THE CASE MANAGER WILL BE RESPONSIBLE FOR VERIFYING THE YOUTH'S ELIGIBILITY, COORDINATING SERVICES, AND ENSURING PARTICIPANTS PROGRESS THROUGH THE VARIOUS ASPECTS OF THEIR SERVICE PLAN. SERVICES WILL INCLUDE, AS APPROPRIATE, NEEDS ASSESSMENT, EMPLOYMENT BARRIER IDENTIFICATION, REFERRAL FOR MENTORING AND VIOLENCE PREVENTION SERVICES AND OTHER COMMUNITY-BASED SERVICES, CAREER EXPLORATION AND PLANNING, JOB PREPARATION, TRAUMA-INFORMED CASE MANAGEMENT (TO INCLUDE FOLLOW-ALONG), WORK EXPERIENCE, AND TRAINING.
