THEY ALSO FUNCTION AS THE PRINCIPAL CONNECTORS ACROSS AND AMONG TRIBAL, REGIONAL, AND STATE PUBLIC HEALTH DEPARTMENTS, KNITTING TOGETHER A MORE COHESIVE, INTEGRATED, NATIONAL PUBLIC HEALTH SYSTEM. HOWEVER, THE COVID-19 PANDEMIC TESTED THE RESILIENCY OF LHDS AS THEY SCALED UP COMMUNICATIONS, TESTING, CONTACT TRACING, AND IMMUNIZATION. STRESSORS OF THE COVID-19 RESPONSE HIGHLIGHTED LONGSTANDING ISSUES THAT NACCHO HAS DOCUMENTED THROUGH OUR SEVEN NATIONAL SURVEYS OF LHDS: LACK OF TIMELY, ACTIONABLE; LACK OF NIMBLE HIRING, TRAINING, AND RETENTION SYSTEMS; INABILITY TO SUSTAIN CRITICAL PROGRAMS AND POLICIES IN THE FACE OF COMPETING DEMANDS; AND THE URGENT NEED TO TACKLE HEALTH INEQUITIES THROUGH SUSTAINED ENGAGEMENT WITH THE MOST IMPACTED COMMUNITIES. RESPONSE EFFORTS ALSO TOOK A TOLL ON STAFF, WITH LHDS EXPERIENCING PERVASIVE HARASSMENT AND STAFF REPORTING SIGNIFICANT MENTAL HEALTH CONCERNS. TO ADDRESS THESE ISSUES, THE NATIONAL ASSOCIATION OF COUNTY AND CITY HEALTH OFFICIALS (NACCHO) PROPOSES USING ITS STRATEGIC, THREE-TIERED CAPACITY BUILDING ASSISTANCE (CBA) FRAMEWORK UNDER THE CDC NOTICE OF FUNDING OPPORTUNITY (NOFO) CDC-RFA-PW-24-0080: STRENGTHENING PUBLIC HEALTH SYSTEMS AND SERVICES THROUGH NATIONAL PARTNERSHIPS TO IMPROVE AND PROTECT THE NATION’S HEALTH (CATEGORY A: LOCAL HEALTH DEPARTMENTS). NACCHO IS THE MEMBERSHIP ORGANIZATION FOR THE MORE THAN 3,300 LHDS ACROSS THE UNITED STATES AND IS THE ONLY ENTITY THAT REPRESENTS ALL OF LOCAL PUBLIC HEALTH, FROM SMALL, RURAL LHDS TO LARGE URBAN AGENCIES. SCIENTISTS WILL BE ABLE TO ACCESS THE MODELS, USE DISCIPLINE-SPECIFIC DATA AND OPTIMIZE THE MODELS. THE PROJECT EMPOWERS RESEARCHERS, PROVIDES DOCUMENTATION TO ACCELERATE RESEARCH AND EDUCATION, AND HAS AN ACTIVE PROGRAM IN EARLY-CAREER TRAINING TO ADVANCE US ECONOMIC AND SCIENTIFIC COMPETITIVENESS. IN ADDITION, OPPORTUNITIES PROVIDED THROUGH PARTNERSHIPS WITH A RANGE OF INSTITUTIONS AND PROGRAMS WILL ENHANCE TRAINING. THE LONG-TERM PLAN IS TO MAKE THE INFRASTRUCTURE AVAILABLE AS A LOW- OR ZERO-COST SERVICE TO THE RESEARCH COMMUNITY IN A MANNER LIKE OPEN-SOURCE CODE REPOSITORIES AND SCIENCE-FOCUSED DIGITAL LIBRARIES, TO MAXIMIZE USAGE. THE OMAI RESEARCH INFRASTRUCTURE CONSISTS