THE POPULATION WE SEEK TO SERVE IS YOUTH IN THE TRIBAL COMMUNITY (ENROLLED CITIZENS AND CHILDREN LIVING WITH ENROLLED CITIZENS), WHICH WE ANTICIPATE WILL CONSIST OF 50 COMMUNITY MEMBERS BETWEEN THE AGES OF 0 AND 24 DURING YEAR 1 OF THE PROJECT AND 170 IN SUBSEQUENT YEARS, FOR AN ESTIMATED CUMULATIVE REACH OF APPROXIMATELY 220 UNIQUE INDIVIDUALS OVER THE LIFETIME OF THE GRANT. IN ADDITION TO THIS TARGET POPULATION, WE ALSO EXPECT TO REACH THE GENERAL CITIZENRY, AS THE PROJECT WILL WIDELY DISTRIBUTE EDUCATIONAL RESOURCES AND CREATE LEARNING OPPORTUNITIES AND TRIBAL EVENTS TO SUPPORT COMMUNITY CONNECTION. CIRCLES OF GRACE WILL EMPLOY STRATEGIES OF PROFESSIONAL DEVELOPMENT FOR TRIBAL STAFF ACROSS DEPARTMENTS WHO ARE IN A POSITION TO BOLSTER PROTECTIVE FACTORS THROUGH EXISTING PROGRAMMING AND THE CREATION AND DISTRIBUTION OF TARGETED MENTAL HEALTH RESOURCES DESIGNED TO EDUCATE TRIBAL CITIZENS AND DE-STIGMATIZE MENTAL HEALTH SERVICES. TO BUILD A FOUNDATION FOR DURABLE WELLNESS AMONG THE CHICKAHOMINY INDIAN TRIBE’S YOUTH, THE PROJECT ESTABLISHED THE FOLLOWING GOALS: 1. TO BUILD THE CHICKAHOMINY INDIAN TRIBE’S CAPACITY TO SUSTAINABLY ADDRESS YOUTH MENTAL HEALTH NEEDS WITHIN THE TRIBAL COMMUNITY. THIS GOAL’S MEASURABLE OBJECTIVE WILL BE THE DEVELOPMENT OF DELIVERABLES INCLUDING THE DISPARITY IMPACT STATEMENT, COMMUNITY NEEDS ASSESSMENT, COMMUNITY READINESS ASSESSMENT, TRIBAL STRATEGIC ACTION PLAN, COMMUNITY ENGAGEMENT ITEMS (I.E., MEETING AGENDAS, INFORMATIONAL FLYERS, SURVEYS), YOUTH ADVISORY BOARD (ATTENDANCE RECORDS AND AGENDAS), SUBSTANCE ABUSE AND MENTAL HEALTH (SAMH) PARTNERSHIP PLAN (PLAN AND MEMORANDUMS OF UNDERSTANDING), AND POSTVENTION PROTOCOLS. 2. TO PROVIDE SUBSTANCE ABUSE AND MENTAL HEALTH EDUCATION TO ORGANIZATIONAL STAFF AND TRIBAL CITIZENS, ENABLING THEM TO RECOGNIZE WARNING SIGNS, AND TO UNDERSTAND PREVENTION, INTERVENTION, AND MENTAL HEALTH CARE OPTIONS RELEVANT TO TRIBAL YOUTH. THIS GOAL’S MEASURABLE OBJECTIVE IS THE NUMBER OF MENTAL HEALTH EDUCATIONAL RESOURCES CREATED (NUMBER OF INDIVIDUAL RESOURCE SHEETS CREATED) AND DISSEMINATED (NUMBER OF RESOURCE SHEETS DISTRIBUTED) AND THE NUMBER OF AWARENESS AND EDUCATION ACTIVITIES ATTENDED (REGISTRATION AND ATTENDANCE SHEETS). 