CAMELLIA BLVD, FORT VALLEY, GA 31030 WEB SITE ADDRESS: HTTPS://CCTHEALTH.ORG CONGRESSIONAL DISTRICT FOR APPLICANT ORGANIZATION PROJECT: GA-008 OVERVIEW OF THE ORGANIZATION: CARECONNECT HEALTH, INC. (CCH) PROVIDES COMPREHENSIVE PRIMARY CARE, DENTAL, BEHAVIORAL HEALTH, OBSTETRICS-GYNECOLOGY, INFECTIOUS DISEASE SERVICES TO 124,293 UNDUPLICATED PATIENTS (FY 2021) AT 39 CCH SITES. DURING THE PAST TWELVE YEARS, PATIENT VISITS HAVE INCREASED 838% FROM 38,574 VISITS IN 2009 TO 361,906 VISITS IN 2021. CCH SERVES PREDOMINANTLY LOW-INCOME PATIENTS WHO ARE UNINSURED OR WHO RELY ON PUBLIC INSURANCE. THE ORGANIZATION RECOGNIZES THAT PROVIDING AFFORDABLE, ACCESSIBLE PRIMARY CARE IS VITAL FOR MAINTAINING THE HEALTH OF THE RESIDENTS THROUGHOUT ITS 26-COUNTY GEORGIA SERVICE AREA. PROPOSED PROJECT: CARECONNECT HEALTH (CCH), A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) WILL BE UTILIZING THE CDS FUNDING ($430,000) FOR CONSTRUCTION/EXPANSION OF 1,088 SQUARE FEET TO THE EXISTING FACILITY OF CARECONNECT HEALTH CENTER – FT VALLEY CONVENIENT CARE, LOCATED IN PEACH COUNTY, GEORGIA. CARECONNECT HEALTH IS A MAJOR PROVIDER FOR COMPREHENSIVE PRIMARY HEALTH CARE SERVICES THROUGH ITS PRIMARY CARE SITE – CARECONNECT CONVENIENT CARE IN FT. VALLEY, PEACH COUNTY, GEORGIA TO HELP MEET THE NEED OF MEDICALLY UNDERSERVED COMMUNITY MEMBERS. DURING LAST 12 MONTHS, CCH HEALTH CENTER HAS INCREASED THE NUMBER OF PATIENTS AND VISITS BY 35% AT ITS FT. VALLEY HEALTH CENTER, COMPARED TO PREVIOUS 12-MONTH TIME-FRAME. THE LIMITED SIZE OF THE EXISTING FACILITY, WHICH IS ONLY 1800 SQUARE FEET IN SIZE, IS DIRECTLY POSING A BARRIER TO PROVIDING SERVICES AND ACCOMMODATE MUCH NEEDED ADDITIONAL STAFF. ACCESS TO PRIMARY HEALTH CARE IS LIMITED IN PEACH COUNTY COMMU NITY. THEREFORE, CCH NEEDS TO EXPAND ITS COMPREHENSIVE PRIMARY HEALTH CARE SERVICES TO THE LOW-INCOME, MEDICALLY DISENFRANCHISED RESIDENTS BY CONSTRUCTION/EXPANSION OF ITS EXISTING FACILITY.
