THEY ALSO FUNCTION AS THE PRINCIPAL CONNECTORS ACROSS AND AMONG TRIBAL, REGIONAL, AND STATE PUBLIC HEALTH DEPARTMENTS, KNITTING TOGETHER A MORE COHESIVE, INTEGRATED, NATIONAL PUBLIC HEALTH SYSTEM. HOWEVER, THE COVID-19 PANDEMIC TESTED THE RESILIENCY OF LHDS AS THEY SCALED UP COMMUNICATIONS, TESTING, CONTACT TRACING, AND IMMUNIZATION. STRESSORS OF THE COVID-19 RESPONSE HIGHLIGHTED LONGSTANDING ISSUES THAT NACCHO HAS DOCUMENTED THROUGH OUR SEVEN NATIONAL SURVEYS OF LHDS: LACK OF TIMELY, ACTIONABLE; LACK OF NIMBLE HIRING, TRAINING, AND RETENTION SYSTEMS; INABILITY TO SUSTAIN CRITICAL PROGRAMS AND POLICIES IN THE FACE OF COMPETING DEMANDS; AND THE URGENT NEED TO TACKLE HEALTH INEQUITIES THROUGH SUSTAINED ENGAGEMENT WITH THE MOST IMPACTED COMMUNITIES. RESPONSE EFFORTS ALSO TOOK A TOLL ON STAFF, WITH LHDS EXPERIENCING PERVASIVE HARASSMENT AND STAFF REPORTING SIGNIFICANT MENTAL HEALTH CONCERNS. TO ADDRESS THESE ISSUES, THE NATIONAL ASSOCIATION OF COUNTY AND CITY HEALTH OFFICIALS (NACCHO) PROPOSES USING ITS STRATEGIC, THREE-TIERED CAPACITY BUILDING ASSISTANCE (CBA) FRAMEWORK UNDER THE CDC NOTICE OF FUNDING OPPORTUNITY (NOFO) CDC-RFA-PW-24-0080: STRENGTHENING PUBLIC HEALTH SYSTEMS AND SERVICES THROUGH NATIONAL PARTNERSHIPS TO IMPROVE AND PROTECT THE NATION’S HEALTH (CATEGORY A: LOCAL HEALTH DEPARTMENTS). NACCHO IS THE MEMBERSHIP ORGANIZATION FOR THE MORE THAN 3,300 LHDS ACROSS THE UNITED STATES AND IS THE ONLY ENTITY THAT REPRESENTS ALL OF LOCAL PUBLIC HEALTH, FROM SMALL, RURAL LHDS TO LARGE URBAN AGENCIES. FOR 30 YEARS, NACCHO HAS CONSISTENTLY STRENGTHENED THE NATION’S LHDS IN WAYS THAT MEET THE GOALS AND FIVE STRATEGIC AREAS OF THIS NOFO. NACCHO’S CBA DRIVES LHD PRACTICE THROUGH DEMONSTRATION SITES, IN-PERSON AND VIRTUAL TRAININGS, PEER MENTORSHIP PROGRAMS, AND TOOLS AND RESOURCES LHDS USE DAILY. NACCHO’S EXTENSIVE KNOWLEDGE OF LHD DIVERSITY—ACROSS FACTORS INCLUDING SIZE, FUNDING, GOVERNANCE, AND RURALITY/URBANICITY—MAKES IT THE MOST TRUSTED PROVIDER OF CBA FOR ALL LHDS.