OF A SERIES OF OPEN, MULTIMODAL LANGUAGE MODELS KEPT UP TO DATE WITH RECENT SCIENTIFIC PUBLICATIONS AND OPEN-SOURCE APPLICATION PROGRAMMING INTERFACES THAT ENABLE SCIENTISTS TO USE, EXPAND, AND MODIFY THOSE MODELS. THE COVID-19 PANDEMIC EXACERBATED LONG-STANDING WEAKNESSES IN GOVERNMENTAL PHWF, DATA SYSTEMS, AND FOUNDATIONAL CAPABILITIES. CDC-RFA-OE22-2203, STRENGTHENING U.S. PUBLIC HEALTH INFRASTRUCTURE, WORKFORCE, & DATA SYSTEMS GRANT PROVIDES RESOURCES FOR S/T/LHAS TO MEET CRITICAL IMMEDIATE NEEDS AND TO PLAN AND MAKE STRATEGIC INVESTMENTS IN PH AGENCY INFRASTRUCTURE FOR THE FUTURE. APPROACH ASTHO PROPOSES THESE ACTIVITIES FOR THE 3 COMPONENT B STRATEGY AREAS: - RAPIDLY COLLECT/REVIEW COMPONENT (COMP) A GRANTEE WORKPLANS TO RESPOND TO NEEDS RELATED TO PHWF, FOUNDATIONAL CAPABILITIES, DATA MODERNIZATION, & GRANT MANAGEMENT. - SUPPORT COMP A RECIPIENTS IN ASSESSMENTS, PLANNING, SYSTEMS, POLICY, & PROCESS IMPROVEMENTS FOR PHWF RECRUITMENT/RETENTION, FOUNDATIONAL PH CAPABILITIES, & ACCREDITATION. - CURATE/DEVELOP TRAININGS TO MEET PHWF NEEDS. - SUPPORT COMP A RECIPIENTS IN PLANNING & IMPLEMENTING AGENCY MANAGEMENT, GOVERNANCE, & SUPPORT STRUCTURES; SYSTEMS & SOLUTIONS; POLICY; & PROCESS/QUALITY IMPROVEMENTS FOR DATA MODERNIZATION & INFORMATICS. - SUPPORT RECRUITMENT/RETENTION OF A DIVERSE PHWF. - SUPPORT UTILIZATION OF NATIONAL PHWF & FINANCE DATA SETS. - COLLABORATE WITH PARTNERS TO DESIGN AN EVALUATION PLAN, SUPPORT DATA COLLECTION, & DISSEMINATE FINDINGS. - FACILITATE CROSS-CUTTING COORDINATION & COMMUNICATION AMONG COMP A RECIPIENTS & CDC. - DEPLOY A COMPREHENSIVE KNOWLEDGE-SHARING & COORDINATION STRATEGY TO IMPROVE COMP A GRANT RECIPIENTS & DMI LEADER SUCCESS. ASTHO WILL LEVERAGE ITS ROBUST INTERNAL CAPACITY AND DEEP ENGAGEMENTS WITH S/T/LHAS AND WILL ENGAGE A NATIONAL COALITION OF PARTNERS, THE NATIONAL EXPERT PARTNER COLLABORATIVE (NEPC). NEPC INCLUDES TRADITIONAL PH PARTNERS (BIG CITIES HEALTH COALITION, PUBLIC HEALTH FOUNDATION, DE BEAUMONT FOUNDATION, MEMBERS OF ASTHO’S AFFILIATE COUNCIL, NATIONAL ASSOCIATION OF STATE BUDGET OFFICERS, NATIONAL CONFERENCE OF STATE LEGISLATURES, NATIONAL ACADEMY FOR STATE HEALTH POLICY) AND NON-TRADITIONAL PARTNERS (DELOITTE, SOCIETY FOR HUMAN RESOURCE MANAGEMENT). AS ESSENTIAL PARTS OF THE COMMUNITIES THEY SERVE, LHDS PLAY A CRITICAL ROLE IN CLINICAL SERVICE PROVISION, POPULATION HEALTH SERVICES, AND EMERGENCY PREPAREDNESS. THEY ALSO FUNCTION AS THE PRINCIPAL CONNECTORS ACROSS AND AMONG TRIBAL, REGIONAL, AND STATE PUBLIC HEALTH DEPARTMENTS, KNITTING TOGETHER A MORE COHESIVE, INTEGRATED, NATIONAL PUBLIC HEALTH SYSTEM. HOWEVER, THE COVID-19 PANDEMIC TESTED THE RESILIENCY OF LHDS AS THEY SCALED UP COMMUNICATIONS, TESTING, CONTACT TRACING, AND IMMUNIZATION. STRESSORS OF THE COVID-19 RESPONSE HIGHLIGHTED LONGSTANDING ISSUES THAT NACCHO HAS DOCUMENTED THROUGH OUR SEVEN NATIONAL SURVEYS OF LHDS: LACK OF TIMELY, ACTIONABLE; LACK OF NIMBLE HIRING, TRAINING, AND RETENTION SYSTEMS; INABILITY TO SUSTAIN CRITICAL PROGRAMS AND POLICIES IN THE FACE OF COMPETING DEMANDS; AND THE URGENT NEED TO TACKLE HEALTH INEQUITIES THROUGH SUSTAINED ENGAGEMENT WITH THE MOST IMPACTED COMMUNITIES. RESPONSE EFFORTS ALSO TOOK A TOLL ON STAFF, WITH LHDS EXPERIENCING PERVASIVE HARASSMENT AND STAFF REPORTING SIGNIFICANT MENTAL HEALTH CONCERNS. TO ADDRESS THESE ISSUES, THE NATIONAL ASSOCIATION OF COUNTY AND CITY HEALTH OFFICIALS (NACCHO) PROPOSES USING ITS STRATEGIC, THREE-TIERED CAPACITY BUILDING ASSISTANCE (CBA) FRAMEWORK UNDER THE CDC NOTICE OF FUNDING OPPORTUNITY (NOFO) CDC-RFA-PW-24-0080: STRENGTHENING PUBLIC HEALTH SYSTEMS AND SERVICES THROUGH NATIONAL PARTNERSHIPS TO IMPROVE AND PROTECT THE NATION’S HEALTH (CATEGORY A: LOCAL HEALTH DEPARTMENTS). NACCHO IS THE MEMBERSHIP ORGANIZATION FOR THE MORE THAN 3,300 LHDS ACROSS THE UNITED STATES AND IS THE ONLY ENTITY THAT REPRESENTS ALL OF LOCAL PUBLIC HEALTH, FROM SMALL, RURAL LHDS TO LARGE URBAN AGENCIES. FOR 30 YEARS, NACCHO HAS CONSISTENTLY STRENGTHENED THE NATION’S LHDS IN WAYS THAT MEET THE GOALS AND FIVE STRATEGIC AREAS OF THIS NOFO. NACCHO’S CBA DRIVES LHD PRACTICE THROUGH DEMONSTRATION SITES, IN-PERSON AND VIRTUAL TRAININGS, PEER MENTORSHIP PROGRAMS, AND TOOLS AND RESOURCES LHDS USE DAILY. NACCHO’S EXTENSIVE KNOWLEDGE OF LHD DIVERSITY—ACROSS FACTORS INCLUDING SIZE, FUNDING, GOVERNANCE, AND RURALITY/URBANICITY—MAKES IT THE MOST TRUSTED PROVIDER OF CBA FOR ALL LHDS.

