THESE NEIGHBORHOODS ARE AMONG THE MOST AT-RISK, HIGH-NEED AREAS IN NYC, WITH DISPROPORTIONATELY POOR HEALTH OUTCOMES. THEY ARE HOME TO PRIMARILY COMMUNITIES OF COLOR, INCLUDING A LARGE NUMBER OF IMMIGRANTS. ICL’S HEALTH HUB, LOCATED IN EAST NEW YORK, BROOKLYN, DELIVERS INTEGRATED, HOLISTIC CARE IN A SINGLE SITE IN PARTNERSHIP WITH OUR CO-LOCATED FQHC PARTNER, COMMUNITY HEALTHCARE NETWORK, INC. (CHN); SERVICES INCLUDE PRIMARY CARE, FAMILY SUPPORT, MH/SUD OUTPATIENT PROGRAMS, RECOVERY PROGRAMS, AND CARE COORDINATION TO ADDRESS BEHAVIORAL AND PHYSICAL HEALTH CONCERNS OF THE HIGHLY DIVERSE, UNDERSERVED LOCAL POPULATION. CCBHC FUNDING WILL ENABLE US TO CONTINUE TO ADDRESS SIGNIFICANT HEALTH DISPARITIES EXPERIENCED BY LATINO FAMILIES WITH CHILDREN, WHO LACK HEALTH INSURANCE, AND EXPERIENCE OTHER BARRIERS TO QUALITY CARE. WE WILL EXTEND OUR OUTREACH EFFORTS TO NEW IMMIGRANTS AND ASYLUM SEEKERS, WHO HAVE COME TO NYC IN GREAT NUMBERS OVER THE PAST FEW MONTHS. THE PROJECT’S GOALS ARE TO: (1) INCREASE ACCESS TO COMPREHENSIVE BH SERVICES FOR INDIVIDUALS AND FAMILIES, WITH LIMITED RESOURCES, LANGUAGE, AND OTHER BARRIERS TO CARE; (2) IMPROVE BH AND OTHER OUTCOMES FOR INDIVIDUALS AND FAMILIES, WITH LIMITED RESOURCES, LANGUAGE, AND OTHER BARRIERS TO CARE; AND (3) ENHANCE CCBHC INFRASTRUCTURE TO PROMOTE CARE COORDINATION, MONITORING, AND ADVANCEMENT. TO ENCOURAGE OUTREACH AND ENGAGEMENT, WE WILL UTILIZE A COMPREHENSIVE SPANISH OUTREACH INITIATIVE, ENGAGE COMMUNITY PARTNERS, SUCH AS SHELTERS, AND CONDUCT BI-WEEKLY PRESENTATIONS TO LOCAL PROVIDERS AND AGENCIES TO STREAMLINE REFERRALS INTO THE CCBHC. WE WILL ENHANCE OUR CAPACITY TO SERVE OUR SPANISH-SPEAKING CLIENTS BY ADDING BILINGUAL CASE MANAGEMENT STAFF. IN ADDITION, WE WILL CONTINUE TO INTENSIFY SERVICES FOR VETERAN FAMILIES. WE WILL COLLABORATE WITH CHN TO PROVIDE PRIMARY CARE SCREENING, MONITORING, AND SERVICES TO OUR CLIENTS. | Department of Health and Human Services | ICL CMHC OLDER ADULT RECOVERY SUPPORTS (OARS) - ICL, INC.’S OARS (OLDER ADULT RECOVERY SUPPORTS) PROVIDES HOME-BASED MENTAL HEALTH TREATMENT AND RECOVERY SERVICES FOR 300 LOW-INCOME OLDER ADULTS AGE 55+ WITH MENTAL ILLNESS, WITH AN EMPHASIS ON LATINX RESIDENTS OF BROOKLYN, NY NEIGHBORHOODS MOST IMPACTED BY COVID-19. VIA A MULTI-DISCIPLINARY TEAM APPROACH, TELEHEALTH AND EVIDENCE-BASED PRACTICES, SENIORS WILL BE (RE-)CONNECTED WITH