OPIOID OVERDOSES (OODS) ARE TREATABLE WITH BASIC CARDIOPULMONARY RESUSCITATION (CPR), BUT THEY CLAIMED OVER 250 LIVES EACH DAY IN THE US IN 2020. PATIENTS MOST AT RISK FOR OOD ARE PEOPLE IN TREATMENT FOR OPIOID USE DISORDER (OUD) OR HIGH IMPACT CHRONIC PAIN (HICP). THE CURRENT STANDARD OF CARE TO OOD IS BYSTANDER RESPONSE (MEDICAL OR NONMEDICAL), BUT THE BYSTANDER NEEDS TO KNOW THAT AN OOD IS OCCURRING. WE PRESENT A WEARABLE MEDICAL DIAGNOSTIC DEVICE TO DETECT OOD AND BRIDGE THE GAP TO BYSTANDER INTERVENTION. OUR DEVICE, PARACHUTE ARMBAND, WILL BE WORN CONTINUOUSLY, AND WILL MONITOR PHYSIOLOGIC PARAMETERS THAT INDICATE OOD, THEN WILL USE AN ON-DEVICE ESCALATING TONAL ALARM TO SUMMON BYSTANDERS, AS WELL AS ON- DEVICE SPEAKER TO VERBALLY IDENTIFY THE URGENCY TO GET MEDICAL HELP. NO WIRELESS CONNECTIVITY IS REQUIRED FOR THESE FUNCTIONS. THE ARMBAND WILL ALSO SEND A BLUETOOTH ALERT TO AN ADJUNCT MOBILE MEDICAL APPLICATION (MMA) ON THE USER’S PHONE, THAT CAN REACH DESIGNATED CONTACTS, PREVIOUSLY CHOSEN BY THE DEVICE USER. THIS FUNCTION REQUIRES MOBILE DEVICE CONNECTIVITY, AND THAT THE MMA BE DOWNLOADED PRIOR TO THE MEDICAL EVENT. COMPANY HAS COMPLETED A PHASE I SBIR, SPONSORED BY NATIONAL INSTITUTE OF DRUG ABUSE (NIDA), THAT SHOWED THE UPPER ARM IS A VIABLE LOCATION FOR WEARABLE OXIMETERS, MAIN COMPONENTS OF NOVEL DEVICE. THE ADDITION OF PHASE I INNOVATION CORPS ENABLED COMPANY TO VALIDATE MINIMAL VIABLE PRODUCT (MVP) KEY FEATURES, INCLUDING ESCALATING ALARM AND WIRELESS ALERT. TOGETHER OUR PHASE I LEARNINGS ALLOWED PRINCIPLE INVESTIGATOR (PI) TO SUBMIT A BREAKTHROUGH DEVICE DESIGNATION (BDD) REQUEST TO THE FDA. THIS PROPOSED PHASE II APPLICATION WILL SHOW FEASIBILITY OF OUR NOVEL DEVICE COMPONENTS TO DETECT GAS- INDUCED HYPOXIA, COMPARED TO ARTERIAL BLOOD GAS (AIM 1); SENSITIVITY OF DEVICE TO DETECT INTRAVENOUS REMIFENTANIL “OOD” AND BEGIN ALARM ALGORITHM (AIM 2); AND ABILITY OF DEVICE COMMUNICATIONS TO SEND A REAL-TIME WIRELESS ALERT (AIM 3). CAPACITY: BUILD LOCAL CAPACITY OF TRAINED AND CERTIFIED INDIVIDUALS AND FIRMS TO ADDRESS LEAD HAZARDS SAFELY AND EFFECTIVELY DURING LEAD HAZARD CONTROL, RENOVATION, REMODELING, AND MAINTENANCE ACTIVITIES. A NALOXONE PARACHUTE: NOVEL WEARABLE DUAL-SENSOR REGIONAL OXIMETER TO DETECT OPIOID-INDUCED HYPOXIA AND ENABLE EMERGENT NALOXONE RESCUE - SBIR PHASE II - ABSTRACT AYUDA MEDICAL HAS THE VISION THAT NO-ONE SHOULD DIE FROM A TREATABLE MEDICAL EMERGENCY. OPIOID OVERDOSES (OODS) ARE TREATABLE WITH BASIC CARDIOPULMONARY RESUSCITATION (CPR), BUT THEY CLAIMED OVER 250 LIVES EACH DAY IN THE US IN 2020. PATIENTS MOST AT RISK FOR OOD ARE PEOPLE IN TREATMENT FOR OPIOID USE DISORDER (OUD) OR HIGH IMPACT CHRONIC PAIN (HICP). THE CURRENT STANDARD OF CARE TO OOD IS