RESEARCH INFRASTRUCTURE: NSF MID-SCALE RI-2: OPEN MULTIMODAL AI INFRASTRUCTURE TO ACCELERATE SCIENCE -LANGUAGE MODELS WITH BILLIONS OF POSSIBLE ADJUSTMENTS AND TRAINED ON TRILLIONS OF WORDS ARE NOW POWERING THE FASTEST-GROWING COMPUTING APPLICATIONS IN HISTORY. LARGE LANGUAGE MODELS (LLM) ARE BUILT USING MASSIVE AMOUNTS OF TEXT, USUALLY OBTAINED BY PULLING DATA FROM MULTIPLE SOURCES ON THE INTERNET. RECENT ADVANCES ENABLE THESE MODELS TO PROCESS OTHER KINDS OF DATA, INCLUDING IMAGES, GRAPHS AND TABLES. MODELS WITH THESE ABILITIES ARE KNOWN AS MULTIMODAL LLMS. THE BEST-PERFORMING LLMS CURRENTLY DEPLOYED ARE PROPRIETARY, SO THEIR PARAMETERS, TRAINING DATA, CODE AND DOCUMENTATION ARE NOT OPENLY AVAILABLE. THUS, MOST ARTIFICIAL INTELLIGENCE (AI) SCIENTISTS CANNOT STUDY, EXPERIMENT DIRECTLY WITH, OR IMPROVE THESE STATE-OF-THE-ART MODELS. THIS PROJECT ? OPEN, MULTIMODAL ARTIFICIAL INTELLIGENCE (OMAI) - WILL PROVIDE INFRASTRUCTURE IN THE FORM OF A SUITE OF POWERFUL, WELL-DOCUMENTED, UP-TO-DATE, OPEN MODELS, AND OPEN-SOURCE INTERFACES DESIGNED FOR SCIENTIFIC WORK. SCIENTISTS WILL BE ABLE TO ACCESS THE MODELS, USE DISCIPLINE-SPECIFIC DATA AND OPTIMIZE THE MODELS. THE PROJECT EMPOWERS RESEARCHERS, PROVIDES DOCUMENTATION TO ACCELERATE RESEARCH AND EDUCATION, AND HAS AN ACTIVE PROGRAM IN EARLY-CAREER TRAINING TO ADVANCE US ECONOMIC AND SCIENTIFIC COMPETITIVENESS. IN ADDITION, OPPORTUNITIES PROVIDED THROUGH PARTNERSHIPS WITH A RANGE OF INSTITUTIONS AND PROGRAMS WILL ENHANCE TRAINING. THE LONG-TERM PLAN IS TO MAKE THE INFRASTRUCTURE AVAILABLE AS A LOW- OR ZERO-COST SERVICE TO THE RESEARCH COMMUNITY IN A MANNER LIKE OPEN-SOURCE CODE REPOSITORIES AND SCIENCE-FOCUSED DIGITAL LIBRARIES, TO MAXIMIZE USAGE. THE OMAI RESEARCH INFRASTRUCTURE CONSISTS OF A SERIES OF OPEN, MULTIMODAL LANGUAGE MODELS KEPT UP TO DATE WITH RECENT SCIENTIFIC PUBLICATIONS AND OPEN-SOURCE APPLICATION PROGRAMMING INTERFACES THAT ENABLE SCIENTISTS TO USE, EXPAND, AND MODIFY THOSE MODELS. IT ADDRESSES PRIORITIES SET FORTH IN THE WHITE HOUSE AI ACTION PLAN (HTTPS://WWW.WHITEHOUSE.GOV/WP-CONTENT/UPLOADS/2025/07/AMERICAS-AI-ACTION-PLAN.PDF) TO ACCELERATE AI-ENABLED SCIENCE AND ENSURE THE UNITED STATES IS PRODUCING THE LEADING OPEN MODELS. THE INFRASTRUCTURE AIMS TO ACCELERATE SCIENTIFIC DISCOVERY ACROSS DISCIPLINES RANGING FROM MATERIALS TO PROTEIN FUNCTION PREDICTION AND WEATHER MODELS. IT WILL ALSO ENABLE NEW UNDERSTANDING AND IMPROVEMENT OF FUTURE LLMS WHILE CONTRIBUTING TO THE DEVELOPMENT