AS ESTABLISHED IN THE PROPOSAL SOURCE DOCUMENTS THE SPRAGUE RIVER WATERSHED IN THE UPPER KLAMATH BASIN UKB ONCE SUPPORTED THRIVING AQUATIC RIPARIAN AND FLOODPLAIN ECOSYSTEMS AND ABUNDANT NATIVE FISH POPULATIONS IN THE ARID HIGH DESERT OF THE EASTERN CASCADES. SETTLERS WHO ARRIVED IN THE BASIN IN THE 19TH AND 20TH CENTURY CHANGED THE LANDSCAPE OFTEN UNDER DIRECTION OF THE FEDERAL GOVERNMENT AND THE LONG-TERM IMPLICATIONS TO THE NATURAL ECOSYSTEM AND THE FIRST PEOPLES WERE RARELY CONSIDERED. WATER SYSTEMS WERE ENGINEERED TO DRAIN WETLANDS CONTROL FLOODS AND DELIVER WATER. TREES WERE CLEARED FIRE SUPPRESSED AND FIRST PEOPLES MANAGEMENT OF LANDS WATERS FISH AND WILDLIFE WAS FORCED INTO MARGINS AND TO NONEXISTENCE. FURTHER CLIMATE CHANGE AND RECENT HIGH SEVERITY WILDFIRES HAVE BEEN ADDING ADDITIONAL PRESSURE TO THIS ALREADY FRAGILE SYSTEM. IN 2021 THE BOOTLEG FIRE BURNED OVER 410000 ACRES OF THE SPRAGUE BASIN EXACERBATING EXCESSIVE SEDIMENT LOADING AND LACK OF RIPARIAN HABITAT. SOILS IN THE KLAMATH BASIN ARE NATURALLY HIGH IN PHOSPHORUS SO EXCESSIVE SEDIMENT LOADING INTO THE SYSTEM FUELS ALGAE BLOOMS AND IMPAIRED WATER QUALITY CONDITIONS IN THE SPRAGUE RIVER UPPER KLAMATH LAKE AND DOWNSTREAM INTO THE MAINSTEM KLAMATH RIVER.WATERSHED HEALTH ON THE SPRAGUE RIVER IS CRITICAL TO THE LONG-TERM RECOVERY OF ESA LISTED ENDANGERED CWAAM (LOST RIVER SUCKER DELTISTES LUXATUS) KOPTU (SHORTNOSE SUCKER CHASMISTES BREVIROSTRIS) AND THREATENED BULL TROUT (SALVELINUS CONFLUENTUS) AS WELL AS ANADROMOUS SALMONIDS FOLLOWING KLAMATH DAM REMOVAL. RECOGNIZING THIS NEED AS WELL AS THE NEED TO SUPPORT ENGAGEMENT AND ECONOMIC RESILIENCE FOR LANDOWNERS OUR DIVERSE GROUP OF PARTNERS HAS AN AMBITIOUS PROPOSAL TO RESTORE THIS WATERSHED. RESTORATION AND BEST MANAGEMENT PRACTICES CAN INCREASE PROPERTY VALUE IMPROVE EFFICIENCIES REDUCE REGULATORY RISK AND SIMPLIFY MANAGEMENT MAKING FARMS AND RANCHES MORE PROFITABLE AND RESILIENT. YET ITS ALSO IMPORTANT TO ACKNOWLEDGE THAT SOME PROJECTS CAN INADVERTENTLY BURDEN LANDOWNERS WITH SLOWED PRODUCTIVITY HIGHER ENERGY COSTS LOSS OF FARMABLE SURFACE AREA OR ADDED TIME AND WAGE REQUIREMENTS. FORTUNATELY, RENEWED COMMUNITY AND PARTNER ENGAGEMENT IN THE UKB HAVE CREATED THE CONDITIONS TO ADDRESS THESE SHORTCOMINGS. THROUGH COLLABORATION WITH PARTNERS AND LANDOWNERS OUR TEAM HAS THE UNIQUE ABILITY TO ADDRESS PROGRAM PRIORITIES WITH TECHNICALLY SOUND COST-EFFECTIVE SOLUTIONS. OUR PROPOSAL WILL DEVELOP A LANDOWNER INCENTIVE FRAMEWORK THAT ENCOURAGES PARTICIPATION IN WATERSHED RESTORATION WHILE VALUING THE DIVERSE NEEDS OF LANDOWNERS IN THE WATERSHED. THIS PHASE I PROPOSAL ENCOMPASSES THREE GOALS TO ADDRESS THE CULTURAL AND LOGISTICAL BARRIERS TO ECOLOGICAL RESTORATION WHILE DEVELOPING AND MAINTAINING ECONOMIC DRIVERS IN THE KLAMATH BASIN. MANUFACTURE OF AN INTRACEREBROVENTRICULAR ENZYME REPLACEMENT THERAPY FOR CLN1 BATTEN DISEASE - SUMMARY THE NEURONAL CEROID LIPOFUSCINOSES (NCLS) ARE A GROUP OF INCURABLE NEURODEGENERATIVE STORAGE DISORDERS PRIMARILY AFFECTING THE BRAIN AND THE RETINA OF CHILDREN AND YOUNG ADULTS, LEADING TO DEMENTIA, BLINDNESS, EPILEPSY, AND EARLY DEATH, WITH A PREVALENCE OF APPROXIMATELY 1.5 TO NINE PER MILLION POPULATION (1.3 TO 7 PER 100,000 LIVE BIRTHS). THE INFANTILE ONSET FORM CLN1 DISEASE IS CAUSED BY MUTATIONS IN THE CLN1/PPT1 GENE, WHICH CODES FOR THE LYSOSOMAL ENZYME PALMITOYL-PROTEIN THIOESTERASE-1 (PPT1) RESULTING IN A REDUCTION OR ABSENCE OF ENZYME ACTIVITY. CLN1 DISEASE USUALLY PRESENTS BETWEEN 6 AND 24 MONTHS OF AGE AND THERE ARE 2-3 CHILDREN WITH THIS FORM IDENTIFIED EACH YEAR AND CURRENTLY 24 KNOWN CHILDREN WITH CLN1 IN THE US, 11 IN BRAZIL AND AT LEAST 10 IN THE UK (WITH LIKELY MANY MORE UNDIAGNOSED). THERE ARE CURRENTLY NO TREATMENTS AVAILABLE OTHER THAN PALLIATIVE THERAPIES AND THE DISEASE