NCOA’S GOAL IS TO DRAMATICALLY EXPAND ACCESS TO FEDERAL AND STATE BENEFITS FOR LOW-INCOME OLDER ADULTS AND PERSONS WITH DISABILITIES, FOCUSING ON PROGRAMS THAT MAKE MEDICARE AFFORDABLE. NCOA WILL EXECUTE FIVE OBJECTIVES: 1) INSIGHT-DRIVEN, USER-CENTERED ENHANCEMENTS TO NCOA’S BENEFITSCHECKUP® (BCU), THE NATION’S MOST COMPREHENSIVE WEB-BASED TOOL CONNECTING INDIVIDUALS TO BENEFITS; 2) MULTI-CHANNEL OUTREACH CAMPAIGNS, STRATEGIC PARTNERSHIPS, AND DATA-DRIVEN TARGETING THROUGH THE BENEFITS PARTICIPATION MAP TO REACH POPULATIONS WITH DEMONSTRATED ENROLLMENT GAPS; 3) COMPETITIVE SUB-GRANTING TO ESTABLISH UP TO 100 BENEFITS ENROLLMENT CENTERS (BECS); 4) CREATION OF A UNIFIED CLEARINGHOUSE INTEGRATING RESOURCES FOR 12 MILLION WEBSITE USERS; AND 5) DELIVERY OF COMPREHENSIVE TRAINING AND TECHNICAL ASSISTANCE TO BENEFITS PROFESSIONALS THROUGH AN ENHANCED LEARNING MANAGEMENT SYSTEM (LMS). NCOA’S APPROACH LEVERAGES NATIONAL, DATA-DRIVEN DIGITAL OUTREACH WITH COMMUNITY-BASED BEST PRACTICES TO REACH A TARGET POPULATION OF INDIVIDUALS WITH GREATEST ECONOMIC NEED AND LIMITED ENGLISH PROFICIENCY. SIGNIFICANT PARTNERSHIPS INCLUDE URBAN INSTITUTE, AARP FOUNDATION, ALLEY, ADVANCING STATES, USAGING, AS WELL AS SHIPS, AAAS, ADRCS, AND NO WRONG DOOR SYSTEMS. ANTICIPATED OUTCOMES INCLUDE: 75% OF GRANTEES IMPLEMENTING NEW OR ENHANCED OUTREACH/ENROLLMENT STRATEGIES; 5% YEAR-OVER-YEAR INCREASES IN BENEFITS ENROLLMENT AND OUT-OF-POCKET COST SAVINGS AMONG ELIGIBLE POPULATIONS; AND ENHANCED BENEFICIARY UNDERSTANDING OF THE VALUE OF PREVENTIVE SERVICES. PRODUCTS INCLUDE AN EXPANDED MULTILINGUAL BCU, A COMPREHENSIVE CLEARINGHOUSE, TRAINING CURRICULA, AND A PIONEERING ENROLLMENT RATE STUDY. PURPOSE: THE OVERALL PURPOSE OF THE OLDER ADULT HOME MODIFICATION PROGRAM (OAHMP) IS TO ASSIST EXPERIENCED NONPROFIT ORGANIZATIONS, STATE AND LOCAL GOVERNMENTS, AND PUBLIC HOUSING AUTHORITIES IN UNDERTAKING COMPREHENSIVE PROGRAMS THAT MAKE SAFETY AND FUNCTIONAL HOME MODIFICATIONS REPAIRS AND RENOVATIONS TO MEET THE NEEDS OF LOW-INCOME ELDERLY HOMEOWNERS. THE GOAL OF THE HOME MODIFICATION PROGRAM IS TO ENABLE LOW-INCOME ELDERLY PERSONS TO REMAIN IN THEIR HOMES THROUGH LOW-COST, LOW BARRIER, HIGH IMPACT HOME MODIFICATIONS TO REDUCE OLDER ADULTS’ RISK OF FALLING, IMPROVE GENERAL SAFETY, INCREASE ACCESSIBILITY, AND TO IMPROVE THEIR