MEDICAL RESERVE CORPS SMALL GRANTS PROGRAM - THE NATIONAL ASSOCIATION OF COUNTY AND CITY HEALTH OFFICIALS (NACCHO) SUPPORTS THIS FIVE-YEAR MEDICAL RESERVE CORPS (MRC) SMALL GRANTS PROGRAM. THE PROJECT GOAL IS TO DEVELOP AND SUSTAIN A TRAINED AND READY VOLUNTEER WORKFORCE TO SUPPORT LOCAL RESPONSE NEEDS THROUGH BUILDING CAPABILITIES OF THE MRC NETWORK, IDENTIFYING AND ADDRESSING RESPONSE BARRIERS, AND DEVELOPING A SUSTAINABILITY PLAN. NACCHO’S OBJECTIVES ARE TO: 1) PROVIDE A FIVE-YEAR MRC UNIT SUB-AWARD FUNDING OPPORTUNITY TO STRENGTHEN UNIT RESPONSE CAPABILITIES; 2) IDENTIFY AND EVALUATE IMPACT OF MRC SUB-AWARD FUNDING TO IMPROVE RESPONSE CAPABILITIES; 3) RESTRUCTURE, UPDATE, AND PROMOTE THE USE OF COMMON DEPLOYMENT READINESS GUIDELINES; 4) IDENTIFY AND PROMOTE THE MRC NETWORK CAPABILITIES; 5) IMPROVE INTEGRATION OF THE MRC IN LOCAL, STATEWIDE, AND/OR INTERSTATE DEPLOYMENTS; 6) PROVIDE RESOURCES AND TRAINING TO SUPPORT SUSTAINABILITY OF UNIT LEADERS, VOLUNTEERS, AND MRC UNITS; AND, 7) INCREASE MRC PROMOTION THROUGH BRAND AWARENESS AND RECOGNITION. RETAIL FOOD SAFETY REGULATION ASSOCIATION COLLABORATION (U18) [CLINICAL TRIAL NOT ALLOWED] - PROJECT SUMMARY/ABSTRACT THE NATIONAL ASSOCIATION OF COUNTY & CITY HEALTH OFFICIALS (NACCHO) COMPRISES OF NEARLY 3,000 LOCAL HEALTH DEPARTMENTS (LHDS) ACROSS THE UNITED STATES. NACCHO STRIVES TO PROTECT AND IMPROVE THE HEALTH OF ALL AMERICANS BY ASSISTING AND ENGAGING LHDS IN PROMOTING NATIONAL PUBLIC HEALTH POLICY, DEVELOPING RESOURCES AND PROGRAMS, SEEKING HEALTH EQUITY, AND SUPPORTING EFFECTIVE LOCAL PUBLIC HEALTH PRACTICE AND SYSTEMS. AN INTEGRAL PIECE OF THIS MISSION IS DEVOTED TO FOOD SAFETY, ACHIEVED THROUGH NACCHO'S PROMOTION AROUND THE ROLE OF LHDS IN FOOD SAFETY REGULATION AND CONTRIBUTING TOWARD AN INTEGRATED FOOD SAFETY SYSTEM, IN COLLABORATION WITH FEDERAL AND PARTNER AGENCIES. NACCHO'S PROPOSAL WILL BE ACCOMPLISHED BY COLLABORATING WITH THE CONFERENCE FOR FOOD PROTECTION (CFP) AND COORDINATING WITH THE RETAIL FOOD SAFETY REGULATORY ASSOCIATION COLLABORATIVE (COLLABORATIVE). THE COLLABORATIVE MEMBERS HAVE SUBMITTED A COORDINATED SET OF FUNDING APPLICATIONS, INCLUDING CONSTANT ONGOING COORDINATION BETWEEN THE ASSOCIATIONS ACROSS THE PROPOSED PROJECTS. NACCHO'S AND CFP'S PROPOSED THREE-YEAR INITIATIVE LAYS OUT THE SIX AIMS OF THE COLLABORATIVE TO ADVANCE STATE, LOCAL, TRIBAL, AND TERRITORIAL (SLTT) RETAIL FOOD PROTECTION PROGRAMS AND REDUCE FOODBORNE ILLNESS. THESE INCLUDE THE FOLLOWING AIMS LED BY NACCHO AND CFP: (1) IMPROVE THE APPROACH, COMPETENCY, AND FOOD SAFETY CULTURE IN THE REGULATORY COMMUNITY; (3) INCREASE ENROLLMENT, ENGAGEMENT, AND CONFORMANCE IN THE VOLUNTARY NATIONAL RETAIL FOOD REGULATORY PROGRAM STANDARDS (PROGRAM STANDARDS); AND (4) INCREASE THE NUMBER OF ESTABLISHMENTS THAT HAVE WELL-DEVELOPED AND IMPLEMENTED FOOD SAFETY MANAGEMENT SYSTEMS (FSMS). THE AIMS WILL BE ACHIEVED THROUGH