KATHRYN FIDDLER 410-912-4923 KATHRYN.FIDDLER@TIDALHEALTH.ORG GRANT PROGRAM FUNDS REQUESTED: $341,000 THE WELLNESS MOBILE HEALTH CLINIC OF TIDALHEALTH WILL SUPPORT THE PURCHASE OF EQUIPMENT AND SUPPORTIVE TECHNOLOGIES AND MATERIALS TO FULLY OUTFIT A MOBILE HEALTH VAN, THAT TIDALHEALTH NANTICOKE ALREADY OWNS, AND PURCHASE A SECOND UPGRADED VAN TO PROVIDE HEALTH SCREENINGS, MULTIDISCIPLINARY HEALTH SERVICES, TELEMEDICINE, SOCIAL DETERMINANT OF HEALTH SCREENING AND ASSISTANCE, EDUCATION AND OUTREACH IN COMMUNITY-BASED SETTINGS. THE CLINIC VAN AND ITS MULTIDISCIPLINARY TEAM WILL CONNECT MARGINALIZED COMMUNITIES WITH OUTREACH, SCREENINGS, CONNECTIONS TO PRIMARY CARE, AS WELL AS PREVENTATIVE HEALTH AND WELLNESS RESOURCES. TIDALHEALTH NANTICOKE IS A HOSPITAL LOCATED IN SEAFORD, DELAWARE. THE MOBILE HEALTH VAN THAT IS OUTFITTED THROUGH THIS FUNDING WILL SERVE THE COMMUNITIES LOCATED IN THE PRIMARY SERVICE AREA AND WILL SUPPORT BOTH WELLNESS CHECKS AND HEALTH CARE SERVICES IN WITHIN WESTERN SUSSEX COUNTY FOCUSED ON GEORGETOWN, MILLSBORO, SELBYVILLE, LAUREL, SEAFORD, DELMAR, BLADES, BETHEL AND BRIDGEVILLE. THE VAN WILL ADDITIONALLY SERVE THREE SCHOOL-BASED WELLNESS CENTERS IN LAUREL, SEAFORD AND DELMAR IN COLLABORATION WITH THE SCHOOL SYSTEMS, AND WORK WITH LOCAL EMPLOYERS TO PROVIDE LARGE SCALE SCREENINGS FOR EMPLOYEES ONSITE. THE VAN WILL BE STAFFED BY A COMMUNITY HEALTH WORKER AND REGISTERED NURSE AND/OR NURSE PRACTITIONER AND WORK TO LINK INDIVIDUALS TO A COORDINATED COMMUNITY WELLNESS TEAM THAT INCLUDES ADDITIONAL COMMUNITY HEALTH WORKERS, SOCIAL WORKER, AND HEALTH CARE PROVIDERS INCLUDING PRIMARY CARE AND BEHAVIORAL HEALTH CARE PROVIDERS. THE INTEGRATED HEALTH CARE SYSTEM IS A REGIONAL CHAMPION OF POPULATION HEALTH AND THE ROLE THAT POPULATION HEALTH PLAYS IN REDUCING HEALTHCARE DISPARITIES AND INEQUITY. THE MOBILE HEALTH UNIT WILL IMPROVE HEALTHCARE ACCESS THROUGH DEPLOYMENT OF RESOURCES FOR RURAL, UNDERSERVED NEIGHBORHOODS AND COMMUNITIES. THIS WILL BE DONE BY OPTIMIZING DATA ANALYTICS TO PINPOINT HIGH RISK COMMUNITIES, WITH SPECIAL ATTENTION AND FOCUS ON ZIP CODES/NEIGHBORHOODS WITH HIGH RATES OF HEALTH DISPARITIES AND POOR HEALTH OUTCOMES. THE PROJECT, IN CONJUNCTION WITH ALIGNED INITIATIVES, WILL CONTRIBUTE TO ACHIEVING A REDUCTION IN ED VISITS, HOSPITAL ADMISSIONS AND READMISSIONS AND POTENTIALLY AVOIDABLE UTILIZATION, AN INCREASE IN THE NUMBER OF PRIMARY CARE/WELLNESS VISITS, AS WELL A REDUCTION IN HEALTHCARE DISPARITIES IN IDENTIFIED VULNERABLE AND HIGH-RISK COMMUNITIES. THIS IS A DISCREET ONE-TIME REQUEST TO FUND THE EQUIPMENT AND STARTUP COSTS OF A MOBILE VAN WITHIN SUSSEX COUNTY, WITH MOST EQUIPMENT BEING PURCHASED BY OCTOBER 1, 2022. ALL FUNDING WILL BE USED WITHIN 3 YEARS OF BEING AWARDED. THERE IS NO PLAN FOR FUTURE REQUESTS FOR THIS PROGRAM AND OPERATING FUNDS WILL COME FROM OTHER SOURCES.
