COVE BEHAVIORAL HEALTH CERTIFIED COMMUNITY BEHAVIORAL HEALTH CLINIC IMPROVEMENT AND ADVANCEMENT - COVE BEHAVIORAL HEALTH, INC. (COVE) IS SEEKING A CERTIFIED COMMUNITY BEHAVIORAL HEALTH CLINIC (CCBHC) IMPROVEMENT AND ENHANCEMENT GRANT. THIS GRANT WILL STRATEGICALLY ENHANCE COVE’S EXISTING ACHIEVEMENTS AND PARTNERSHIPS EXPERIENCED THROUGH THE ORIGINAL CCBHC AWARD TO INCREASE ACCESS TO AND IMPROVE THE QUALITY OF COMMUNITY BEHAVIORAL HEALTH TREATMENT. WE INTEND TO BUILD ON OUR EMERGING SUCCESS IN IDENTIFYING AND SERVING OUR POPULATION OF FOCUS – MINORITY INDIVIDUALS WITH CO-OCCURRING MENTAL HEALTH AND SUBSTANCE USE DISORDERS WITH SPECIFIC FOCUS ON THOSE WITH OPIOID USE DISORDERS, AS WELL AS ADOLESCENTS, YOUNG ADULTS, PREGNANT WOMEN, AND VETERANS WITH EMERGING MENTAL HEALTH AND SUBSTANCE USE ISSUES IN HILLSBOROUGH COUNTY, FLORIDA. OVER THE FOUR-YEAR FUNDING PERIOD, WE WILL IDENTIFY 4,000 PARTICIPANTS, SCREEN 3,000 PATIENTS FOR BEHAVIORAL HEALTH ISSUES USING OUR PATIENT PATHWAYS AND DIGITAL DOORWAY ESTABLISHED DURING OUR INITIAL CCBHC IMPLEMENTATION AND ENGAGE 2,250 PATIENTS FOR INDICATED TREATMENT AND CARE COORDINATION SERVICES. COVE HAS SERVED RESIDENTS OF HILLSBOROUGH COUNTY SINCE 1973 AND HAS ESTABLISHED STRONG COMMUNITY PARTNERSHIPS WITH OTHER PROVIDERS, SOCIAL SERVICE ORGANIZATIONS, SCHOOLS, FAITH-BASED ORGANIZATIONS, AND BUSINESSES. THROUGH THE YEARS OF COLLABORATION WITH OUR STAKEHOLDERS COVE HAS DEMONSTRATED THE ABILITY TO ENGAGE AND MAINTAIN PARTNERSHIPS AND HAS ACHIEVED A REPUTATION FOR EXCELLENCE IN THE FIELD OF BEHAVIORAL HEALTH. COVE LOOKS FORWARD TO USING THIS GRANT OPPORTUNITY TO IMPROVE AND ADVANCE OUR CCBHC.

COVE BEHAVIORAL HEALTH FAMILY TREATMENT SERVICES - COVE BEHAVIORAL HEALTH IN TAMPA FLORIDA WILL UTILIZE ITS CURRENT RESIDENTIAL AND OUTPATIENT SUBSTANCE ABUSE TREATMENT, PREVENTION, AND RECOVERY SERVICES THROUGH THE PROPOSED FAMILY TREATMENT SERVICES TO 90 PREGNANT AND POSTPARTUM WOMEN AND 90 CHILDREN. THE UNIQUE DUPLEX SETTING ON-SITE WITH 8 BEDROOMS IN 2 DUPLEXES IS GROUNDWORK FOR COVE’S SUCCESSFUL TREATMENT PROGRAM OUTSIDE OF THE IN-HOUSE RESIDENTIAL COMMUNITY. GROUPING WOMEN WITH LIKE STRUGGLES AND THEIR CHILDREN WILL ENHANCE THE RECOVERY ENVIRONMENT, CAMARADERIE AND VERY MUCH SIMULATE LIVING IN AN APARTMENT AFTER DISCHARGE. THE PROGRAM WILL ALSO PROVIDE TRAUMA-INFORMED SERVICES TO STABILIZE, STRENGTHEN, PRESERVE, AND REUNITE FAMILIES FOR THE WOMEN, MINOR CHILDREN, FATHERS OF THE CHILDREN, PARTNERS OF THE WOMEN, AND EXTENDED FAMILY MEMBERS. THROUGH THE USE OF EVIDENCED-BASED STRATEGIES COVE WILL ACCOMPLISH ITS GOALS OF (1) WORK WITH COMMUNITY PARTNERS TO IDENTIFY AND ENGAGE HIGH RISK PREGNANT AND POSTPARTUM WOMEN IN NEED OF BEHAVIORAL HEALTH TREATMENT SERVICES; (2) PROVIDE SCREENING AND ASSESSMENT FOR WOMEN WITH SUBSTANCE USE DISORDERS, ALCOHOL ABUSE, FETAL