ACCESS’S SERVICE AREA INCLUDES THE FOLLOWING SIX RURAL COUNTIES IN SOUTHWEST MISSOURI: BARRY, BARTON, JASPER, LAWRENCE, MCDONALD, AND NEWTON. THE SERVICE AREA INCLUDES JOPLIN, A CITY THAT COMPRISES 51,762 RESIDENTS AND IS THE MAIN HUB FOR THE AREA. THIS APPLICATION FOCUSES ON HIV PREVENTION SERVICES IN ACCESS’S SERVICE AREA, LOCATED IN MISSOURI—A DESIGNATED GEOGRAPHIC LOCATION IDENTIFIED IN ENDING THE HIV EPIDEMIC: A PLAN FOR AMERICA. PCHP PROJECT PLAN: MISSOURI IS CONSIDERED A PRIORITY AREA FOR MINIMIZING HIV INFECTIONS. THROUGH THIS FISCAL YEAR 2022 ENDING THE HIV EPIDEMIC (EHE)—PRIMARY CARE HIV PREVENTION FUNDING, ACCESS IS RESPONDING TO THE NEED TO INCREASE ACCESS TO HIV TESTING, PREP PRESCRIPTIONS, AND LINKAGE TO CARE FOR PERSONS WHO TEST POSITIVE FOR HIV. ACCESS’S PROJECT OUTLINES ACTIVITIES AND STRATEGIES TO ADDRESS EHE. THE PROJECT WILL LEVERAGE PARTNERSHIPS WITH OTHER MISSOURI PROGRAMS, INCLUDING THE AIDS PROJECT OF THE OZARKS, A RYAN WHITE-FUNDED ENTITY PROVIDING HIV TREATMENT AND MEDICAL CASE MANAGEMENT SERVICES FOR THE SOUTHWEST REGION; MIDWEST AIDS TRAINING AND EDUCATION CENTER (MATEC) PROVIDING NUMEROUS HIV TRAINING OPPORTUNITIES; AND THE MISSOURI PRIMARY CARE ASSOCIATION’S EHE LEARNING COLLABORATIVE TO PROVIDE TRAINING FOR PROVIDERS AND STAFF IN ORDER TO INCREASE THE IR KNOWLEDGE AND IMPROVE ACCESS TO PREP. TRAINING AND CONTINUING EDUCATION WILL BE PROVIDED FOR STAFF REGARDING PREP PRESCRIBING AND HIV PREVENTION, HOW TO MINIMIZE BARRIERS THAT IMPEDE ACCESS TO HIV PREVENTION SERVICES, AS WELL AS HIV TESTING AND LINKAGE TO TREATMENT FOR PERSONS WHO TEST POSITIVE. ACCESS’S ONSITE PHARMACY SERVICES WILL COMPLEMENT THE PREP PRESCRIBING INITIATIVE, CONNECTING PATIENTS WITH MEDICATION MANAGEMENT SERVICES TO IMPROVE PREP ADHERENCE. ACCESS WILL ALSO WORK ON INTERNAL PROCESSES AND PROTOCOLS, INCLUDING OPTIMIZATION OF HEALTH INFORMATION TECHNOLOGY, TO IMPROVE THE PERCENTAGE OF PATIENTS AGE 15-65 WHO HAVE BEEN TESTED FOR HIV.

ACCESS’S SERVICE AREA INCLUDES THE FOLLOWING SIX RURAL COUNTIES IN SOUTHWEST MISSOURI: BARRY, BARTON, LAWRENCE, JASPER, MCDONALD, AND NEWTON. THE SERVICE AREA’S POPULATION CENTER IS JOPLIN, MO, COMPRISED OF 52,518 RESIDENTS AND IS THE MAIN HUB FOR COMMERCE IN THE SERVICE AREA. ACCESS IS DESIGNATED AS A PATIENT CENTERED MEDICAL HOME BY THE NATIONAL COMMITTEE ON QUALITY ASSURANCE. AS SUCH, CARE IS FOCUSED ON WHOLE-PERSON WELLNESS, AND PROVIDER TEAMS EMPOWER PATIENTS BY ENGAGING THEM IN THEIR HEALTH CARE PLAN. BEHAVIORAL HEALTH SERVICES, INCLUDING MENTAL HEALTH, SUBSTANCE USE DISORDER (SUD), AND MEDICATION FOR OPIOID USE DISORDER (MOUD) ARE PROVIDED IN AN INTEGRATED MANNER. THROUGH THIS BEHAVIORAL HEALTH SERVICE EXPANSION PROJECT, ACCESS WILL ADD A MOBILE UNIT TO SERVE THE JOPLIN SCHOOL DISTRICT AND PERSONS WHO ARE HOMELESS IN THE SERVICE AREA. BEHAVIORAL HEALTH SERVICES (MENTAL HEALTH AND SUD) WILL ALSO BE EXPANDED FOR THE NEOSHO SCHOOL DISTRICT WHERE ACCESS CURRENTLY PROVIDES SCHOOL-BASED SERVICES. IN ADDITION, BEHAVIORAL HEALTH STAFF WILL BE ADDED TO EXPAND ACCESS TO MENTAL HEALTH, SUD, AND MOUD SERVICES AT ACCESS’S NEOSHO AND JOPLIN CLINIC LOCATIONS. THROUGH THIS PROJECT, ACCESS WILL ADD 3.0 FTE BEHAVIORAL HEALTH PROVIDERS (2.0 FTE IN YEAR 1, INCREASING TO 3.0 FTE IN YEAR 2), 1.0 FTE SUBSTANCE USE COUNSELOR (YEAR 2), AND 1.0 FTE CARE COORDIN ATOR (YEAR 2). THE CARE COORDINATOR WILL WORK CLOSELY WITH THE JOPLIN SCHOOL DISTRICT AND SOCIAL SERVICE ORGANIZATIONS SERVING PERSONS WHO ARE HOMELESS TO PROVIDE SUPPORTIVE RESOURCES AND IMPROVE ACCESS TO BEHAVIORAL HEALTH SERVICES. ACCESS WILL INCREASE THE NUMBER OF MOUD MEDICAL PROVIDERS, PROVIDING TRAINING AND SUPPORT TO A PHYSICIAN TO BECOME DEA REGISTERED AND ABLE TO PROVIDE MOUD TREATMENT SERVICES. THEIR TIME IS ALLOCATED AT .25 FTE/YEAR TO INCREASE ACCESS TO MOUD TREATMENT.
