SOWEGA COA IS A SINGLE AGENCY THAT SPONSORS THESE 20 PROGRAMS THROUGHOUT THE COMMUNITY. ITS MISSION AND VALUES ARE AS FOLLOWS: VISION: LIVING LONGER, LIVING SAFELY, LIVING WELL MISSION: TO COORDINATE A SYSTEM OF SERVICES THAT PROMOTES THE WELL-BEING AND INDEPENDENCE OF OLDER AND DISABLED GEORGIANS, HELPING THEM ACHIEVE HEALTHY AND SELF-SUFFICIENT LIVES. VALUES: LISTEN TO OLDER PEOPLE AND OUR PARTNERS WHO SERVE THEM. ADVOCATE ON THE BEHALF OF OUR CONSUMERS BY ANTICIPATING THEIR NEEDS. PRODUCE MEASURABLE OUTCOMES THAT SIGNIFICANTLY IMPACT THE WELL-BEING OF OLDER PEOPLE AND THEIR FAMILY CAREGIVERS. RESPOND TO CHANGING NEEDS AND PREFERENCES OF OUR DIVERSE AND GROWING ELDERLY POPULATION. ACCOUNTABILITY AND RESULTS. AN ESTIMATED 116 VOLUNTEERS WILL SERVE. THEY WILL SERVE FRAIL AND ISOLATED SENIORS, VETERANS, PUBLIC SAFETY, THE HOMELESS, TAX FILING FOR ELDERLY, HOSPITAL AND HOSPICE NEEDS. THESE VOLUNTEERS SERVE IN 7 VOLUNTEER STATIONS INCLUDING SENIOR CENTERS, WELLNESS PROGRAMS, TRANSPORTATION AND TELEPHONE RESPONSE. THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES. AT THE END OF THE 12-MONTH PERFORMANCE PERIOD, ANTICIPATED OUTCOMES INCLUDE INCREASED PHYSICAL ACTIVITY, IMPROVED NUTRITION, ACCESS TO HEALTH CARE, INCREASED SOCIAL SUPPORT TO DISABLED AND LONELY SENIORS, SERVICES THAT INCREASE A SENIOR'S ABILITY TO REMAIN IN THEIR OWN HOMES (AGE IN PLACE) WITH IMPROVED QUALITY OF LIFE. THE CNCS FEDERAL INVESTMENT OF $45,010 WILL BE SUPPLEMENTED BY $19,654 NON-FEDERAL RESOURCES.
