OF ST. CLOUD, MINNESOTA’S METROPOLITAN AREA; AND, THE CITY OF WADENA WITH A POPULATION OF 4,325 IN WADENA COUNTY. EACH SITE IS HEALTH PROFESSIONAL SHORTAGE AREAS (HPSA) FOR PRIMARY CARE HPSA, DENTAL HEALTH AND MENTAL HEALTH. MELROSE IS IN A MEDICALLY UNDERSERVED AREA. ALL SITES ARE RURAL PER HRSA’S ELIGIBILITY ANALYZER. NETWORK PARTNERS ARE THE FOLLOWING: RURAL PRIMARY CLINIC SITES CC-MELROSE, CC-PAYNESVILLE AND TRI-COUNTY HEALTH CARE-WADENA CLINIC. EACH RURAL SITE WILL RECEIVE TELE-MFM SERVICES AT THESE PRIMARY CARE PROVIDER SITES. NONE OF THESE SITES HAVE ACCESS TO ON-SITE PERINATAL CARE. PATIENTS MUST TRAVEL TO CC-ST. CLOUD HOSPITAL MFM UNIT OR TO OTHER PERINATOLOGISTS IN MINNESOTA TO RECEIVE CARE FOLLOWING A LEVEL 1 SONOGRAM THAT INDICATES AN ISSUE WITH THE MOTHER OR FETUS AT A PRIMARY CARE APPOINTMENT OR AFTER SEEING AN OBGYN. NETWORK SERVICES AND ACTIVITIES FOR THE PORT PROJECT INCLUDE: ESTABLISH PERINATOLOGY TELEHEALTH AT THREE RURAL SITES TO INCREASE ACCESS TO PERINATOLOGY SERVICES; PROVIDE HOME HEALTH MONITORING FOR HIGH-RISK PREGNANCIES; IMPLEMENT A TEAM-BASED APPROACH FOR PERINATOLOGY; AND, PROVIDE GENETIC COUNSELING REFERRALS FOR CURRENT AND FUTURE PREGNANCIES; INCREASED REFERRALS TO PRIMARY CARE FOR COMORBIDITIES SUCH AS HYPERTENSION, CARDIOVASCULAR AND DIABETES. PROJECTED OUTCOMES INCLUDE: 1,872 PREGNANT WOMEN PER YEAR ACCESS PORT PROJECT SERVICES; MINIMUM OF 25 PREGNANT WOMEN PER YEAR ACCESS HOME HEALTH MONITORING; 100% OF PORT PATIENT HAVE TEAM-BASED CARE ACCESS; 60% OF PATIENTS RECEIVE GENETIC COUNSELING; AND, A MINIMUM OF 100 PATIENTS RECEIVE PRIMARY CARE SERVICES ANNUALLY WHO HAVE NOT PREVIOUSLY USED THESE SERVICES. CLINICAL SERVICES INCLUDE TELEHEALTH MATERNAL-FETAL CLINICAL EVALUATIONS, TREATMENT AND MATERNAL-FETAL MONITORING WILL SERVE ARE A PROJECTED 1,872 PREGNANT WOMEN EACH YEAR OF THE PROJECT PERIOD WHO CURRENTLY DO NOT HAVE ACCESS TO THESE SERVICES AT THREE PROVIDER SITES IN RURAL CENTRAL MINNESOTA. A MINIMUM OF 1,000 ULTRASOUND SCANS PER YEAR; MFM MONITORING OF 25 PATIENTS PER YEAR; ALL PATIENTS RECEIVE TEAM ACCESS AND ARE REFERRED FOR GENETIC COUNSELING. PORT IS SUSTAINABLE AS SERVICES ARE BILLED AND COSTS ARE REDUCED DUE TO NON-DUPLICATION OF SERVICES FROM LEVEL 1 TO LEVEL 2 ULTRASOUNDS. CC'S BEHAVIORAL HEALTH IMPLEMENTED A HRSA EBTBHNP GRANT AND SHARED ITS EXPERIENCE WITH THE PORT TEAM. PORT DOES NOT DUPLICATE THESE SERVICES AS ITS FOCUS IS ON MFM SERVICES. THE APPLICANT IS ELIGIBLE FOR A FUNDING PREFERENCE UNDER 42 U.S.C. 254C(H)(3)), QUALIFICATION 1 AS ALL RURAL N ETWORK SITES ARE HPSAS.
