REQUESTED FUNDING FOR FY22: $705,000 AGENCY: HEALTH RESOURCES AND SERVICES ADMINISTRATION (HRSA) ACCOUNT: HEALTH FACILITIES CONSTRUCTION AND EQUIPMENT PROJECT DESCRIPTION: UF HEALTH JACKSONVILLE HAS A LONG-STANDING COMMITMENT TO CARING FOR UNDERSERVED PATIENTS WHO MAY OTHERWISE NOT GET THE CARE THEY NEED. THAT COMMITMENT IS THE CATALYST FOR THE DEVELOPMENT OF CREATIVE MODELS OF CARE WHERE THEY ARE TAKING BOLD STEPS TO PROVIDE ONE-STOP HEALTHCARE RESOURCES FOR THEIR MOST VULNERABLE PATIENTS AT THEIR UF HEALTH TOTAL CARE CLINIC – JACKSONVILLE WHICH ADDRESSES THE NEEDS OF PATIENTS WHOSE CARE IS RENDERED THROUGH THE CITY CONTRACT PROGRAM. THE CONTRACT WITH THE CITY OF JACKSONVILLE PROVIDES HEALTH CARE FOR A SUBSET OF UNINSURED INDIVIDUALS WHO MET SPECIFIC CRITERIA, SUCH AS RESIDENCY IN DUVAL COUNTY, FINANCIAL ASSISTANCE REQUIREMENTS (I.E., THE FAMILY UNIT MAY NOT EXCEED 200% OF THE FEDERAL POVERTY GUIDELINES), A LACK OF HEALTH INSURANCE AND INELIGIBILITY FOR MEDICAID. ABOUT 7,000 TO 8,000 INDIVIDUALS QUALIFY AT ANY GIVEN TIME WHICH INCLUDES THE CITY AND COUNTY PRISON POPULATION. UF HEALTH JACKSONVILLE RECEIVES $30.27 MILLION IN FUNDING ANNUALLY FROM THE CITY OF JACKSONVILLE, AND EXPERIENCES A COST FOR CARE OF $53.5 MILLION. A CRITICAL ISSUE IS THE RELOCATION OF THE FINANCIAL EVALUATION DEPARTMENT (FED) WHERE PATIENT’S ELIGIBILITY FOR SERVICES IS DETERMINED. WITH OVER 20,000 PATIENTS EVALUATED YEARLY, FED HAS NOT ONLY REACHED ITS PHYSICAL LIMITATION BUT LACKS THE ABILITY TO EFFICIENTLY CONNECT WITH THE ONE-STOP CARE MODEL SINCE IT IS CURRENTLY LOCATED IN AN OUTDATED BUILDING AT THE EDGE OF THE CAMPUS. REQUESTED FUNDING OF $705,000 WOULD BE FOR RENOVATIONS TO MOVE THE FINANCIAL EVALUATION DEPARTMENT TO A NEW SPACE IN THE PROFESSIONAL OFFICE BUILDING (POB), WHERE THE TOTAL CARE CLINIC AND OTHER NECESSARY SERVICES FOR OUR PATIENTS ARE LOCATED. UF HEALTH JACKSONVILLE HAS A LONG-STANDING COMMITMENT TO CARING FOR UNDERSERVED PATIENTS WHO MAY OTHERWISE NOT GET THE CARE THEY NEED. PATIENTS SEEN AT THE TOTAL CARE CLINIC HAVE A PANEL OF DOCTORS, NURSES AND OTHER PROVIDERS DEDICATED TO MAINTAINING AND IMPROVING THEIR HEALTH, AS WELL AS, ASSISTING THEM WITH SPECIALTY CARE. IN COLLABORATION WITH COMMUNITY-BASED AGENCIES, UF HEALTH TOTAL CARE CLINIC HELPS PATIENTS MEET THEIR SOCIAL NEEDS INCLUDING ACCESS TO OPPORTUNITIES FOR EMPLOYMENT, FOOD, AND HOUSING. CONSTRUCTION ACTIVITIES TO RELOCATE THE FINANCIAL EVALUATION DEPARTMENT (FED) WILL INCLUDE THE COMPLETE DEMOLITION OF AN EXISTING VACANT OFFICE SUITE LOCATED ON THE FIRST FLOOR OF THE PROFESSIONAL OFFICE BUILDING (POB), AND DESIGN/CONSTRUCTION OF A NEW SUITE THAT MEETS THE REQUIREMENTS OF THE FINANCIAL EVALUATION DEPARTMENT (FED), INCLUDING FURNITURE, FIXTURES & EQUIPMENT (FF&E).
