RURAL PROGRAM FUNDS ARE USED TO SUPPORT PROGRAMS THAT: A) IDENTIFY, ASSESS, AND APPROPRIATELY RESPOND TO CHILD, YOUTH, AND ADULT VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING IN RURAL COMMUNITIES; B) ESTABLISH AND EXPAND VICTIM SERVICES IN RURAL COMMUNITIES FOR CHILD, YOUTH, AND ADULT VICTIMS; AND/OR C) INCREASE THE SAFETY AND WELL-BEING OF WOMEN AND CHILDREN IN RURAL COMMUNITIES BY DEALING DIRECTLY AND IMMEDIATELY WITH DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING, AND CREATING AND IMPLEMENTING STRATEGIES TO INCREASE AWARENESS AND PREVENT THESE CRIMES. GRANTEES MUST USE AT LEAST ONE OF THE FOLLOWING STRATEGIES IN IMPLEMENTING THEIR PROJECTS: A) IMPLEMENT, EXPAND, AND ESTABLISH COOPERATIVE EFFORTS AND PROJECTS AMONG LAW ENFORCEMENT OFFICERS, PROSECUTORS, VICTIM SERVICE PROVIDERS, AND OTHER RELATED PARTIES TO INVESTIGATE AND PROSECUTE INCIDENTS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING; B) PROVIDE TREATMENT, COUNSELING, ADVOCACY, LEGAL ASSISTANCE, AND OTHER LONG- AND SHORT-TERM ASSISTANCE TO ADULT AND MINOR VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING IN RURAL COMMUNITIES; C) WORK IN COOPERATION WITH THE COMMUNITY TO DEVELOP EDUCATION AND PREVENTION STRATEGIES DIRECTED TOWARD SUCH ISSUES; D) DEVELOP, ENLARGE, OR STRENGTHEN PROGRAMS ADDRESSING SEXUAL ASSAULT; AND E) DEVELOP PROGRAMS AND STRATEGIES THAT FOCUS ON THE SPECIFIC NEEDS OF VICTIMS WHO RESIDE IN REMOTE RURAL AND GEOGRAPHICALLY ISOLATED AREAS. AS OF 2019, IT WAS ESTIMATED THAT 463 MILLION PEOPLE WORLDWIDE OR ALMOST 9% OF THE WORLD ADULT POPULATION, HAD DIABETES, WITH RATES CONTINUING TO RISE. THE HALLMARK OF DIABETES IS THE FAILURE OF INSULIN SECRETING BETA CELLS LOCATED IN THE PANCREAS. THE BETA CELL IS THE ONLY CELL IN THE BODY THAT SECRETES INSULIN, WHICH IN TURN IS THE ONLY HORMONE ABLE TO LOWER PLASMA GLUCOSE LEVELS. GIVEN THE IMPORTANCE OF BETA CELLS, NUMEROUS METHODS HAVE BEEN APPLIED TO STUDY THEIR PHYSIOLOGY. BETA CELLS STORE INSULIN IN SMALL COMPARTMENTS CALLED INSULIN GRANULES AND WHEN BLOOD SUGAR LEVELS RISE, THESE GRANULES FUSE WITH THE CELL MEMBRANE, ALLOWING BETA CELLS TO RELEASE LARGE QUANTITIES OF INSULIN AT ONCE. UNFORTUNATELY, DIRECT VISUALIZATION OF INSULIN GRANULES IS NOT STRAIGHTFORWARD, SINCE CURRENT METHODS REQUIRE EXTERNAL LABELS THAT CAN AFFECT GRANULE FUNCTION AND DYNAMICS. THE TEAM OF INVESTIGATORS HAS BEEN DEVELOPING A STRAIGHTFORWARD OPTICAL MICROSCOPIC TECHNIQUE CALLED CONFOCAL BROADBAND BACKSCATTERING MICROSCOPY (CBBM) THAT CAN VISUALIZE INSULIN GRANULES WITHOUT ANY LABELS AND MONITOR THEIR DYNAMICS OVER THE COURSE OF HOURS AND EVEN DAYS, WHICH COULD POTENTIALLY LEAD TO A NEW TREATMENT FOR DIABETES. PROJECT OUTCOMES AND EFFORTS WILL BE INTEGRATED INTO A COURSE ON OPTICAL MICROSCOPY IN BIOLOGY; PROVIDE TRAINING OPPORTUNITIES FOR UNDERGRADUATE, GRADUATE, AND POST GRADUATE STUDENTS; AND ENHANCE RESEARCH EXPERIENCES FOR K-12 STUDENTS. THE OVERARCHING GOAL OF THIS PROJECT IS TO DEVELOP A LABEL-FREE MICROSCOPY SYSTEM FOR FUNCTIONAL IMAGING AND CONTINUOUS MONITORING OF INSULIN GRANULES IN LIVE PANCREATIC ISLETS IN A