3. TO ENHANCE PROTECTIVE FACTORS THROUGH EXISTING AND NEW TRIBAL PROGRAMMING THROUGH PROGRAMS THAT ENCOURAGE PARENTAL RESILIENCE AND CULTURAL AND SOCIAL CONNECTION AMONG TRIBAL YOUTH. WE ARE PLEASED THAT THE COUNTY IS CURRENTLY INSTALLING FIBER OPTIC LINES, AND COX COMMUNICATIONS WILL PROVIDE COMMERCIAL BROADBAND SERVICE TO HOMES AND COMMERCIAL BUILDINGS IN THE COUNTY BEGINNING THIS SUMMER. WE PLAN TO USE TBCP FUNDS TO HELP SUBSIDIZE THE COST FOR OUR TRIBAL MEMBERS TO SUBSCRIBE TO THESE NEW COX BROADBAND SERVICES AT GIG SPEEDS BY PAYING FOR MONTHLY SUBSCRIPTION FEES. WITHOUT A LOCAL GROCERY STORE AND ONLY 1 HEALTH FACILITY, COMPLIANCE WITH DIABETES AND HEALTH CARE IN GENERAL, TO INCLUDE HEALTHY EATING AND PHYSICAL ACTIVITY, MAY OFTEN HIT A ROADBLOCK. IMPLEMENTATION OF PROGRAMS AND SERVICES RELATED TO DIABETES EDUCATION WILL PROVIDE A ROADMAP FOR THE TRIBAL COMMUNITY TO NAVIGATE DIABETES, AND IN SOME CASES, PREVENT IT BEFORE IT’S TOO LATE. TRIBAL CITIZENS WITH PRE-DIABETES, GESTATIONAL DIABETES, AND DIABETES WILL GAIN KNOWLEDGE AND EDUCATION RELATED TO DIABETES AND HOW IT IMPACTS THEIR OVERALL HEALTH. KEY ELEMENTS TO DIABETES PREVENTION, MANAGEMENT, AND COMPLIANCE WILL BE HIGHLIGHTED: (A) HEALTHY EATING/ACCESS TO NUTRITIOUS FOODS, (B) PHYSICAL ACTIVITY, AND (C) BLOOD GLUCOSE MONITORING/CONTROL. THROUGH A SERIES OF WORKSHOPS, CLINICS, AND CLASSES, CITIZENS WILL: (A) LEARN TO SELECT AND PREPARE HEALTHY FOODS, INCLUDING TRADITIONAL FOODS, (B) ENGAGE IN PHYSICAL ACTIVITY (E.G., WALKING GROUPS, CHAIR EXERCISES, AND NATIVE DANCE), (C) PARTICIPATE IN CLINICS WITH TRIBAL NURSES (E.G., ASSESSMENT OF VITAL SIGNS, BLOOD GLUCOSE LEVELS, AND FOOT HEALTH), AND (D) PARTICIPATE IN SUPPORT GROUPS TO LEARN FROM AND SUPPORT ONE ANOTHER IN THE DIABETES JOURNEY. IN ORDER TO HAVE A COMPREHENSIVE AND HIPAA COMPLIANT TRACKING MECHANISM, THE PROJECT WILL INCLUDE IMPLEMENTING CASE MANAGEMENT SOFTWARE. CHICKAHOMINY CIRCLES OF GRACE: HEALING YOUTH THROUGH COMMUNITY CONNECTIONS - THE CHICKAHOMINY CIRCLES OF GRACE: HEALING YOUTH THROUGH COMMUNITY CONNECTIONS PROJECT WILL HELP ADDRESS SUBSTANCE MISUSE AND MENTAL HEALTH CHALLENGES BY BUILDING THE CHICKAHOMINY INDIAN TRIBE’S CAPACITY TO EDUCATE THE COMMUNITY, REDUCE THE STIGMA THAT CURRENTLY PREVENTS TRIBAL CITIZENS FROM SEEKING MENTAL HEALTH SERVICES, AND INCORPORATE PROTECTIVE FACTORS TO SAFEGUARD TRIBAL YOUTH IN THE AREA SURROUNDING THE TRIBAL HEADQUARTERS IN CHARLES CITY COUNTY, VIRGINIA. THE POPULATION WE SEEK TO SERVE IS YOUTH IN THE TRIBAL COMMUNITY (ENROLLED CITIZENS AND CHILDREN LIVING WITH ENROLLED CITIZENS), WHICH WE ANTICIPATE WILL CONSIST OF 50 COMMUNITY MEMBERS BETWEEN THE AGES OF 0 AND 24 DURING YEAR 