IS FATAL. HUMAN RECOMBINANT PPT1 (RHPPT1) EXPRESSED IN CHO CELLS HAS BEEN PREVIOUSLY REPORTED TO MODIFY DISEASE PHENOTYPES FOLLOWING A SINGLE INTRATHECAL (IT) AND INTRAVENOUS (IV) ADMINISTRATION IN PPT1 DEFICIENT MICE (PPT1-/-). AFTER SUCCESSFUL COMPLETION OF OUR PHASE I SBIR IN WHICH WE EXCEEDED OUR MILESTONES MONTHLY INTRACEREBROVENTRICULAR (ICV) ADMINISTRATION OF RHPPT1 PRODUCED STATISTICALLY SIGNIFICANT TREATMENT EFFECTS IN PPT1-/- MICE, SUCH AS RESCUE OF MORE THAN 60% PPT1 ENZYME ACTIVITY DECREASED SECONDARY ENZYME LEVELS, DECREASED THE LOSS OF NEURONS IN ALL REGIONS OF BRAIN AND SPINAL CORD AND IMPROVED GAIT AND ROTAROD RESULTS. THE CLN1 ERT DATA CLEARLY POINTED TO ICV DOSING AS IDEAL FOR FUTURE STUDIES, WHICH IS ALSO A PREFERRED ROUTE ACCORDING TO PHYSICIANS. OUR PHASE II SBIR HAS ENABLED US TO 1. PREPARE OUR QUALITY DOCUMENTS, 2 IDENTIFY A CDMO (STC BIOLOGICS) WHO HAS MANUFACTURED OUR GLP QUALITY PROTEIN FOR PREIND TOXICITY STUDIES IN RAT. 3. SUBMITTED A PREIND REQUEST TO THE FDA (WITH THE ASSISTANCE OF RTI INTERNATIONAL,). WE NOW PROPOSE IN THIS PROJECT 1. PRODUCTION OF GMP PROTEIN (WITH STC BIOLOGICS), 2. CONTINUED DEVELOPMENT OF QUALITY PROCEDURES, 3. PREPARE A CLINICAL TRIAL PROTOCOL (WITH ASSISTANCE FROM CTI CLINICAL TRIAL SERVICES, INC.), 4 AND SUBMIT AN IND (WITH ASSISTANCE FROM RTI). THESE COLLABORATIONS WILL ENABLE US TO COST EFFECTIVELY AND MORE RAPIDLY TRANSLATE THIS POTENTIAL TREATMENT TO THE CLINIC THAT CAN POTENTIALLY SAVE THE LIVES OF CHILDREN LIVING WITH THIS DEVASTATING DISEASE. WE HAVE ALREADY OBTAINED AN ORPHAN DRUG DESIGNATION AND RARE PEDIATRIC DISEASE DESIGNATION FROM THE FDA FOR RHPPT1 AS A BIOLOGICAL PRODUCT FOR A “RARE PEDIATRIC DISEASE” WHICH OFFERS SEVERAL BENEFITS IN FUTURE UPON FDA APPROVAL, INCLUDING MARKETING EXCLUSIVITY FOR 7 YEARS AND THE POTENTIAL TO OBTAIN A RARE PEDIATRIC DISEASE VOUCHER AND THUS PROVIDE A RETURN ON INVESTMENT (CURRENT VALUE ~$100 M). WE ARE ENGAGED WITH CLN1 FAMILIES AND WE ARE NOW WELL POSITIONED TO CONTINUE THE DEVELOPMENT OF THIS POTENTIAL TREATMENT FOR A DEVASTATING DISEASE. THIS PROPOSED WORK WOULD BRING US CLOSER TO A CLINICAL TRIAL. OUR WORK ON DEVELOPING RHPPT1 AS AN ERT FOR CLN1 WOULD POSITION US WELL TO WORK ON FURTHER RARE DISEASES IN FUTURE AND GROW CPI. THERE IS AN IMPORTANT ROLE TO BRING TREATMENTS TO PATIENTS WITH ULTRA-RARE DISEASES AS ILLUSTRATED BY OTHER MUCH LARGER COMPANIES SUCH AS BIOMARIN AND ULTRAGENYX WHO WE ASPIRE TO EMULATE.

PULESA-UGANDA-STRENGTHENING THE BLOOD PRESSURE CARE AND TREATMENT CASCADE FOR UGANDANS LIVING WITH HIV-IMPLEMENTATION STRATEGIES TO SAVE LIVES - PROJECT SUMMARY CHRONIC HIV INFECTION IS A WELL-ESTABLISHED RISK FACTOR FOR CARDIOVASCULAR DISEASE. IN SUB-SAHARAN AFRICA)—A REGION THAT MAY ACCOUNT FOR HALF OF THE GLOBAL BURDEN OF CVD ATTRIBUTABLE TO HIV—HYPERTENSION IS THE MOST IMPORTANT DRIVER OF CVD RISK. PROFOUND BARRIERS TO EFFECTIVE HYPERTENSION MANAGEMENT EXIST, INCLUDING LIMITED KNOWLEDGE, INCONSISTENT BP MEASUREMENT, AND POOR ACCESS TO MEDICATIONS. HIV CARE INNOVATIONS SUCH AS ACCESS TO NO-COST ANTIRETROVIRAL THERAPY, DIFFERENTIATED SERVICE DELIVERY AND USE OF PLHIV PEERS IN CARE MODELS MAY IMPROVE CARE OF COMORBID CONDITIONS SUCH AS HYPERTENSION. THE OVERARCHING GOAL OF PULESA-UGANDA STUDY IS TO IMPROVE THE BP TREATMENT CASCADE FOR PEOPLE LIVING WITH HIV (PLHIV) IN URBAN AND PERI-URBAN UGANDA IN A SCALABLE AND SUSTAINABLE MANNER. THIS HYBRID IS TYPE 3 STUDY PROPOSES TO FIRST EXPLORE CURRENT PRACTICE, ROUTINES, BARRIERS, AND FACILITATORS OF EVIDENCE–BASED BP CARE IN HIV CLINICAL SETTINGS IN KAMPALA AND WAKISO DISTRICTS (AIM 1). THEN, USING A HUMAN-CENTERED DESIGN APPROACH, A DESIGN TEAM OF KEY STAKEHOLDERS WILL USE DATA FROM THE FORMATIVE ASSESSMENT TO DEVELOP A MULTI-COMPONENT IMPLEMENTATION STRATEGY (HTN-PLUS) TO IMPROVE UPTAKE AND ADHERENCE TO EVIDENCE-BASED BP TREATMENTS, CONTEXTUALLY ADAPTED TO THESE UGANDAN HIV CLINICS (SUB-AIM 1.1). THE DESIGN TEAM WILL ADAPT DIFFERENTIATED SERVICE DELIVERY MODELS, USE OF HYPERTENSIVE PLHIV PEER CHAMPIONS, AND METHODS OF BP MONITORING THAT ADDRESS SPECIFIC BARRIERS AND FACILITATORS OF BP CARE. IN A STEPPED-WEDGE CLUSTER RANDOMIZED TRIAL OF 16 CLINICS FROM KAMPALA AND WAKISO, WE WILL DETERMINE THE EFFECTIVENESS OF IMPLEMENTATION STRATEGIES TO IMPROVE BP CASCADE METRICS (AIM 2). CLINICS WILL BE RANDOMIZED TO RECEIVE FREE AND CONSISTENT ACCESS TO DIAGNOSTIC EQUIPMENT AND EVIDENCE-BASED ANTIHYPERTENSIVE DRUGS (HTN-BASIC) WITH AND WITHOUT THE MULTI-COMPONENT IMPLEMENTATION STRATEGY DEVELOPED IN SUB-AIM 1 (HTN-PLUS). THE PRIMARY EFFECTIVENESS OUTCOME WILL BE % OF PATIENTS WITH HYPERTENSION DIAGNOSIS WHO ARE CONTROLLED (<140MMHG SYSTOLIC). WE HYPOTHESIZE THAT THE HTN-BASIC INTERVENTION WILL INCREASE CONTROL FROM 25% AT BASELINE TO 35%, AND THAT HTN-PLUS WILL FURTHER INCREASE CONTROL TO 40%. WE WILL CONDUCT AN EXTENSIVE MIXED-METHODS PROCESS EVALUATION. WE WILL ASSESS SCALABILITY AS OUR MAIN IMPLEMENTATION OUTCOME, AND WILL ALSO ASSESS ACCEPTABILITY, ADOPTION, AND IMPLEMENTATION CLIMATE. FINALLY, WE WILL EVALUATE THE ECONOMIC AND FINANCIAL SUSTAINABILITY OF THE INTEGRATED CARE STRATEGIES IN A COST-EFFECTIVENESS ANALYSIS FROM A SOCIETAL PERSPECTIVE THAT WILL INCLUDE HOUSEHOLD OUT-OF-POCKET EXPENDITURES. THE PRIMARY OUTCOME OF THIS AIM WILL BE INCREMENTAL COST PER BP- CONTROLLED PATIENT. THIS STUDY WILL PROVIDE MUCH NEEDED EVIDENCE TO SSA GOVERNMENT STAKEHOLDERS FOR A STRATEGY TO PRESERVE THE HEALTH GAINS OF HIV TREATMENT BY PREVENTING DEATH AND DISABILITY FROM CVD. IMPORTANTLY, IT WILL OFFER ECONOMIC EVIDENCE OF THE SCALABILITY, SUSTAINABILITY, AND EQUITY OF A MODEL OF HIV- HYPERTENSION MANAGEMENT INTEGRATION.

MANUFACTURE OF AN INTRACEREBROVENTRICULAR ENZYME REPLACEMENT THERAPY FOR CLN1 BATTEN DISEASE - SUMMARY THE NEURONAL CEROID LIPOFUSCINOSES (NCLS) ARE A GROUP OF INCURABLE NEURODEGENERATIVE STORAGE DISORDERS PRIMARILY AFFECTING THE BRAIN AND THE RETINA OF CHILDREN AND YOUNG ADULTS, LEADING TO DEMENTIA, BLINDNESS, EPILEPSY, AND EARLY DEATH, WITH A PREVALENCE OF APPROXIMATELY 1.5 TO NINE PER MILLION POPULATION (1.3 TO 7 PER 100,000 LIVE BIRTHS). THE INFANTILE ONSET FORM CLN1 DISEASE IS CAUSED BY MUTATIONS IN THE CLN1/PPT1 GENE, WHICH CODES FOR THE LYSOSOMAL ENZYME PALMITOYL-PROTEIN THIOESTERASE-1 (PPT1) RESULTING IN A REDUCTION OR ABSENCE OF ENZYME ACTIVITY. CLN1 DISEASE USUALLY PRESENTS BETWEEN 6 AND 24 MONTHS OF AGE AND THERE ARE 2-3 CHILDREN WITH THIS FORM IDENTIFIED EACH YEAR AND CURRENTLY 24 KNOWN CHILDREN WITH CLN1 IN THE US AND 11 IN BRAZIL (WITH LIKELY MANY MORE UNDIAGNOSED). THERE ARE CURRENTLY NO TREATMENTS AVAILABLE OTHER THAN PALLIATIVE THERAPIES AND THE DISEASE IS FATAL. HUMAN RECOMBINANT PPT1 (RHPPT1) EXPRESSED IN CHO CELLS HAS BEEN PREVIOUSLY REPORTED TO MODIFY DISEASE PHENOTYPES FOLLOWING A SINGLE INTRATHECAL (IT) AND INTRAVENOUS (IV) ADMINISTRATION IN PPT1 DEFICIENT MICE (PPT1-/-). AFTER SUCCESSFUL COMPLETION OF OUR PHASE I SBIR IN WHICH WE MET OR EXCEEDED OUR MILESTONES IT, INTRACEREBROVENTRICULAR (ICV) AS WELL AS COMBINED ROUTES OF DELIVERY WERE COMPARED. WE DEMONSTRATED THAT MONTHLY ADMINISTRATION OF RHPPT1 VIA ICV PRODUCED STATISTICALLY SIGNIFICANT TREATMENT EFFECTS IN PPT1-/- MICE, SUCH AS RESCUE OF MORE THAN 60% PPT1 ENZYME ACTIVITY DECREASED SECONDARY ENZYME LEVELS, DECREASED THE LOSS OF NEURONS IN ALL REGIONS OF BRAIN AND SPINAL CORD AND IMPROVED GAIT AND ROTAROD RESULTS. THIS ALSO ILLUSTRATED THAT DELIVERY OF ENZYME VIA THIS ROUTE ALONE MAY BE SUFFICIENT RATHER THAN USING IN COMBINATION WITH IT DOSING. WE HAVE NOW DEVELOPED OUR OWN PPT1 CELL LINE USING CHO-DG44, THAT