CUSTOM ISLET-ON-A-CHIP. THE APPROACH WILL COMBINE NOVEL CONFOCAL BROADBAND BACKSCATTERING MICROSCOPY (CBBM) WITH THE EXISTING CONFOCAL FRET AND SUPER-RESOLUTION STORM MODALITIES, WITH ALL THREE SHARING A MAJOR PART OF THE OPTICAL TRAIN AND HAVING A COMMON FIELD OF VIEW. THE CBBM TECHNIQUE WILL PROVIDE A FUNCTIONAL IMAGE OF A BETA CELL, RESOLVE INSULIN GRANULES, MONITOR THE CONVERSION OF PROINSULIN INTO INSULIN INSIDE THE GRANULES, AND VISUALIZE GRANULE-BASED INSULIN INTERCELLULAR TRANSPORT. THE PROJECT WILL ALSO DEVELOP A PHYSICAL AND MATHEMATICAL MODEL TO DESCRIBE LIGHT SCATTERING FROM INSULIN GRANULES WITHIN THE CONFOCAL VOLUME OF THE LABEL-FREE MICROSCOPY SYSTEM AND AN INVERSE ALGORITHM TO DIFFERENTIATE INSULIN AND NON-INSULIN GRANULES LABEL-FREE. THESE ALGORITHMS WILL BE USED TO MONITOR TEMPORAL CHANGES IN GRANULES AND CONTINUOUSLY VISUALIZE THEM IN 3D DURING INSULIN INTERCELLULAR TRANSPORT AND RELEASE. TO BETTER UNDERSTAND ISLET FUNCTION AND THE OPTIMAL ENVIRONMENTS FOR TRANSPLANTABLE ISLETS, GLUCOSE EXCHANGES WILL BE COMBINED WITH IN VIVO FUNCTIONAL IMAGING IN HUMAN PANCREATIC ISLETS, ALLOWING INVESTIGATION OF IMPORTANT FEEDBACK LOOPS IN GLUCOSE/INSULIN CYCLES. EXPERIMENTS WILL ALSO BE PERFORMED TO INVESTIGATE CRITICAL DIFFERENCES BETWEEN ISLETS DERIVED FROM DIABETIC AND HEALTHY SUBJECTS. THIS PROJECT BRINGS TOGETHER EXPERTS IN MACHINE LEARNING AND HUMAN-COMPUTER INTERACTION TO DEVELOP A NOVEL DYNAMIC CONTEXTUAL USER INTERFACE THAT TRANSFORMS THE MEDICAL NOTE-TAKING INTERFACE FROM A SIMPLE RECORDING DEVICE TO A TOOL THAT CAN HELP CLINICIANS QUICKLY FIND AND SYNTHESIZE INFORMATION. THE PROJECT?S NOVELTIES ARE IN ADVANCING THE FOUNDATIONS OF HUMAN-AI INTERACTION AND IN ADVANCING THE STATE-OF-THE-ART IN ARTIFICIAL INTELLIGENCE FOR HEALTH CARE WITH NOVEL METHODS THAT AUTONOMOUSLY RETRIEVE, SUMMARIZE, AND SURFACE RELEVANT INFORMATION FOR CLINICIANS, AT THE RIGHT TIME. THE PROJECT?S IMPACTS ARE IN AN AREA OF NATIONAL PRIORITY, HEALTH IT, AS IT AIMS TO MODERNIZE ELECTRONIC HEALTH RECORDS. THE RESULTING SYSTEM WILL HELP PREVENT SUBTLE FINDINGS FROM BEING OVERLOOKED, PATIENTS FROM BEING MISDIAGNOSED, AND CRITICAL INTERVENTIONS FROM BEING MISSED, ULTIMATELY RESULTING IN A DECREASE IN MORBIDITY, MORTALITY, AND OVERALL COST OF HEALTH CARE. IT WILL ADDITIONALLY DECREASE DOCUMENTATION BURDEN AND MITIGATE PHYSICIAN BURNOUT. PRIOR ATTEMPTS AT DEVELOPING CONTEXTUAL DISPLAYS OF PATIENT INFORMATION WERE MANUAL, LABOR-INTENSIVE PROCESSES THAT RELIED ON DOMAIN EXPERTISE AND WERE NEITHER SCALABLE, MAINTAINABLE, NOR CUSTOMIZED TO INDIVIDUAL USERS. AUTOMATING CONTEXTUAL DISPLAYS IS CHALLENGING BECAUSE WHAT INFORMATION IS RELEVANT HIGHLY DEPENDS ON THE USER, PATIENT, AND SPECIFIC CLINICAL CONTEXT. TRADITIONAL MACHINE LEARNING APPROACHES ARE INFEASIBLE BECAUSE OF THE LACK OF LABELED TRAINING DATA. THIS PROJECT DEVELOPS NEW USER INTERFACES THAT ENABLE THE LARGE-SCALE COLLECTION OF IMPLICIT USAGE-BASED TRAINING DATA AS PART OF ROUTINE USER WORKFLOWS. SPECIFICALLY, THIS PROJECT DEVELOPS A NOVEL INFORMATION FORAGING INTERFACE, THE ?SEMANTIC CLIPBOARD?, WHICH CLINICIANS WILL