CURRENTLY REACHING OVER 8000 CONSUMERS, SWCS IS A 501(C)(3), CARF ACCREDITED, MEDICAID CERTIFIED, FULLY LICENSED MENTAL HEALTH SERVICE PROVIDER WITH NEARLY FIVE DECADES OF EXPERIENCE SERVICING THE NEEDS OF SOUTHWEST DETROIT. SWCS IS LICENSED TO PROVIDE SUD SERVICES AND IS ONE OF SEVEN CMH CENTERS THAT SERVE WAYNE COUNTY. THE COMMUNITY PROJECT FUNDING WILL FOCUS ON SWCS’ FACILITY LOCATED AT 1700 WATERMAN, DETROIT, MICHIGAN. 1700 WATERMAN WAS BUILT IN THE 1960S. THE ORIGINAL TWO-STORY 16,669 SQUARE FOOT BUILDING HOUSED TRANS AMERICAN FREIGHT. IN 1988 A SINGLE-STORY 14,962 SQUARE FOOT ADDITION WAS ADDED TO THE BACK OF THE BUILDING INCREASING THE SQUARE FOOTAGE TO 31,661 (INCLUDES BASEMENT). THE PROPOSED COMMUNITY PROJECT, SWCS’ NEW CRISIS RESPONSE AND OUTPATIENT BEHAVIORAL HEALTH CLINIC FOR YOUTH AND ADULTS AT THE WATERMAN CAMPUS, IS DESIGNED TO ADDRESS THE SIGNIFICANT AND GROWING NEED FOR INTEGRATED BEHAVIORAL HEALTH SERVICES IN WAYNE COUNTY. THE PROJECT COSTS, $2,565,000 WILL REVOLUTIONIZE THE WATERMAN CAMPUS TO SERVE CONSUMERS IN CRISIS. BUILDING IMPROVEMENTS AND INFRASTRUCTURE CHANGES WILL MAKE THE FACILITY: - ACCESSIBLE TO INCREASED NUMBERS OF YOUTH AND ADULT CLIENTS. - SAFER FOR CONSUMERS WITH EMERGENCY BEHAVIORAL NEEDS AND THOSE WHO SERVE THEM. - A NEW CRISIS RESPONSE ENTRANCE WILL SUPPORT SEPARATE CLINICAL CARE SPACES FOR ADULTS AND YOUTH THAT WILL SERVE BOTH MILD/MODERATE CLIENTS AND THOSE WITH SEVERE AND IMMEDIATE MENTAL HEALTH CRISES. - ADAPTABLE TO DELIVERY SYSTEMS THAT ADDRESS CURRENT AND FUTURE BEHAVIORAL HEALTH NEEDS. - DELIVERY OF TELEHEALTH CARE WILL BE IMPROVED WITH INFRASTRUCTURE (E.G. JOHN BRENNE, LONG-TIME FGP AND SCP PROJECT DIRECTOR FROM 1975 TO 2018, PASSED AWAY SUDDENLY ON JANUARY 22, 2018. HE HAS SINCE BEEN REPLACED BY TERESA E. O?HALLORAN, WHO BRINGS NEW PERSPECTIVE AND ORGANIZATION TO THE PROGRAM. ROBERT BURDETT REMAINS THE PROJECT ASSISTANT AND A VITAL PART OF THE PROGRAM PROVIDING ORIENTATION TO THE NEW PROJECT DIRECTOR. IN THE UPCOMING PROGRAM YEAR, WE PROPOSE A REDUCTION IN VSYS DUE TO THESE UNEXPECTED CHANGES. BY REDUCING OUR VSYS TO 53 IT WILL ALLOW THE FLEXIBILITY NEEDED TO REORGANIZE AND SOLIDIFY THE PROGRAM UNDER NEW PROJECT LEADERSHIP. WE ANTICIPATE THE EO-CO FGP VOLUNTEERS WILL SERVE APPROXIMATELY 55,332 HOURS DURING THE 2019-20 PROGRAM YEAR. OREGON DEPARTMENT OF EDUCATION (ODE) TESTING RECORDS FOR THE SCHOOL YEAR 2017-18 SHOW THERE ARE 580,684 REGISTERED STUDENTS IN THE STATE. OF THESE STUDENTS, 261,637 ARE IN ELEMENTARY SCHOOL. 