DBA CARESOUTH MEDICAL AND DENTAL (CARESOUTH) LOCATED IN THE BATON ROUGE METROPOLITAN STATISTICAL AREA (MSA), WAS ESTABLISHED IN 1997 AS A PRIVATE, NOT-FOR-PROFIT FEDERALLY QUALIFIED HEALTH CENTER (FQHC). FOR THE PAST TWENTY-SIX (26) YEARS, CARESOUTH HAS PROVIDED HIGH-QUALITY, COMPREHENSIVE HEALTH SERVICES FOR RESIDENTS LIVING IN EAST BATON ROUGE AND THE EIGHT SURROUNDING PARISHES, PROVIDING MEDICAL CARE SERVICES TO NEARLY 17,000 UNDUPLICATED CLIENTS IN 2023. CARESOUTH FOCUSES ON PROVIDING CARE FOR CLIENTS THAT HAVE TRADITIONALLY BEEN UNDERSERVED DUE TO SYSTEMIC ISSUES SUCH AS RACISM, SEXISM, POVERTY, HOMOPHOBIA/TRANSPHOBIA, HISTORY OF INCARCERATION, AND GEOGRAPHIC INEQUALITIES THAT PLAGUE THE DEEP SOUTH. CARESOUTH OFFERS COMPREHENSIVE BEHAVIORAL HEALTH SERVICES FOR CHILDREN (AGED 5 AND UP) AND ADULTS, AS WELL AS SCREENING, BRIEF INTERVENTION AND REFERRAL FOR TREATMENT (SBIRT) FOR THOSE ADULTS AT-RISK FOR SUBSTANCE USE DISORDERS. BEHAVIORAL HEALTH SERVICES ARE CURRENTLY AVAILABLE BOTH IN-PERSON AT HEALTH CENTER SITES ON FORM 5B VIA TELEHEALTH, AND INCLUDE CLINICAL ASSESSMENT BY LICENSED CLINICAL SOCIAL WORKER, PSYCHIATRIC EVALUATIONS, INDIVIDUALIZED CARE PLANNING, MEDICATION MANAGEMENT, CASE MANAGEMENT, CRISIS INTERVENTION AND INDIVIDUAL, GROUP AND/OR FAMILY THERAPY. CARESOUTH ALSO PROVIDES HEALTH SERVICES TO INMATES IN ASCENSION PARISH JAIL AND SEEKS TO EXPAND SERVICES AT ASCENSION PARISH JAIL AND EAST BATON ROUGE PRISON TO PROVIDE WRAPAROUND CARE TO JI-R INDIVIDUALS WITHIN 90 DAYS OF THEIR RELEASE FROM CUSTODY. CRITICAL HEALTH NEEDS TO BE ADDRESSED THROUGH PROGRAMMING INCLUDE ADDRESSING MENTAL HEALTH AND SUBSTANCE USE DISORDERS TREATMENT NEEDS AND REDUCING RISK OF OVERDOSE. PREVENTION, SCREENING, DIAGNOSIS AND TREATMENT OF HCV, HIV, AND OTHER INFECTIOUS DISEASES WILL CONTINUE THROUGH CURRENTLY OPERATIONAL CARESOUTH PROGRAMMING. CARESOUTH WILL ALSO FACILITATE CASE MANAGEMENT SERVICES AND COLLABORATE WITH COMMUNITY PARTNERS TO ADDRESS HEALTH RELATED SOCIAL NEEDS, INCLUDING HOUSING INSECURITY, FOOD INSECURITY, FINANCIAL STRAIN, LACK OF TRANSPORTATION, INTIMATE PARTNER VIOLENCE, AND OTHER NEEDS AS IDENTIFIED. TO ADEQUATELY ADDRESS INDIVIDUAL NEEDS, CARESOUTH WILL PARTNER WITH ASCENSION PARISH SHERIFF’S OFFICE (ASCENSION PARISH JAIL) TO COORDINATE CARE DURING INCARCERATION AND TRANSITIONS IN CARE TO COMMUNITY-BASED PROVIDERS UPON REENTRY. CARESOUTH WILL ALSO ESTABLISH REFERRAL PROCESSES WITH COMMUNITY-BASED PROVIDERS FAMILY SERVICES OF GREATER BATON ROUGE, LOUISIANA PAROLE PROJECT, AND GREATER BATON ROUGE FOOD BANK, TO MEET INDIVIDUALS’ HEALTH-RELATED SOCIAL NEEDS. OTHER PROGRAM ACTIVITIES WILL INCLUDE THE PROVISION OF: - HEALTH EVALUATION(S), TO IDENTIFY ACUTE AND CHRONIC PHYSICAL HEALTH, BEHAVIORAL HEALTH, OR ORAL HEALTH ISSUES AND ASSOCIATED NEEDS. - PRIMARY HEALTH SERVICES INCLUDING DIAGNOSIS, TREATMENT, AND MEDICATION MANAGEMENT. IMMUNIZATIONS. - BEHAVIORAL HEALTH SERVICES, INCLUDING MAT/MOUD AND PEER RECOVERY SUPPORT. - PREVENTIVE DENTAL SERVICES - CASE MANAGEMENT SERVICES CONNECTING J-IR INDIVIDUALS WITH FEDERAL, STATE AND LOCAL BENEFITS PROGRAMS TO ADDRESS CRITICAL HEALTH AND HEALTH-RELATED NEEDS. THROUGH HRSA QIF-TJI FUNDING, CARESOUTH WILL INCREASE THE NUMBER OF JI-R INDIVIDUALS WHO: - RECEIVE WRAPAROUND SUPPORT FROM COMMUNITY-ORGANIZATIONS POST-INCARCERATION. - RECEIVE IN-SCOPE HEALTH CENTER SERVICES POST INCARCERATION. - ARE ENROLLED IN ELIGIBLE LOCAL, STATE, AND FEDERAL BENEFITS PROGRAMS POST-INCARCERATION.
