DFU IS THE LEADING CAUSE OF AMPUTATION, MORE COSTLY THAN THE MOST EXPENSIVE CANCER, AND SURVIVORS OF LOWER LIMB AMPUTATION HAVE A HIGHER MORTALITY RATE THAN HALF OF THE TOP 10 CANCERS. INVENTION: TOGETHER WITH PATIENT INPUT, TEAM HAS DESIGNED A NOVEL ASSISTIVE EXOSKELETON THAT HAS BEEN SHOWN IN PH I STUDIES TO OFF-LOAD PRESSURE FROM THE FOOT BY DEPLOYING A DISRUPTIVE DESIGN FOR REHABILITATION ROBOTICS. IT INTEGRATES A STRUCTURAL EXO-SKELETON TO EXTERNALIZE LOWER LIMB FORCES OUTSIDE OF THE BODY. IN TWO PHASE I STUDIES, THE EXOSKELETON HAS DEMONSTRATED REDUCTION OF FORCE UNDER THE FOREFOOT. UNEXPECTEDLY, THE DEVICE HAS ALSO DEMONSTRATED IMPROVED PROPULSION BY INCREASING STRIDE LENGTH AND WALKING SPEED CONCURRENT TO REDUCING PLANTAR PRESSURES. THE DEVICE HAS RECEIVED ENTHUSIASTIC PATIENT FEEDBACK FROM MULTIPLE PARTICIPANTS WHO ENJOYED FEELING A “SPRING IN THEIR STEP” AND SHOWED NO NEGATIVE IMPACT ON MEASURES OF FALL RISK. AIM 1) PRODUCT REFINEMENT ENGINEERING: TEAM WILL IMPROVE UPON PH I PROTOTYPES TO OVERCOME ISSUES WITH COMFORT AND TO ENSURE DURABILITY AND ROBUSTNESS. AIM 2) MOTOR RESTORATION OF GAIT & BALANCE, ACTIVITY, PSYCHO-SOCIAL: THE PROTOTYPES WILL BE EVALUATED FOR THEIR ABILITY TO HELP RESTORE CLINICAL GAIT FUNCTION AND BALANCE AT 1, 3 AND 6 MONTHS AFTER COMPLETION OF WOUND HEALING SUBSEQUENT TO DFU TREATMENT AS WELL AS THEIR IMPACT ON THE RESTORATION OF DAILY PHYSICAL ACTIVITY LEVEL AND PSYCHO SOCIAL MEASURES. EFFICACY (RECURRENCE RATES) ARE BEYOND THE AVAILABLE BUDGET OF SBIR AND WILL BE PURSUED WITH COLLABORATIVE UNIVERSITY LED RESEARCH. RATIONALE: THE TEAM RECENTLY COMPLETED TWO PHASE I SBIR FUNDED STUDIES THROUGH NIH/NATIONAL INSTITUTE OF NURSING RESEARCH AND ACL/NATIONAL INSTITUTE FOR DISABILITY, INDEPENDENT LIVING AND REHABILITATION RESEARCH. RESULTS SHOW THAT THE PROTOTYPES WERE ABLE TO OFF-LOAD SIGNIFICANT PRESSURE BY EXTERNALIZING FORCES. TEAM HAS BEEN AWARDED 4 ISSUED US PATENTS AND INTERNATIONAL IP. THE GOAL IS TO DECELERATE CMT PROGRESSION BY CREATING A NEW ALTERNATIVE TO ANKLE/FOOT BRACING WHICH CAN BE STARTED YEARS EARLIER THAN IS POSSIBLE WITH TODAY’S ANKLE FOOT ORTHOSES (AFOS) WHICH HAVE MULTIPLE ISSUES OF DISCOMFORT, BULKINESS, HIGH COSTS AND STIGMA. CURRENT CLINICAL CARE GUIDELINES ACKNOWLEDGE THE DIFFICULTY OF WEARING AFOS AND DO NOT PRESCRIBE THEM UNTIL AFTER SIGNIFICANT DISEASE PROGRESSION, LEAVING 10+ YEARS OF MISSED OPPORTUNITY FOR EARLIER TREATMENT. OUR OBJECTIVES ARE: 1) TO DESIGN A NEW EXOSKELETON BRACING DEVICE HIDDEN WITHIN A BOOT WITH GREATER DROP-FOOT RESISTANCE 2) TO CONDUCT