COLLABORATIVE WORKGROUP EFFORTS, ENVIRONMENTAL SCANS, KEY INFORMANT INTERVIEWS, FOCUS GROUPS, ESTABLISHING A RETAIL FOOD ALLIANCE, QUANTITATIVE AND QUALITATIVE RESEARCH METHODS, EDUCATIONAL CURRICULUM/TRAINING DEVELOPMENT, AND UTILIZING COMMUNICATION CHANNELS TO COMMUNICATE THE COLLABORATIVE'S JOINT MESSAGES AND RESOURCES. THE RETAIL FOOD SAFETY ADVISORY GROUP (SLTT PROGRAM REPRESENTATIVES FROM DIVERSE GEOGRAPHICAL LOCATIONS AND JURISDICTIONAL SIZES) AND COLLABORATIVE WILL INFORM THE THREE-YEAR INITIATIVE TO PROVIDE DIRECTION AND FEEDBACK ON PROJECT ACTIVITIES. UNDER THE GUIDANCE OF THESE GROUPS AND FDA, NACCHO, AND CFP WILL DEVELOP TOOLS AND RESOURCES ESSENTIAL FOR SLTTS TO IMPROVE RETAIL FOOD SAFETY PROGRAMS AND REDUCE FOODBORNE ILLNESS. THE PLAN WILL ALSO ALLOW NACCHO AND CFP TO SHARE STRATEGIES ON SUCCESSFUL ADOPTION OF THE LATEST FOOD CODE EDITIONS, IMPLEMENTATION OF EFFECTIVE INTERVENTION STRATEGIES, APPLICATION OF EFFECTIVE RISK-BASED INSPECTION METHODS, CONFORMANCE TO THE PROGRAM STANDARDS, AND OTHER COLLABORATIVE EFFORTS BASED ON THE ENDEAVORS OF INNOVATIVE LEADERS ENGAGED THROUGH THIS INITIATIVE.

NACCHO IS PLEASED TO SUBMIT THIS PROPOSAL IN RESPONSE TO THE HEALTH RESOURCES & SERVICES ADMINISTRATION (HRSA) NOTICE OF FUNDING OPPORTUNITY ENTITLED “MATERNAL AND CHILD HEALTH POLICY INNOVATION PROGRAM” (MCH PIP), HRSA-24-037 TO SUPPORT THE STAKEHOLDER GROUP, LOCAL HEALTH OFFICIALS (LHOS), TO DEVELOP AND IMPLEMENT INNOVATIVE CITY AND COUNTY-LEVEL INITIATIVES THAT IMPROVE OUTCOMES AND REDUCE ASSOCIATED DISPARITIES FOR MATERNAL AND CHILD HEALTH (MCH) POPULATIONS. IN PARTNERSHIP WITH HRSA, NACCHO WILL PROVIDE TECHNICAL ASSISTANCE TO LHOS TO MEET THE FOLLOWING GOALS: 1) INCREASE STAKEHOLDER CAPACITY TO EFFECTIVELY USE POLICY APPROACHES TO IMPROVE OUTCOMES, REDUCE DRIVERS OF HEALTH DISPARITIES IN MCH, ADDRESS MATERNAL AND CHILD HEALTH BUREAU (MCHB) PRIORITY TOPICS, AND RESPOND TO EMERGING ISSUES FOR MCH POPULATIONS; 2) INCREASE THE NUMBER OF POLICY INITIATIVES DEVELOPED TO ADDRESS SOCIAL DETERMINATES OF HEALTH (SDOH); AND 3) INCREASE COLLABORATION WITH AWARD RECIPIENTS OF THE MATERNAL AND CHILD HEALTH POLICY INNOVATION PROGRAM AND NATIONAL ORGANIZATIONS OF STATE AND LOCAL OFFICIALS (NOSLO), AND SIMILAR HRSA-FUNDED PROGRAMS. NACCHO WILL BUILD THE CAPACITY OF LHOS TO USE POLICY APPROACHES TO REDUCE DRIVERS OF DISPARITIES, ADDRESS SDOH AND OTHER MCHB PRIORITIES, AND SUPPORT LHOS IN THEIR ROLE ENGAGING CROSS-SECTOR PARTNERS AND PEOPLE WITH LIVED EXPERIENCE (PWLE). THIS WILL BE ACCOMPLISHED BY ESTABLISHING A LEARNING COMMUNITY (LC), DEVELOPING TA PRODUCTS FOR BROAD DISSEMINATION, AND PROVIDING TARGETED TECHNICAL ASSISTANCE ON POLICY DEVELOPMENT AND IMPLEMENTATION THAT ADDRESS SDOH IN THE DOMAINS INCLUDING: HOUSING, TRANSPORTATION, FOOD AND NUTRITION SECURITY, AND OTHER MCHB PRIORITIES, SUCH AS CONGENITAL SYPHILIS, VACCINE PROMOTION AND PLACE-BASED STRATEGIES.