53% OF ALL STUDENTS IN GRADE 3 DO NOT MEET OR EXCEED GRADE LEVEL FOR ENGLISH, AND 54% OF ALL STUDENTS IN GRADE 3 FALL SHORT OF THE LEVEL FOR MATHEMATICS. 48.6 % OF STUDENTS IN GRADES 3-5 DO NOT MEET STATE GRADE-LEVEL EXPECTATIONS FOR ENGLISH LANGUAGE ARTS. FOR STUDENTS IN GRADES 6-8, THE NUMBER IS 44.8% WHO DO NOT MEET THE STANDARD FOR LITERACY. THE EO SCP PROGRAM OF CCS IS CURRENTLY BUDGETED FOR 30 SENIOR COMPANION VOLUNTEERS (33 VSYS), BUT FACING ABRUPT AND UNEXPECTED CHANGES HAS SEEN A DECLINE IN VOLUNTEERS. JOHN BRENNE, LONG-TIME FGP AND SCP PROJECT DIRECTOR FROM 1975 TO 2018, PASSED AWAY SUDDENLY ON JANUARY 22, 2018. HE HAS SINCE BEEN REPLACED BY TERESA E. O?HALLORAN, WHO BRINGS NEW PERSPECTIVE AND ORGANIZATION TO THE PROGRAM. ROBERT BURDETT REMAINS THE PROJECT ASSISTANT AND A VITAL PART OF THE PROGRAM PROVIDING ORIENTATION TO THE NEW PROJECT DIRECTOR. IN THE UPCOMING PROGRAM YEAR, WE PROPOSE A REDUCTION IN VSYS DUE TO THESE UNEXPECTED CHANGES. BY REDUCING OUR VSYS TO 24 IT WILL ALLOW THE FLEXIBILITY NEEDED TO REORGANIZE AND SOLIDIFY THE PROGRAM 25,056 HOURS DURING THE 2019-20 PROGRAM YEAR. CCS CURRENTLY HAS 24 EO SCP VOLUNTEER STATIONS IN MORROW, MALHEUR, UMATILLA, AND BAKER COUNTIES. VOLUNTEER STATION TYPES INCLUDE MULTI-PURPOSE CENTERS (INCLUDING SENIOR CENTERS), CONVALESCENT CENTERS/HOSPICES, AND PUBLIC/CONGREGATE HOUSING, AND HOSPITAL OUTREACH PROGRAMS. THE SCP PROGRAM OF CCS WILL PROVIDE SERVICE TO 240 CLIENTS WITH AN ESTIMATED 25 UNDUPLICATED VOLUNTEERS (24 VSYS). THE EO SCP PROGRAM OF CCS IS BECOMING WELL ESTABLISHED IN THE AREA IT SERVES. WE CONTINUE TO REACH OUT TO ESTABLISH MORE MEMORANDUMS OF UNDERSTANDING WITH SOCIAL SERVICE ORGANIZATIONS AND HOSPITALS IN OUR SERVICE AREA WHILE AT THE SAME TIME STRENGTHEN EXISTING ONES. WE EXPECT TO RECRUIT MORE SCP VOLUNTEERS IN THE EASTERN OREGON AREAS IN ORDER TO STABILIZE AND ACHIEVE OUR VSY GOAL. CURRENTLY REACHING OVER 8000 CONSUMERS, SWCS WILL SUPPORT A MINIMUM OF 500 ADDITIONAL UNDUPLICATED ADULTS AND ADOLESCENTS OVER TWO-YEARS, INCLUDING THOSE WITH SUBSTANCE USE DISORDER (SUD), SERIOUS MENTAL ILLNESS (SMI), SERIOUS EMOTIONAL DISTURBANCE (SED), OR CO-OCCURRING DISORDER (COD). TO REDUCE DISPARITIES, THE SWCS CAMPUS WILL SERVICE UNDERINSURED AND UNINSURED ADULTS AND YOUTH, INCLUDING LIMITED ENGLISH SPEAKING RESIDENTS, VETERANS, AND THE HOMELESS. CURRENTLY REACHING OVER 8000 CONSUMERS, SWCS IS A 501(C)(3), CARF ACCREDITED, MEDICAID CERTIFIED, FULLY LICENSED MENTAL HEALTH SERVICE PROVIDER WITH NEARLY FIVE DECADES OF EXPERIENCE SERVICING THE NEEDS OF SOUTHWEST DETROIT. SWCS IS LICENSED TO PROVIDE SUD SERVICES AND IS ONE OF SEVEN CMH CENTERS THAT SERVE WAYNE COUNTY. THE COMMUNITY PROJECT FUNDING WILL FOCUS ON SWCS’ FACILITY LOCATED AT 1700 WATERMAN, DETROIT, MICHIGAN. 