USE WHILE READING PATIENTS? PAST MEDICAL RECORDS AND WHILE WRITING NOTES. THIS WILL BE ACHIEVED BY USING PROPERTIES OF SCATTERED LIGHT, WHICH CARRIES INFORMATION ABOUT THE OBJECT IT IS SCATTERED BY, EVEN IF THE STRUCTURE OF THE OBJECT IS SO TINY THAT REGULAR MICROSCOPES CANNOT RESOLVE IT. AN IMPORTANT PART OF THIS PROJECT WILL BE AN EDUCATIONAL OUTREACH PROGRAM TO SHARE THE TECHNOLOGY WITH UNDERREPRESENTED GROUPS AND K-12 STUDENTS. THIS OUTREACH PROGRAM IS DESIGNED TO REACH K-12 EDUCATIONALLY DISADVANTAGED STUDENTS FROM LOCAL NEIGHBORHOODS WITH PREDOMINANTLY ETHNIC-MINORITY STUDENT POPULATIONS. THE NOVEL LABEL-FREE TECHNOLOGY FOR DYNAMIC BIOSENSING OF CHROMATIN PACKING IN LIVE CELLS, PROPOSED IN THIS APPLICATION WILL ADDRESS FUNDAMENTAL UNSOLVED PROBLEMS IN GENETICS AND EPIGENETICS. IT SHOULD HELP TO ANSWER A POORLY UNDERSTOOD QUESTION OF HOW SIGNAL TRANSDUCTION PATHWAYS DIRECTLY COMMUNICATE WITH CHROMATIN TO CHANGE THE EPIGENETIC LANDSCAPE. IT CAN ALSO FACILITATE A NOVEL SYNTHETIC BIOLOGY BIOSENSING APPROACH AS CHROMATIN CAN SERVE AS A SENSOR OF METABOLIC CHANGES DURING EARLY DEVELOPMENT. IT SHOULD ALSO HAVE IMPORTANT IMPLICATIONS IN PUBLIC HEALTH, INCLUDING CHROMATIN CHANGES IN CANCER OR THE ROLE OF EPIGENETICS IN INFECTIOUS DISEASES. THE APPROACH WILL ADVANCE BOTH ENGINEERING AND LIFE SCIENCES AND WILL BE BASED ON A COMBINATION OF THE PRINCIPLES OF FRACTAL SENSING, SINCE IT IS KNOWN THAT CHROMATIN IN LIVE CELLS IS ORGANIZED AS A FRACTAL WITH HETEROCHROMATIN AND EUCHROMATIN DOMAINS EXHIBITING DIFFERENT FRACTAL DIMENSIONS, AND COHERENT CONFOCAL LIGHT ABSORPTION AND SCATTERING SPECTROSCOPIC MICROSCOPY. APPLICATION OF IMMUNOTHERAPIES HAS LED TO SIGNIFICANT IMPROVEMENT IN SURVIVAL OF CANCER PATIENTS. HOWEVER, THERE IS FURTHER NEED TO ACHIEVE BETTER RESPONSES TO CURRENT IMMUNOTHERAPIES. THE MOST PROMISING RESULTS HAD BEEN ACHIEVED WHEN COMBINING ANTI-PD-L1/PD-1 WITH TREATMENTS THAT ACTIVATE IMMUNE FUNCTION. WE PROPOSE THAT EXOGENOUS CARBON MONOXIDE (COEX) APPLIED AT LOW NON-TOXIC DOSES IS A POTENTIAL ADJUVANT FOR PATIENTS WITH LUNG CANCER TREATED WITH IMMUNOTHERAPY. CO IS GENERATED ENDOGENOUSLY BY HEME OXYGENASE-1 (HO-1) THAT ACTS DURING HOMEOSTASIS TO BREAK DOWN HEME TO THREE ACTIVE METABOLITES. WE HAVE DEMONSTRATED THAT COEX APPLIED AT 250 PPM FOR 1 HOUR PER DAY (THAT CORRESPONDS TO THE DOSE OF CO OBTAINED FROM BURNING 2 CIGARETTES) BLOCKS PROGRESSION OF LUNG AND PROSTATE CANCERS. COEX CAN BLOCK TUMOR GROWTH VIA TARGETING METABOLISM OF CANCER CELLS. HOWEVER, SINCE COEX BLOCKS GROWTH OF DIFFERENT TUMOR TYPES AND AT VARIOUS METABOLIC RATES AS WELL AS IS MORE EFFICIENT IN VIVO THAN IN VITRO IN INDUCING APOPTOSIS OF CANCER CELLS, WE REASONED COEX HAS A BROADER EFFECT IN THE TME THAT INCLUDES IMMUNE CELLS. OUR PRELIMINARY DATA SUGGEST THAT COEX PROMOTES MORE ‘IMMUNO-RECEPTIVE’ TUMOR MICROENVIRONMENT (TME). WE PROPOSE THAT REPROGRAMING OF THE TME BY REGULATING HEME METABOLISM REGULATES RESPONSES TO IMMUNOTHERAPY. OUR AIM IS TO: TO DETERMINE THE ROLE OF COEX IN REDIRECTING IMMUNE RESPONSES TO INCREASE EFFICACY OF IMMUNOTHERAPY. SPECIFICALLY, WE INTENT TO: I) DETERMINE EFFECTIVENESS OF THE COMBINATION OF COEX AND ANTI-CTLA-4, ANTI-PD-L1 OR ANTI-PD-1 TREATMENT IN A MOUSE MODEL OF LUNG CANCER. II) DETERMINE COEX-MEDIATED INDUCTION OF CD86 VIA MTOR-NOTCH1 SIGNALING DURING RESPONSE TO IMMUNOTHERAPY IN THE LUNG CANCER MODEL. III) DEFINE THE IMPORTANCE OF ENDOGENOUS HO-1-DERIVED CO DURING RESPONSE TO IMMUNOTHERAPY. THE INVESTIGATIONS PROPOSED IN THIS APPLICATION WILL DELINEATE THE ROLE OF CO AND HO-1 IN ACTIVATION OF THE HOST IMMUNE RESPONSES VIA SELECTIVE SIGNALING PATHWAYS LEADING TO INCREASE IN MYELOID FUNCTION IN THE TME. RURAL PROGRAM FUNDS ARE USED TO SUPPORT PROGRAMS THAT: A) IDENTIFY, ASSESS, AND APPROPRIATELY RESPOND TO CHILD, YOUTH, AND ADULT VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING IN RURAL COMMUNITIES; B) ESTABLISH AND EXPAND VICTIM SERVICES IN RURAL COMMUNITIES FOR CHILD, YOUTH, AND ADULT VICTIMS; AND/OR C) INCREASE THE SAFETY AND WELL-BEING OF WOMEN AND CHILDREN IN RURAL COMMUNITIES BY DEALING DIRECTLY AND IMMEDIATELY WITH DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING, AND CREATING AND IMPLEMENTING STRATEGIES TO INCREASE AWARENESS AND PREVENT THESE CRIMES. GRANTEES MUST USE AT LEAST ONE OF THE FOLLOWING STRATEGIES IN IMPLEMENTING THEIR PROJECTS: A) IMPLEMENT, EXPAND, AND ESTABLISH COOPERATIVE EFFORTS AND PROJECTS AMONG LAW ENFORCEMENT OFFICERS, PROSECUTORS, VICTIM SERVICE PROVIDERS, AND OTHER RELATED PARTIES TO INVESTIGATE AND PROSECUTE INCIDENTS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING; B) PROVIDE TREATMENT, COUNSELING, ADVOCACY, LEGAL ASSISTANCE, AND OTHER LONG- AND SHORT-TERM ASSISTANCE TO ADULT AND MINOR VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING IN RURAL COMMUNITIES; C) WORK IN COOPERATION WITH THE COMMUNITY TO DEVELOP EDUCATION AND PREVENTION STRATEGIES DIRECTED TOWARD SUCH ISSUES; D) DEVELOP, ENLARGE, OR STRENGTHEN PROGRAMS ADDRESSING SEXUAL ASSAULT; AND E) DEVELOP PROGRAMS AND STRATEGIES THAT FOCUS ON THE SPECIFIC NEEDS OF VICTIMS WHO RESIDE IN REMOTE RURAL AND GEOGRAPHICALLY ISOLATED AREAS. BROADBAND MULTISPECTRAL MICROSCOPY OF INSULIN GRANULE DYNAMICS IN LIVE PANCREATIC ISLETS -DIABETES IS A CHRONIC DISEASE CHARACTERIZED BY A HIGH BLOOD SUGAR LEVEL OVER AN EXTENDED PERIOD OF TIME. AS OF 2019, IT WAS ESTIMATED THAT 463 MILLION PEOPLE WORLDWIDE OR ALMOST 9% OF THE WORLD ADULT POPULATION, HAD DIABETES, WITH RATES CONTINUING TO RISE. THE HALLMARK OF DIABETES IS THE FAILURE OF INSULIN SECRETING BETA CELLS LOCATED IN THE PANCREAS. THE BETA CELL IS THE ONLY CELL IN THE BODY THAT SECRETES INSULIN, WHICH IN TURN IS THE ONLY HORMONE ABLE TO LOWER PLASMA GLUCOSE LEVELS. GIVEN THE IMPORTANCE OF BETA CELLS, NUMEROUS METHODS HAVE BEEN APPLIED TO STUDY THEIR PHYSIOLOGY. BETA CELLS STORE INSULIN IN SMALL COMPARTMENTS CALLED INSULIN GRANULES AND WHEN BLOOD SUGAR LEVELS RISE, THESE GRANULES FUSE WITH THE CELL MEMBRANE, ALLOWING BETA CELLS TO RELEASE LARGE QUANTITIES OF INSULIN AT ONCE. UNFORTUNATELY, DIRECT VISUALIZATION OF INSULIN GRANULES IS NOT STRAIGHTFORWARD, SINCE CURRENT METHODS REQUIRE EXTERNAL LABELS THAT CAN AFFECT GRANULE FUNCTION