THE COVID-19 PANDEMIC HAS ONLY EXACERBATED THE FOOD INSECURITY OF AREA RESIDENTS. THE MERCY HEALTH PRX HAS SUCCESSFULLY PROVIDED SIGNIFICANT ASSISTANCE TO LOW-INCOME PATIENTS, WITH AVERAGE COHORT SIZES OF 200 ANNUALLY FROM 2013-2017 AND AN AVERAGE SIZE OF 606 FROM 2018-2020, DUE TO SUPPORT FROM THE 2017 USDA FINI AWARD 2017-70025-26687. HOWEVER, WITHOUT FUNDING FROM GUSNIP CRR PRX, THE NUMBER OF PARTICIPANTS WOULD LIKELY BE REDUCED TO PRE-FINI NUMBERS, LEAVING HUNDREDS OF LOW-INCOME PATIENTS WITHOUT AFFORDABLE ACCESS TO FRESH PRODUCE.THE ORIGINAL GOALS OF THE 2017 FINI AWARD WERE 1) TO INCREASE THE PURCHASE OF FRESH FRUITS AND VEGETABLES BY LOW-INCOME CONSUMERS PARTICIPATING IN SNAP; 2) TO INSTITUTIONALIZE NUTRITION EDUCATION INTO ALL LEVELS OF A REGIONAL HEALTH SYSTEM TO IMPROVE OUTCOMES; AND 3) TO INCREASE THE VIABILITY OF FARMERS' MARKETS AND THE NUMBER OF VENDORS PARTICIPATING. THESE GOALS WERE ACHIEVED BY SIGNIFICANTLY INCREASING THE MERCY HEALTH PRX COHORT SIZE ANNUALLY--FROM 200 TO 800--AND BY OFFERING PRX ENROLLMENT AT 24 MERCY HEALTH CLINICS IN THE YOUNGSTOWN REGION. FOOD INSECURITY SCREENING, NUTRITION EDUCATION, AND EVALUATION BECAME AN INTEGRAL PART OF THE PROGRAM. FARMERS MARKETS AND FARMER COOPERATIVES WERE STRENGTHENED BY THE INCREASED CUSTOMER BASE OF LOW-INCOME PATIENTS ABLE TO PURCHASE FRESH PRODUCE. EVEN THROUGHOUT THE COVID-19 PANDEMIC, PROJECT PARTNERS WERE ABLE TO ADJUST TO CHALLENGING CIRCUMSTANCES BY WORKING WITH LOCAL FARMERS TO ASSEMBLE, DISTRIBUTE AND DELIVER BOXES OF FRESH PRODUCE DIRECTLY TO ENROLLED PATIENTS, AS SEVERAL LOCAL FARMERS MARKETS CEASED OPERATIONS IN 2020.THE PROPOSED 2021 GUSNIP CRR PRX PROJECT WILL EXPAND THE GOALS OF ORIGINAL AWARD BY INCREASING THE NUMBER OF MERCY HEALTH PRX PARTICIPANTS, EXTENDING THE PARTICIPATION PERIOD TO SIX (6) MONTHS PER YEAR, OFFERING A NEW BRICK-AND-MORTAR FACILITY FOR PRESCRIPTION REDEMPTION WITH EXTENDED HOURS, PROVIDE DELIVERY SERVICE TO PARTICIPANTS WITH TRANSPORTATION ISSUES, OFFER DIRECT ASSISTANCE AND NUTRITION EDUCATION TO FOOD INSECURE PATIENTS WITH DIET-RELATED ILLNESS, AND CONTINUE TO SUPPORT THE VIABILITY OF LOCAL FARMERS THROUGH THE PRIORITIZATION OF PROVIDING ACCESS TO FRESH, LOCALLY-GROWN PRODUCE TO PROGRAM PARTICIPANTS.THE 2017 FINI AWARD INCLUDED BOTH SNAP INCENTIVES AND PRODUCE PRESCRIPTIONS; HOWEVER, FOR PURPOSES OF THE 2021 GUSNIP CRR, OUR PROPOSED PROJECT FOCUSES ON THE MERCY HEALTH PRX. A BUDGET COMPARISON OF THE 2017 FINI AND THE 2021 GUSNIP CRR IS INCLUDED IN THE TABLE BELOW. AS