IS SCALABLE FOR GMP MANUFACTURE AND PERFORMED INITIAL PURIFICATION DEVELOPMENT STRATEGIES AND WE ARE DEVELOPING A DIAGNOSTIC TOOL FOR CLN1. THE CLN1 ERT DATA CLEARLY POINTS TO ICV DOSING AS IDEAL FOR FUTURE STUDIES, WHICH IS ALSO A PREFERRED ROUTE ACCORDING TO PHYSICIANS. COLLABORATIONS PHARMACEUTICALS, INC (CPI) NOW PROPOSES IN THIS PHASE II SBIR TO FILE A PREIND (WITH THE ASSISTANCE OF RTI INTERNATIONAL AND FORESIGHT BIOSCIENCES), PERFORM MANUFACTURING OF PPT1 (WITH THE ASSISTANCE OF GOODWIN BIOTECHNOLOGY, INC. AND CMC CONSULTANT DR. STEFAN PRONIUK), CONDUCT RAT AND DOG IND ENABLING TOXICOLOGY STUDIES (WITH THE ASSISTANCE OF CHARLES RIVER LABORATORIES) AND ULTIMATELY FILING AN IND (WITH THE ASSISTANCE OF RTI INTERNATIONAL AND FORESIGHT BIOSCIENCES). WE WILL HIRE THESE EXPERIENCED CONSULTANTS AND CLINICAL RESEARCH ORGANIZATIONS TO ASSIST US THROUGHOUT THE PROCESS AS THEY HAVE YEARS OF EXPERIENCE. THESE COLLABORATIONS WILL ENABLE US TO COST EFFECTIVELY AND MORE RAPIDLY TRANSLATE THIS POTENTIAL TREATMENT TO THE CLINIC THAT CAN POTENTIALLY SAVE THE LIVES OF CHILDREN LIVING WITH THIS DEVASTATING DISEASE. WE HAVE ALREADY OBTAINED AN ORPHAN DRUG DESIGNATION AND RARE PEDIATRIC DISEASE DESIGNATION FROM THE FDA FOR RHPPT1 AS A BIOLOGICAL PRODUCT FOR A “RARE PEDIATRIC DISEASE” WHICH OFFERS SEVERAL BENEFITS IN FUTURE UPON FDA APPROVAL, INCLUDING MARKETING EXCLUSIVITY FOR 7 YEARS AND THE POTENTIAL TO OBTAIN A RARE PEDIATRIC DISEASE VOUCHER AND THUS PROVIDE A RETURN ON INVESTMENT (CURRENT VALUE ~$100 M). WE ARE ENGAGED WITH CLN1 FAMILIES COMMUNICATING WITH THEM FREQUENTLY TO PROVIDE PROGRESS UPDATES AND WE ARE NOW WELL POSITIONED TO CONTINUE THE DEVELOPMENT OF THIS POTENTIAL TREATMENT FOR A DEVASTATING DISEASE. THERE IS AN IMPORTANT ROLE TO BRING TREATMENTS TO PATIENTS WITH ULTRA-RARE DISEASES AS ILLUSTRATED BY OTHER COMPANIES SUCH AS BIOMARIN AND ULTRAGENYX. OUR WORK ON DEVELOPING RHPPT1 AS AN ERT FOR CLN1 WOULD POSITION US WELL TO WORK ON FURTHER RARE DISEASES IN FUTURE AND GROW CPI.

ENSURING THESE YOUTH RECEIVE WRAPAROUND SUPPORT, ROAD TO SUCCESS FOCUSES ON ENHANCING THE SKILLS AND CAPACITIES OF PARENTS TO SUPPORT THEIR YOUTH/YOUNG ADULTS THROUGH ONE-TO-ONE SUPPORT, EDUCATION, AND RESOURCE NAVIGATION. ROAD TO SUCCESS PROPOSES TO SERVE 300 PARENTS/GUARDIANS OVER THE FIVE YEARS THROUGH THE IMPLEMENTATION OF AN EDUCATIONAL SEMINAR SERIES, ONE TO ONE SUPPORT SESSIONS, COMMUNITY OUTREACH THROUGH CORRESPONDENCE AND SOCIAL MEDIA, AND BY FACILITATING PARENT NETWORKING GROUPS AND EDUCATIONAL OPPORTUNITIES. WE WILL TEACH OUR PARTICIPANTS WHERE RESOURCES CAN BE SHARED AMONGST PARENTS, PROVIDERS, STAFF, AND COMMUNITY MEMBERS. ADDITIONALLY, THIS PROPOSAL ESTABLISHES A MULTIAGENCY ADVISORY COUNCIL WITHIN SIX MONTHS TO OPERATE OVER THE FIVE-YEAR PERIOD AS A STEERING COMMITTEE FOCUSED ON COLLECTING DATA AND THEN PROPOSING STRATEGY TO ADDRESS SERVICE DELIVERY GAPS, FUNDING SHORTFALLS, AND INSTITUTIONALIZATION OF THE PROJECT’S MODEL INTO THE STRUCTURE OF THE COUNTIES’ SOCIAL SERVICES NETWORKS. PUTTING SUSTAINABILITY AT THE HEART OF THIS PROJECT REINFORCES PACT’S COMMITMENT TO CHANGING THE SYSTEM OF CARE AS IT EXISTS TODAY, THEREBY DEMONSTRATING TO RURAL MINNESOTA COMMUNITIES HOW THEY CAN SUPPORT THIS VULNERABLE POPULATION. ROAD TO SUCCESS REPRESENTS A SHIFT IN SERVICE DELIVERY AND WRAPAROUND SUPPORT THAT WILL DECREASE SED/SMI DIAGNOSES BY INCREASING THE RESILIENCE OF OUR TARGETED RURAL YOUTH, NURTURING MORE DURABLE PARENT-CHILD BONDS, AND SWAYING COMMUNITIES TO ADOPT SIMILAR SYSTEMS OF CARE. THIS WILL CULMINATE IN THEIR RECOGNITION OF THE VALUE OF INVESTING RESOURCES INTO ADDRESSING THIS PARTICULAR LIFE TRANSITION POINT TO ENHANCE THE SUPPORT NETWORK, TRANSFORM THE CURRENT SERVICE SYSTEM MODEL, AND SUSTAIN THESE SYSTEMS CHANGES INDEFINITELY.