AND DYNAMICS. THE TEAM OF INVESTIGATORS HAS BEEN DEVELOPING A STRAIGHTFORWARD OPTICAL MICROSCOPIC TECHNIQUE CALLED CONFOCAL BROADBAND BACKSCATTERING MICROSCOPY (CBBM) THAT CAN VISUALIZE INSULIN GRANULES WITHOUT ANY LABELS AND MONITOR THEIR DYNAMICS OVER THE COURSE OF HOURS AND EVEN DAYS, WHICH COULD POTENTIALLY LEAD TO A NEW TREATMENT FOR DIABETES. PROJECT OUTCOMES AND EFFORTS WILL BE INTEGRATED INTO A COURSE ON OPTICAL MICROSCOPY IN BIOLOGY; PROVIDE TRAINING OPPORTUNITIES FOR UNDERGRADUATE, GRADUATE, AND POST GRADUATE STUDENTS; AND ENHANCE RESEARCH EXPERIENCES FOR K-12 STUDENTS. THE OVERARCHING GOAL OF THIS PROJECT IS TO DEVELOP A LABEL-FREE MICROSCOPY SYSTEM FOR FUNCTIONAL IMAGING AND CONTINUOUS MONITORING OF INSULIN GRANULES IN LIVE PANCREATIC ISLETS IN A CUSTOM ISLET-ON-A-CHIP. THE APPROACH WILL COMBINE NOVEL CONFOCAL BROADBAND BACKSCATTERING MICROSCOPY (CBBM) WITH THE EXISTING CONFOCAL FRET AND SUPER-RESOLUTION STORM MODALITIES, WITH ALL THREE SHARING A MAJOR PART OF THE OPTICAL TRAIN AND HAVING A COMMON FIELD OF VIEW. THE CBBM TECHNIQUE WILL PROVIDE A FUNCTIONAL IMAGE OF A BETA CELL, RESOLVE INSULIN GRANULES, MONITOR THE CONVERSION OF PROINSULIN INTO INSULIN INSIDE THE GRANULES, AND VISUALIZE GRANULE-BASED INSULIN INTERCELLULAR TRANSPORT. THE PROJECT WILL ALSO DEVELOP A PHYSICAL AND MATHEMATICAL MODEL TO DESCRIBE LIGHT SCATTERING FROM INSULIN GRANULES WITHIN THE CONFOCAL VOLUME OF THE LABEL-FREE MICROSCOPY SYSTEM AND AN INVERSE ALGORITHM TO DIFFERENTIATE INSULIN AND NON-INSULIN GRANULES LABEL-FREE. THESE ALGORITHMS WILL BE USED TO MONITOR TEMPORAL CHANGES IN GRANULES AND CONTINUOUSLY VISUALIZE THEM IN 3D DURING INSULIN INTERCELLULAR TRANSPORT AND RELEASE. HOWEVER, USER INTERFACES IN HEALTHCARE ARE UNWIELDY TO NAVIGATE AND ARE LARGELY A DIGITIZATION OF STATIC PAPER FORMS FROM LEGACY CLINICAL WORKFLOWS, A PARADIGM THAT HAS CONTRIBUTED TO POOR USABILITY AND CLINICIAN BURNOUT. THIS PROJECT BRINGS TOGETHER EXPERTS IN MACHINE LEARNING AND HUMAN-COMPUTER INTERACTION TO DEVELOP A NOVEL DYNAMIC CONTEXTUAL USER INTERFACE THAT TRANSFORMS THE MEDICAL NOTE-TAKING INTERFACE FROM A SIMPLE RECORDING DEVICE TO A TOOL THAT CAN HELP CLINICIANS QUICKLY FIND AND SYNTHESIZE INFORMATION. THE PROJECT?S NOVELTIES ARE IN ADVANCING THE FOUNDATIONS OF HUMAN-AI INTERACTION AND IN ADVANCING THE STATE-OF-THE-ART IN ARTIFICIAL INTELLIGENCE FOR HEALTH CARE WITH NOVEL METHODS THAT AUTONOMOUSLY RETRIEVE, SUMMARIZE, AND SURFACE RELEVANT INFORMATION FOR CLINICIANS, AT THE RIGHT TIME. THE PROJECT?S IMPACTS ARE IN AN AREA OF NATIONAL PRIORITY, HEALTH IT, AS IT AIMS TO MODERNIZE ELECTRONIC HEALTH RECORDS. THE RESULTING SYSTEM WILL HELP PREVENT SUBTLE FINDINGS FROM BEING OVERLOOKED, PATIENTS FROM BEING MISDIAGNOSED, AND CRITICAL INTERVENTIONS FROM BEING MISSED, ULTIMATELY RESULTING IN A DECREASE IN MORBIDITY, MORTALITY, AND OVERALL COST OF HEALTH CARE. IT WILL ADDITIONALLY DECREASE DOCUMENTATION BURDEN AND MITIGATE PHYSICIAN BURNOUT. PRIOR ATTEMPTS AT DEVELOPING CONTEXTUAL DISPLAYS OF PATIENT INFORMATION WERE MANUAL, LABOR-INTENSIVE PROCESSES THAT RELIED ON DOMAIN EXPERTISE AND WERE