ANTICIPATES SERVING 2,755 CLIENTS, OF WHICH 20% (551) ARE PROJECTED TO BE SERVED VIA TELEHEALTH WHO ARE UNDERSERVED IN COCHISE COUNTY. - THE ARIZONA FAMILY HEALTH PARTNERSHIP (AFHP) IS A PRIVATE NON-PROFIT ORGANIZATION THAT HAS SERVED AS A TITLE X ARIZONA GRANTEE SINCE 1983. AFHP WILL ENHANCE AND EXPAND TELEHEALTH INFRASTRUCTURE AND CAPACITY OF QUALITY TITLE X FAMILY PLANNING SERVICES (FPS) THROUGHOUT COCHISE COUNTY, ARIZONA VIA ITS SUBRECIPIENT CHIRICAHUA COMMUNITY HEALTH CENTERS, INC. (CCHCI). UNIQUE CHALLENGES FOR RURAL AREA RESIDENTS IN COCHISE COUNTY TO ACCESS FPS INCLUDE LACK OF TRANSPORTATION, PREVALENCE OF POVERTY, LIMITED ACCESS TO TECHNOLOGY AND OTHER COMPLEX SOCIAL DETERMINANTS OF HEALTH. THE COVID-19 PANDEMIC HAS CREATED ADDITIONAL BARRIERS TO SERVING FAMILY PLANNING CLIENTS, AND SUPPORTING EVIDENCE DEMONSTRATES THAT COCHISE COUNTY IS IN GREAT NEED OF FPS AND A DEDICATED TELEHEALTH COORDINATOR TO STRENGTHEN CCHCI’S TELEHEALTH INFRASTRUCTURE TO PROVIDE A BROAD RANGE OF ACCEPTABLE AND EFFECTIVE MEDICALLY APPROVED FAMILY PLANNING METHODS AND SERVICES. CCHCI ANTICIPATES SERVING 2,755 UNDUPLICATED FAMILY PLANNING CLIENTS, OF WHICH 20% (551) ARE PROJECTED TO BE SERVED VIA TELEHEALTH. CCHCI WILL EXPAND AND IMPROVE OUTREACH AND PROMOTION OF TITLE X FPS TO PEOPLE WHO ARE UNDERSERVED BY TELEHEALTH SERVICES TO ENHANCE ACCESS TO HIGH-QUALITY, CLIENT-CENTERED, AND EQUITABLE CARE. CCHCI ANTICIPATES REACHING 5,000 INDIVIDUALS THROUGH OUTREACH AND PROMOTION EFFORTS. AS THE LEADING EXPERT ON FPS DELIVERY IN ARIZONA, AND A TRUSTED COMMUNITY, STATE-WIDE, AND NATIONAL RESOURCE, AFHP IS DEDICATED TO PROVIDING AND IMPROVING ACCESS TO HIGH-QUALITY REPRODUCTIVE HEALTHCARE AND ADVANCING HEALTH EQUITY. AFHP WILL ENSURE TELEHEALTH INFRASTRUCTURE ENHANCEMENT IN COCHISE COUNTY INCREASES OPPORTUNITIES FOR RURAL, LOW-INCOME INDIVIDUALS TO ACCESS EQUITABLE, AFFORDABLE, CLIENT-CENTERED, CONFIDENTIAL, AND QUALITY FPS.