1 OF THE PROJECT AND 170 IN SUBSEQUENT YEARS, FOR AN ESTIMATED CUMULATIVE REACH OF APPROXIMATELY 220 UNIQUE INDIVIDUALS OVER THE LIFETIME OF THE GRANT. IN ADDITION TO THIS TARGET POPULATION, WE ALSO EXPECT TO REACH THE GENERAL CITIZENRY, AS THE PROJECT WILL WIDELY DISTRIBUTE EDUCATIONAL RESOURCES AND CREATE LEARNING OPPORTUNITIES AND TRIBAL EVENTS TO SUPPORT COMMUNITY CONNECTION. CIRCLES OF GRACE WILL EMPLOY STRATEGIES OF PROFESSIONAL DEVELOPMENT FOR TRIBAL STAFF ACROSS DEPARTMENTS WHO ARE IN A POSITION TO BOLSTER PROTECTIVE FACTORS THROUGH EXISTING PROGRAMMING AND THE CREATION AND DISTRIBUTION OF TARGETED MENTAL HEALTH RESOURCES DESIGNED TO EDUCATE TRIBAL CITIZENS AND DE-STIGMATIZE MENTAL HEALTH SERVICES. TO BUILD A FOUNDATION FOR DURABLE WELLNESS AMONG THE CHICKAHOMINY INDIAN TRIBE’S YOUTH, THE PROJECT ESTABLISHED THE FOLLOWING GOALS: 1. TO BUILD THE CHICKAHOMINY INDIAN TRIBE’S CAPACITY TO SUSTAINABLY ADDRESS YOUTH MENTAL HEALTH NEEDS WITHIN THE TRIBAL COMMUNITY. THIS GOAL’S MEASURABLE OBJECTIVE WILL BE THE DEVELOPMENT OF DELIVERABLES INCLUDING THE DISPARITY IMPACT STATEMENT, COMMUNITY NEEDS ASSESSMENT, COMMUNITY READINESS ASSESSMENT, TRIBAL STRATEGIC ACTION PLAN, COMMUNITY ENGAGEMENT ITEMS (I.E., MEETING AGENDAS, INFORMATIONAL FLYERS, SURVEYS), YOUTH ADVISORY BOARD (ATTENDANCE RECORDS AND AGENDAS), SUBSTANCE ABUSE AND MENTAL HEALTH (SAMH) PARTNERSHIP PLAN (PLAN AND MEMORANDUMS OF UNDERSTANDING), AND POSTVENTION PROTOCOLS. 2. TO PROVIDE SUBSTANCE ABUSE AND MENTAL HEALTH EDUCATION TO ORGANIZATIONAL STAFF AND TRIBAL CITIZENS, ENABLING THEM TO RECOGNIZE WARNING SIGNS, AND TO UNDERSTAND PREVENTION, INTERVENTION, AND MENTAL HEALTH CARE OPTIONS RELEVANT TO TRIBAL YOUTH. THIS GOAL’S MEASURABLE OBJECTIVE IS THE NUMBER OF MENTAL HEALTH EDUCATIONAL RESOURCES CREATED (NUMBER OF INDIVIDUAL RESOURCE SHEETS CREATED) AND DISSEMINATED (NUMBER OF RESOURCE SHEETS DISTRIBUTED) AND THE NUMBER OF AWARENESS AND EDUCATION ACTIVITIES ATTENDED (REGISTRATION AND ATTENDANCE SHEETS). 3. TO ENHANCE PROTECTIVE FACTORS THROUGH EXISTING AND NEW TRIBAL PROGRAMMING THROUGH PROGRAMS THAT ENCOURAGE PARENTAL RESILIENCE AND CULTURAL AND SOCIAL CONNECTION AMONG TRIBAL YOUTH. CHICKAHOMINY INDIAN TRIBE - EASTERN DIVISION OUR MODERN COMMUNITY IN RURAL NEW KENT COUNTY HAS NOT HAD THE OPPORTUNITY FOR BROADBAND CONNECTIVITY PRIOR TO 2024. TO DATE, WE HAVE BEEN RELIANT ON CELLULAR HOTSPOTS OR CIT-ED TBCP ROUND 2 2 SATELLITE IN ALMOST ALL AREAS OF THE