NEITHER SCALABLE, MAINTAINABLE, NOR CUSTOMIZED TO INDIVIDUAL USERS. AUTOMATING CONTEXTUAL DISPLAYS IS CHALLENGING BECAUSE WHAT INFORMATION IS RELEVANT HIGHLY DEPENDS ON THE USER, PATIENT, AND SPECIFIC CLINICAL CONTEXT. TRADITIONAL MACHINE LEARNING APPROACHES ARE INFEASIBLE BECAUSE OF THE LACK OF LABELED TRAINING DATA. THIS PROJECT DEVELOPS NEW USER INTERFACES THAT ENABLE THE LARGE-SCALE COLLECTION OF IMPLICIT USAGE-BASED TRAINING DATA AS PART OF ROUTINE USER WORKFLOWS. SPECIFICALLY, THIS PROJECT DEVELOPS A NOVEL INFORMATION FORAGING INTERFACE, THE ?SEMANTIC CLIPBOARD?, WHICH CLINICIANS WILL USE WHILE READING PATIENTS? PAST MEDICAL RECORDS AND WHILE WRITING NOTES. USING THE DATA COLLECTED THROUGH THIS NEW INTERFACE, THE INVESTIGATORS WILL DEVELOP NEW MACHINE LEARNING METHODOLOGIES TO PREDICT THE RELEVANT PIECES OF INFORMATION THAT SHOULD APPEAR IN THESE CONTEXTUAL DISPLAYS, CUSTOMIZED TO THE CLINICAL SCENARIO AS WELL AS THE USER. THROUGH THIS PROJECT AND THE INVESTIGATORS? SOME PARTS OF CHROMATIN DISPLAY A MORE OPEN FORM CALLED EUCHROMATIN, WHICH IS CHARACTERISTIC OF HIGH GENE EXPRESSION, WHILE OTHERS EXHIBIT A MORE DENSELY PACKED CHROMATIN CALLED HETEROCHROMATIN. CURRENTLY, THERE IS NO BIOSENSING TECHNOLOGY CAPABLE OF SENSING THE CONVERSION FROM HETEROCHROMATIN TO EUCHROMATIN AND BACK IN REAL TIME, WHICH IS A CRITICALLY IMPORTANT QUESTION FOR UNDERSTANDING HOW CELLS FUNCTION AND HOW THE HUMAN GENOME IS REGULATED. THUS, THE GOAL OF THIS PROJECT IS TO DEVELOP A METHOD FOR DYNAMIC BIOSENSING OF CHROMATIN PACKING IN LIVE CELLS AND TO ACHIEVE THIS WITHOUT EMPLOYING EXTERNAL MARKERS THAT CAN AFFECT CELL FUNCTION AND, AS A RESULT, CHROMATIN DYNAMICS. THIS WILL BE ACHIEVED BY USING PROPERTIES OF SCATTERED LIGHT, WHICH CARRIES INFORMATION ABOUT THE OBJECT IT IS SCATTERED BY, EVEN IF THE STRUCTURE OF THE OBJECT IS SO TINY THAT REGULAR MICROSCOPES CANNOT RESOLVE IT. AN IMPORTANT PART OF THIS PROJECT WILL BE AN EDUCATIONAL OUTREACH PROGRAM TO SHARE THE TECHNOLOGY WITH UNDERREPRESENTED GROUPS AND K-12 STUDENTS. THIS OUTREACH PROGRAM IS DESIGNED TO REACH K-12 EDUCATIONALLY DISADVANTAGED STUDENTS FROM LOCAL NEIGHBORHOODS WITH PREDOMINANTLY ETHNIC-MINORITY STUDENT POPULATIONS. THE NOVEL LABEL-FREE TECHNOLOGY FOR DYNAMIC BIOSENSING OF CHROMATIN PACKING IN LIVE CELLS, PROPOSED IN THIS APPLICATION WILL ADDRESS FUNDAMENTAL UNSOLVED PROBLEMS IN GENETICS AND EPIGENETICS. IT SHOULD HELP TO ANSWER A POORLY UNDERSTOOD QUESTION OF HOW SIGNAL TRANSDUCTION PATHWAYS DIRECTLY COMMUNICATE WITH CHROMATIN TO CHANGE THE EPIGENETIC LANDSCAPE. IT CAN ALSO FACILITATE A NOVEL SYNTHETIC BIOLOGY BIOSENSING APPROACH AS CHROMATIN CAN SERVE AS A SENSOR OF METABOLIC CHANGES DURING EARLY DEVELOPMENT. IT SHOULD ALSO HAVE IMPORTANT IMPLICATIONS IN PUBLIC HEALTH, INCLUDING CHROMATIN CHANGES IN CANCER OR THE ROLE OF EPIGENETICS IN INFECTIOUS DISEASES. THE APPROACH WILL ADVANCE BOTH ENGINEERING AND LIFE SCIENCES AND WILL BE BASED ON A COMBINATION OF THE PRINCIPLES OF FRACTAL SENSING, SINCE IT IS KNOWN THAT CHROMATIN IN LIVE CELLS IS ORGANIZED AS A FRACTAL WITH HETEROCHROMATIN AND EUCHROMATIN DOMAINS