LOUIS REGION - AND IS REGISTERED WITH THE SECRETARY OF STATE TO CONDUCT BUSINESS IN MISSOURI, AS A 501(C)(3) NON-PROFIT ORGANIZATION. CARESTL HEALTH IS PRESENTLY NCQA CERTIFIED AS A LEVEL 3 PATIENT CENTERED MEDICAL HOME, PROVIDING INTEGRATED HEALTH CARE SERVICES ACROSS PRIMARY CARE, BEHAVIORAL HEALTH, ANCILLARY SERVICES, AND ORAL HEALTH CARE FOR NEARLY TWENTY-FIVE THOUSAND (25,000) UNIQUE PATIENTS ANNUALLY. AT PRESENT THE ORGANIZATIONS FOOTPRINT SPANS ACROSS FIVE (5) UNIQUE OUTPATIENT HEALTH CENTER LOCATIONS AND SEVEN (7) SCHOOL-BASED HEALTH CENTER LOCATIONS DISBURSED WITHIN SOME OF THE POOREST COMMUNITIES ACROSS BOTH THE GREATER ST. LOUIS REGION, AND THE STATE OF MISSOURI. THE MISSION OF CARESTL HEALTH IS TO IMPROVE HEALTH OUTCOMES IN VULNERABLE COMMUNITIES BY PROVIDING COMPREHENSIVE, MULTI-GENERATIONAL HEALTHCARE, EDUCATION AND SUPPORT TO ALL WHO NEED US REGARDLESS OF INCOME OR SOCIOECONOMIC STATUS. IN RESPONSE TO THE ABOVE TITLED GRANT OPPORTUNITY, WE ARE REQUESTING $1,000,000.00 IN FUNDING SUPPORT OUR $18,000,000.00 PROJECT AND THE MISSION TO IMPROVE HEALTH OUTCOMES FOR NORTH ST. LOUIS RESIDENTS. CARESTL HEALTH WILL CREATE A NEW STAND-ALONE STRUCTURE AS PART OF THE VILLE WELLNESS CAMPUS. THIS WILL BE A 45,000 SQUARE FOOT STATE-OF-THE-ART FACILITY. DRAMATICALLY EXPANDING ACC ESS TO PRIMARY, ORAL, AND BEHAVIORAL HEALTHCARE FOR NORTH ST. LOUIS RESIDENTS. THE VILLE WELLNESS CAMPUS (VWC) WILL ENSURE THE HEALTHCARE OFFERINGS AT THE NEW FACILITY MEETS BOTH CURRENT AND PROJECTED COMMUNITY NEEDS. CARESTL HEALTH WILL EXPAND THE SERVICES OFFERED ON-SITE TO INCLUDE DESPERATELY NEEDED COVID-19 TESTING, VACCINATIONS, AND PHARMACY SERVICES. SPECIFICALLY, THE NEW FACILITY WILL SERVE A MINIMUM OF 1,500 ADDITIONAL PATIENTS AND AN ADDITIONAL 6,000 CONSULTATIONS PER YEAR. THE DESIGN AND CONSTRUCTION OF THE VILLE WELLNESS CAMPUS WILL BE EXECUTED BY MISSOURI BASED FIRMS, WITH OVER $10 MILLION IN ECONOMIC. DURING THE CONSTRUCTION PROCESS, 36 FULL TIME JOBS WILL BE CREATED, LARGELY SERVED BY MISSOURI BASED PROFESSIONALS. UPON PROJECT COMPLETION, A MINIMUM OF 5 FTES WILL BE EMPLOYED AT THE DEVELOPMENT, CONSISTING OF A CASE MANAGER, PROPERTY MANAGER AND MAINTENANCE PERSONNEL. WE WILL HIRE FROM WITHIN THE COMMUNITY.