MACHINE LEARNING APPROACHES TO PREDICT ACETYLCHOLINESTERASE INHIBITION AND MODEL APPLICATIONS - SUMMARY ORGANOPHOSPHORUS (OP) COMPOUNDS ARE ONE OF THE MOST COMMON CAUSES OF POISONING WORLDWIDE. THERE ARE NEARLY 3 MILLION POISONINGS PER YEAR RESULTING IN THREE HUNDRED THOUSAND DEATHS. OPS BIND TO ACETYLCHOLINESTERASE (ACHE), RENDERING THE ENZYME INCAPABLE OF HYDROLYZING ACETYLCHOLINE (ACH) IN THE CHOLINERGIC SYNAPSES AND NEUROMUSCULAR JUNCTIONS. SUBSEQUENT ACCUMULATION OF ACH LEADS TO OVERSTIMULATION OF THE AFFECTED NEURONS ACTING THROUGH MUSCARINIC AND NICOTINIC RECEPTORS. SOME OF THE ADVERSE EFFECTS OF PESTICIDES ON NON-TARGET ORGANISMS SUCH AS FISH, AMPHIBIANS AND HUMANS HAVE ALSO OCCURRED AS A RESULT OF BIOMAGNIFICATIONS OF THE TOXIC COMPOUNDS. SINCE ACHE IS A VERY WELL-STUDIED TARGET, THE LIKELIHOOD OF FINDING FURTHER NEW INHIBITORS AMONGST WELL-KNOWN DRUGS AND MOLECULES IS LIKELY UNLIKELY AND THEREFORE WE NEED TO LOOK FURTHER AFIELD AND DO THIS IN A MORE EFFICIENT MANNER. COMPUTATIONAL APPROACHES SUCH AS MACHINE LEARNING ARE AN APPROACH THAT CAN BE USED TO LEARN FROM DIVERSE LITERATURE TO ENABLE VIRTUAL SCREENING. FOLLOWING OUR PRELIMINARY WORK GENERATING BAYESIAN MODELS FOR HUMAN ACHE WE HAVE PERFORMED A HIGH THROUGHPUT SCREEN OF 2431 COMPOUNDS AND IDENTIFIED 66 COMPOUNDS THAT WERE “ACTIVE” AGAINST EEL ACHE. WE RECENTLY COMPLETED A PHASE I SBIR IN WHICH WE EXPANDED THE USE OF THESE MACHINE LEARNING MODELS TO CURATE ADDITIONAL ACHE DATA, BUILT AND VALIDATED ACHE MODELS WITH MANY ALGORITHMS IN PARALLEL AND USED THEM TO IDENTIFY ACHE INHIBITORS FROM VARIOUS SPECIES TO PREDICT OP INDUCED POISONING AND POSSIBLE THREATS TO THE ENVIRONMENT. IN ADDITION, WE HAVE RECENTLY CURATED FURTHER LITERATURE DATA AND GENERATED MODELS FOR THE CLOSELY RELATED ENZYME BCHE USING A CONTRASTIVE LEARNING ALGORITHM APPROACH TO SCORE A LIBRARY OF 5 MILLION MOLECULES FROM VARIOUS VENDORS IN ORDER TO SELECT MOLECULES FOR TESTING AND HAVE IDENTIFIED SEVERAL NOVEL SELECTIVE INHIBITORS. IN PHASE II WE WILL DEVELOP THE MEGAACHE SOFTWARE PRODUCT FURTHER FOR PREDICTION OF ACHE INHIBITION AND INCORPORATE UNCERTAINLY PREDICTION FEATURES AND NEW ALGORITHMS SUCH AS LARGE LANGUAGE MODELS. WE WILL ALSO USE THIS SOFTWARE TO DESIGN AND DEVELOP ACHE REACTIVATORS THAT COULD PROVIDE A NEW TREATMENT FOR OP PESTICIDE AND NERVE AGENT EXPOSURE. WE NOW PROPOSE THE FOLLOWING AIMS TO FURTHER DEVELOP AND VALIDATE THE MEGAACHE SOFTWARE AS A COMMERCIAL PRODUCT: AIM 1: MODELING UNCERTAINTY IN ACHE MODELS AIM 2: LARGE LANGUAGE MODELS FOR ACHE AND BCHE DATASETS AIM 3: DEVELOP NEW REACTIVATORS TO TREAT ACHE POISONING USING GENERATIVE DESIGN OUR ULTIMATE GOAL WILL BE TO PROVIDE SOFTWARE AND MODELS TO PREDICT ACHE INHIBITION WHICH WILL BE A COMMERCIAL PRODUCT. IN ADDITION WE WILL DEVELOP NEW MOLECULE INTELLECTUAL PROPERTY WHICH WE CAN THEN LICENSE TO A LARGER COMPANY. ENSURING THESE YOUTH RECEIVE WRAPAROUND SUPPORT, JTI FOCUSES ON ENHANCING THE SKILLS AND CAPACITIES OF PARENTS TO SUPPORT THEIR YOUTH/YOUNG ADULTS THROUGH ONE-TO-ONE SUPPORT, EDUCATION, AND RESOURCE NAVIGATION. JTI PROPOSES TO SERVE 300 PARENTS/GUARDIANS OVER THE FOUR YEARS THROUGH THE IMPLEMENTATION OF AN EDUCATIONAL SEMINAR SERIES, ONE TO ONE SUPPORT SESSIONS, COMMUNITY OUTREACH THROUGH CORRESPONDENCE AND SOCIAL MEDIA, AND BY FACILITATING PARENT NETWORKING GROUPS AND EDUCATIONAL OPPORTUNITIES. WE WILL HELP OUR PARTICIPANTS LEARN ABOUT MENTAL HEALTH AND COMMUNITY RESOURCES THAT CAN BE SHARED TO SUPPORT PARENTS, LEARN ABOUT PROVIDERS AND STAFF INVOLVED WITH THE PROJECT. THIS PROPOSAL WILL LEVERAGE PACT’S FIVE COUNTY LOCAL CHILDREN’S MENTAL HEALTH ADVISORY COUNCIL (LAC) TO ESTABLISH A SUBCOMMITTEE THAT WILL SERVE AS THE JTI ADVISORY COMMITTEE. THIS COMMITTEE WILL BE DEDICATED TO COLLECTING DATA AND PROPOSING STRATEGIES AIMED AT ADDRESSING SERVICE DELIVERY GAPS AND FUNDING SHORTFALLS, WITH THE OVERARCHING GOAL OF ENHANCING THE SYSTEM OF CARE ACROSS THE FIVE-COUNTY SERVICE REGION. PUTTING SUSTAINABILITY AT THE HEART OF THIS PROJECT REINFORCES PACT’S COMMITMENT TO CHANGING THE SYSTEM OF CARE AS IT EXISTS TODAY, THEREBY DEMONSTRATING TO RURAL MINNESOTA COMMUNITIES HOW THEY CAN SUPPORT THIS VULNERABLE POPULATION. JTI REPRESENTS AN APPROACH TO SERVICE DELIVERY AND WRAPAROUND SUPPORT AIMED AT REDUCING SED/SMI DIAGNOSES AMONG RURAL YOUTH. HUMAN TRANSPORTER MACHINE