EXHIBITING DIFFERENT FRACTAL DIMENSIONS, AND COHERENT CONFOCAL LIGHT ABSORPTION AND SCATTERING SPECTROSCOPIC MICROSCOPY. THE PROPOSED APPROACH IS MOTIVATED BY THE NEW FUNDAMENTAL INSIGHTS THAT SELF-SIMILAR PROPERTIES OF THE CHROMATIN FRACTAL ARE REFLECTED IN THE CHROMATIN LIGHT SCATTERING SPECTRA, AND THESE SPECTRA CAN BE CONTINUOUSLY AND RAPIDLY RECORDED CREATING A 3D DYNAMIC MAP OF CHROMATIN ON A MULTITUDE OF SPATIAL AND TEMPORAL SCALES. DETERMINING THE ROLE OF GAS METABOLITE IN RESPONSE TO IMMUNOTHERAPY - PROJECT SUMMARY LUNG CANCER IS A LEADING CAUSE OF DEATH AND THE SECOND MOST COMMON CANCER IN THE UNITED STATES. APPLICATION OF IMMUNOTHERAPIES HAS LED TO SIGNIFICANT IMPROVEMENT IN SURVIVAL OF CANCER PATIENTS. HOWEVER, THERE IS FURTHER NEED TO ACHIEVE BETTER RESPONSES TO CURRENT IMMUNOTHERAPIES. THE MOST PROMISING RESULTS HAD BEEN ACHIEVED WHEN COMBINING ANTI-PD-L1/PD-1 WITH TREATMENTS THAT ACTIVATE IMMUNE FUNCTION. WE PROPOSE THAT EXOGENOUS CARBON MONOXIDE (COEX) APPLIED AT LOW NON-TOXIC DOSES IS A POTENTIAL ADJUVANT FOR PATIENTS WITH LUNG CANCER TREATED WITH IMMUNOTHERAPY. CO IS GENERATED ENDOGENOUSLY BY HEME OXYGENASE-1 (HO-1) THAT ACTS DURING HOMEOSTASIS TO BREAK DOWN HEME TO THREE ACTIVE METABOLITES. WE HAVE DEMONSTRATED THAT COEX APPLIED AT 250 PPM FOR 1 HOUR PER DAY (THAT CORRESPONDS TO THE DOSE OF CO OBTAINED FROM BURNING 2 CIGARETTES) BLOCKS PROGRESSION OF LUNG AND PROSTATE CANCERS. COEX CAN BLOCK TUMOR GROWTH VIA TARGETING METABOLISM OF CANCER CELLS. HOWEVER, SINCE COEX BLOCKS GROWTH OF DIFFERENT TUMOR TYPES AND AT VARIOUS METABOLIC RATES AS WELL AS IS MORE EFFICIENT IN VIVO THAN IN VITRO IN INDUCING APOPTOSIS OF CANCER CELLS, WE REASONED COEX HAS A BROADER EFFECT IN THE TME THAT INCLUDES IMMUNE CELLS. OUR PRELIMINARY DATA SUGGEST THAT COEX PROMOTES MORE ‘IMMUNO-RECEPTIVE’ TUMOR MICROENVIRONMENT (TME). WE PROPOSE THAT REPROGRAMING OF THE TME BY REGULATING HEME METABOLISM REGULATES RESPONSES TO IMMUNOTHERAPY. OUR AIM IS TO: TO DETERMINE THE ROLE OF COEX IN REDIRECTING IMMUNE RESPONSES TO INCREASE EFFICACY OF IMMUNOTHERAPY. SPECIFICALLY, WE INTENT TO: I) DETERMINE EFFECTIVENESS OF THE COMBINATION OF COEX AND ANTI-CTLA-4, ANTI-PD-L1 OR ANTI-PD-1 TREATMENT IN A MOUSE MODEL OF LUNG CANCER. II) DETERMINE COEX-MEDIATED INDUCTION OF CD86 VIA MTOR-NOTCH1 SIGNALING DURING RESPONSE TO IMMUNOTHERAPY IN THE LUNG CANCER MODEL. III) DEFINE THE IMPORTANCE OF ENDOGENOUS HO-1-DERIVED CO DURING RESPONSE TO IMMUNOTHERAPY. THE INVESTIGATIONS PROPOSED IN THIS APPLICATION WILL DELINEATE THE ROLE OF CO AND HO-1 IN ACTIVATION OF THE HOST IMMUNE RESPONSES VIA SELECTIVE SIGNALING PATHWAYS LEADING TO INCREASE IN MYELOID FUNCTION IN THE TME. OUR FINDINGS WILL HAVE FUNDAMENTAL AND THERAPEUTIC IMPLICATIONS FOR PATIENTS WITH LUNG CANCER, ESPECIALLY THAT COEX IS IN CLINICAL TRIALS. THIS STUDY IS WELL-ALIGNED WITH THE MISSION OF THE NCI AND THIS RFA “NCI CLINICAL AND TRANSLATIONAL EXPLORATORY/DEVELOPMENTAL STUDIES (R21 CLINICAL TRIAL OPTIONAL)” (PAR-19-356).