DETAILED IN THE BUDGET JUSTIFICATION, FUNDS FOR EQUIPMENT, TRAVEL, MATERIALS AND SUPPLIES, PUBLICATIONS, AND A PORTION OF PERSONNEL COSTS WILL BE PROVIDED BY A GRANT FROM THE MERCY HEALTH FOUNDATION MAHONING VALLEY, IN ORDER TO MAXIMIZE THE AMOUNT OF 2021 GUSNIP CRR FUNDS REACHING PARTICIPANTS DIRECTLY. PRX FUNDS WILL SUPPORT PATIENT COHORTS FOR THREE YEARS, BEGINNING ON JUNE 15, 2021 THROUGH JUNE 14, 2024. THE PROPOSED 2021 GUSNIP CRR PROJECT WILL INCORPORATE HOME DELIVERY SERVICES OF PRODUCE BOXES AS PART OF THE MERCY HEALTH PRX, AS LACK OF TRANSPORTATION WAS A SIGNIFICANT BARRIER TO PATIENT PARTICIPATION. IN 2020, IN RESPONSE TO THE PANDEMIC, MORE THAN 1,000 PRODUCE BOXES WERE DELIVERED DIRECTLY TO THE HOMES OF LOW-INCOME PATIENTS. MERCY HEALTH HAS DEDICATED FUTURE FUNDING TOWARD DELIVERY SERVICES FOR ENROLLED PATIENTS. ANOTHER SIGNIFICANT BARRIER IDENTIFIED BY PARTICIPANTS WAS THE DIFFICULTY OF FARMERS MARKET ATTENDANCE DUE TO LIMITED HOURS OF OPERATION. IN RESPONSE, THE PROJECT TEAM PLANS TO OPEN A NEW BRICK-AND-MORTAR FACILITY THAT WILL BE OPEN YEAR-ROUND AND PRIORITIZE LOCALLY- AND REGIONALLY-PRODUCED FRUITS AND VEGETABLES, PROVIDING INCREASED ACCESS AND CHOICE FOR PROGRAM PARTICIPANTS. THE BRICK-AND-MORTAR FACILITY--TO BE OPENED IN 2021 BY YNDC AND MERCY HEALTH--WILL PROVIDE SPACE FOR NUTRITION EDUCATION AND HEALTH SCREENINGS BY MERCY HEALTH STAFF. THE COVID-19 PANDEMIC HAS ONLY EXACERBATED THE FOOD INSECURITY OF AREA RESIDENTS. THE MERCY HEALTH PRX HAS SUCCESSFULLY PROVIDED SIGNIFICANT ASSISTANCE TO LOW-INCOME PATIENTS, WITH AVERAGE COHORT SIZES OF 200 ANNUALLY FROM 2013-2017 AND AN AVERAGE SIZE OF 606 FROM 2018-2020, DUE TO SUPPORT FROM THE 2017 USDA FINI AWARD 2017-70025-26687. HOWEVER, WITHOUT FUNDING FROM GUSNIP CRR PRX, THE NUMBER OF PARTICIPANTS WOULD LIKELY BE REDUCED TO PRE-FINI NUMBERS, LEAVING HUNDREDS OF LOW-INCOME PATIENTS WITHOUT AFFORDABLE ACCESS TO FRESH PRODUCE.THE ORIGINAL GOALS OF THE 2017 FINI AWARD WERE 1) TO INCREASE THE PURCHASE OF FRESH FRUITS AND VEGETABLES BY LOW-INCOME CONSUMERS PARTICIPATING IN SNAP; 2) TO INSTITUTIONALIZE NUTRITION EDUCATION INTO ALL LEVELS OF A REGIONAL HEALTH SYSTEM TO IMPROVE OUTCOMES; AND 3) TO INCREASE THE VIABILITY OF FARMERS' MARKETS AND THE NUMBER OF VENDORS PARTICIPATING. THESE GOALS WERE ACHIEVED BY SIGNIFICANTLY INCREASING THE MERCY HEALTH PRX COHORT SIZE ANNUALLY--FROM 200 TO 800--AND BY OFFERING PRX ENROLLMENT AT 24 MERCY HEALTH CLINICS IN THE YOUNGSTOWN REGION. FOOD INSECURITY SCREENING, NUTRITION