LEARNING MODELS - SUMMARY BEING ABLE TO PREDICT INTERACTIONS WITH IMPORTANT HUMAN TRANSPORTERS WOULD BE OF VALUE TO NEW DRUG DESIGN TO AVOID COMPOUNDS THAT INTERACT WITH THEM AND CAUSE UNDESIRABLE SIDE EFFECTS. CONVERSELY, SOME DRUG TRANSPORTERS CAN BE USED FOR TARGETING MOLECULES TO SPECIFIC ORGANS AND THIS MAY HAVE CONSIDERABLE UTILITY. UNDERSTANDING THE INTERACTIONS OF NOVEL DRUGS, NATURAL PRODUCTS AND ENVIRONMENTAL TOXICANTS AND THEIR INTERACTIONS WITH AN ARRAY OF SUCH TRANSPORTERS IS, THEREFORE, IMPORTANT FOR SEVERAL INDUSTRIES, AS WELL AS FROM A REGULATORY PERSPECTIVE (E.G. FDA, EPA AND EMA). BEING ABLE TO PREDICT SUCH INTERACTIONS IN A FAST AND RELIABLE MANNER EFFECTIVELY REQUIRES USING COMPUTATIONAL APPROACHES AND LEARNING FROM IN VITRO DATA, THE LATTER A RESOURCE THAT IS RAPIDLY GROWING. OVER THE PAST 20 YEARS, WE HAVE BEEN AT THE FOREFRONT OF APPLYING DIFFERENT MACHINE LEARNING APPROACHES TO MODELING DRUG TRANSPORTERS AND, IN MANY CASES, DEVELOPING DATASETS FOR TRANSPORTERS FOR WHICH THERE WAS SCANT AVAILABLE DATA. WE NOW PROPOSE DOING THIS FOR SEVERAL TRANSPORTERS THAT MAY BE IMPORTANT FOR DRUG DISCOVERY. IN PHASE I WE FOCUSED ON OATP1B1 (SLCO1B1), WHICH IS AN UPTAKE TRANSPORTER LARGELY RESTRICTED TO THE SINUSOIDAL ASPECT OF HEPATOCYTES WHERE IT MEDIATES TRANSPORT OF A VARIETY OF STRUCTURALLY UNRELATED COMPOUNDS, INCLUDING MEMBERS OF SEVERAL CLINICALLY IMPORTANT DRUG FAMILIES (INCL. STATINS, SARTANS AND ANGIOTENSIN CONVERTING ENZYME (ACE) INHIBITORS). WE TESTED 476 DRUGS AGAINST ONE SUBSTRATE IN VITRO. WE THEN CURATED THESE DATA AND BUILT MACHINE LEARNING MODELS USING MULTIPLE MACHINE LEARNING METHODS AS WELL AS MODEL EVALUATION METRICS. THIS ENABLED US TO DEVELOP MODELS FOR INTEGRATION IN A WEB-BASED SOFTWARE TOOL CALLED MEGATRANS® THAT ENABLES THE USER TO INPUT THEIR OWN COMPOUND STRUCTURES AND GENERATE PREDICTIONS FOR INTERACTIONS WITH TRANSPORTER/S OF INTEREST, AS WELL AS VISUALIZE THE SIMILARITY TO THE TRAINING SET OF EACH MODEL USING SEVERAL DIFFERENT VISUALIZATION METHODS. IN ADDITION, DURING PHASE I WE ALSO PERFORMED PRELIMINARY DATA CURATION, MODEL BUILDING AND VALIDATION FOR TWO EQUILIBRATIVE NUCLEOSIDE TRANSPORTERS (ENTS), ENT1 AND ENT2, THAT ARE PRESENT AT THE BLOOD TESTES BARRIER (BTB), WHERE THEY CAN FACILITATE DRUG DISPOSITION (E.G. FOR ANTIVIRALS, THEREBY POTENTIALLY ELIMINATING A SANCTUARY SITE FOR VIRUSES DETECTABLE IN SEMEN). WE GENERATED BAYESIAN AND PHARMACOPHORE MODELS AND USED THESE TO PREDICT NUMEROUS COMPOUNDS THAT WERE THEN TESTED IN VITRO AGAINST ENTS. WE USED THESE ENT MODELS TO PREDICT (I) THE ANTIVIRALS USED IN TREATING COVID-19, REMDESIVIR AND MOLNUPIRAVIR, INHIBIT ENT ACTIVITY, AND THAT (II) REMDESIVIR IS AN ENT SUBSTRATE, AS WELL AS VALIDATING THESE PREDICTIONS. IN PHASE II WE PLAN ON BUILDING ON THE FOUNDATION OF PHASE I AND PROPOSE GREATLY EXPANDING THE ENT1 AND ENT2 MODELS THROUGH IN VITRO TESTING (AT THE UNIVERSITY OF ARIZONA) OF >2000 APPROVED DRUGS, NATURAL PRODUCTS, AND ENVIRONMENTAL TOXICANTS AS INHIBITORS OF ENT TRANSPORT. WE WILL USE THESE DATA TO BUILD AND VALIDATE MACHINE LEARNING MODELS USING SEVERAL ALGORITHMS, AT COLLABORATIONS PHARMACEUTICALS, INC. WE WILL ALSO TEST THESE MODELS USING EXTERNAL VALIDATION WITH ADDITIONAL MOLECULES FROM VENDOR LIBRARIES AND DRUG COLLECTIONS THAT ARE NOT IN THE MODEL. IN THIS PROCESS WE WILL ALSO BUILD OUT THE CAPABILITIES OF MEGATRANSÒ TO USE 3D PHARMACOPHORE DESCRIPTORS TO INCORPORATE MOLECULAR SHAPE FEATURES AND ALLOW 3D SEARCHES. THE RETURN ON INVESTMENT OF SUCH A COMMERCIAL TOOL WOULD BE THAT IT COULD ASSIST IN THE DESIGN AND SELECTION OF MORE FAVORABLE COMPOUNDS BY AVOIDING TRANSPORTERS OF INTEREST (OR, CONVERSELY, ALLOW THE TARGETING OF SPECIFIC TRANSPORTERS TO INCREASE UPTAKE INTO ORGANS). IT COULD ALSO IDENTIFY COMPOUNDS THAT ARE ALREADY APPROVED THAT MIGHT PRESENT A DRUG-INTERACTION RISK. PREDICTING SUCH BEHAVIOR SEEN IN VIVO IS IDEAL AND WILL LEAD TO THE PRIORITIZATION OF COMPOUNDS TO TEST

