COMMUNITY PROJECT FUNDING/CONGRESSIONALLY DIRECTED SPENDING - CONSTRUCTION - EAU CLAIRE COOPERATIVE HEALTH CENTER, INC. (HEREIN KNOWN AS “COOPERATIVE HEALTH”), IS A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) WITH A HEALTHCARE NETWORK PROVIDING MEDICAL SERVICES IN 30 LOCATIONS THROUGHOUT THE MIDLANDS OF SOUTH CAROLINA. IN 2020, COOPERATIVE HEALTH SERVED 51,377 UNDUPLICATED MEDICALLY UNDERSERVED PATIENTS RESULTING IN OVER 171,693 CLINICAL VISITS. OF THIS TOTAL, 11,680 (22.73%) WERE UNINSURED AND 37,371 (72.73%) WERE AT OR BELOW 100% OF THE FEDERAL POVERTY LEVEL (FPL). COOPERATIVE HEALTH IS RESPONSIBLE FOR 40% OF ALL FAMILIES LIVING UNDER THE FEDERAL POVERTY LEVEL IN THREE OF ITS FOUR COUNTY SERVICE AREA. THERE IS TREMENDOUS EXPERIENCE IN THE DELIVERY OF A FULL-CONTINUUM OF PRIMARY CARE SERVICES TO INCLUDE ADULT, PEDIATRIC AND FAMILY PRACTICE, OBSTETRICS & GYNECOLOGY, DENTAL, PODIATRY, BEHAVIORAL MEDICINE, PSYCHIATRY, COUNSELING, MAT SUBSTANCE ABUSE TREATMENT, ORTHOPEDICS, AND 340B PHARMACY. COOPERATIVE HEALTH ALSO ADMINISTERS THE HEALTHCARE FOR THE HOMELESS AND MIGRANT SEASONAL AGRICULTURE WORKER PROGRAMS. COOPERATIVE HEALTH SEEKS TO EXPAND SERVICES TO ADDRESS THE PRIMARY HEALTH CARE NEEDS OF ADOLESCENT AND YOUNG ADULTS THROUGH THE ADDITION OF A CLINIC SPECIFICALLY DESIGNED TO ADDRESS THE NEEDS OF THIS PARTICULAR AGE GROUP WITHIN THE NORTH COLUMBIA COMMUNITY. THE INSTITUTION OF ADOLESCENT HEALTH SERVICES IN THE GREATER COLUMBIA AREA WILL CREATE A NEW COMMUNITY HEALTHCARE ACCESS POINT THAT AIMS TO ADDRESS THE NEEDS THAT ARE SPECIFIC TO THIS SUBGROUPS BIOLOGICAL AND PSYCHOLOGICAL DEVELOPMENT. COOPERATIVE HEALTH WILL CONSTRUCT A NEW STATE-OF-THE-ART COMMUNITY HEALTH CENTER IN THE 29203 ZIP OF COLUMBIA, SOUTH CAROLINA. THE TWO-STORY FACILITY WILL CONSIST OF APPROXIMATELY 7,500 SQUARE FEET OF CLINICAL, COUNSELING & WELLNESS, EDUCATIONAL, AND BUSINESS OFFICE SPACE. THE FACILITY WILL COMPLY WITH ACCESSIBILITY REQUIREMENTS UNDER THE AMERICANS WITH DISABILITIES ACT AND LOCAL BUILDING CODES. WE WILL OFFER PRIMARY AND PREVE NTATIVE HEALTH CARE SERVICES TO ADOLESCENT AND YOUNG ADULTS FROM THIS FACILITY. SERVICES WILL INCLUDE PRIMARY CARE, BEHAVIORAL HEALTH, OBSTETRICS & GYNECOLOGICAL SERVICES AS WELL AS WELL-CHILD/SICK CARE, IMMUNIZATIONS, HEALTH EDUCATION, SCREENINGS, FAMILY PLANNING, LIFE SKILLS TRAINING/COUNSELING, AND WIC TO LOW-INCOME TO MODERATE-INCOME YOUNG PEOPLE. THE TOTAL CONSTRUCTION COST IS $2,175,000. COOPERATIVE HEALTH IS REQUESTING $2,175,000 TO SUPPORT THE TOTAL CONSTRUCTION AND SITE WORK COSTS. PROJECT PLANNING WILL BEGIN JUNE 2022. CONSTRUCTION IS ANTICIPATED TO START IN DECEMBER OF 2022 AND COMPLETED BY NOVEMBER 2023.