OF A WELL-TRAINED WORKFORCE CAPABLE OF BUILDING, CUSTOMIZING, AND MAINTAINING SUCH MODELS. IN ALLOWING RESEARCHERS TO FINE TUNE THE MODELS, RESEARCHERS CAN OPTIMIZE PERFORMANCE AND UNDERSTAND DESIGN DECISIONS THAT INFLUENCE TRAINING SPEED AND STABILITY, WHICH CAN IMPACT THE SHORT-TERM AND LONG-TERM ECONOMIC COSTS OF LLM DEVELOPMENT. THE PROJECT EMPHASIZES REPRODUCIBILITY, TRANSPARENCY, OPEN DATA, OPEN AND EVOLVING EVALUATIONS, MULTIMODALITY, AND SCIENTIFIC USE-CASES. IT WILL ENABLE A BROAD POPULATION OF SCIENTIST-USERS ACROSS ALL DISCIPLINES TO USE AND ADAPT ARTIFICIAL INTELLIGENCE MODELS TO THEIR OWN NEEDS AND LAYS THE FOUNDATION FOR FUTURE RESEARCH IN AI FOR SCIENCE. BY SUPPORTING WORK IN THESE NOVEL, CRITICAL RESEARCH AREAS, OMAI CAN ULTIMATELY BENEFIT BOTH SCIENCE AND THE PUBLIC. THIS AWARD REFLECTS NSF'S STATUTORY MISSION AND HAS BEEN DEEMED WORTHY OF SUPPORT THROUGH EVALUATION USING THE FOUNDATION'S INTELLECTUAL MERIT AND BROADER IMPACTS REVIEW CRITERIA.- SUBAWARDS ARE PLANNED FOR THIS AWARD.

AFDO HAS BEEN REPRESENTING STATE REGULATORY FOOD PROGRAMS SINCE 1896. THE PRINCIPAL INVESTIGATOR AND PROJECT STAFF ASSIGNED TO THIS PROJECT INCLUDE FORMER STATE FOOD PROGRAM MANAGERS THAT HAVE EXTENSIVE EXPERIENCE DEVELOPING STATE FOOD SAFETY PROGRAMS, IMPLEMENTING PROGRAMMATIC CHANGE TO ACHIEVE CONFORMANCE WITH THE REQUIREMENTS OF THE MFRPS, AND COLLABORATING WITH FDA TO ACHIEVE A NATIONAL IFSS. MEDICAL RESERVE CORPS SMALL GRANTS PROGRAM - THE NATIONAL ASSOCIATION OF COUNTY AND CITY HEALTH OFFICIALS (NACCHO) SUPPORTS THIS FIVE-YEAR MEDICAL RESERVE CORPS (MRC) SMALL GRANTS PROGRAM. THE PROJECT GOAL IS TO DEVELOP AND SUSTAIN A TRAINED AND READY VOLUNTEER WORKFORCE TO SUPPORT LOCAL RESPONSE NEEDS THROUGH BUILDING CAPABILITIES OF THE MRC NETWORK, IDENTIFYING AND ADDRESSING RESPONSE BARRIERS, AND DEVELOPING A SUSTAINABILITY PLAN. NACCHO’S OBJECTIVES ARE TO: 1) PROVIDE A FIVE-YEAR MRC UNIT SUB-AWARD FUNDING OPPORTUNITY TO STRENGTHEN UNIT RESPONSE CAPABILITIES; 2) IDENTIFY AND EVALUATE IMPACT OF MRC SUB-AWARD FUNDING TO IMPROVE RESPONSE CAPABILITIES; 3) RESTRUCTURE, UPDATE, AND PROMOTE THE USE OF COMMON DEPLOYMENT READINESS GUIDELINES; 4) IDENTIFY AND PROMOTE THE MRC NETWORK CAPABILITIES; 5) IMPROVE INTEGRATION OF THE MRC IN LOCAL, STATEWIDE, AND/OR INTERSTATE DEPLOYMENTS; 6) PROVIDE RESOURCES AND TRAINING TO SUPPORT SUSTAINABILITY OF UNIT LEADERS, VOLUNTEERS, AND MRC UNITS; AND, 7) INCREASE MRC PROMOTION THROUGH BRAND AWARENESS AND RECOGNITION.
