BOX 765, CEDAR BLUFF, VA 24609 PROJECT DIRECTOR: REGINA SAYERS CONTACT PHONE NUMBER: 276-964-4915 EMAIL ADDRESS: RSAYERS@AASC.ORG WEBSITE ADDRESS: WWW.AASC.ORG GRANT FUNDS REQUESTED: $25,000 PROJECT DESCRIPTION APPALACHIAN AGENCY HAS COMPLETED CONSTRUCTION ON THE FALLS MILLS ADULT DAYCARE CENTER, LOCATED IN THE NORTHERN PORTION OF TAZEWELL COUNTY. THIS IS AASC’S THIRD STATE-LICENSED, MULTI-SERVICE DAYCARE CENTER PROVIDING DAILY CARE THAT INCLUDES SUPERVISION, AND PERSONAL AND SOCIAL SUPPORT. THE CENTER HOUSES A MEDICAL CLINIC, ALONG WITH AN AREA FOR PHYSICAL THERAPY AND OCCUPATIONAL THERAPY, SUPERVISED BY AN RN CENTER MANAGER. CONSTRUCTION OF THE DAY CENTER IS COMPLETE; HOWEVER, FUNDING IS NEEDED TO HELP EQUIP THE MEDICAL CLINIC AND THERAPY AREA. SKILLED STAFF CREATE INDIVIDUAL CARE PLANS FOR ADULTS 18 AND OVER WHO HAVE ALZHEIMER’S, POST-STROKE SYMPTOMS, AGE-RELATED DISABILITIES OR OTHER NEEDS REQUIRING ASSISTANCE OR SUPERVISION. THE PROGRAM PROVIDES A SAFE DAYTIME ENVIRONMENT TO KEEP PARTICIPANTS INVOLVED IN THE COMMUNITY WHILE FAMILY MEMBERS WORK OR TAKE A BREAK FROM CAREGIVING. RESIDENTS OF THE FALLS MILLS AREA AND THE SURROUNDING COMMUNITIES ARE PREDOMINANTLY OLDER, MANY HAVE MULTIPLE HEALTH ISSUES, AND THERE IS CURRENTLY NO PRIMARY HEALTH CARE PROVIDER IN THE AREA. THIS FACILITY ALSO FEATURES A COMMERCIAL KITCHEN AND WILL PREPARE HOT, NUTRITIOUS MEALS FOR PARTICIPANTS AS WELL AS TO HOMEBOUND INDIVIDUALS WHO LIVE WITHIN 5 MINUTES OR 5 MILES OF THE FACILITY. THE FACILITY HOUSES A CONGREGATE NUTRITION SITE FOR THOSE INDIVIDUALS WHO ENJOY GATHERING TOGETHER FOR A DELICIOUS AND NUTRITIOUS MEAL, ACTIVITIES, NUTRITION EDUCATION AND FELLOWSHIP. CONSTRUCTION OF THIS NEW FACILITY WAS FUNDED BY A VARIETY OF GRANTS — VIRGINIA DEPARTMENT OF HOUSING COMMUNITY DEVELOPMENT BLOCK GRANT, APPALACHIAN REGIONAL COMMISSION, VIRGINIA COALFIELD ECONOMIC DEVELOPMENT AUTHORITY, VIRGINIA TOBACCO REVITALIZATION COMMISSION, USDA RURA L DEVELOPMENT, THE COMMUNITY FOUNDATION OF THE VIRGINIAS AND OTHER LOCAL FOUNDATIONS. THE VIRGINIA DEPARTMENT OF MEDICAL ASSISTANCE SERVICES HAS APPROVED THE CENTER TO BE AN ALTERNATIVE CARE SITE FOR ALLCARE FOR SENIORS — AASC’S PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY (PACE). PACE IS A LONG-TERM CARE SOLUTION FOR INDIVIDUALS WHO NEED HELP TAKING CARE OF THEMSELVES AT HOME. THE CLINIC AT FALLS MILLS WILL HELP TO PROVIDE HEALTH CARE AND SOCIAL SERVICES FOR OLDER ADULTS WHO NEED NURSING HOME SERVICES BUT WANT TO CONTINUE LIVING IN THEIR HOMES.