IN SOME CASES, THE COMPUTATIONAL MODELS ARE DERIVED FROM DISCRETE MOLECULAR ENDPOINTS (E.G. ESTROGEN RECEPTOR AGONISM) WHILE IN OTHERS THEY ARE QUITE BROAD IN SCOPE (E.G. DRUG INDUCED LIVER INJURY, DILI). CONSIDERABLE PROGRESS HAS BEEN MADE IN COMPUTATIONAL TOXICOLOGY IN A DECADE BOTH IN MODEL DEVELOPMENT AND AVAILABILITY SUCH THAT THE LATEST GENERATION OF LARGER SCALE MACHINE LEARNING (ML) MODELS WILL FURTHER FOCUS IN VITRO AND IN VIVO TESTING ON VERIFICATION OF SELECT PREDICTIONS. PHARMACEUTICAL, CONSUMER PRODUCTS, AGROCHEMICAL AND OTHER CHEMISTRY FOCUSED COMPANIES POSSESS STRUCTURE-ACTIVITY DATA GENERATED OVER MANY DECADES OF SCREENING THAT IS NOT IN THE PUBLIC DOMAIN, AND THIS DATA IS PRIMARILY ONLY ACCESSIBLE TO THE CHEMINFORMATICS EXPERTS IN EACH COMPANY. OUTSIDE OF THESE COMPANIES SMALL PHARMACEUTICAL, BIOTECH COMPANIES AND ACADEMICS MUST RELY ON DATA FROM PUBLIC DATABASES, COMMERCIAL DATABASES AND THEIR OWN DATA. INTEGRATING SUCH DATA FROM DIVERSE SOURCES AND PROCESSING WITH ALGORITHMS TO BUILD MACHINE LEARNING (ML) MODELS THAT CAN HELP TO ENABLE PREDICTIONS FOR NEW COMPOUNDS IS A VAST UNDERTAKING. OVER PHASE I OF THIS PROJECT TO DEVELOP THE PROTOTYPE FOR MEGATOXÒ, WE CURATED TOXICITY DATASETS THEN GENERATED AND TESTED WELL OVER 200 ML MODELS INITIALLY FOCUSED ON THE BAYESIAN APPROACH. WE HAVE ALSO DEVELOPED APPROACHES TO UNDERSTAND TRAINING AND TEST SET APPLICABILITY AND ULTIMATELY PERFORMED PROSPECTIVE PREDICTIONS AGAINST SEVERAL TOXICITY TARGETS. HAVING COMPLETED THESE AIMS, WE ALSO COLLABORATED WITH NUMEROUS ACADEMIC LABORATORIES AND PERFORMED FEE-FOR-SERVICE WORK WITH FIVE COMMERCIAL COMPANIES. WE CURRENTLY HAVE SEVERAL PHARMACEUTICAL, AGROCHEMICAL AND CONSUMER PRODUCT COMPANIES EVALUATING OUR COMPUTATIONAL TOXICITY MODELS PRIOR TO LICENSING. THESE DISCUSSIONS WITH POTENTIAL CUSTOMERS HAVE INFLUENCED THIS PHASE II PROPOSAL TO INCLUDE THE FOLLOWING AIMS: 1. COMPARE AND INTEGRATE NOVEL GRAPH-BASED MODELS SUCH AS GRAPHSAGE VERSUS OUR SUITE OF 15 DIFFERENT ML REGRESSION AND CLASSIFICATION ALGORITHMS FOR MODELING TOXICOLOGY DATASETS SUCH AS THOSE GENERATED IN PHASE I. 2. INTEGRATE READ ACROSS AND ADVERSE OUTCOME PATHWAY METHODS WITH OUR COMPUTATIONAL MODELS FOR DILI AND OTHER TOXICITY MODELS AS NEEDED. 3. GENERATE VALIDATED ML MODELS FROM IN VIVO DATA FOR NON-MAMMALIAN SPECIES (INITIALLY USING ZEBRAFISH) WHICH WILL ENABLE IN VITRO AND IN VIVO CORRELATIONS AND CAN BE VALIDATED RELATIVELY COST EFFECTIVELY. IN THIS PROPOSAL OVER 2 YEARS WE EXPECT TO DEVELOP MODELS WITH 15 DIFFERENT ALGORITHMS FOR AT LEAST 100 IN VITRO AND IN VIVO DATASETS, LEADING TO > 1500 TOXICITY ML MODELS. WE ARE NOT AWARE OF ANY OTHER COMPANY PURSUING SUCH AN APPROACH TO BOTH GENERATE NEW HIGH VALUE DATASETS OR MODELS, PERFORMING TESTING OF THEIR OWN MODELS AND CREATING A WIDE ARRAY OF TOXICITY ML MODELS. (CDD) PROPOSES TO DEVELOP CLOUD WORKSPACES FOR DRUG DISCOVERY – A NOVEL INFORMATICS FRAMEWORK THAT WILL ENABLE SCIENTISTS ENGAGED IN DRUG DISCOVERY AND TRANSLATION TO EFFORT- LESSLY, ROBUSTLY, AND SECURELY INTEGRATE DISPARATE DATABASES AND COMPUTATIONAL TOOLS DISTRIBUTED ACROSS MULTIPLE SYSTEMS AND VENDORS INTO HIGHLY-EFFICIENT, CUSTOM-TAILORED COMPUTATIONAL WORKFLOWS. OUR INNOVATIVE TECHNOL- OGY WILL SOLVE A CRITICAL PROBLEM THAT HINDERS DRUG DISCOVERY AND TRANSLATION EFFORTS: SCIENTISTS IN THIS FIELD TYPI- CALLY NEED TO COMBINE CHEMICAL AND BIOLOGICAL DATA FROM SEVERAL SOURCES, RUN THEM THROUGH MULTIPLE SOFTWARE PACKAGES THAT SPECIALIZE IN DIFFERENT TYPES OF ANALYSES AND VISUALIZATION, THEN IDEALLY STORE THE RESULTS OF THE ANALYSES TOGETHER WITH THEIR UNDERLYING EXPERIMENTAL DATA. TODAY, THIS TYPE OF INTEGRATION IS DIFFICULT AND EXPENSIVE TO ACCOMPLISH AND TYPICALLY FRAGILE, CREATING A LARGE BARRIER TO (I) EXPLOITING THE RAPIDLY INCREASING NUMBER OF HIGH-QUALITY PUBLIC-ACCESS DATA REPOSITORIES AND (II) EVALUATING PROMISING NEW ANALYTICAL TOOLS AND STRATEGIES. MONOLITHIC PLATFORMS OFFER TO SOLVE THIS PROBLEM BY BRINGING EVERYTHING TOGETHER UNDER ONE ROOF, BUT THEY ARE EXTREMELY EXPENSIVE AND THEY LIMIT FLEXIBILITY: NO SINGLE PLATFORM CAN OFFER EVERY CAPABILITY. THE ALTER- NATIVE APPROACH – STRINGING TOGETHER DISCRETE RESOURCES – EVOLVED DURING THE ERA OF DESKTOP COMPUTING AND DOES NOT TRANSLATE WELL TO MODERN CLOUD-BASED WORKFLOWS AND IN PARTICULAR TO THE CHALLENGES OF PERFORMING COMPUTATIONALLY INTENSIVE OPERATIONS THAT REQUIRE COMBINING LARGE DATASETS DISTRIBUTED ACROSS REMOTE SYSTEMS. CLOUD WORKSPACES (CW) AIMS TO COMBINE THE STRENGTHS AND AVOID THE WEAKNESSES OF THESE TWO EXTREMES. CW WILL IN ESSENCE ALLOW USERS TO EASILY CREATE THEIR OWN INDIVIDUALIZED CLOUD-HOSTED SOLUTIONS TAILORED TO THEIR UNIQUE REQUIREMENTS AND WORKFLOWS. OUR APPROACH OFFERS THE