1700 WATERMAN WAS BUILT IN THE 1960S. THE ORIGINAL TWO-STORY 16,669 SQUARE FOOT BUILDING HOUSED TRANS AMERICAN FREIGHT. IN 1988 A SINGLE-STORY 14,962 SQUARE FOOT ADDITION WAS ADDED TO THE BACK OF THE BUILDING INCREASING THE SQUARE FOOTAGE TO 31,661 (INCLUDES BASEMENT). THE PROPOSED COMMUNITY PROJECT, SWCS’ NEW CRISIS RESPONSE AND OUTPATIENT BEHAVIORAL HEALTH CLINIC FOR YOUTH AND ADULTS AT THE WATERMAN CAMPUS, IS DESIGNED TO ADDRESS THE SIGNIFICANT AND GROWING NEED FOR INTEGRATED BEHAVIORAL HEALTH SERVICES IN WAYNE COUNTY. THE PROJECT COSTS, $2,565,000 WILL REVOLUTIONIZE THE WATERMAN CAMPUS TO SERVE CONSUMERS IN CRISIS. BUILDING IMPROVEMENTS AND INFRASTRUCTURE CHANGES WILL MAKE THE FACILITY: - ACCESSIBLE TO INCREASED NUMBERS OF YOUTH AND ADULT CLIENTS. - SAFER FOR CONSUMERS WITH EMERGENCY BEHAVIORAL NEEDS AND THOSE WHO SERVE THEM. - A NEW CRISIS RESPONSE ENTRANCE WILL SUPPORT SEPARATE CLINICAL CARE SPACES FOR ADULTS AND YOUTH THAT WILL SERVE BOTH MILD/MODERATE CLIENTS AND THOSE WITH SEVERE AND IMMEDIATE MENTAL HEALTH CRISES. - ADAPTABLE TO DELIVERY SYSTEMS THAT ADDRESS CURRENT AND FUTURE BEHAVIORAL HEALTH NEEDS. - DELIVERY OF TELEHEALTH CARE WILL BE IMPROVED WITH INFRASTRUCTURE (E.G. BROADBAND) ENHANCEMENTS. SWCS’ WATERMAN CAMPUS HAS TREMENDOUS POTENTIAL FOR THE PROVISION OF NEW AND EXPANDED SERVICES FOR CONSUMERS ACRO SS SOUTHEAST MICHIGAN. THE NEW CRISIS RESPONSE ENTRANCE, DESIGNED TO ACCEPT 24/7 REFERRALS WILL FACILITATE SWCS’ GOAL OF BEING THE STATE OF MICHIGAN’S PRIMARY SPANISH LANGUAGE REFERRAL PARTNER FOR CRISIS INTERVENTION. THE CCBHC AWARD, GROWING DEMAND FOR SERVICES, AND ANTICIPATING FUTURE STRATEGIES FOR DELIVERING BEHAVIORAL HEALTH NEEDS IN THE FUTURE IS THE IMPETUS FOR THIS FUNDING REQUEST. SWCS IS SEEKING SUPPORT FOR A MODERNIZATION PROJECT WITHOUT EQUIPMENT. SPECIFICALLY, SWCS WILL RENOVATE ITS CURRENT FACILITY BY MODERNIZING ITS CURRENT FACILITY INTERIOR AND REPAIR EXTERIOR BUILDING CONCERNS. CCS IS REQUESTING $319,354.35 IN FEDERAL FUNDING FOR THE EO-CO FGP PROGRAM AND PROPOSES TO SUPPLEMENT THESE FEDERAL FUNDS WITH AN ADDITIONAL $35,510 IN NON-FEDERAL CASH AND IN-KIND SUPPORT. JOHN BRENNE, LONG-TIME FGP AND SCP PROJECT DIRECTOR FROM 1975 TO 2018, PASSED AWAY SUDDENLY ON JANUARY 22, 2018. HE HAS SINCE BEEN REPLACED BY TERESA E. O?HALLORAN, WHO BRINGS NEW PERSPECTIVE AND