COUNTY. WE ARE PLEASED THAT THE COUNTY IS CURRENTLY INSTALLING FIBER OPTIC LINES, AND COX COMMUNICATIONS WILL PROVIDE COMMERCIAL BROADBAND SERVICE TO HOMES AND COMMERCIAL BUILDINGS IN THE COUNTY BEGINNING THIS SUMMER. WE PLAN TO USE TBCP FUNDS TO HELP SUBSIDIZE THE COST FOR OUR TRIBAL MEMBERS TO SUBSCRIBE TO THESE NEW COX BROADBAND SERVICES AT GIG SPEEDS BY PAYING FOR MONTHLY SUBSCRIPTION FEES. WE WILL ALSO DISTRIBUTE END-USER DEVICES (E.G., IPADS AND/OR LAPTOPS) TO FAMILIES RECEIVING BILL PAYMENT SUBSIDIES SO THAT THEY ARE ABLE TO TAKE FULL ADVANTAGE OF THEIR NEW CONNECTIVITY OPTIONS. IN ADDITION TO ASSISTING OUR MEMBERS WITH THEIR CONNECTIONS, WE PLAN TO UPGRADE THE CONNECTIVITY AT OUR TRIBAL ADMINISTRATIVE FACILITY, AND UPDATE THE OBSOLETE DEVICES THAT OUR STAFF ARE CURRENTLY USING. THE CHICKAHOMINY INDIAN TRIBE IS NO EXCEPTION, WITH 51 OUT OF 64 DIABETES NEEDS ASSESSMENT RESPONDENTS REPORTING SOME FORM OF DIABETES (E.G., PRE, GESTATIONAL, DIABETES). WITH MANY TRIBAL CITIZENS LIVING WITHIN MINUTES OF THE CHICKAHOMINY TRIBAL CENTER IN CHARLES CITY COUNTY, VIRGINIA, THEIR ACCESS TO NUTRITIOUS FOODS AND DIABETES-RELATED HEALTH SERVICES CAN BE CHALLENGING DUE TO THE RURAL LOCATION. WITHOUT A LOCAL GROCERY STORE AND ONLY 1 HEALTH FACILITY, COMPLIANCE WITH DIABETES AND HEALTH CARE IN GENERAL, TO INCLUDE HEALTHY EATING AND PHYSICAL ACTIVITY, MAY OFTEN HIT A ROADBLOCK. IMPLEMENTATION OF PROGRAMS AND SERVICES RELATED TO DIABETES EDUCATION WILL PROVIDE A ROADMAP FOR THE TRIBAL COMMUNITY TO NAVIGATE DIABETES, AND IN SOME CASES, PREVENT IT BEFORE IT’S TOO LATE. TRIBAL CITIZENS WITH PRE-DIABETES, GESTATIONAL DIABETES, AND DIABETES WILL GAIN KNOWLEDGE AND EDUCATION RELATED TO DIABETES AND HOW IT IMPACTS THEIR OVERALL HEALTH. KEY ELEMENTS TO DIABETES PREVENTION, MANAGEMENT, AND COMPLIANCE WILL BE HIGHLIGHTED: (A) HEALTHY EATING/ACCESS TO NUTRITIOUS FOODS, (B) PHYSICAL ACTIVITY, AND (C) BLOOD GLUCOSE MONITORING/CONTROL. THROUGH A SERIES OF WORKSHOPS, CLINICS, AND CLASSES, CITIZENS WILL: (A) LEARN TO SELECT AND PREPARE HEALTHY FOODS, INCLUDING TRADITIONAL FOODS, (B) ENGAGE IN PHYSICAL ACTIVITY (E.G., WALKING GROUPS, CHAIR EXERCISES, AND NATIVE DANCE), (C) PARTICIPATE IN CLINICS WITH TRIBAL NURSES (E.G., ASSESSMENT OF VITAL SIGNS, BLOOD GLUCOSE LEVELS, AND FOOT HEALTH), AND (D) PARTICIPATE IN SUPPORT GROUPS TO LEARN FROM AND SUPPORT ONE ANOTHER IN THE DIABETES JOURNEY. WEAVING PATHWAYS OF CARE AIMS TO IMPROVE THE PROVISION