MENTAL HEALTH SERVICES AND IMPROVE THEIR WELLBEING. MOST RESIDENTS IN THE TARGET AREAS OF EAST NEW YORK, BROWNSVILLE, AND BEDFORD STUYVESANT ARE FROM COMMUNITIES OF COLOR: 61% ARE BLACK AND 27% ARE LATINX. APPROXIMATELY 25% OF RESIDENTS ARE AGE 55+, AND 14% ARE AGE 65+, WITH ALMOST 3 OUT OF 10 SENIORS EXPERIENCING POVERTY (29%). DETRIMENTAL SOCIAL DETERMINANTS OF HEALTH LED TO CASE AND DEATH RATES SIGNIFICANTLY ABOVE THE NYC AVERAGES; EAST NEW YORK, IN PARTICULAR, SAW THE HIGHEST RATE OF COVID-19 DEATHS IN NYC, WITH 945/100,000 FATALITIES. THE CDC REPORTS THAT 20% OF PEOPLE AGE 55+ EXPERIENCE SOME TYPE OF MENTAL HEALTH CONCERN. THE COVID-19 PANDEMIC HAS WORSENED MENTAL HEALTH AND REDUCED TREATMENT ACCESS FOR OLDER ADULTS, DESPITE AN INCREASE IN TELEHEALTH OFFERINGS. LATINX, IN PARTICULAR, ARE LESS LIKELY TO RECEIVE TREATMENT FOR MENTAL HEALTH ISSUES THAN WHITES, INCLUDING DURING THE PANDEMIC, WHILE SUFFERING FROM ELEVATED RATES OF INFECTION AND DEATH RATES AS WELL AS DEPRESSION AND ANXIETY. PROJECT OARS IS DESIGNED TO IDENTIFY, REENGAGE, AND BRING ICL COMMUNITY MENTAL HEALTH CLINIC SERVICES TO OLDER ADULTS WITH SIGNIFICANT MENTAL HEALTH AND/OR SUBSTANCE USE DISORDERS. ICL WILL USE GRANT FUNDING TO DELIVER FLEXIBLE, IN-HOME, AND TRAUMA-INFORMED OUTREACH, TREATMENT, AND PEER SUPPORT SERVICES TO SENIORS WHO DETERIORATED AND/OR HAVE NOT CONSISTENTLY BEEN ENGAGED IN MENTAL HEALTH CLINIC SERVICES DURING THE PANDEMIC. THE OARS TEAM, CONSISTING OF A REGISTERED NURSE, MENTAL HEALTH CLINICIANS, AND PEER SPECIALISTS WILL FACILITATE ACCESS TO TELEHEALTH THAT CONNECTS CLIENTS WITH CLINIC-BASED PSYCHIATRISTS, PRIMARY CARE PHYSICIANS, AND OFF-SITE SOCIAL SERVICES; PROVIDE IN-HOME/OUTPATIENT TREATMENT THAT IS EVIDENCE-BASED; PSYCHOSOCIAL REHABILITATION AND INTENSIVE CASE MANAGEMENT VIA CRITICAL TIME INTERVENTION; AND PEER SUPPORTS. SERVICES ARE AIMED AT BOTH DIRECTLY IMPROVING CLIENTS’ MENTAL HEALTH AND HELPING THEM TRANSITION TO CLINIC- AND COMMUNITY-BASED MENTAL HEALTH SERVICES. OARS FIRST GOAL IS TO ESTABLISH HIGH-INTENSITY OUTREACH AND ENGAGEMENT STRATEGIES THAT LEAD TO THE ENROLLMENT OF 150 NEW YORKERS AGES 55+ ANNUALLY, AND 300 CLIENTS OVER THE COURSE OF THE PROJECT. THE SECOND GOAL IS TO INCREASE OLDER ADULTS’ ACCESS TO AND PARTICIPATION IN MENTAL AND PHYSICAL HEALTH CARE AND RECOVERY SUPPORTS. AS A RESULT OF OARS SERVICES, THE THIRD GOAL IS TO IMPROVE