FUNCTIONAL ABILITIES IN THEIR HOME. THIS WILL ENABLE OLDER ADULTS TO REMAIN IN THEIR HOMES, THAT IS, TO “AGE IN PLACE,” RATHER THAN MOVE TO NURSING HOMES OR OTHER ASSISTED CARE FACILITIES.; ACTIVITIES TO BE PERFORMED: HUD’S OFFICE OF LEAD HAZARD CONTROL AND HEALTHY HOMES IS MAKING AVAILABLE GRANT FUNDS AND TRAINING RESOURCES TO NON-FEDERAL ENTITIES. UNDER THE OAHMP AWARD, EXPERIENCED NONPROFIT ORGANIZATIONS, STATE AND LOCAL GOVERNMENTS, AND PUBLIC HOUSING AUTHORITIES WILL DELIVER HOME MODIFICATION SERVICES TO QUALIFIED BENEFICIARIES. THE OAHMP MODEL FOCUSES ON LOW-COST, HIGH-IMPACT HOME MODIFICATIONS. EXAMPLES OF THESE HOME MODIFICATIONS INCLUDE INSTALLATION OF GRAB BARS, RAILINGS, AND LEVER-HANDLED DOORKNOBS AND FAUCETS, AS WELL AS THE INSTALLATION OF ADAPTIVE EQUIPMENT, SUCH AS TEMPORARY RAMP, TUB/SHOWER TRANSFER BENCH, HANDHELD SHOWER HEAD, RAISED TOILET SEAT, RISERS FOR CHAIRS AND SOFAS, AND NON-SLIP STRIPS FOR TUB/SHOWER OR STAIRS. THE OAHMP MODEL PRIMARILY RELIES ON THE EXPERTISE OF A LICENSED OCCUPATIONAL THERAPIST (OT) TO ENSURE THAT THE HOME MODIFICATION ADDRESSES THE CLIENT’S SPECIFIC GOALS AND NEEDS AND PROMOTES THEIR FULL PARTICIPATION IN DAILY LIFE ACTIVITIES. THE OT IS TRAINED TO EVALUATE CLIENTS’ FUNCTIONAL ABILITIES AND THE HOME ENVIRONMENT AND HAS KNOWLEDGE OF THE RANGE OF LOW-COST, HIGH-IMPACT ENVIRONMENTAL MODIFICATIONS AND ADAPTIVE EQUIPMENT USED TO OPTIMIZE THE HOME ENVIRONMENT AND INCREASE INDEPENDENCE. THE GRANTEES, WHICH ARE EXPERIENCED IN PROVIDING SERVICES TO SENIORS, WILL DELIVER HOME MODIFICATION SERVICES TO MORE THAN 1,900 SENIOR FAMILIES IN BOTH URBAN COMMUNITIES AND COMMUNITIES WITH SUBSTANTIAL RURAL POPULATIONS.; EXPECTED OUTCOMES: PROVIDED THROUGH HUD’S OLDER ADULTS HOME MODIFICATION PROGRAM (OAHMP), THESE GRANTS ENABLE LOW-INCOME ELDERLY PERSONS TO REMAIN IN THEIR HOMES THROUGH LOW-COST, LOW BARRIER, HIGH IMPACT HOME MODIFICATIONS TO REDUCE OLDER ADULTS’ RISK OF FALLING, IMPROVE GENERAL SAFETY, INCREASE ACCESSIBILITY, AND IMPROVE THEIR FUNCTIONAL ABILITIES IN THEIR HOME. THE COUNCIL SUPPORTS INDEPENDENCE, SELF-CHOICE AND SECURITY FOR ALL ELDERS THROUGH A VARIETY OF PROGRAMS AND SERVICES, SOME OF WHICH ARE PROVIDED DIRECTLY THROUGH CVCOA STAFF, OTHERS PROVIDED VIA CONTRACT WITH PARTNERING ORGANIZATIONS. WE ARE APPLYING FOR 60 VOLUNTEER SERVICE YEARS (VSYS). THIS IS 11 MORE THAN THE SENIOR COMPANION PROGRAM PERFORMANCE MEASURE REQUIREMENTS CALCULATION OF 49 (BASED ON THE BASE LEVEL FUNDING AMOUNT OF $296,474), BECAUSE 49 VOLUNTEERS WOULD BE INSUFFICIENT TO MEET CLIENT NEED IN VERMONT. WE ANTICIPATE THAT THIS 60-VSY LEVEL, WHICH IS LESS THAN THE 68 VSYS FROM THE PREVIOUS GRANT PERIOD, WILL ALLOW US TO OBTAIN VOLUNTEERS WHILE EASING THE DIFFICULTY OF RECRUITMENT AND RETENTION THAT THE PROGRAM HAS PREVIOUSLY EXPERIENCED. THE GOAL OF THE NATIONAL SENIOR CORPS IS TO EMPOWER PEOPLE 55 YEARS OF AGE OR OLDER TO CONTRIBUTE TO THEIR COMMUNITIES THROUGH SERVICE, ENHANCE THE LIVES OF THOSE WHO SERVE AND THOSE WHOM THEY SERVE, AND PROVIDE COMMUNITIES WITH VALUABLE SERVICES (DOMESTIC VOLUNTEER SERVICE ACT, CHAPTER 66, SUBCHAPTER II). THE LOCAL FOSTER GRANDPARENT PROGRAM WILL PROVIDE ACCESS TO SERVICES, RESOURCES, AND STRUCTURED VOLUNTEER OPPORTUNITIES THAT WILL IMPROVE THE QUALITY OF LIFE FOR LOW-INCOME SENIORS AND ENABLE THEM TO BE ACTIVELY ENGAGED IN THEIR COMMUNITIES. IN TURN, THEY WILL CONTRIBUTE TO IMPROVED EDUCATIONAL OUTCOMES FOR AT-RISK CHILDREN AND YOUTH. THE LOCAL 57 FOSTER GRANDPARENT VSY POSITIONS WILL BE FULFILLED BY 65 TO 70 SENIORS ANNUALLY. THEY WILL FOCUS ON SCHOOL READINESS SKILLS FOR ECONOMICALLY DISADVANTAGED YOUNG CHILDREN AND ON EDUCATIONAL AND BEHAVIORAL OUTCOMES OF K-12 YOUTH WITH SPECIAL OR EXCEPTIONAL NEEDS. 100% OF VOLUNTEERS WILL BE PLACED IN OUTCOME BASED ASSIGNMENTS IN NON-PROFIT CHILD-SERVING ORGANIZATIONS. VOLUNTEERS WILL ASSIST PRESCHOOL CHILDREN AGES 0 TO 5 WITH EMOTIONAL/SOCIAL/ACADEMIC AND MOTOR SKILL DEVELOPMENT FOR SCHOOL READINESS SKILLS, WITH A STRONG FOCUS ON PHONEMIC AND PHONOLOGICAL AWARENESS AND BEGINNING READING ACTIVITIES. THIS PROJECT WILL ADDRESS THE INDEPENDENT LIVING AND RESPITE FOR CAREGIVERS SERVICE CATEGORIES. AS A REPLACEMENT GRANTEE, PIMA COUNCIL ON AGING PROPOSES TO HONOR THE FUNDAMENTAL PROGRAM MODEL THAT HAS ASSISTED SO MANY OLDER PEOPLE IN OUR COMMUNITY SINCE 2001, WHILE INCORPORATING ELEMENTS TO MAXIMIZE LIMITED RESOURCES AND FOCUS ON THE GREATEST NEEDS OF PIMA COUNTY'S OLDER ADULTS. RESEARCH SHOWS THAT RESPITE CARE REDUCES ABUSE AND NEGLECT AND IMPROVES OVERALL QUALITY OF CARE, AND EXTENDS THE TIME OLDER ADULTS CAN LIVE IN THEIR HOME OF CHOICE. THIS PROJECT IS PROPOSING TO COLLECTIVELY