EDUCATION, AND EVALUATION BECAME AN INTEGRAL PART OF THE PROGRAM. FARMERS MARKETS AND FARMER COOPERATIVES WERE STRENGTHENED BY THE INCREASED CUSTOMER BASE OF LOW-INCOME PATIENTS ABLE TO PURCHASE FRESH PRODUCE. EVEN THROUGHOUT THE COVID-19 PANDEMIC, PROJECT PARTNERS WERE ABLE TO ADJUST TO CHALLENGING CIRCUMSTANCES BY WORKING WITH LOCAL FARMERS TO ASSEMBLE, DISTRIBUTE AND DELIVER BOXES OF FRESH PRODUCE DIRECTLY TO ENROLLED PATIENTS, AS SEVERAL LOCAL FARMERS MARKETS CEASED OPERATIONS IN 2020.THE PROPOSED 2021 GUSNIP CRR PRX PROJECT WILL EXPAND THE GOALS OF ORIGINAL AWARD BY INCREASING THE NUMBER OF MERCY HEALTH PRX PARTICIPANTS, EXTENDING THE PARTICIPATION PERIOD TO SIX (6) MONTHS PER YEAR, OFFERING A NEW BRICK-AND-MORTAR FACILITY FOR PRESCRIPTION REDEMPTION WITH EXTENDED HOURS, PROVIDE DELIVERY SERVICE TO PARTICIPANTS WITH TRANSPORTATION ISSUES, OFFER DIRECT ASSISTANCE AND NUTRITION EDUCATION TO FOOD INSECURE PATIENTS WITH DIET-RELATED ILLNESS, AND CONTINUE TO SUPPORT THE VIABILITY OF LOCAL FARMERS THROUGH THE PRIORITIZATION OF PROVIDING ACCESS TO FRESH, LOCALLY-GROWN PRODUCE TO PROGRAM PARTICIPANTS.THE 2017 FINI AWARD INCLUDED BOTH SNAP INCENTIVES AND PRODUCE PRESCRIPTIONS; HOWEVER, FOR PURPOSES OF THE 2021 GUSNIP CRR, OUR PROPOSED PROJECT FOCUSES ON THE MERCY HEALTH PRX. A BUDGET COMPARISON OF THE 2017 FINI AND THE 2021 GUSNIP CRR IS INCLUDED IN THE TABLE BELOW. AS DETAILED IN THE BUDGET JUSTIFICATION, FUNDS FOR EQUIPMENT, TRAVEL, MATERIALS AND SUPPLIES, PUBLICATIONS, AND A PORTION OF PERSONNEL COSTS WILL BE PROVIDED BY A GRANT FROM THE MERCY HEALTH FOUNDATION MAHONING VALLEY, IN ORDER TO MAXIMIZE THE AMOUNT OF 2021 GUSNIP CRR FUNDS REACHING PARTICIPANTS DIRECTLY. PRX FUNDS WILL SUPPORT PATIENT COHORTS FOR THREE YEARS, BEGINNING ON JUNE 15, 2021 THROUGH JUNE 14, 2024. THE PROPOSED 2021 GUSNIP CRR PROJECT WILL INCORPORATE HOME DELIVERY SERVICES OF PRODUCE BOXES AS PART OF THE MERCY HEALTH PRX, AS LACK OF TRANSPORTATION WAS A SIGNIFICANT BARRIER TO PATIENT PARTICIPATION. IN 2020, IN RESPONSE TO THE PANDEMIC, MORE THAN 1,000 PRODUCE BOXES WERE DELIVERED DIRECTLY TO THE HOMES OF LOW-INCOME PATIENTS. MERCY HEALTH HAS DEDICATED FUTURE FUNDING TOWARD DELIVERY SERVICES FOR ENROLLED PATIENTS. ANOTHER SIGNIFICANT BARRIER IDENTIFIED BY PARTICIPANTS WAS THE DIFFICULTY OF FARMERS MARKET ATTENDANCE DUE TO LIMITED HOURS OF OPERATION. IN RESPONSE, THE PROJECT TEAM PLANS TO OPEN A NEW BRICK-AND-MORTAR FACILITY THAT WILL BE OPEN YEAR-ROUND AND PRIORITIZE LOCALLY- AND REGIONALLY-PRODUCED FRUITS