MENTAL HEALTH, SUBSTANCE USE, PHYSICAL, AND ENVIRONMENTAL WELLBEING FOR AT LEAST 80% OF PARTICIPANTS. OREGON LIQUOR CONTROL COMMISSION (OLCC) DATA SHOWED WEST LINN HAS A HIGH ALCOHOL OUTLET DENSITY DEMONSTRATING SOCIAL ACCEPTANCE OF ALCOHOL CONSUMPTION IS HIGH WITHIN THE DEFINED AREA OF THE GRANT PROPOSAL AREA. ACCORDING TO THE OREGON IDAHO HIGH-INTENSITY DRUG TRAFFICKING AREA (HIDTA) PROGRAM, THERE HAS BEEN AN INCREASE IN COUNTERFEIT PILLS DESIGNED TO APPEAR LIKE OPIOIDS AND BENZODIAZEPINES THROUGHOUT THE STATE. THE ENFORCEMENT TEAMS IN OREGON SEIZED OVER 1.3 MILLION DOSAGE UNITS OF COUNTERFEIT PILLS CONTAINING FENTANYL DURING 2021 WHICH IS DOUBLE THE AMOUNT SEIZED IN 2020. THE PILLS ARE ALMOST CERTAIN TO CONTAIN THE DEADLY SUBSTANCE FENTANYL OR ITS EQUALLY DANGEROUS ANALOGS. THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. WILL IMPROVE AND ADVANCE OUTREACH, ENGAGEMENT, AND INTEGRATED BEHAVIORAL HEALTH TREATMENT FOR 1,000 AT-RISK RESIDENTS OF BROOKLYN, NY ACROSS THE LIFESPAN. I.E., CHILDREN, YOUTH, ADULTS AND THEIR FAMILIES THROUGH A WHOLE FAMILY HEALTH MODEL (WFHM), WITH A FOCUS ON SPANISH-SPEAKING POPULATIONS, RECENT IMMIGRANTS AND ASYLUM SEEKERS, AND VETERAN FAMILIES. THE HEALTH HUB CCBHC SERVES THE GEOGRAPHIC CATCHMENT AREA OF BEDFORD-STUYVESANT, EAST NEW YORK/STARRETT CITY/CYPRESS HILLS, AND BROWNSVILLE/OCEAN HILL IN BROOKLYN, NEW YORK. THESE NEIGHBORHOODS ARE AMONG THE MOST AT-RISK, HIGH-NEED AREAS IN NYC, WITH DISPROPORTIONATELY POOR HEALTH OUTCOMES. THEY ARE HOME TO PRIMARILY COMMUNITIES OF COLOR, INCLUDING A LARGE NUMBER OF IMMIGRANTS. ICL’S HEALTH HUB, LOCATED IN EAST NEW YORK, BROOKLYN, DELIVERS INTEGRATED, HOLISTIC CARE IN A SINGLE SITE IN PARTNERSHIP WITH OUR CO-LOCATED FQHC PARTNER, COMMUNITY HEALTHCARE NETWORK, INC. (CHN); SERVICES INCLUDE PRIMARY CARE, FAMILY SUPPORT, MH/SUD OUTPATIENT PROGRAMS, RECOVERY PROGRAMS, AND CARE COORDINATION TO ADDRESS BEHAVIORAL AND PHYSICAL HEALTH CONCERNS OF THE HIGHLY DIVERSE, UNDERSERVED LOCAL POPULATION. CCBHC FUNDING WILL ENABLE US TO CONTINUE TO ADDRESS SIGNIFICANT HEALTH DISPARITIES EXPERIENCED BY LATINO FAMILIES WITH CHILDREN, WHO LACK HEALTH INSURANCE, AND EXPERIENCE OTHER BARRIERS TO QUALITY CARE. WE WILL EXTEND OUR OUTREACH EFFORTS TO NEW IMMIGRANTS AND ASYLUM SEEKERS, WHO HAVE COME TO NYC IN GREAT NUMBERS OVER THE PAST FEW MONTHS. THE PROJECT’S GOALS ARE TO: (1) INCREASE