ENGAGE 220 VOLUNTEERS IN THE PRIMARY FOCUS AREA OF THE HEALTHY FUTURES OF OLDER VERMONTERS. THEIR SERVICE IS IN PROGRAMING THAT IMPROVES SUCCESSFUL AGING IN PLACE (H4A) OF 1095 VERMONTERS. THIS PROJECT PROPOSES -THREE WORK PLANS WHERE OUTCOMES WILL BE MEASURED: PHYSICAL ACTIVITY (H18), NUTRITION/FOOD SUPPORT (H12) AND COMPANIONSHIP (H9A) BUILDING ON LONG-TERM COMMUNITY RELATIONSHIPS, RSVP VOLUNTEERS CONTINUE TO ENGAGE THEIR COMMUNITIES THROUGH SERVICE PROGRAMS THAT SUPPORT OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES TO LIVE INDEPENDENTLY. ONE 120 RSVP VOLUNTEERS WILL ASSIST WITH ACCESS TO FOOD RESOURCES BY PARTNERING WITH 26 INDEPENDENT MEALS ON WHEELS PROGRAMS (H12) TO DELIVER NUTRITIOUS MEALS TO 640 HOMEBOUND VERMONTERS. RSVP VOLUNTEERS WILL CONTINUE TO GROW 30 PEER-LED WELLNESS PROGRAMS THAT FOCUS ON EVIDENCE-BASED PROGRAMMING AND PROVIDE LEARNING EXPERIENCES THAT IMPROVE THE HEALTH OUTCOMES OF 325 INDIVIDUALS (H18). 30 VOLUNTEERS WILL PROVIDE COMPANIONSHIP (H9A) TO 30 INDIVIDUALS WHO LIVE IN RURAL COMMUNITIES THROUGHOUT OUR SERVICE AREA.

THE CENTRAL VERMONT COUNCIL ON AGING (CVCOA) WISHES TO CONTINUE SPONSORING RSVP OF CENTRAL VERMONT AND THE NORTHEAST KINGDOM (RSVP). CVCOA HAS BEEN A SPONSOR AND SUPPORTER OF SENIOR CORPS PROJECTS SINCE 1988. THE MISSION OF CVCOA IS TO SUPPORT ELDERS AND FAMILY CAREGIVERS IN LEADING INDEPENDENT, MEANINGFUL, AND DIGNIFIED LIVES IN THEIR HOMES AND COMMUNITIES. IN ALIGNMENT WITH OUR SPONSOR'S MISSION, AND THE NEEDS OF THE COMMUNITIES WE SERVE, OUR PROPOSAL IS TO HAVE 170 VOLUNTEERS SERVING IN THE PRIMARY FOCUS AREA OF HEALTHY FUTURES, WITH AN EMPHASIS ON AGING IN PLACE (H8/H9) AND ACCESS TO CARE (H4), AND CAPACITY BUILDING (G3-3.3) BUILDING ON LONG-TERM COMMUNITY RELATIONSHIPS, RSVP VOLUNTEERS IMPROVE LIVES BY ENGAGING IN SERVICE THAT SUPPORTS OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES TO LIVE INDEPENDENTLY. VOLUNTEERS ASSIST WITH ACCESS TO FOOD RESOURCES BY PARTNERING WITH 26 INDEPENDENT MEALS ON WHEELS PROGRAMS (H8) TO DELIVER NUTRITIOUS MEALS TO HOME BOUND ELDERS. THEY PROVIDE TRANSPORTATION SERVICES IN CONJUNCTION WITH TWO TRANSPORTATION PROVIDERS (H8). RSVP VOLUNTEERS WILL CONTINUE TO GROW PEER-LED WELLNESS PROGRAMS THAT FOCUS ON EVIDENCE-BASED PROGRAMMING AND PROVIDE LEARNING EXPERIENCES THAT IMPROVE THE HEALTH