ORGANIZATION TO THE PROGRAM. ROBERT BURDETT REMAINS THE PROJECT ASSISTANT AND A VITAL PART OF THE PROGRAM PROVIDING ORIENTATION TO THE NEW PROJECT DIRECTOR. IN THE UPCOMING PROGRAM YEAR, WE PROPOSE A REDUCTION IN VSYS DUE TO THESE UNEXPECTED CHANGES. BY REDUCING OUR VSYS TO 53 IT WILL ALLOW THE FLEXIBILITY NEEDED TO REORGANIZE AND SOLIDIFY THE PROGRAM UNDER NEW PROJECT LEADERSHIP. WE ANTICIPATE THE EO-CO FGP VOLUNTEERS WILL SERVE APPROXIMATELY 55,332 HOURS DURING THE 2019-20 PROGRAM YEAR. VOLUNTEER STATION TYPES INCLUDE BOTH PUBLIC AND PRIVATE SCHOOLS AS WELL AS HEAD START AND A VARIETY OF AFTERSCHOOL OR SUMMER PROGRAMS. THE EO-CO FGP VOLUNTEERS WILL PROVIDE SERVICE TO 560 STUDENTS WITH AN ESTIMATED 55 VOLUNTEERS (53 VSYS). THE EO-CO FGP PROGRAM OF CCS IS WELL ESTABLISHED IN THE AREA IT SERVES. OREGON DEPARTMENT OF EDUCATION (ODE) TESTING RECORDS FOR THE SCHOOL YEAR 2017-18 SHOW THERE ARE 580,684 REGISTERED STUDENTS IN THE STATE. OF THESE STUDENTS, 261,637 ARE IN ELEMENTARY SCHOOL. 53% OF ALL STUDENTS IN GRADE 3 DO NOT MEET OR EXCEED GRADE LEVEL FOR ENGLISH, AND 54% OF ALL STUDENTS IN GRADE 3 FALL SHORT OF THE LEVEL FOR MATHEMATICS. 48.6 % OF STUDENTS IN GRADES 3-5 DO NOT MEET STATE GRADE-LEVEL EXPECTATIONS FOR ENGLISH LANGUAGE ARTS. FOR STUDENTS IN GRADES 6-8, THE NUMBER IS 44.8% WHO DO NOT MEET THE STANDARD FOR LITERACY. CCS HAS 150 EMPLOYEES AND ITS MISSION IS TO PROVIDE DYNAMIC, PROGRESSIVE, AND DIVERSE SUPPORTS TO IMPROVE THE WELL-BEING OF OUR COMMUNITIES. CCS IS REQUESTING $142,896.60 IN FEDERAL FUNDING FOR THE EO SCP PROGRAM AND PROPOSES TO SUPPLEMENT THESE FEDERAL FUNDS WITH AN ADDITIONAL $15,844 IN NON-FEDERAL CASH AND IN-KIND SUPPORT. THE EO SCP PROGRAM OF CCS IS CURRENTLY BUDGETED FOR 30 SENIOR COMPANION VOLUNTEERS (33 VSYS), BUT FACING ABRUPT AND UNEXPECTED CHANGES HAS SEEN A DECLINE IN VOLUNTEERS. JOHN BRENNE, LONG-TIME FGP AND SCP PROJECT DIRECTOR FROM 1975 TO 2018, PASSED AWAY SUDDENLY ON JANUARY 22, 2018. HE HAS SINCE BEEN REPLACED BY TERESA E. O?HALLORAN, WHO BRINGS NEW PERSPECTIVE AND ORGANIZATION TO THE PROGRAM. ROBERT BURDETT REMAINS THE PROJECT ASSISTANT AND A VITAL PART OF THE PROGRAM PROVIDING ORIENTATION TO THE NEW PROJECT DIRECTOR. IN THE UPCOMING PROGRAM YEAR, WE PROPOSE A REDUCTION IN VSYS DUE TO THESE UNEXPECTED CHANGES. BY REDUCING OUR VSYS TO 24 IT WILL ALLOW THE FLEXIBILITY NEEDED TO REORGANIZE AND SOLIDIFY THE PROGRAM 25,056 HOURS DURING THE 2019-20 PROGRAM YEAR. CCS CURRENTLY HAS 24 EO SCP VOLUNTEER STATIONS IN MORROW, MALHEUR, UMATILLA, AND BAKER COUNTIES. VOLUNTEER