ACCESS TO COMPREHENSIVE BH SERVICES FOR INDIVIDUALS AND FAMILIES, WITH LIMITED RESOURCES, LANGUAGE, AND OTHER BARRIERS TO CARE; (2) IMPROVE BH AND OTHER OUTCOMES FOR INDIVIDUALS AND FAMILIES, WITH LIMITED RESOURCES, LANGUAGE, AND OTHER BARRIERS TO CARE; AND (3) ENHANCE CCBHC INFRASTRUCTURE TO PROMOTE CARE COORDINATION, MONITORING, AND ADVANCEMENT. TO ENCOURAGE OUTREACH AND ENGAGEMENT, WE WILL UTILIZE A COMPREHENSIVE SPANISH OUTREACH INITIATIVE, ENGAGE COMMUNITY PARTNERS, SUCH AS SHELTERS, AND CONDUCT BI-WEEKLY PRESENTATIONS TO LOCAL PROVIDERS AND AGENCIES TO STREAMLINE REFERRALS INTO THE CCBHC. WE WILL ENHANCE OUR CAPACITY TO SERVE OUR SPANISH-SPEAKING CLIENTS BY ADDING BILINGUAL CASE MANAGEMENT STAFF. IN ADDITION, WE WILL CONTINUE TO INTENSIFY SERVICES FOR VETERAN FAMILIES. WE WILL COLLABORATE WITH CHN TO PROVIDE PRIMARY CARE SCREENING, MONITORING, AND SERVICES TO OUR CLIENTS. AS A PART OF OUR WFHM, WE WILL CONTINUE TO DEVELOP A FAMILY CARE PLAN IN OUR EHR AND IMPROVE CARE COORDINATION WITH OTHER PROVIDERS VIA ELECTRONIC DATA SHARING. HNNP COMMITS TO LEVERAGING THE CCBHC FUNDING TO ADDRESS THE BEHAVIORAL HEALTH DISPARITIES IN MACOMB COUNTY THROUGH: 1) EXPANDED RESEARCH-DRIVEN BEST PRACTICES, 2) BUILDING COMMUNITY CAPACITY, 3) INTEGRATED PRIMARY AND BEHAVIORAL HEALTH TREATMENT, 4) BETTER COMMUNITY SURVEILLANCE AND ACCESS, AND 5) ENHANCED OUTREACH SERVICES THAT WILL BE FOCUSED TO ALL RESIDENTS. US CENSUS DATA REPORTS 11% OF RESIDENTS LIVE BELOW FEDERAL POVERTY LEVEL (FPL) AND 16% OF YOUTH UNDER 18 YEARS ARE BELOW FPL AS COMPARED TO MICHIGAN (10.2% FPL) AND NATIONAL AVERAGES (9.2% FPL). (ACS, 2020) NOTE THAT THE FEDERAL STIMULUS EFFORTS DUE TO THE PANDEMIC OVERSTATE THE REAL FPL BY ALMOST 4%. HNNP AND THEIR COMMITTED DESIGNATED COLLABORATING ORGANIZATIONS (DCOS) THAT COMPRISE THE MHP INITIATIVE HAVE IDENTIFIED THE FOLLOWING CCBHC PRIORITIES: GOAL ONE: IMPROVE HNNP'S SERVICE CONTINUUM AND QUALITY BY OBTAINING THE CORE CCBHC STANDARDS PER SAMHSA AND AS DESCRIBED IN HNNP'S (CCBHC) PROPOSAL. GOAL TWO: INCREASE ACCESS TO CARE BY OFFERING EXPANDED COMPREHENSIVE TREATMENT AVAILABILITY. ICL CMHC OLDER ADULT RECOVERY SUPPORTS (OARS) - ICL, INC.’S OARS (OLDER ADULT RECOVERY SUPPORTS) PROVIDES HOME-BASED MENTAL HEALTH TREATMENT AND RECOVERY SERVICES FOR 300 LOW-INCOME OLDER ADULTS AGE 55+ WITH MENTAL ILLNESS, WITH AN EMPHASIS ON LATINX RESIDENTS OF