AND VEGETABLES, PROVIDING INCREASED ACCESS AND CHOICE FOR PROGRAM PARTICIPANTS. THE BRICK-AND-MORTAR FACILITY--TO BE OPENED IN 2021 BY YNDC AND MERCY HEALTH--WILL PROVIDE SPACE FOR NUTRITION EDUCATION AND HEALTH SCREENINGS BY MERCY HEALTH STAFF. PRIVATE SPACE WILL ALSO BE PROVIDED FOR PROGRAM PARTICIPANTS TO COMPLETE PRE- AND POST- SURVEYS REQUIRED BY THE NTAE EVALUATION. THE YOUNGSTOWN NEIGHBORHOOD DEVELOPMENT CORPORATION (YNDC) AND HEALTHCARE PARTNER MERCY HEALTH YOUNGSTOWN (MERCY) REQUEST A GUSNIP PRODUCE PRESCRIPTION (PPR) PROGRAM AWARD TO DEVELOP THE EATING FOR HEALTH: GLENWOOD FRESH MARKET PRODUCE PRESCRIPTION PROGRAM, WHICH EXPANDS UPON OUR EXISTING PPR PROGRAM BASED ON PARTICIPANT FEEDBACK AND PARTNERSHIP WITH THE USDA. THE PROGRAM WILL PROVIDE $40/MONTH PRODUCE PRESCRIPTIONS FOR 6-MONTH DURATIONS TO 2,000 HOUSEHOLDS (4,000 PARTICIPANTS) LOCATED IN YOUNGSTOWN AND MAHONING COUNTY, OHIO, FROM A SINGLE BRICK-AND-MORTAR SITE WITH WEEKLY HOURS THAT OFFERS FRESH PRODUCE YEAR-ROUND THAT IS LOCALLY GROWN AND/OR SOURCED IN ORDER TO ENHANCE THE VIABILITY OF OUR LOCAL FOOD SYSTEM. EVIDENCE-BASED NUTRITION EDUCATION, HEALTH SCREENINGS, AND EVALUATION WILL BE CONDUCTED ON SITE IN COORDINATION BETWEEN YNDC AND MERCY. THE SITE IS LOCATED IN A RETAIL PLAZA OWNED BY YNDC IN A COMMUNITY WHERE HUNDREDS OF HOUSEHOLDS REPORTED THAT ACCESS TO FRESH FRUITS AND VEGETABLES IS A HIGH PRIORITY FOR IMPROVING THEIR HEALTH.THE PROGRAM WILL STRENGTHEN THE LOCAL FOOD AND HEALTHCARE SYSTEMS' CAPACITIES TO SUPPORT THE NUTRITIONAL AND FOOD SECURITY NEEDS OF PEOPLE PREDISPOSED TO DIETARY ILLNESS AND HELP TO REDUCE HEALTHCARE USAGE IN ALIGNMENT WITH LOCAL HEALTH PLAN PRIORITIES BY MAKING FRESH, LOCALLY GROWN AND SOURCED PRODUCE FREE AND ACCESSIBLE TO THEM ON A ROUTINE BASIS IN A SETTING THAT IS COMFORTABLE, INVITING, AND WHERE PARTICIPANTS CAN BUILD STRONG RELATIONSHIPS AND TRUST WITH STAFF AND COMMUNITY HEALTH WORKERS.

THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. LIMITATION ON USE OF FUNDS: NO ASSISTANCE PROVIDED UNDER PROGRAM (OR ANY STATE OR LOCAL GOVERNMENT FUNDS USED TO SUPPLEMENT THIS ASSISTANCE) MAY BE USED TO REPLACE STATE OR LOCAL FUNDS PREVIOUSLY USED, OR DESIGNATED FOR USE, TO ASSIST HOMELESS PERSONS OR PERSONS AT-RISK OF HOMELESSNESS.; EXPECTED OUTCOMES: DECREASE IN THE NUMBER INDIVIDUALS AND FAMILIES EXPERIENCING HOMELESSNESS, MORE SPECIFICALLY USING PERFORMANCE INDICATORS SUCH AS THE LENGTH OF TIME HOMELESS, RETURNS TO HOMELESSNESS OVER TIME, AND EXITS TO PERMANENT HOUSING.