BENCHTOP TESTING AND COMPARE BOOT PERFORMANCE TO TODAY’S BEST AFOS 3) TO DESIGN A PHASE II CLINICAL STUDY THAT OVERCOMES BARRIERS TO PARTICIPATION OF UNDERSERVED POPULATIONS. PRODUCTS GENERATED INCLUDE: 1) A PENDULUM TESTING RIG TO EVALUATE TRIP HAZARDS FROM DROP FOOT, 2) THREE EXOSKELETON SHOE PROTOTYPES, 3) BENCHTOP TEST EVALUATION REPORTS, AND 4) A PHASE II STUDY PLAN. UNDER THIS PROGRAM, SUCH SERVICES MAY BE PROVIDED TO ADULT, YOUTH, AND CHILD VICTIMS OF SEXUAL ASSAULT, FAMILY AND HOUSEHOLD MEMBERS OF SUCH VICTIMS, AND THOSE COLLATERALLY AFFECTED BY THE VICTIMIZATION. SURVIVORS OF SEXUAL ASSAULT FROM CULTURALLY SPECIFIC COMMUNITIES FREQUENTLY CONFRONT UNIQUE CHALLENGES WHEN SEEKING ASSISTANCE, SUCH AS LINGUISTIC AND CULTURAL BARRIERS. CULTURALLY SPECIFIC COMMUNITY-BASED ORGANIZATIONS ARE MORE LIKELY TO UNDERSTAND THESE CHALLENGES BECAUSE THEY ARE FAMILIAR WITH THE CULTURE, LANGUAGE, AND BACKGROUND OF VICTIMS FROM THEIR COMMUNITIES, WHO IN TURN ARE MORE INCLINED TO SEEK SERVICES FROM SUCH ORGANIZATIONS. THE GOAL OF THE SAS CULTURALLY SPECIFIC PROGRAM IS TO ESTABLISH, MAINTAIN, AND EXPAND CULTURALLY SPECIFIC INTERVENTION AND RELATED ASSISTANCE FOR VICTIMS OF SEXUAL ASSAULT, WHICH SERVE A VITAL ROLE IN PROVIDING SERVICES THAT ARE RELEVANT FOR THEIR COMMUNITIES. IN FY 2023, OVW WILL AWARD PLANNINGGRANTS FOR UP TO$100,000 FOR A PERIOD OF UP TO 12 MONTHS TO ASSIST ORGANIZATIONS IN ESTABLISHING COMPREHENSIVE, CULTURALLY SPECIFIC, SEXUAL ASSAULT SERVICES FOR VICTIMS. AIM 2) MOTOR RESTORATION OF GAIT & BALANCE, ACTIVITY, PSYCHO-SOCIAL: THE PROTOTYPES WILL BE EVALUATED FOR THEIR ABILITY TO HELP RESTORE CLINICAL GAIT FUNCTION AND BALANCE AT 1, 3 AND 6 MONTHS AFTER COMPLETION OF WOUND HEALING SUBSEQUENT TO DFU TREATMENT AS WELL AS THEIR IMPACT ON THE RESTORATION OF DAILY PHYSICAL ACTIVITY LEVEL AND PSYCHO SOCIAL MEASURES. EFFICACY (RECURRENCE RATES) ARE BEYOND THE AVAILABLE BUDGET OF SBIR AND WILL BE PURSUED WITH COLLABORATIVE UNIVERSITY LED RESEARCH. RATIONALE: THE TEAM RECENTLY COMPLETED TWO PHASE I SBIR FUNDED STUDIES THROUGH NIH/NATIONAL INSTITUTE OF NURSING RESEARCH AND ACL/NATIONAL INSTITUTE FOR DISABILITY, INDEPENDENT LIVING AND REHABILITATION RESEARCH. RESULTS SHOW THAT THE PROTOTYPES WERE ABLE TO OFF-LOAD SIGNIFICANT PRESSURE BY EXTERNALIZING FORCES. TEAM HAS BEEN AWARDED 4 ISSUED US PATENTS AND INTERNATIONAL IP. IF SUCCESSFUL, THIS WILL BE THE FIRST DISRUPTIVE INNOVATION IN OFF-LOADING IN DECADES AND ILLUMINATES KEY DESIGN FACTORS FOR FUTURE EVALUATION OF ROBOTIC OFF- LOADING DEVICES AND SENSOR CONTROLLED DEVICES.

