HEALTH CENTER PROGRAM GRANT NUMBER: H80CS00179 MEDCURA HEALTH, INC. (MCH), A NON-PROFIT COMMUNITY HEALTH CENTER, HAS BEEN PROVIDING ACCESSIBLE, AFFORDABLE, HIGH-QUALITY, PATIENT FOCUSED HEALTH CARE TO RESIDENTS OF DEKALB, FULTON AND ROCKDALE COUNTIES, GEORGIA, INCLUDING THE CITY OF ATLANTA, SINCE 1980. IN 2023, MCH SERVED 30,100 PATIENTS WITH 61,334 MEDICAL CLINIC VISITS, 8,129 MEDICAL VIRTUAL VISITS, 3,515 DENTAL CLINIC VISITS, 765 MENTAL HEALTH VISITS, AND 2,444 MENTAL HEALTH VIRTUAL VISITS (UDS, 2023). THE PROPOSED EXPANDED HOURS FUNDING WILL ALLOW MCH TO EXPAND OPERATIONAL HOURS AT THE MAIN SITE IN STONE MOUNTAIN, GEORGIA, WHICH HAS THE HIGHEST PATIENT POPULATION OUT OF THE 14 LOCATIONS. NEED: DATA FROM THE SERVICE AREA REVEAL SYSTEMIC, WIDESPREAD HEALTH DISPARITIES AND EMPHASIZE THE NEED FOR COMPREHENSIVE PRIMARY AND PREVENTATIVE HEALTH SERVICES THAT ARE ACCESSIBLE TO THOSE WITH LOW INCOMES AND THE UNINSURED. THE SERVICE AREA POPULATION HAS GROWN BY 12.0% SINCE 2012. MORE THAN A QUARTER (28.6% OR 607,829 INDIVIDUALS) OF RESIDENTS ARE IN THE MCH TARGET POPULATION OR LIVE ON INCOMES AT OR BELOW 200% OF THE FEDERAL POVERTY GUIDELINES (FPG). THE POPULATION TO PROVIDER RATIO IN THE SERVICE AREA DEMONSTRATES A SEVERE SHORTAGE OF PRIMARY CARE AND MENTAL HEALTH PROVIDERS, PRESENTING A SIGNIFICANT BARRIER TO CARE, PARTICULARLY FOR LOW-INCOME RESIDENTS. PATIENT BARRIERS SUCH AS WORKING HOURS, INABILITY TO MISS WORK FOR LOST WAGES OR THREAT OF LOSING THEIR JOB ALTOGETHER, AND LACK OF AVAILABLE CHILDCARE CONTRIBUTE TO DIFFICULTIES IN ACCESSING CARE DURING NORMAL BUSINESS HOURS. THIS CAN RESULT IN COSTLY OUTCOMES SUCH AS DELAYING NEEDED CARE OR SEEKING NON-URGENT CARE IN THE LOCAL EMERGENCY DEPARTMENT. MCH PROPOSES TO INCREASE ACCESS TO CARE FOR EXISTING PATIENTS AND VULNERABLE SERVICE AREA RESIDENTS THROUGH EXPANDED HOURS, THEREBY REDUCING PREVENTABLE ED VISITS. SERVICES: MCH IS REQUESTING FUNDING TO SUPPORT EXPANDING OPERATIONAL HOURS AT THE MAIN SITE IN STONE MOUNTAIN, GA. THE SITE WILL OFFER FACE-TO-FACE GENERAL PRIMARY MEDICAL CARE AND MENTAL HEALTH SERVICES, AS WELL AS TELE-HEALTH. IN SUPPORT OF THESE SERVICES, MCH IS PROPOSING TO HIRE .55 FTE FAMILY MEDICINE PHYSICIAN, 1.1 FTE OTHER MEDICAL PERSONNEL, 1.4 FTE PATIENT SUPPORT STAFF AND .50 FTE LICENSED CLINICAL SOCIAL WORKER. BY INCREASING AVAILABILITY OF NON-TRADITIONAL HOURS, MCH WILL ADDRESS THE GAP IN HEALTHCARE ACCESS THAT OFTEN RESULTS IN INAPPROPRIATE ED USE OR UNTREATED HEALTH CONDITIONS. TARGET POPULATION: MCH IS AVAILABLE TO SERVE THE GENERAL COMMUNITY, BUT PRIORITIZES SERVING LOW-INCOME INDIVIDUALS IN THE SERVICE AREA, INCLUDING MEDICAID RECIPIENTS AND THE UNDERINSURED/UNINSURED. ALL PRESENTING PATIENTS WILL RECEIVE HIGH-QUALITY SERVICES, REGARDLESS OF ABILITY TO PAY, AND MCH WILL WORK TO REDUCE BARRIERS TO CARE WITH AN ARRAY ENABLING AND FINANCIAL SERVICES INCLUDING A SLIDING FEE DISCOUNT SCHEDULE. EXPANDED HOURS FUNDING WILL FOSTER A MORE ACCESSIBLE AND PATIENT-CENTERED SERVICE DELIVERY SYSTEM, WHILE REDUCING DISPARITIES WITHIN THE COMMUNITY.