OF CULTURALLY COMPETENT AND AGE-FRIENDLY CARE, SERVICES, AND OUTCOMES FOR BOTH CHICKAHOMINY CITIZENS LIVING WITH DEMENTIA AND THEIR CAREGIVERS. - WEAVING PATHWAYS OF CARE SEEKS TO INCREASE THE PROVISION OF CULTURALLY COMPETENT AND AGE-FRIENDLY CARE, SERVICES, AND OUTCOMES FOR BOTH CHICKAHOMINY TRIBAL CITIZENS LIVING WITH DEMENTIA AND THEIR CAREGIVERS, AND TO RAISE BRAIN HEALTH AWARENESS AND COGNITIVE SCREENING RATES AMONG THE TRIBAL CITIZENS. THE CHICKAHOMINY INDIAN TRIBE’S LEADERSHIP RECOGNIZES THAT AMERICAN INDIANS EXPERIENCE COGNITIVE IMPAIRMENT AND DEMENTIA AT HIGHER RATES THAN OTHER RACIAL AND ETHNIC GROUPS. THEY HAVE AN UNWAVERING COMMITMENT TO ADDRESS THIS CRITICAL ISSUE TO IMPROVE THE QUALITY OF LIFE FOR BOTH TRIBAL CITIZENS LIVING WITH DEMENTIA AND THEIR CAREGIVERS, AND TO EASE ANY BURDENS ASSOCIATED WITH RESOURCE AND CARE NAVIGATION. WHILE THE TRIBE CURRENTLY PROVIDES BRAIN HEALTH EDUCATION AND CAREGIVER SUPPORT, THERE IS A NEED TO INCREASE PROVISIONS AND CREATE A ROBUST SYSTEM THAT ENCOMPASSES A DEDICATED STAFF MEMBER ALONG WITH AGING SERVICES PROVIDERS/AGENCIES AND HEALTHCARE PROVIDERS. TO DO SO, ONE KEY ASPECT OF THE PROJECT IS TO ONBOARD A BRAIN HEALTH NAVIGATOR WHO WILL WORK SPECIFICALLY WITH BOTH TRIBAL CITIZENS LIVING WITH DEMENTIA AND THEIR CAREGIVERS. SOME OF THE RESPONSIBILITIES OF THE NAVIGATOR INCLUDE PROVIDING BRAIN HEALTH EDUCATION, INFORMATION ABOUT SERVICES OFFERED VIA THE NEWLY-OPENED MID-ATLANTIC TRIBAL HEALTH CENTER (INDIAN HEALTH SERVICE), COMMUNITY AGENCIES, AND LOCAL HEALTH SYSTEMS, AND CASE MANAGEMENT TRACKING. IN ORDER TO HAVE A COMPREHENSIVE AND HIPAA COMPLIANT TRACKING MECHANISM, THE PROJECT WILL INCLUDE IMPLEMENTING CASE MANAGEMENT SOFTWARE. FORGING NEW RELATIONSHIPS AND ALSO BUILDING UPON EXISTING RELATIONSHIPS WITH THE MID-ATLANTIC TRIBAL HEALTH CENTER (INDIAN HEALTH SERVICE), COMMUNITY AGENCIES, AND LOCAL HEALTH SYSTEMS ARE KEY COMPONENTS OF THE PROJECT. THROUGH COLLABORATION THAT INCLUDES EDUCATION, TRAINING, AND BI-DIRECTIONAL REFERRALS, THE PROJECT WILL WEAVE TOGETHER AN ECOSYSTEM THAT CREATES A STRONG PATH FORWARD FOR CULTURALLY RELEVANT AND AGE-FRIENDLY CARE FOR BOTH TRIBAL CITIZENS LIVING WITH DEMENTIA AND THEIR CAREGIVERS. EXPECTED OUTCOMES OF THE PROJECT INCLUDE, BUT ARE NOT LIMITED TO, INCREASED KNOWLEDGE AND AWARENESS OF BRAIN HEALTH RISK FACTORS AND DEMENTIA SIGNS AND SYMPTOMS, INCREASED NUMBER OF COGNITIVE SCREENINGS, AND INCREASED ASSESSMENTS FOR SOCIAL