BROOKLYN, NY NEIGHBORHOODS MOST IMPACTED BY COVID-19. VIA A MULTI-DISCIPLINARY TEAM APPROACH, TELEHEALTH AND EVIDENCE-BASED PRACTICES, SENIORS WILL BE (RE-)CONNECTED WITH MENTAL HEALTH SERVICES AND IMPROVE THEIR WELLBEING. MOST RESIDENTS IN THE TARGET AREAS OF EAST NEW YORK, BROWNSVILLE, AND BEDFORD STUYVESANT ARE FROM COMMUNITIES OF COLOR: 61% ARE BLACK AND 27% ARE LATINX. APPROXIMATELY 25% OF RESIDENTS ARE AGE 55+, AND 14% ARE AGE 65+, WITH ALMOST 3 OUT OF 10 SENIORS EXPERIENCING POVERTY (29%). DETRIMENTAL SOCIAL DETERMINANTS OF HEALTH LED TO CASE AND DEATH RATES SIGNIFICANTLY ABOVE THE NYC AVERAGES; EAST NEW YORK, IN PARTICULAR, SAW THE HIGHEST RATE OF COVID-19 DEATHS IN NYC, WITH 945/100,000 FATALITIES. THE CDC REPORTS THAT 20% OF PEOPLE AGE 55+ EXPERIENCE SOME TYPE OF MENTAL HEALTH CONCERN. THE COVID-19 PANDEMIC HAS WORSENED MENTAL HEALTH AND REDUCED TREATMENT ACCESS FOR OLDER ADULTS, DESPITE AN INCREASE IN TELEHEALTH OFFERINGS. LATINX, IN PARTICULAR, ARE LESS LIKELY TO RECEIVE TREATMENT FOR MENTAL HEALTH ISSUES THAN WHITES, INCLUDING DURING THE PANDEMIC, WHILE SUFFERING FROM ELEVATED RATES OF INFECTION AND DEATH RATES AS WELL AS DEPRESSION AND ANXIETY. PROJECT OARS IS DESIGNED TO IDENTIFY, REENGAGE, AND BRING ICL COMMUNITY MENTAL HEALTH CLINIC SERVICES TO OLDER ADULTS WITH SIGNIFICANT MENTAL HEALTH AND/OR SUBSTANCE USE DISORDERS. ICL WILL USE GRANT FUNDING TO DELIVER FLEXIBLE, IN-HOME, AND TRAUMA-INFORMED OUTREACH, TREATMENT, AND PEER SUPPORT SERVICES TO SENIORS WHO DETERIORATED AND/OR HAVE NOT CONSISTENTLY BEEN ENGAGED IN MENTAL HEALTH CLINIC SERVICES DURING THE PANDEMIC. THE OARS TEAM, CONSISTING OF A REGISTERED NURSE, MENTAL HEALTH CLINICIANS, AND PEER SPECIALISTS WILL FACILITATE ACCESS TO TELEHEALTH THAT CONNECTS CLIENTS WITH CLINIC-BASED PSYCHIATRISTS, PRIMARY CARE PHYSICIANS, AND OFF-SITE SOCIAL SERVICES; PROVIDE IN-HOME/OUTPATIENT TREATMENT THAT IS EVIDENCE-BASED; PSYCHOSOCIAL REHABILITATION AND INTENSIVE CASE MANAGEMENT VIA CRITICAL TIME INTERVENTION; AND PEER SUPPORTS. SERVICES ARE AIMED AT BOTH DIRECTLY IMPROVING CLIENTS’ MENTAL HEALTH AND HELPING THEM TRANSITION TO CLINIC- AND COMMUNITY-BASED MENTAL HEALTH SERVICES. OARS FIRST GOAL IS TO ESTABLISH HIGH-INTENSITY OUTREACH AND ENGAGEMENT STRATEGIES THAT LEAD TO THE ENROLLMENT OF 150 NEW YORKERS AGES 55+ ANNUALLY, AND 300 CLIENTS OVER THE COURSE OF THE PROJECT. THE SECOND