THIS PROGRAM SUPPORTS THE MAINTENANCE AND REPLICATION OF EXISTING SUCCESSFUL COMMUNITY-BASED PROGRAMS PROVIDING CULTURALLY SPECIFIC SERVICES TO VICTIMS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING, AS WELL AS THE DEVELOPMENT OF INNOVATIVE CULTURALLY SPECIFIC STRATEGIES TO ENHANCE ACCESS TO SERVICES AND RESOURCES FOR VICTIMS WHO FACE OBSTACLES TO USING MORE TRADITIONAL PROGRAMS. CULTURALLY SPECIFIC SERVICES MEANS COMMUNITY-BASED SERVICES THAT INCLUDE CULTURALLY RELEVANT AND LINGUISTICALLY SPECIFIC SERVICES AND RESOURCES TO CULTURALLY SPECIFIC COMMUNITIES WHICH ARE STATUTORILY DEFINED AS “AMERICAN INDIANS (INCLUDING ALASKA NATIVES, ESKIMOS, AND ALEUTS); ASIAN AMERICANS; NATIVE HAWAIIANS AND OTHER PACIFIC ISLANDERS; BLACKS; AND HISPANICS.” PROJECTS MUST: 1) WORK WITH STATE AND LOCAL GOVERNMENTS AND SOCIAL SERVICE AGENCIES TO DEVELOP AND ENHANCE EFFECTIVE STRATEGIES TO PROVIDE CULTURALLY SPECIFIC VICTIM SERVICES ; 2) INCREASE COMMUNITIES’ CAPACITY TO PROVIDE CULTURALLY SPECIFIC RESOURCES AND SUPPORT FOR VICTIMS AND THEIR FAMILIES; 3) STRENGTHEN CRIMINAL JUSTICE INTERVENTIONS BY PROVIDING TRAINING FOR LAW ENFORCEMENT, PROSECUTION, COURTS, PROBATION, AND CORRECTIONAL FACILITIES ON CULTURALLY SPECIFIC RESPONSES; 4) ENHANCE TRADITIONAL SERVICES TO VICTIMS THROUGH THE LEADERSHIP OF CULTURALLY SPECIFIC VICTIM SERVICES PROGRAMS ; 5) WORK IN COOPERATION WITH THE COMMUNITY TO DEVELOP EDUCATION AND PREVENTION STRATEGIES HIGHLIGHTING CULTURALLY SPECIFIC ISSUES AND RESOURCES; 6) PROVIDE CULTURALLY SPECIFIC PROGRAMS FOR CHILDREN EXPOSED TO DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, AND STALKING; 7) PROVIDE CULTURALLY SPECIFIC RESOURCES AND SERVICES THAT ADDRESS THE SAFETY, ECONOMIC, HOUSING, AND WORKPLACE NEEDS OF VICTIMS, INCLUDING EMERGENCY ASSISTANCE; AND/OR 8) EXAMINE THE DYNAMICS OF CULTURE AND ITS IMPACT ON VICTIMIZATION AND HEALING.

