(ACHC) IS A STABLE, JOINT COMMISSION & PCMH ACCREDITED, 330 FUNDED FQHC THAT PROVIDES COMPREHENSIVE PRIMARY CARE SERVICES TO VULNERABLE POPULATIONS IN POTTAWATTAMIE COUNTY AND OTHER AREAS OF WESTERN IOWA. ACHC ACHIEVED FQHC STATUS IN 1998 AND IS AN INTEGRAL COMPONENT OF THE REGION’S HEALTH DELIVERY NETWORK. ACHC PROVIDES A SEAMLESS CONTINUUM OF CARE FOR VULNERABLE PEOPLE THAT HAS IMPROVED THE HEALTH STATUS OF ITS PATIENTS. IN 2023, ACHC SERVED OVER 5,600 INDIVIDUALS AND PROVIDED OVER 24,000 MEDICAL, DENTAL, BEHAVIORAL HEALTH AND ENABLING VISITS. ACHC’S PRIMARY FOCUS IS TREATING MEDICALLY VULNERABLE PEOPLE LIVING UNDER 200% FPG, WHO ARE EITHER UNINSURED, UNDERINSURED OR HAVE PUBLIC INSURANCE. THIS DEMOGRAPHIC IS REFLECTED IN THE HEALTH CENTER’S CURRENT USERS, OF WHICH APPROXIMATELY 71% LIVE BELOW 200% FPG, 18% ARE UNINSURED, 52% ARE INSURED THROUGH MEDICAID, AND 15% ARE LOW-INCOME MEDICARE USERS. ACHC’S BOARD CONTINUALLY EVALUATES THE HEALTH NEEDS AND BARRIERS TO CARE IN THE COMMUNITY AND EXPANDS ITS SERVICE DELIVERY CAPACITY AS NEEDED BY ADDING PROGRAMS, SERVICES AND PROVIDERS THAT ADDRESS COMMUNITY NEEDS. THROUGH THESE BOARD EFFORTS, THE HEALTH CENTER HAS IDENTIFIED A NEED TO DEVELOP A COMPREHENSIVE PROGRAM TO ADDRESS BOTH BEHAVIORAL HEALTH (BH) AND SUD/MAT NEEDS IN THE SERVICE AREA. THE SERVICE AREA IS STRUGGLING WITH A BH CRISIS. WITHIN THE SERVICE AREA, 30% OF ADULTS HAD A DIAGNOSED DEPRESSIVE DISORDER &/OR SYMPTOMS OF CHRONIC DEPRESSIO N, AND SUICIDE RATES ARE HIGH. WHILE DRUG OVERDOSE DEATHS HAVE DECLINED SINCE 2022, A SIGNIFICANT PORTION OF THE POPULATION AGED 12+ (18.8%) HAVE A SUBSTANCE USE DISORDER, PUTTING THEM AT HIGHER RISK FOR OTHER MEDICAL AND MH/BH ISSUES, INCLUDING OVERDOSE. DESPITE GROWING NEEDS, MANY PEOPLE ARE UNABLE TO ACCESS CARE DUE TO LACK OF CAPACITY AND BARRIERS TO CARE, WITH 25.4% OF ADULTS SEEKING BUT NOT RECEIVING CARE IN 2023. WITH THIS BH SERVICE EXPANSION FUNDING, ALL CARE WILL STRENGTHEN ITS ABILITY TO PROVIDE AN INTEGRATED CONTINUUM OF CARE FOR BH & SUD PATIENTS, INCLUDING THOSE PATIENTS WHO REQUIRE MAT TO ACHIEVE SOBRIETY. THE INTEGRATED CARE CONTINUUM STARTS WITH ONE-ON-ONE TREATMENT WITH AN ALL CARE PROVIDER (EITHER A BH PROVIDER OR MAT PROVIDER) THAT IS COORDINATED WITH A PRIMARY CARE PROVIDER TO ENSURE THAT ALL ASPECTS OF THE PATIENT’S HEALTH ARE ADDRESSED. PATIENTS WILL HAVE ACCESS TO INTENSIVE CASE MANAGEMENT, EVALUATION FOR SDOH AND CONNECTION TO SERVICES THAT WILL ADDRESS ACCESS BARRIERS, PEER RECOVERY SERVICES INCLUDING GROUP THERAPY, AND ACCESS TO INPATIENT AND RESIDENTIAL SERVICES THROUGH THE HEALTH CENTER’S COLLABORATIONS FOR THESE SERVICES. TO SUPPORT THIS CARE CONTINUUM, ALL CARE WILL HIRE A 0.20 FTE NURSE PRACTITIONER TO PROVIDE MAT SERVICES; A 1.0 FTE LICENSED ALCOHOL & DRUG COUNSELOR AND 2.0 FTE LICENSED THERAPISTS TO SUPPORT BOTH MAT AND BH PATIENTS; A 1.0 FTE SOCIAL WORKER TO ASSIST PATIENTS WITH SIGNIFICANT SOCIAL SERVICES NEEDS REMOVE ACCESS BARRIERS; A 1.0 FTE COMMUNITY HEALTH WORKER/CASE MANAGER TO ASSESS ALL PATIENTS FOR SDOH, ASSIST PATIENTS ADDRESS ACCESS BARRIERS AND PROVIDE INTENSIVE CASE MANAGEMENT TO ENSURE THAT PATIENTS REMAIN IN CARE; AND 3.0 FTE MEDICAL SUPPORT STAFF. THROUGH THESE EFFORTS, ALL CARE WILL BE ABLE TO EXPAND ACCESS TO BOTH SUD/MAT AND BH SERVICES AND EXPECTS TO BE ABLE TO TREAT 28 MAT PATIENTS, PROVIDE BH SERVICES TO 78 SUD/MAT PATIENTS AND PROVIDE BH SERVICES TO 242 PATIENTS. EXPANDED SERVICES WILL BE AVAILABLE FOR BOTH EXISTING AND NEW PATIENTS.
