| Department of Health and Human Services | LABORATORY SYSTEMS STRENGTHENING IN ZIMBABWE - THE BIOMEDICAL RESEARCH AND TRAINING INSTITUTE (BRTI) AND ITS PARTNERS, CLINICAL AND LABORATORY STANDARDS INSTITUTE (CLSI) AND INFECTION CONTROL ASSOCIATION OF ZIMBABWE (ICAZ) IN ZIMBABWE WILL BE IMPLEMENTING A 5-YEAR PROJECT ON SUPPORTING LABORATORY SYSTEMS STRENGTHENING ACTIVITIES IN ZIMBABWE UNDER THE PRESIDENT'S EMERGENCY PLAN FOR AIDS RELIEF (PEPFAR) TO ADDRESS EXISTING GAPS IN ACCESS TO QUALITY-ASSURED DIAGNOSTIC SERVICES.THE OVERARCHING GOAL OF THE PROJECT IS TO ENSURE UNIVERSAL ACCESS TO QUALITY DIAGNOSTIC SERVICES FOR HIV VIRAL LOAD (VL)/EARLY INFANT DIAGNOSIS (EID)/TUBERCULOSIS (TB)/SEXUALLY TRANSMITTED INFECTIONS (STIS) IN A BID TO SAVE LIVES OF PEOPLE LIVING WITH HIV (PLHIV), AVERT NEW INFECTIONS AND ACHIEVE HIV EPIDEMIC CONTROL WITH SUPPORT FROM THE CENTRES FOR DISEASE CONTROL AND PREVENTION (CDC)/PEPFAR IN ZIMBABWE. THE PURPOSE OF THE PROJECT IS TO SUPPORT THE MINISTRY OF HEALTH AND CHILD CARE (MOHCC) IN EXPANDING AND IMPROVING THE QUALITY OF DIAGNOSTIC AND SURVEILLANCE LABORATORY SERVICES FOR HIV VL/EID/TB/STI AND EMERGING DISEASES OF PUBLIC HEALTH CONCERN (EDPHC). BRTI WILL IMPLEMENT A COMPREHENSIVE APPROACH TO LABORATORY SYSTEMS STRENGTHENING, INITIALLY IN THE 40 PEPFAR-SUPPORTED DISTRICTS AND EVENTUALLY IN ALL 63 NATIONAL DISTRICTS, BY LEVERAGING PREVIOUS PEPFAR INVESTMENTS, SUPPORT FROM OTHER MOHCC, FUNDING AGENCIES/PARTNERS, PUBLIC-PRIVATE PARTNERSHIPS AND MOHCC INITIATIVES FOR COUNTRY-LED SUSTAINABLE PROGRAMMING. THE 5 KEY STRATEGIES THAT WILL BE IMPLEMENTED ARE BUILDING CAPACITY AND EXPANDING SERVICES FOR KEY DIAGNOSTIC METHODS; STRENGTHENING LIMS AND ITS INTEGRATION WITH OTHER E-HEALTH SYSTEMS; IMPLEMENTING INNOVATIVE APPROACHES FOR CONTINUAL QUALITY IMPROVEMENT IN VL/EID/STI/TB/EDPHC TESTING ACCORDING TO INTERNATIONAL STANDARDS; ESTABLISHING THE CAPACITY FOR LAB-BASED DISEASE SURVEILLANCE AND EDPHC; DEVELOPING AND IMPLEMENTING NATIONAL POLICY GUIDELINES AND STANDARDS. UNDER THE WIOA ADULT PROGRAM, ELIGIBLE INDIVIDUALS CAN RECEIVE CAREER SERVICES, WHICH ARE CLASSIFIED INTO TWO CATEGORIES: BASIC AND INDIVIDUALIZED SERVICES. WHILE SOME JOB SEEKERS MAY ONLY NEED SELF-SERVICE OR OTHER BASIC CAREER SERVICES SUCH AS LABOR EXCHANGE SERVICES, OTHERS MAY NEED MORE COMPREHENSIVE SERVICES, SUCH AS INDIVIDUALIZED SERVICES, WHICH INCLUDE CAREER PLANNING, AND DEVELOPING AN INDIVIDUAL EMPLOYMENT PLAN OUTLINING NEEDS AND GOALS OF THE JOB SEEKER. PARTICIPANTS WILL ALSO RECEIVE TRAINING SERVICES LINKED TO JOB OPPORTUNITIES IN THEIR COMMUNITIES. TO PROMOTE CUSTOMER CHOICE AND INVOLVEMENT IN CAREER DECISIONS, PARTICIPANTS USE AN INDIVIDUAL TRAINING ACCOUNT TO SELECT AN APPROPRIATE TRAINING PROGRAM FROM AN ELIGIBLE TRAINING PROVIDER LIST. WIOA ALSO AUTHORIZES THE PROVISION OF SUPPORTIVE SERVICES (E.G. TRANSPORTATION AND CHILDCARE ASSISTANCE) TO ENABLE AN INDIVIDUAL TO PARTICIPATE IN THE PROGRAM.EXPECTED OUTCOMES: THE WIOA ADULT PROGRAM OUTCOMES ARE TO ENSURE