STATION TYPES INCLUDE MULTI-PURPOSE CENTERS (INCLUDING SENIOR CENTERS), CONVALESCENT CENTERS/HOSPICES, AND PUBLIC/CONGREGATE HOUSING, AND HOSPITAL OUTREACH PROGRAMS. THE SCP PROGRAM OF CCS WILL PROVIDE SERVICE TO 240 CLIENTS WITH AN ESTIMATED 25 UNDUPLICATED VOLUNTEERS (24 VSYS). THE EO SCP PROGRAM OF CCS IS BECOMING WELL ESTABLISHED IN THE AREA IT SERVES. WE CONTINUE TO REACH OUT TO ESTABLISH MORE MEMORANDUMS OF UNDERSTANDING WITH SOCIAL SERVICE ORGANIZATIONS AND HOSPITALS IN OUR SERVICE AREA WHILE AT THE SAME TIME STRENGTHEN EXISTING ONES. WE EXPECT TO RECRUIT MORE SCP VOLUNTEERS IN THE EASTERN OREGON AREAS IN ORDER TO STABILIZE AND ACHIEVE OUR VSY GOAL. THE PRIMARY FOCUS AREA FOR THIS PROJECT IS HEALTHY FUTURES WITH THE OBJECTIVE OF AGING IN PLACE. THE SERVICE ACTIVITIES INCLUDE COMPANIONSHIP / TRANSPORTATION / RESPITE SERVICES. COMMUNITY NEED TO BE ADDRESSED: OUR GOAL IS TO ADDRESS THE LONELINESS AND ISOLATION OF HUNDREDS OF FRAIL AND ISOLATED SENIORS WHO COULD USE COMPANIONSHIP AND/OR TRANSPORTATION TO GROCERY SHOP. HUNDREDS OF SENIORS IN EASTERN OREGON HAVE BEEN IDENTIFIED (BY SENIOR-SERVING AGENCIES) WHO NEED HELP WITH ACTIVITIES OF DAILY LIFE AND ACHIEVING INDEPENDENT SKILLS SO THEY CAN DO MORE FOR THEMSELVES, AS DOCUMENTED BY OREGON STATE AGING AND DISABILITY SERVICES (2013). THE SSVF PROGRAM'S PURPOSE IS TO PROVIDE SUPPORTIVE SERVICES GRANTS TO PRIVATE NON-PROFIT ORGANIZATIONS AND CONSUMER COOPERATIVES, WHO WILL COORDINATE OR PROVIDE SUPPORTIVE SERVICES TO VERY LOW-INCOME VETERAN FAMILIES WHO ARE RESIDING IN PERMANENT HOUSING, ARE HOMELESS AND SCHEDULED TO BECOME RESIDENTS OF PERMANENT HOUSING WITHIN A SPECIFIED TIME PERIOD; OR AFTER EXITING PERMANENT HOUSING WITHIN A SPECIFIED TIME PERIOD, ARE SEEKING OTHER HOUSING THAT IS RESPONSIVE TO SUCH VERY LOW-INCOME VETERAN FAMILY'S NEEDS AND PREFERENCES. GRANTEES WILL USE SUPPORTIVE SERVICES GRANT FUNDS TO PROVIDE SUPPORTIVE SERVICE. ALL GRANTEES ARE REQUIRED TO PROVIDE OUTREACH SERVICES, CASE MANAGEMENT SERVICES, ASSISTANCE IN OBTAINING VA BENEFITS AND ASSISTANCE IN OBTAINING AND COORDINATING OTHER PUBLIC BENEFITS. IN ADDITION TO THE REQUIRED SERVICES, GRANTEES MAY ALSO PROVIDE TEMPORARY FINANCIAL ASSISTANCE PAID DIRECTLY TO A THIRD PARTY ON BEHALF OF A PARTICIPANT FOR CHILD CARE, EMERGENCY HOUSING ASSISTANCE, TRANSPORTATION, RENTAL ASSISTANCE, UTILITY-FEE PAYMENT ASSISTANCE, SECURITY DEPOSITS, UTILITY DEPOSITS, MOVING COSTS, AND GENERAL HOUSING STABILITY ASSISTANCE (WHICH INCLUDES EMERGENCY SUPPLIES), IN ACCORDANCE WITH 38 CFR PART 62.
