CITY/STATE: MARTINSBURG, WV COUNTIES SERVED: WEST VIRGINIA: BERKELEY, JEFFERSON, MORGAN. VIRGINIA: CLARKE, FREDERICK, SHENANDOAH, WARREN. SUMMARY: SHENANDOAH VALLEY MEDICAL SYSTEMS, INC. (SVMS) IS A FEDERALLY QUALIFIED HEALTH CENTER PROVIDING HEALTH AND WELLNESS SERVICES TO PEOPLE IN 7 COUNTIES IN NORTHWESTERN VIRGINIA AND NORTHEASTERN WEST VIRGINIA, SERVING OVER 30,364 PATIENTS IN 2023. SVMS PROVIDES A LARGE ARRAY OF HEALTH SERVICES TO ITS PRIMARILY LOW-INCOME PATIENT POPULATION, INCLUDING 60% THAT HAVE A HOUSEHOLD INCOME BELOW THE POVERTY LEVEL AND 88% THAT HAVE A HOUSEHOLD INCOME BELOW 200%. SVMS IS PROUD TO BE A SAFETY NET PROVIDER FOR THE PEOPLE IN ITS SERVICE AREA, PROVIDING FULL ADULT PRIMARY CARE, OB/GYN, PRENATAL CARE INCLUDING HOSPITAL DELIVERY, PEDIATRICS, URGENT CARE, MENTAL HEALTH COUNSELING, PSYCHIATRIC ASSESSMENT AND TREATMENT, PHARMACY, DENTAL, RYAN WHITE HIV SERVICES, WIC NUTRITION, AND MANY SUPPORT SERVICES. SVMS HAS IDENTIFIED A SIGNIFICANT NEED FOR SUBSTANCE USE DISORDER SERVICES, INCLUDING MEDICATION FOR OPIOID USE DISORDER AND MENTAL HEALTH SERVICES IN THE LOCAL POPULATION AND ITS PATIENT POPULATION. ALL COUNTIES IN THE SERVICE AREA EXPERIENCE FREQUENT MENTAL DISTRESS AT RATES UP TO NEARLY 6% HIGHER THAN THE NATIONAL RATE, AND WV HAS ONE OF THE HIGHEST DEPRESSION RATES IN THE COUNTRY (RANKING 48TH). ALTHOUGH SVMS PROVIDED 2,934 MENTAL HEALTH SERVICES IN 2023, THERE IS STILL A PROJECTED GAP OF 10,020 PATIENTS WITH LIKELY MENTAL HEALTH DISORDERS. SIMILARLY, SVMS PROJECTS A GAP IN SUBSTANCE ABUSE DISORDER SERVICES FOR 5,162 PATIENTS. THE COMMUNITIES IN THIS BEAUTIFUL PART OF AMERICA HAVE RATES OF SUBSTANCE USE AND MENTAL HEALTH PREVALENCE ABOVE NATIONAL AND STATE RATES. FOR THESE REASONS, SVMS PROPOSES TO EXPAND ITS MENTAL HEALTH AND SUBSTANCE USE DISORDER PROGRAM BY ADDING 2.0 FTE PSYCHIATRIC NURSE PRACTITIONERS, 2.0 FTE SUD THERAPIST (LCSW), 1.0 FTE PEER RECOVERY COACH, 1.0 FTE CASE MANAGER, 2.0 FTE RN, 1.0 FTE FRONT DESK SUPPORT, AND .50 FTE CUSTODIAN, AND CREATING A FULL-TIME MENTAL HEALTH/SUD/MOUD PROGRAM AND PHARMACY AT THEIR WINCHESTER, VA SITE. THE PSYCHIATRIC NURSE PRACTITIONER AND SUD THERAPIST WILL TREAT MENTAL HEALTH AND SUBSTANCE USE DISORDERS, INCLUDING PRESCRIBING MEDICATION FOR OPIOID USE DISORDERS. TRAINING WILL BE PROVIDED TO ALL STAFF IN THE USE OF EVIDENCE-BASED PRACTICES FOR TOPICS SUCH AS SUICIDAL IDEATION AND TO PRIMARY CARE PROVIDERS FOR THE TREATMENT OF MENTAL HEALTH DISORDERS. THROUGH CROSS-TRAINING OF PCPS, SVMS ANTICIPATES INCREASING THE NUMBER OF MOUD PRESCRIBERS BY FIVE. IN ADDITION, SVMS INTENDS TO IMPLEMENT STANDARDIZED SUD SCREENING AND REFERRAL TO TREATMENT, AND TO ADD A PHARMACY TO ITS SERVICE LINE AT THE WINCHESTER SITE TO SUPPORT SUD/MOUD AND INCREASE ACCESS TO PHARMACEUTICALS. TO SUPPORT PATIENT TREATMENT AND RECOVERY, A PEER RECOVERY COACH WILL PARTICIPATE IN COMMUNITY OUTREACH EVENTS TO EDUCATE ABOUT SUD/MOUD AND THE AVAILABILITY OF TREATMENT. THE SVMS CASE MANAGER WILL IMPLEMENT AN OUTREACH AND REFERRAL PROGRAM THROUGH INFORMAL PARTNERSHIPS WITH VALLEY HEALTH AND WEST VIRGINIA UNIVERSITY, INCLUDING THE DEVELOPMENT OF MATERIALS FOR PATIENT EDUCATION ON HOW TO ACCESS SERVICES AND THE AFFORDABILITY OF THE PROGRAM FOR THOSE UNDER 200% FPL. SVMS EXPECTS TO INCREASE THE NUMBER OF UNDUPLICATED PATIENTS BY 500, INCLUDING 613 MH PATIENTS AND 299 SUD PATIENTS. THIS WOULD INCLUDE 110 PATIENTS FOR MOUD. THESE PROJECTIONS ARE BASED ON HISTORICAL DATA AND 2023 UDS NUMBERS. SVMS BELIEVES THAT EXPANDING MENTAL HEALTH CARE AND SUBSTANCE USE DISORDER TREATMENT SERVICES IN THIS WAY WILL POSITIVELY IMPACT THE SERVICE AREA AND FURTHER STRENGTHEN THE INTEGRATED ENVIRONMENT IN WHICH THEY PROVIDE COMPREHENSIVE CARE FOR ALL.