GOAL IS TO INCREASE OLDER ADULTS’ ACCESS TO AND PARTICIPATION IN MENTAL AND PHYSICAL HEALTH CARE AND RECOVERY SUPPORTS. AS A RESULT OF OARS SERVICES, THE THIRD GOAL IS TO IMPROVE MENTAL HEALTH, SUBSTANCE USE, PHYSICAL, AND ENVIRONMENTAL WELLBEING FOR AT LEAST 80% OF PARTICIPANTS. AS OUTREACH AND SERVICES WILL BE CULTURALLY AND LINGUISTICALLY RESPONSIVE AND TARGETED TO LOCAL LATINS RESIDENTS, THE PROJECT ADDRESSES PERSISTENT RACIAL/ETHNIC INEQUITIES IN ACCESS TO HEALTHCARE.

THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. A MOBILIZED COMMUNITY TO ENGAGE YOUTH FOR PREVENTION SUCCESS TODAY - PROJECT ABSTRACT SUMMARY COMMUNITY LIVING ABOVE (CLA), A DRUG PREVENTION COALITION IN WEST LINN, OREGON (IN CLACKAMAS COUNTY), WORKS WITH COMMUNITY STAKEHOLDERS TO STRATEGICALLY ADDRESS RISK FACTORS THAT CONTRIBUTE TO THE INITIATION OF YOUTH SUBSTANCE USE. WEST LINN IS A SMALL AFFLUENT COMMUNITY WITH UNIQUE NORMS, INCLUDING COMMON DENIAL OF THE SEVERITY OF LOCAL YOUTH SUBSTANCE USE, LIBERAL STATE DRUG LAWS CREATING A LOW PERCEPTION OF HARM, AND EASY ACCESSIBILITY TO ADDICTIVE SUBSTANCES. CLA PROPOSES TO ENGAGE IN A MORE COMPREHENSIVE ASSESSMENT, STRENGTHEN CAPACITY AND COLLABORATION WITH DIVERSE STAKEHOLDERS, UTILIZE SCIENCE-BASED IMPLEMENTATION, AND EVALUATE REGULARLY FOR IMPROVEMENT OF ACTIVITIES. THROUGH THE SUBSTANCE ABUSE AND MENTAL HEALTH SERVICES ADMINISTRATIONS’ (SAMHSA) STRATEGIC PREVENTION FRAMEWORK (SPF) PROCESS, THE COALITION WILL FINALIZE A STRATEGIC PLAN TO FURTHER DEFINE ACTIVITIES TO PREVENT EARLY ONSET OF YOUTH SUBSTANCE USE SPECIFIC TO THE SUBSTANCES DETERMINED TO BE THE MOST PREVALENT THREAT TO OUR COMMUNITY. A PRELIMINARY ASSESSMENT REVEALED LOCAL CONDITIONS THAT CONTRIBUTE TO A LOW PERCEPTION OF HARM AND EXCESSIVE ACCESSIBILITY TO THE SELECTED SUBSTANCES. OREGON LIQUOR CONTROL COMMISSION (OLCC) DATA SHOWED WEST LINN HAS A HIGH ALCOHOL OUTLET DENSITY DEMONSTRATING SOCIAL ACCEPTANCE OF ALCOHOL CONSUMPTION IS HIGH WITHIN THE DEFINED AREA OF THE GRANT PROPOSAL AREA. THE OLCC IS ALSO RESPONSIBLE FOR MINOR DECOY OPERATIONS TO TEST LOCAL ESTABLISHMENTS ON THE EFFICACY OF CHECKING IDENTIFICATION AND REFUSAL TO SELL ALCOHOL TO MINORS. THE MOST RECENT OPERATION WAS MORE THAN TWO YEARS AGO AND RESULTED IN ONLY 54% COMPLIANCE. NO FOLLOW-UP OR EDUCATIONAL OUTREACH HAS BEEN PROVIDED SINCE THAT TIME. IN 2014 OREGON LEGALIZED MARIJUANA FOR “RECREATIONAL” USE, AND MARIJUANA DISPENSARIES QUICKLY SATURATED THE STATE. PRODUCERS GREW RETAIL MARIJUANA AT A RATE THAT QUICKLY EXCEEDED EXPECTATIONS, AND PRICES PLUMMETED TO PRICES COMPARABLE TO THE BLACK MARKET 40 YEARS AGO. SIGNAGE GLAMORIZING MARIJUANA USE AND ADVERTISING PRE-ROLLED JOINTS FOR $2 WERE PLASTERED ACROSS COMMUNITIES EVERYWHERE. WEST LINN WAS PROACTIVE AND QUICKLY ADOPTED AN ORDINANCE BANNING MARIJUANA DISPENSARIES. STILL, AS OF THIS WRITING, THEY ARE ALLOWED TO DELIVER, AND AT LEAST THREE HAVE BEEN CONFIRMED TO DELIVER TO WEST LINN. THE SOCIAL ACCEPTABILITY OF MARIJUANA USE HAS BECOME MAINSTREAM. YOUTH IN THE REGION COMMONLY MISUSE PRESCRIPTION PILLS. OPIOIDS AND BENZODIAZEPINES ARE THE MOST POPULAR, FOR YOUTH ATTEMPTING TO GET HIGH. IN OUR AREA, YOUTH KNOW THEY CAN OBTAIN PILLS THROUGH SOCIAL MEDIA OR A BUS RIDE AWAY ON A STREET CORNER. ACCORDING TO THE OREGON IDAHO HIGH-INTENSITY DRUG TRAFFICKING AREA (HIDTA) PROGRAM, THERE HAS BEEN AN INCREASE IN COUNTERFEIT PILLS DESIGNED TO APPEAR LIKE OPIOIDS AND BENZODIAZEPINES THROUGHOUT THE STATE. THE ENFORCEMENT TEAMS IN OREGON SEIZED OVER 1.3 MILLION DOSAGE UNITS OF COUNTERFEIT PILLS CONTAINING FENTANYL DURING 2021 WHICH IS DOUBLE THE AMOUNT SEIZED IN 2020. THE PILLS ARE ALMOST CERTAIN TO CONTAIN THE DEADLY SUBSTANCE FENTANYL OR ITS EQUALLY DANGEROUS ANALOGS. CLA SEEKS FUNDING TO ADDRESS EARLY-ONSET (OFTEN TOBACCO/NICOTINE) AND RISKY BEHAVIORS TO PREVENT YOUTH USE OF ALCOHOL, MARIJUANA, AND PRESCRIPTION DRUGS THROUGH MEANINGFUL STAKEHOLDER COLLABORATION AND EVIDENCE-BASED INTERVENTIONS. THE GOAL IS TO INCREASE AWARENESS OF THE PREVALENT DRUG THREATS TO WEST LINN YOUTH AND STRENGTHEN COMMUNITY READINESS TO ADDRESS IT WHILE DECREASING YOUTH SUBSTANCE USE AND EARLY INITIATION OF RISKY BEHAVIORS.

IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. LIMITATION ON USE OF FUNDS: NO ASSISTANCE PROVIDED UNDER PROGRAM (OR ANY STATE OR LOCAL GOVERNMENT FUNDS USED TO SUPPLEMENT THIS ASSISTANCE) MAY BE USED TO REPLACE STATE OR LOCAL FUNDS PREVIOUSLY USED, OR DESIGNATED FOR USE, TO ASSIST HOMELESS PERSONS OR PERSONS AT-RISK OF HOMELESSNESS.; EXPECTED OUTCOMES: DECREASE IN THE NUMBER INDIVIDUALS AND FAMILIES EXPERIENCING HOMELESSNESS, MORE SPECIFICALLY USING PERFORMANCE INDICATORS SUCH AS THE LENGTH OF TIME HOMELESS, RETURNS TO HOMELESSNESS OVER TIME, AND EXITS TO PERMANENT HOUSING.