EXPANDING ACCESS TO ASSISTIVE WALKING DEVICES: MANAGING SYMPTOMS FOR CHILD AND ADOLESCENT CMT PATIENTS WITH EXOSKELETAL BOOTS - RESULTS GROUP LLC TOGETHER WITH THE CHARCOT-MARIE-TOOTH ASSOCIATION (CMTA) AND CMT CENTERS OF EXCELLENCE AT MASS GENERAL HOSPITAL, ST. JUDE HOSPITAL AND CHILDREN’S HOSPITAL OF COLORADO WILL CONDUCT A 6-MONTH PROJECT TO RE-IMAGINE ASSISTIVE TECHNOLOGY TO ADDRESS A CRITICAL GAP IN THE START OF TREATMENT OF YOUTH WITH CMT. THE GOAL IS TO DECELERATE CMT PROGRESSION BY CREATING A NEW ALTERNATIVE TO ANKLE/FOOT BRACING WHICH CAN BE STARTED YEARS EARLIER THAN IS POSSIBLE WITH TODAY’S ANKLE FOOT ORTHOSES (AFOS) WHICH HAVE MULTIPLE ISSUES OF DISCOMFORT, BULKINESS, HIGH COSTS AND STIGMA. CURRENT CLINICAL CARE GUIDELINES ACKNOWLEDGE THE DIFFICULTY OF WEARING AFOS AND DO NOT PRESCRIBE THEM UNTIL AFTER SIGNIFICANT DISEASE PROGRESSION, LEAVING 10+ YEARS OF MISSED OPPORTUNITY FOR EARLIER TREATMENT. OUR OBJECTIVES ARE: 1) TO DESIGN A NEW EXOSKELETON BRACING DEVICE HIDDEN WITHIN A BOOT WITH GREATER DROP-FOOT RESISTANCE 2) TO CONDUCT BENCHTOP TESTING AND COMPARE BOOT PERFORMANCE TO TODAY’S BEST AFOS 3) TO DESIGN A PHASE II CLINICAL STUDY THAT OVERCOMES BARRIERS TO PARTICIPATION OF UNDERSERVED POPULATIONS. THE SEXUAL ASSAULT SERVICES CULTURALLY SPECIFIC PROGRAM (SAS CULTURALLY SPECIFIC PROGRAM) WAS CREATED BY THE VIOLENCE AGAINST WOMEN AND DEPARTMENT OF JUSTICE REAUTHORIZATION ACT OF 2005 (VAWA 2005), 34 U.S.C. 12511(C), AND IS PART OF THE FIRST FEDERAL FUNDING STREAM SOLELY DEDICATED TO THE PROVISION OF DIRECT INTERVENTION AND RELATED ASSISTANCE FOR VICTIMS OF SEXUAL ASSAULT. INTERVENTION AND RELATED ASSISTANCE INCLUDE ADVOCACY, ACCOMPANIMENT (E.G., ACCOMPANYING VICTIMS TO COURT, MEDICAL FACILITIES, POLICE DEPARTMENTS, ETC.), CRISIS INTERVENTION AND SUPPORT SERVICES, AND REFERRALS, AMONG OTHER SERVICES. UNDER THIS PROGRAM, SUCH SERVICES MAY BE PROVIDED TO ADULT, YOUTH, AND CHILD VICTIMS OF SEXUAL ASSAULT, FAMILY AND HOUSEHOLD MEMBERS OF SUCH VICTIMS, AND THOSE COLLATERALLY AFFECTED BY THE VICTIMIZATION. SURVIVORS OF SEXUAL ASSAULT FROM CULTURALLY SPECIFIC COMMUNITIES FREQUENTLY CONFRONT UNIQUE CHALLENGES WHEN SEEKING ASSISTANCE, SUCH AS LINGUISTIC AND CULTURAL BARRIERS. CULTURALLY SPECIFIC COMMUNITY-BASED ORGANIZATIONS ARE MORE LIKELY TO UNDERSTAND THESE CHALLENGES BECAUSE THEY ARE FAMILIAR WITH THE CULTURE, LANGUAGE, AND BACKGROUND OF VICTIMS FROM THEIR COMMUNITIES, WHO IN TURN ARE MORE INCLINED TO SEEK SERVICES FROM SUCH ORGANIZATIONS. THE GOAL OF THE SAS CULTURALLY SPECIFIC PROGRAM IS TO ESTABLISH, MAINTAIN, AND EXPAND CULTURALLY SPECIFIC INTERVENTION AND RELATED ASSISTANCE FOR VICTIMS OF SEXUAL ASSAULT, WHICH SERVE A VITAL ROLE IN PROVIDING SERVICES THAT ARE RELEVANT FOR THEIR COMMUNITIES. IN FY 2023, OVW WILL AWARD PLANNINGGRANTS FOR UP TO$100,000 FOR A PERIOD OF UP TO 12 MONTHS TO ASSIST ORGANIZATIONS IN ESTABLISHING COMPREHENSIVE, CULTURALLY SPECIFIC, SEXUAL ASSAULT SERVICES FOR VICTIMS.