BYSTANDER RESPONSE (MEDICAL OR NONMEDICAL), BUT THE BYSTANDER NEEDS TO KNOW THAT AN OOD IS OCCURRING. WE PRESENT A WEARABLE MEDICAL DIAGNOSTIC DEVICE TO DETECT OOD AND BRIDGE THE GAP TO BYSTANDER INTERVENTION. OUR DEVICE, PARACHUTE ARMBAND, WILL BE WORN CONTINUOUSLY, AND WILL MONITOR PHYSIOLOGIC PARAMETERS THAT INDICATE OOD, THEN WILL USE AN ON-DEVICE ESCALATING TONAL ALARM TO SUMMON BYSTANDERS, AS WELL AS ON- DEVICE SPEAKER TO VERBALLY IDENTIFY THE URGENCY TO GET MEDICAL HELP. NO WIRELESS CONNECTIVITY IS REQUIRED FOR THESE FUNCTIONS. THE ARMBAND WILL ALSO SEND A BLUETOOTH ALERT TO AN ADJUNCT MOBILE MEDICAL APPLICATION (MMA) ON THE USER’S PHONE, THAT CAN REACH DESIGNATED CONTACTS, PREVIOUSLY CHOSEN BY THE DEVICE USER. THIS FUNCTION REQUIRES MOBILE DEVICE CONNECTIVITY, AND THAT THE MMA BE DOWNLOADED PRIOR TO THE MEDICAL EVENT. COMPANY HAS COMPLETED A PHASE I SBIR, SPONSORED BY NATIONAL INSTITUTE OF DRUG ABUSE (NIDA), THAT SHOWED THE UPPER ARM IS A VIABLE LOCATION FOR WEARABLE OXIMETERS, MAIN COMPONENTS OF NOVEL DEVICE. THE ADDITION OF PHASE I INNOVATION CORPS ENABLED COMPANY TO VALIDATE MINIMAL VIABLE PRODUCT (MVP) KEY FEATURES, INCLUDING ESCALATING ALARM AND WIRELESS ALERT. TOGETHER OUR PHASE I LEARNINGS ALLOWED PRINCIPLE INVESTIGATOR (PI) TO SUBMIT A BREAKTHROUGH DEVICE DESIGNATION (BDD) REQUEST TO THE FDA. THIS PROPOSED PHASE II APPLICATION WILL SHOW FEASIBILITY OF OUR NOVEL DEVICE COMPONENTS TO DETECT GAS- INDUCED HYPOXIA, COMPARED TO ARTERIAL BLOOD GAS (AIM 1); SENSITIVITY OF DEVICE TO DETECT INTRAVENOUS REMIFENTANIL “OOD” AND BEGIN ALARM ALGORITHM (AIM 2); AND ABILITY OF DEVICE COMMUNICATIONS TO SEND A REAL-TIME WIRELESS ALERT (AIM 3). THESE THREE AIMS SUPPORT DEVICE SAFETY AND EFFECTIVENESS. CAPACITY: BUILD LOCAL CAPACITY OF TRAINED AND CERTIFIED INDIVIDUALS AND FIRMS TO ADDRESS LEAD HAZARDS SAFELY AND EFFECTIVELY DURING LEAD HAZARD CONTROL, RENOVATION, REMODELING, AND MAINTENANCE ACTIVITIES. THIS PROJECT WILL FOCUS HEAVILY ON MONTGOMERY COUNTY AND PRINCE GEORGE’S COUNTY, WHICH ARE THE MOST POPULOUS COUNTIES IN MARYLAND WITH NEARLY TWO MILLION RESIDENTS IN TOTAL AND HAVE THE LARGEST PERCENTAGE OF IMMIGRANTS. SERVICES WILL ALSO BE AVAILABLE TO TRAFFICKING VICTIMS THROUGHOUT THE ENTIRE STATE OF MARYLAND, WHICH INCLUDES RURAL AREAS WITH LITTLE ACCESS TO CULTURALLY SPECIFIC SERVICES FOR TRAFFICKING VICTIMS. FUNDING WILL ENABLE AYUDA TO CONTINUE PROVIDING CULTURALLY AND LINGUISTICALLY SPECIFIC LEGAL AND SOCIAL SERVICES TO CURRENT AND NEW CLIENTS WHO ARE TRAFFICKING VICTIMS, REGARDLESS OF GENDER, SEXUAL ORIENTATION, RACE, COUNTRY-OF-ORIGIN, OR TYPE OF TRAFFICKING EXPERIENCED. ALL SERVICES WILL BE TRAUMA-INFORMED, VICTIM-CENTERED, SURVIVOR-INFORMED, CULTURALLY COMPETENT, VOLUNTARY, AND