OF INDIVIDUALS, AND THE COMMUNITY (H4).ALL HELP ADDRESS THE GROWING PROBLEM OF SOCIAL ISOLATION OF SENIORS. RSVP STAFF WILL CONTINUE TO FOSTER THE SUCCESS OF OUR PARTNER AGENCIES BY ENGAGING IN CAPACITY BUILDING SERVICES EMPHASIZING EFFECTIVE VOLUNTEER MANAGEMENT PRACTICES SUCH AS RECRUITING AND MARKETING TO PROSPECTIVE VOLUNTEERS; INTERVIEWING, SCREENING, AND SELECTING VOLUNTEERS. THEY WILL ALSO WORK WITH PARTNERS TO DEVELOP METHODS FOR MEASURING OUTCOMES AND EVALUATING THE PROCESS. (G3-3.3) THE GOAL IS TO STRENGTHEN OUR PARTNER AGENCIES' VOLUNTEER PROGRAMS; ALSO BY INCREASING THE POOL OF VOLUNTEERS, THE NUMBER OF MEALS DELIVERED BY EACH VOLUNTEER CAN BE REDUCED, ALLOWING FOR MORE TIME TO VISIT WITH MEAL RECIPIENTS AND INCREASING SOCIAL TIES AND SUPPORT. MACOA IS IN ITS 46TH YEAR OF OPERATION. THE MISSION OF OUR RSVP PROJECT IS TO LINK THE SKILLS, KNOWLEDGE, AND EXPERIENCE OF THE VOLUNTEER AGE 55 AND OLDER WITH IDENTIFIED, CRITICAL NEEDS IN THE COMMUNITY, AND TO PROVIDE REWARDING OPPORTUNITIES FOR SENIORS. 14% OF THE POPULATION OF OUR FIVE COUNTY PROJECT IS MADE UP OF PEOPLE OVER THE AGE OF 65 AND 17% IS FOOD INSECURE. OUR PRIMARY FOCUS AREA IS HEALTHY FUTURES WHICH ADDRESSES THESE SEGMENTS OF OUR POPULATION. AN ESTIMATED 1,064 RSVP VOLUNTEERS WILL SERVE IN ALL SIX FOCUS AREAS AT 137 STATIONS ACROSS OUR SERVICE AREA. OF THIS 1,064, 441 OR 41%, OF OUR VOLUNTEERS ARE PLACED IN OUTCOME BASED ASSIGNMENTS BEGINNING YEAR ONE. 661 VOLUNTEERS OR 62% ARE PLACED IN THE PRIMARY FOCUS AREA OF HEALTHY FUTURES. ACTIVITIES OF OUR VOLUNTEERS IN OUTCOME BASES ASSIGNMENTS INCLUDE DELIVERING HOMEBOUND MEALS, PREPARING AND PACKAGING FROZEN MEALS, PROVIDING TRANSPORTATION FOR SENIORS TO DOCTOR VISITS, THE PHARMACY, AND GROCERY STORE, AND PROVIDING COMPANIONSHIP TO THE HOMEBOUND. AT THE END OF THE THREE YEAR GRANT PERIOD WE PLAN TO HAVE 555 INDIVIDUALS WITH INCREASED SOCIAL TIES/PERCEIVED SOCIAL SUPPORT DUE TO THE EFFORTS OF THESE VOLUNTEERS. SOME OF THEIR ACTIVITIES INCLUDE DELIVERING, PREPARING, AND PACKAGING HOME BOUND MEALS, PROVIDING TRANSPORTATION FOR SENIORS TO DOCTOR VISITS, THE PHARMACY, AND GROCERY STORE, PROVIDING COMPANIONSHIP TO THE HOME BOUND, PROVIDING TAX ASSISTANCE, FREE MEDICAL CARE AT MEDICAL OUTREACH MINISTRIES, FOOD ASSISTANCE AT LOCAL PANTRIES, AND BENEFITS ASSISTANCE TO VETERANS.
