TARGET POPULATION: THE DELTA NETWORK’S POOL OF CERTIFIED DOULAS WILL PROVIDE IN-HOME PERINATAL CARE TO PREGNANT MOTHERS IN ALL 18 TN RURAL DELTA COUNTIES, WITH EM PHASIS ON LOW-INCOME FAMILIES, ESPECIALLY THOSE RELIANT ON MEDICAID. NETWORK PARTNERSHIPS: THE FOUNDING MEMBERS OF THE DELTA NETWORK INCLUDE RHA, 4 RURAL HOSPITALS, AN RHC, AND A BHP. PROJECT ACTIVITIES: OUR STRATEGIES FOR ACHIEVING THESE OBJECTIVES INVOLVE RECRUITING AND ONBOARDING 30 RURAL DELTA COUNTY RESIDENTS TO BECOME CERTIFIED LABOR DOULAS, CONDUCTING INTENSE PROVIDER EDUCATION AND OUTREACH, AND PROVIDING 280-300 DOULA SERVICES SCHOLARSHIPS, INCLUSIVE OF TELEHEALTH SERVICES. EXPECTED OUTCOMES: RHA WILL ESTABLISH A NETWORK OF 30 CERTIFIED DOULAS, PROVIDER PARTNERS IN EACH COMMUNITY, A DOULA TRAINING CURRICULUM SPECIFIC TO THE NEEDS OF RURAL TN, AND A SUSTAINED NETWORK THAT MATCHES DOULAS WITH EXPECTING MOTHERS. EXPERIENCE: RHA HAS A 28-YEAR HISTORY OF PROVIDING RURAL PROFESSIONALS WITH EDUCATIONAL OPPORTUNITIES, RURAL RELEVANT COMMUNICATIONS, AND FACILITATING CROSS-SECTOR COLLABORATIONS. RHA’S 600+ MEMBERS ARE COMMITTED TO IMPROVING THE HEALTH OF RURAL TENNESSEANS. THROUGH A 2022 HRSA RHND PLANNING GRANT, RHA CONDUCTED SITE VISITS, FOCUS GROUPS, AND SURVEYS TO ASSESS THE NEEDS AND DEVELOP A NETWORK FOR RHCS. FUNDING PREFERENCE: RHA REQUESTS A FUNDING PREFERENCE BASED ON QUALIFICATIONS 1 & 2. DECATUR COUNTY IS IN A DESIGNATED HPSA & MUA. TENNESSEE HAS NEITHER A STAND-ALONE RHC ASSOCIATION NOR STRONG REPRESENTATION IN THEIR RURAL HEALTH ASSOCIATION; OUR 241 RHCS THUS LACK A HISTORY OF COLLABORATING, SHARING RESOURCES, OR A CENTRALIZED LOCATION TO RECEIVE INFORMATION, RAISING CONCERN ABOUT QUALITY AND ACCESS TO CARE. THE PURPOSE OF THE TN-RHC NETWORK IS TO STRENGTHEN THE RURAL HEALTHCARE SYSTEM BY INTEGRATING RHCS INTO THE BROADER HEALTHCARE COMMUNITY. RHA REQUESTS A 4-YEAR, $1.2 MILLION RURAL HEALTH NETWORK DEVELOPMENT GRANT TO SUPPORT THE START-UP OF THE TN-RHC NETWORK. SERVICES WILL INCLUDE CONDUCTING VALUE BASED CARE (VBC) READINESS ASSESSMENTS, DEVELOPING A COMMUNITY OF PRACTICE TO ADVANCE PROFESSIONAL KNOWLEDGE, AND PROVIDING TECHNICAL ASSISTANCE TO IMPROVE OPERATIONAL EFFICIENCIES AND CARE COORDINATION. THESE EVIDENCE-BASED SERVICES WILL SUPPORT THE FINANCIAL VIABILITY OF RHCS AND IMPROVE THE HEALTH OF RURAL TENNESSEANS. AS THE NETWORK INCREASES ITS VALUE TO RHCS THROUGH ACTIVITIES, DATA SHARING, ADVOCACY, AND TECHNICAL ASSISTANCE, IT IS EXPECTED TO GROW INTO A FULLY SELF-SUSTAIN ING ORGANIZATION.