ALCOHOL SPECTRUM DISORDERS AND CO-OCCURRING MENTAL HEALTH DISORDERS, AGE 18 AND OVER, WHO ARE PREGNANT OR POSTPARTUM; (3) PROVIDE COMPREHENSIVE SUD TREATMENT SERVICES TO PREGNANT AND POSTPARTUM WOMEN AT A RESIDENTIAL LEVEL OF CARE, BASED ON INDIVIDUALIZED TREATMENT PLANS; AND (4) IMPROVE PARENTING SKILLS, ECONOMIC STABILITY AND FAMILY FUNCTIONING FOR ENROLLED FAMILIES. COVE WILL MEET THESE GOALS AND REACH PROPOSED GOALS BY (A) DECREASING THE USE AND/OR ABUSE OF PRESCRIPTION, ALCOHOL, TOBACCO, ILLICIT AND OTHER HARMFUL DRUGS AMONG PREGNANT AND POSTPARTUM WOMEN; (B) INCREASING SAFE AND HEALTHY PREGNANCIES; (C) IMPROVING BIRTH OUTCOMES; (D) REDUCING RELATED EFFECTS OF MATERNAL AND /OR PATERNAL DRUG USE ON INFANTS AND CHILDREN; (E) IMPROVING THE MENTAL AND PHYSICAL HEALTH OF THE WOMEN AND CHILDREN; (F) PREVENTING MENTAL, EMOTIONAL, AND BEHAVIORAL DISORDERS AMONG THE CHILDREN; (G) IMPROVING PARENTING SKILLS, FAMILY FUNCTIONING, ECONOMIC STABILITY, AND QUALITY OF LIFE FOR FAMILIES; (H) LINKING WOMEN TO ELIGIBLE BENEFITS, HOUSING, EMPLOYMENT, TRANSPORTATION, AND OTHER COMMUNITY SOCIAL SERVICES. COVE WILL PROVIDE AND/OR COORDINATE TREATMENT SERVICES TO PREGNANT AND POSTPARTUM WOMEN AND THEIR FAMILIES INCLUDING MINOR CHILDREN, AS WELL AS AN ARRAY OF SERVICES FOR NON-RESIDENTIAL FAMILY MEMBERS OF BOTH THE WOMEN AND CHILDREN DELIVERED ON-SITE AT COVE’S MAIN CAMPUS OR IN-HOME. OTHER COMPONENTS WILL INCLUDE FOLLOW-UP CARE AFTER FAMILIES LEAVE THE RESIDENTIAL SETTING BY CONTINUING THE INDIVIDUAL AND FAMILY WELLNESS AND RECOVERY PLANS IN THE HOME AND ADDING PARENTING AND WORKSHOPS TO ENHANCE FAMILY FUNCTIONING AND MITIGATE RE-LAPSE. COVE WILL INCLUDE CULTURALLY COMPETENT, TRAUMA-INFORMED SERVICES DESIGNED TO MEET THE SPECIFIC NEEDS OF PPW AND THEIR CHILDREN. THE APPLICATION BUILDS ON THE STRONG 50-YEAR SERVICE RECORD OF COVE BEHAVIORAL HEALTH. COVE HAS SUCCESSFULLY RECEIVED AND IMPLEMENTED SUBSTANCE ABUSE PREVENTION, TREATMENT, AND RECOVERY GRANTS FROM LOCAL, STATE, AND FEDERAL SOURCES, INCLUDING PREVIOUS PREGNANT AND POSTPARTUM WOMEN PROGRAM. IN ADDITION, COVE WAS AWARDED THE SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES ADMINISTRATION (SAMHSA) 2013 SCIENCE AND SERVICE AWARD AND THE 2013 IAWARD FOR INNOVATION IN BEHAVIORAL HEALTH SERVICES NATIONAL AWARDS FOR COMPREHENSIVE COORDINATED CARE FOR OPIATE-ADDICTED PREGNANT WOMEN.

CHIP WILL STRATEGICALLY BUILD ON THE AGENCY’S EXISTING STRENGTHS AND COMBINE THE EXPERTISE OF COMMUNITY PARTNERS TO PROVIDE SUBSTANCE ABUSE AND HIV PREVENTION SERVICES TO RACIAL/ETHNIC MINORITY MALES AT RISK FOR HIV/AIDS IN HILLSBOROUGH COUNTY FLORIDA, TO REDUCE INFECTIONS AND BUILD PROTECTIVE FACTORS. CHIP WILL USE COMMUNITY OUTREACH AND A NAVIGATION APPROACH TO PROVIDE TO BRING PREVENTION SERVICES DIRECTLY TO COMMUNITIES USING COVE’S MOBILE UNIT.