THAT INDIVIDUALS CAN ACCESS AND RECEIVE THE EMPLOYMENT AND TRAINING SERVICES THEY ARE ELIGIBLE FOR, SO THAT THEY MAY BECOME EMPLOYED OR ADVANCE IN EMPLOYMENT. LABORATORY SYSTEMS STRENGTHENING IN ZIMBABWE - THE BIOMEDICAL RESEARCH AND TRAINING INSTITUTE (BRTI) AND ITS PARTNERS, CLINICAL AND LABORATORY STANDARDS INSTITUTE (CLSI) AND INFECTION CONTROL ASSOCIATION OF ZIMBABWE (ICAZ) IN ZIMBABWE WILL BE IMPLEMENTING A 5-YEAR PROJECT ON SUPPORTING LABORATORY SYSTEMS STRENGTHENING ACTIVITIES IN ZIMBABWE UNDER THE PRESIDENT'S EMERGENCY PLAN FOR AIDS RELIEF (PEPFAR) TO ADDRESS EXISTING GAPS IN ACCESS TO QUALITY-ASSURED DIAGNOSTIC SERVICES.THE OVERARCHING GOAL OF THE PROJECT IS TO ENSURE UNIVERSAL ACCESS TO QUALITY DIAGNOSTIC SERVICES FOR HIV VIRAL LOAD (VL)/EARLY INFANT DIAGNOSIS (EID)/TUBERCULOSIS (TB)/SEXUALLY TRANSMITTED INFECTIONS (STIS) IN A BID TO SAVE LIVES OF PEOPLE LIVING WITH HIV (PLHIV), AVERT NEW INFECTIONS AND ACHIEVE HIV EPIDEMIC CONTROL WITH SUPPORT FROM THE CENTRES FOR DISEASE CONTROL AND PREVENTION (CDC)/PEPFAR IN ZIMBABWE. THE PURPOSE OF THE PROJECT IS TO SUPPORT THE MINISTRY OF HEALTH AND CHILD CARE (MOHCC) IN EXPANDING AND IMPROVING THE QUALITY OF DIAGNOSTIC AND SURVEILLANCE LABORATORY SERVICES FOR HIV VL/EID/TB/STI AND EMERGING DISEASES OF PUBLIC HEALTH CONCERN (EDPHC). BRTI WILL IMPLEMENT A COMPREHENSIVE APPROACH TO LABORATORY SYSTEMS STRENGTHENING, INITIALLY IN THE 40 PEPFAR-SUPPORTED DISTRICTS AND EVENTUALLY IN ALL 63 NATIONAL DISTRICTS, BY LEVERAGING PREVIOUS PEPFAR INVESTMENTS, SUPPORT FROM OTHER MOHCC, FUNDING AGENCIES/PARTNERS, PUBLIC-PRIVATE PARTNERSHIPS AND MOHCC INITIATIVES FOR COUNTRY-LED SUSTAINABLE PROGRAMMING. THE 5 KEY STRATEGIES THAT WILL BE IMPLEMENTED ARE BUILDING CAPACITY AND EXPANDING SERVICES FOR KEY DIAGNOSTIC METHODS; STRENGTHENING LIMS AND ITS INTEGRATION WITH OTHER E-HEALTH SYSTEMS; IMPLEMENTING INNOVATIVE APPROACHES FOR CONTINUAL QUALITY IMPROVEMENT IN VL/EID/STI/TB/EDPHC TESTING ACCORDING TO INTERNATIONAL STANDARDS; ESTABLISHING THE CAPACITY FOR LAB-BASED DISEASE SURVEILLANCE AND EDPHC; DEVELOPING AND IMPLEMENTING NATIONAL POLICY GUIDELINES AND STANDARDS. AT THE END OF THE PROJECT PERIOD, THE KEY EXPECTE D OUTCOMES WILL BE UNIVERSAL ACCESS TO QUALITY, UNINTERRUPTED HIV VL/EID/TB/STI TESTING SERVICES THROUGH AN EFFICIENT LABORATORY DIAGNOSTIC NETWORK IN LINE WITH APPROPRIATE POLICIES AND LABORATORY STRATEGIC PLANS IMPLEMENTED AT ALL LEVELS OF THE LABORATORY DIAGNOSTIC NETWORK, IMPROVED LIMS RELATED DATA DRIVEN DECISION MAKING FOR PLANNING, MANAGEMENT AND SURVEILLANCE, SUSTAINABLE ORGANIZATIONAL INFRASTRUCTURE WITH A QUALIFIED, OPTIMIZED LABORATORY WORKFORCE, INCREASED CAPACITY TO DELIVER AND MANAGE QUALITY LABORATORY SERVICES AND IMPROVED SURVEILLANCE SYSTEM INCLUDING DATA DRIVEN PLANNING AND DECISION MAKING, STRENGTHENED QUALITY OF BLOOD TRANSFUSION SERVICES AS PART OF PREVENTION STRATEGIES FOR HIV, THE USE OF BLOOD TRANSFUSION FOR THERAPEUTIC PURPOSES BRINGS IN BLOOD SAFETY AT THE CORE OF LABORATORY