PROVIDED AT NO COST TO CLIENTS. THROUGH THIS PROJECT, VICTIMS WILL HAVE ACCESS TO LEGAL AND SOCIAL SERVICES PROFESSIONALS WITH SPECIALIZED EXPERTISE IN THE NEEDS AND CHALLENGES THAT IMMIGRANT VICTIMS OF HUMAN TRAFFICKING FACE WHEN RECOVERING FROM THE TRAUMA OF TRAFFICKING. THE PROGRAM IS INTENDED TO PROVIDE ASSISTANCE TO VICTIMS FOR WHOM EMERGENCY SHELTER SERVICES OR OTHER CRISIS INTERVENTION SERVICES ARE UNAVAILABLE OR INSUFFICIENT. THE PROGRAM SUPPORTS VICTIM-CENTERED APPROACHES TO PROVIDING TRANSITIONAL HOUSING, SHORT-TERM HOUSING ASSISTANCE, AND VOLUNTARY SUPPORT SERVICES TO ASSIST WITH LOCATING AND SECURING PERMANENT HOUSING, OBTAINING EMPLOYMENT, AND INTEGRATING INTO A COMMUNITY. AYUDA, A NON-PROFIT ORGANIZATION LOCATED IN WASHINGTON, D.C., WILL PROVIDE VICTIM-CENTERED APPROACHES TO TRANSITIONAL HOUSING NEEDS, IN COLLABORATION WITH THE NEXT STEP PUBLIC CHARTER SCHOOL. THE PROJECT WILL PROVIDE 30 PRIVATE LANDLORD, SCATTERED SITE TRANSITIONAL HOUSING RESIDENCES FOR 30 SURVIVORS AND THEIR FAMILIES IN URBAN AND SUBURBAN REGIONS OF WASHINGTON, D.C. THIS NEW PROJECT WILL USE FUNDS TO PROVIDE BOTH HOUSING AND SUPPORTIVE SERVICES TO MOVE SURVIVORS OF DOMESTIC VIOLENCE, DATING VIOLENCE, SEXUAL ASSAULT, OR STALKING WHO ARE HOMELESS OR IN NEED OF HOUSING ASSISTANCE TO PERMANENT HOUSING. THE PROJECT WILL ASSIST CLIENTS FOR AN ANTICIPATED MINIMUM OF 6 MONTHS AND A MAXIMUM LENGTH OF 24 MONTHS. SERVICES OFFERED WILL FOCUS ON PRIMARILY SERVING UNDERSERVED POPULATIONS IN THE REGION AND WILL BE TAILORED TO ADDRESS THE NEEDS OF VICTIMS WHO ARE SERVED. THE RANGE OF OPTIONAL SUPPORT SERVICES INCLUDES RENTAL AND UTILITY ASSISTANCE, CASE MANAGEMENT, SAFETY PLANNING, CHILDCARE, TRANSPORTATION, CAREER COUNSELING, FINANCIAL AND CREDIT COUNSELING, SUPPORT GROUPS, INDIVIDUAL COUNSELING, JOB TRAINING, EDUCATION ATTAINMENT, AND HOUSING ADVOCACY. INTERVENTION AND RELATED ASSISTANCE INCLUDE ADVOCACY, ACCOMPANIMENT (E.G., ACCOMPANYING VICTIMS TO COURT, MEDICAL FACILITIES, POLICE DEPARTMENTS, ETC.), CRISIS INTERVENTION AND SUPPORT SERVICES, AND REFERRALS, AMONG OTHER SERVICES. UNDER THIS PROGRAM, SUCH SERVICES MAY BE PROVIDED TO ADULT, YOUTH, AND CHILD VICTIMS OF SEXUAL ASSAULT, FAMILY AND HOUSEHOLD MEMBERS OF SUCH VICTIMS, AND THOSE COLLATERALLY AFFECTED BY THE VICTIMIZATION. SURVIVORS OF SEXUAL ASSAULT FROM CULTURALLY SPECIFIC COMMUNITIES FREQUENTLY CONFRONT UNIQUE CHALLENGES WHEN SEEKING ASSISTANCE, SUCH AS LINGUISTIC AND CULTURAL BARRIERS. CULTURALLY SPECIFIC COMMUNITY-BASED ORGANIZATIONS ARE MORE LIKELY TO UNDERSTAND THESE CHALLENGES BECAUSE THEY ARE FAMILIAR WITH THE CULTURE, LANGUAGE, AND BACKGROUND OF VICTIMS FROM THEIR COMMUNITIES, WHO IN TURN ARE MORE INCLINED TO SEEK SERVICES FROM SUCH ORGANIZATIONS.