INTERVENTION AND RELATED ASSISTANCE INCLUDE ADVOCACY, ACCOMPANIMENT (E.G., ACCOMPANYING VICTIMS TO COURT, MEDICAL FACILITIES, POLICE DEPARTMENTS, ETC.), CRISIS INTERVENTION AND SUPPORT SERVICES, AND REFERRALS, AMONG OTHER SERVICES. UNDER THIS PROGRAM, SUCH SERVICES MAY BE PROVIDED TO ADULT, YOUTH, AND CHILD VICTIMS OF SEXUAL ASSAULT, FAMILY AND HOUSEHOLD MEMBERS OF SUCH VICTIMS, AND THOSE COLLATERALLY AFFECTED BY THE VICTIMIZATION. SURVIVORS OF SEXUAL ASSAULT FROM CULTURALLY SPECIFIC COMMUNITIES FREQUENTLY CONFRONT UNIQUE CHALLENGES WHEN SEEKING ASSISTANCE, SUCH AS LINGUISTIC AND CULTURAL BARRIERS. CULTURALLY SPECIFIC COMMUNITY-BASED ORGANIZATIONS ARE MORE LIKELY TO UNDERSTAND THESE CHALLENGES BECAUSE THEY ARE FAMILIAR WITH THE CULTURE, LANGUAGE, AND BACKGROUND OF VICTIMS FROM THEIR COMMUNITIES, WHO IN TURN ARE MORE INCLINED TO SEEK SERVICES FROM SUCH ORGANIZATIONS. THE GOAL OF THE SAS CULTURALLY SPECIFIC PROGRAM IS TO ESTABLISH, MAINTAIN, AND EXPAND CULTURALLY SPECIFIC INTERVENTION AND RELATED ASSISTANCE FOR VICTIMS OF SEXUAL ASSAULT, WHICH SERVE A VITAL ROLE IN PROVIDING SERVICES THAT ARE RELEVANT FOR THEIR COMMUNITIES.

THE GRANTS TO ENHANCE CULTURALLY SPECIFIC SERVICES FOR VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING PROGRAM IS AUTHORIZED BY 34 U.S.C. § 20124. THIS PROGRAM SUPPORTS THE MAINTENANCE AND REPLICATION OF EXISTING SUCCESSFUL COMMUNITY-BASED PROGRAMS PROVIDING CULTURALLY SPECIFIC SERVICES TO VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING, AS WELL AS THE DEVELOPMENT OF INNOVATIVE CULTURALLY SPECIFIC STRATEGIES TO ENHANCE ACCESS TO SERVICES AND RESOURCES FOR VICTIMS WHO FACE OBSTACLES TO USING MORE TRADITIONAL PROGRAMS. CULTURALLY SPECIFIC SERVICES MEANS COMMUNITY-BASED SERVICES THAT INCLUDE CULTURALLY RELEVANT AND LINGUISTICALLY SPECIFIC SERVICES AND RESOURCES TO CULTURALLY SPECIFIC COMMUNITIES WHICH ARE STATUTORILY DEFINED AS “AMERICAN INDIANS (INCLUDING ALASKA NATIVES, ESKIMOS, AND ALEUTS); ASIAN AMERICANS; NATIVE HAWAIIANS AND OTHER PACIFIC ISLANDERS; BLACKS; AND HISPANICS.” PROJECTS MUST: 1) WORK WITH STATE AND LOCAL GOVERNMENTS AND SOCIAL SERVICE AGENCIES TO DEVELOP AND ENHANCE EFFECTIVE STRATEGIES TO PROVIDE CULTURALLY SPECIFIC VICTIM SERVICES ; 2) INCREASE COMMUNITIES’ CAPACITY TO PROVIDE CULTURALLY SPECIFIC RESOURCES AND SUPPORT FOR VICTIMS AND THEIR FAMILIES; 3) STRENGTHEN CRIMINAL JUSTICE INTERVENTIONS BY PROVIDING TRAINING FOR LAW ENFORCEMENT, PROSECUTION, COURTS, PROBATION, AND CORRECTIONAL FACILITIES ON CULTURALLY SPECIFIC RESPONSES; 4) ENHANCE TRADITIONAL SERVICES TO VICTIMS THROUGH THE LEADERSHIP OF CULTURALLY SPECIFIC VICTIM SERVICES PROGRAMS ; 5) WORK IN COOPERATION WITH THE COMMUNITY TO DEVELOP EDUCATION AND PREVENTION STRATEGIES HIGHLIGHTING CULTURALLY SPECIFIC ISSUES AND RESOURCES; 6) PROVIDE CULTURALLY SPECIFIC PROGRAMS FOR CHILDREN EXPOSED TO DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING; 7) PROVIDE CULTURALLY SPECIFIC RESOURCES AND SERVICES THAT ADDRESS THE SAFETY, ECONOMIC, HOUSING, AND WORKPLACE NEEDS OF VICTIMS, INCLUDING EMERGENCY ASSISTANCE; AND/OR 8) EXAMINE THE DYNAMICS OF CULTURE AND ITS IMPACT ON VICTIMIZATION AND HEALING. THE TIMING FOR PERFORMANCE OF THIS AWARD IS 24 MONTHS.