NEEDS AND IMPROVED ACCESS TO RESOURCES TO ADDRESS IDENTIFIED NEEDS. MOREOVER, THE PROJECT IS EXPECTED TO HELP DEMYSTIFY THE PROCESS OF OBTAINING COGNITIVE SCREENINGS AND ASSOCIATED SERVICES IF A POSITIVE SCREEN OCCURS. THROUGH CULTURALLY APPROPRIATE, THOUGHTFUL, AND CONCERTED EFFORTS, THE PROJECT WILL LIFT UP AND SUPPORT OUR MOST VULNERABLE POPULATION. INITIAL PROJECT IMPLEMENTATION FOLLOWED BY FUTURE GROWTH WILL PROVIDE IMMEASURABLE VALUE FOR THE NEXT SEVEN GENERATIONS.

DESCRIPTION: THIS PURPOSE OF THIS AWARD IS TO FUND THE CHICKAHOMINY INDIAN TRIBE EASTERN DIVISION DECREASING THE AMOUNT OF FOOD WASTE DISPOSAL BY ESTABLISHING A COMMUNITY COMPOST PROGRAM. THE COMPOST PRODUCED WILL BE UTILIZED IN A COMMUNITY GARDEN ON TRIBAL LANDS. THE PROJECT WILL CONTRIBUTE AND SUPPORT TRIBAL CITIZENS EDUCATION AND INVOLVEMENT, CONNECT CITIZENS TO ANCESTRAL FOOD PRACTICES, DECREASE FOOD INSECURITIES, AND INCREASE AVAILABILITY OF LOCAL, FRESH PRODUCE IN THE TRIBAL COMMUNITY. ACTIVITIES: ACTIVITIES INCLUDE KEEPING TRACK OF FOOD WASTES THAT ARE DIVERTED FROM LANDFILLS, UTILIZED IN AN ORGANIC COMPOSTING PROGRAM AND TRIBAL COMMUNITY GARDEN. THIS EFFORT WILL INCREASE FOOD AVAILABILITY TO TRIBAL CITIZENS, ALLOWING CITIZENS TO LEARN CULTURAL AND LONG-TERM KNOWLEDGE REGARDING FOOD WASTE, PRODUCTION, AND HEALTHY EATING. OUTCOMES: THE ANTICIPATED OUTCOMES AND OUTPUTS OF THE PROJECT INCLUDE THE DIVERSION OF FOOD WASTE TOWARDS SUSTAINABLE MANAGEMENT PRACTICES INCREASING THE AVAILABILITY OF LOCALLY GROWN FOODS, COMMUNITY ENGAGEMENT AND EDUCATION. THE INTENDED BENEFICIARY OF THE PROJECT ARE THE TRIBAL CITIZENS. SUBRECIPIENT: NO SUBAWARDS ARE INCLUDED IN THIS ASSISTANCE AGREEMENT.

STATES, TERRITORIES, AND TRIBAL GOVERNMENTS WILL PURCHASE LOCAL, UNPROCESSED OR MINIMALLY PROCESSED DOMESTIC FOODS FROM LOCAL PRODUCERS, TARGETING HISTORICALLY UNDERSERVED FARMERS/PRODUCERS/FISHERS AND SMALL BUSINESSES INCLUDING PROCESSORS, AGGREGATORS, AND DISTRIBUTORS. FOOD PURCHASED WILL TARGET DISTRIBUTION TO UNDERSERVED COMMUNITIES. QUARTERLY PERFORMANCE REPORTS WILL BE SUBMITTED TO CAPTURE THE VALUE OF PURCHASES (INCLUDING THE VALUE OF PURCHASES FROM HISTORICALLY UNDERSERVED PRODUCERS AND THE VALUE OF PURCHASES FROM SMALL BUSINESSES), THE NUMBER OF PRODUCERS (INCLUDING THE NUMBER OF HISTORICALLY UNDERSERVED PRODUCERS AND THE NUMBER OF SMALL BUSINESSES), THE VALUE OF FOOD DISTRIBUTIONS TO DATE, AND THE NUMBER OF UNIQUE LOCATIONS DISTRIBUTING FOOD TO DATE.