SYSTEMS, ENSURING QUALITY SCREENING OF BLOOD AND BLOOD PRODUCTS FOR HIV AND OTHER INFECTIOUS DISEASES WILL CONTRIBUTE TO SUSTAINED HIV EPIDEMIC CONTROL AND REDUCED NEW INFECTIONS, INCREASED STANDARDIZATION ACROSS LABORATORY SERVICES FOR HIV VL/EID/TB/STIS AND OTHER EMERGING DISEASES OF PUBLIC HEALTH CONCERN THAT ALIGN WITH NATIONAL POLICIES AND GUIDELINES (STEPWISE LABORATORY IMPROVEMENT PROCESS TOWARD ACCREDITATION (SLIPTA) AND STRENGTHENING LABORATORY MANAGEMENT TOWARD ACCREDITATION (SLMTA), RAPID TEST CONTINUOUS QUALITY IMPROVEMENT (RTCQI) AND OTHER CQI ACTIVITIES)

DISSEMINATION AND SHARING OF STRATEGIC INFORMATION PROMOTE BOTH UNDERSTANDING OF THE DYNAMICS OF EPIDEMICS AND CONSENSUS ABOUT HOW BEST TO RESPOND TO HIV. ALSO, THE CONSISTENCY AND AVAILABILITY OF INFORMATION IS CENTRAL TO THE TRANSPARENCY OF DECISIONS IN THE HEALTH SECTOR, FURTHER STRENGTHENED BY ANALYSIS AND FORMAL REVIEWS OF THESE DATA INVOLVING KEY STAKEHOLDERS.THE OVERALL GOAL OF THE PROJECT IS TO ACHIEVE SUSTAINABLE HIV EPIDEMIC CONTROL BY MEANS OF STRENGTHEN NATIONAL HIV PREVENTION, CASE FINDING AND TREATMENT PROGRAM, IMPROVING UTILIZATION OF HIV STRATEGIC INFORMATION (SI), STRENGTHENING HIV EPIDEMIOLOGICAL SURVEILLANCE SYSTEMS AND IMPLEMENTING A LABORATORY QUALITY MANAGEMENT/QUALITY IMPROVEMENT (QM/QI) SYSTEM.PROJECT GOAL WILL BE ACHIEVED THROUGH: STRENGTHEN NATIONAL HIV PREVENTION, CASE FINDING AND TREATMENT THROUGH TECHNICAL ASSISTANCE TO REGIONAL AND NATIONAL CENTERS AMONG KPS; DEVELOPMENT OF NATIONAL POLICY AND REGULATION TO SUPPORT TEST AND START, MAT, LAB, PREVENTION (INCLUDING PREP); ENHANCE NATIONAL MAT FRAMEWORK AND CAPACITY; ENHANCE LABORATORY CAPACITY AND QUALITY; ENHANCE AVAILABILITY, QUALITY AND TIMELINESS OF HIV SURVEILLANCE DATA.ADDITIONALLY, PHC WILL PROVIDE TECHNICAL ASSISTANCE TO ENSURE INITIALIZATION OF INNOVATIVE RISK NETWORK RECRUITMENT AND CASE MANAGEMENT DELIVERY MODELS WHICH ARE IMPLEMENTED BY PARTNERS ACROSS THE 12 PEPFAR REGIONS. THE PHC TEAM WILL PROVIDE TA TO STRENGTHEN HIV STATISTICAL CAPABILITIES AT THE NATIONAL AND REGIONAL LEVELS. THE PHC WILL CONTINUE LEAD THE REVISION O F HIV SERVICE PACKAGES GUIDELINES, CREATE AN ENABLING POLICY ENVIRONMENT, AND FULLY TRANSITION TO THE HIV MEDICAL INFORMATION SYSTEM (MIS) ELECTRONIC DATA ENTRY SYSTEM. THE PHC WILL PROVIDE TA TO SPEARHEAD THE EXPANSION OF INDEX TESTING AND PREP IN UKRAINE, HIV TESTING AND TREATMENT PROVIDERS, AND COORDINATE IMPLEMENTING PARTNERS. THE PHC WILL CREATE A NATIONAL MAT HUB AND WILL SUPPORT THE IMPLEMENTATION OF MAT MODULE IN HIV MIS AND INTEGRATION INTO E-HEALTH SYSTEMS AND ALSO DEVELOP MAT REGULATORY FRAMEWORK NEEDED FOR SUCCESSFUL MAT EXPANSION. THE PHC IN COOPERATION WITH MINISTRY OF HEALTH (MOOH) WILL LEAD IN LABORATORY NETWORK OPTIMIZATION TO ENSURE UNINTERRUPTED ACCESS TO PROPER QUALITY LABORATORY AND HIV TESTING SERVICES FOR PLHIV AND HIV SURVEILLANCE AT CENTRAL AND REGIONAL LEVELS. THE PHC WILL CONTINUE TO IMPROVE THE COMPETENCIES OF THE LAB WORKFORCE IN MULTIPLE DISEASE TESTING.