PERFORMANCE, ROBUSTNESS, AND EASE OF USE OF A MONOLITHIC SOFTWARE SOLUTION, BUT WITHOUT THE ASSOCIATED INFLEXIBILITY AND VENDOR LOCK IN. THIRTEEN TYPES OF ASSISTANCE MAY BE PROVIDED THROUGH THE CONTINUUM OF CARE (COC) PROGRAM: (1) COC PLANNING ACTIVITIES/COSTS FOR DESIGNING AND CARRYING OUT A COLLABORATIVE PROCESS FOR THE DEVELOPMENT OF AN APPLICATION TO HUD; (2) UNITED FUNDING AGENCY (UFA) COSTS FOR FISCAL CONTROL AND ACCOUNTING NECESSARY TO ASSURE THE PROPER DISBURSAL OF, AND ACCOUNTING FOR, FEDERAL FUNDS AWARDED TO SUBRECIPIENTS UNDER THE CONTINUUM OF CARE PROGRAM, (3) ACQUISITION OF REAL PROPERTY (INCLUDING STRUCTURES) FOR USE IN THE PROVISION OF HOUSING OR SUPPORTIVE SERVICES; (4) REHABILITATION OF STRUCTURES TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (5) NEW CONSTRUCTION, INCLUDING THE BUILDING OF A NEW STRUCTURE OR BUILDING AN ADDITION TO AN EXISTING STRUCTURE FOR USE AS SUPPORTIVE HOUSING; (6) LEASING OF A STRUCTURE OR STRUCTURES, OR PORTIONS THEREOF, TO PROVIDE HOUSING OR SUPPORTIVE SERVICES; (7) RENTAL ASSISTANCE, WHICH MAY BE SHORT-TERM, MEDIUM-TERM, OR LONG-TERM, AS WELL AS TENANT-BASED, PROJECT-BASED, OR SPONSOR-BASED, FOR TRANSITIONAL OR PERMANENT HOUSING; (8) SUPPORTIVE SERVICES TO ASSIST PROGRAM PARTICIPANTS OBTAIN AND MAINTAIN HOUSING; (9) OPERATING COSTS OF SUPPORTIVE HOUSING; (10) COSTS OF IMPLEMENTING AND OPERATING HMIS; (11) PROJECT ADMINISTRATIVE COSTS; (12) RELOCATION COSTS; AND (13) INDIRECT COSTS IN ACCORDANCE WITH 2 CFR PARTS 200, AS APPLICABLE. IN ADDITION TO USING GRANT FUNDS FOR THE ELIGIBLE COSTS DESCRIBED ABOVE, RECIPIENTS AND SUBRECIPIENTS IN CONTINUUMS OF CARE DESIGNATED AS HIGH PERFORMING COMMUNITIES MAY ALSO USE GRANT FUNDS TO PROVIDE HOUSING RELOCATION AND STABILIZATION SERVICES AND SHORT- AND/OR MEDIUM-TERM RENTAL ASSISTANCE TO INDIVIDUALS AND FAMILIES AT RISK OF HOMELESSNESS AS SET FORTH IN 24 CFR 576.103 AND 24 CFR 576.104, IF NECESSARY TO PREVENT THE INDIVIDUAL OR FAMILY FROM BECOMING HOMELESS. CPCQC'S STATEWIDE EXPANSION OF QI PROJECTS TO INCREASE CLINICAL-COMMUNITY LINKAGES AND IMPROVE PERINATAL OUTCOMES EQUITABLY. - MATERNAL AND INFANT HEALTH ARE TWO OF THE MOST CRITICAL INDICATORS OF A STATE’S HEALTH. IN COLORADO, DISPARITIES EXIST IN PERINATAL HEALTH OUTCOMES THAT CORRELATE WITH RACIAL, SOCIO-ECONOMIC, GEOGRAPHIC, AND OTHER FACTORS, LEADING TO CHRONIC STRESS AND POOR HEALTH AND RESULTING IN MORE ADVERSE OUTCOMES FOR HISTORICALLY MARGINALIZED POPULATIONS. COLORADO PERINATAL CARE QUALITY COLLABORATIVE (CPCQC) WORKS TO ENSURE THAT BIRTHING INDIVIDUALS AND THEIR FAMILIES RECEIVE CULTURALLY-RELEVANT, SAFE, EQUITABLE, AND HIGH-QUALITY CARE REGARDLESS OF WHO THEY ARE OR WHERE THEY LIVE. WE LEAD SYSTEMS CHANGE IN COLORADO’S PERINATAL HEALTH CARE FACILITIES THROUGH EQUITY-CENTERED BEST PRACTICES, DATA-DRIVEN DECISION-MAKING, AND TRAUMA-INFORMED CARE. CPCQC CURRENTLY LEADS FOUR ACTIVE QUALITY IMPROVEMENT (QI) INITIATIVES THAT ADDRESS HIGH PRIORITY NEEDS ACROSS COLORADO’S PERINATAL POPULATION. THESE PROJECTS SEEK TO SCREEN, SUPPORT, AND TREAT PREGNANT AND BIRTHING PEOPLE FOR SUBSTANCE USE DISORDERS (COLORADO AIM SUBSTANCE USE DISORDER LEARNING COLLABORATIVE-CO AIM: SUD LC); SCREEN AND TREAT SUBSTANCE-EXPOSED NEWBORNS AND SUPPORT PARENTS IN THE PROCESS (COLORADO HOSPITAL SUBSTANCE EXPOSED NEWBORNS COLLABORATIVE-CHOSEN); INCREASE PARENTAL ENGAGEMENT IN THEIR INFANT’S CARE DURING NICU STAYS (DATA-DRIVEN ENGAGEMENT OF FAMILIES TO IMPROVE THE NICU EXPERIENCE IN COLORADO-DEFINE COLORADO); AND REDUCE UNNECESSARY CESAREAN SECTIONS IN LOW-RISK POPULATIONS (SUPPORTING VAGINAL DELIVERY FOR LOW-RISK MOTHERS-SOAR). CPCQC HAS CREATED A CARE NETWORK ACROSS CLINICAL AND COMMUNITY SETTINGS CALLED IMPROVE PERINATAL ACCESS, COORDINATION, AND TREATMENT FOR BEHAVIORAL HEALTH (IMPACT BH) TO HELP FAMILIES OVERCOME BEHAVIORAL HEALTH AND SOCIAL NEEDS. AS PART OF THIS WORK, CPCQC INCLUDES PARTICIPATING HOSPITALS IN THE SUBSTANCE USE AND BEHAVIORAL HEALTH QI PROJECTS MENTIONED ABOVE. CPCQC HAS ALSO LAUNCHED THE FAMILY INTEGRATION TO RESTORE TRUST (FIRST) PROGRAM, A TWO-PHASE, COMMUNITY-FOCUSED EFFORT TO TRAIN AND INTEGRATE PATIENT-FAMILY PARTNERS FROM DIVERSE BACKGROUNDS INTO LEADERSHIP COMMITTEES AND WORKGROUPS WITHIN OUR ORGANIZATION. CPCQC IS APPLYING FOR CDC-RFA-DP22-2207 COMPONENT A FUNDING TO ADDRESS THE PERINATAL HEALTH CARE NEEDS IN COLORADO BY EXPANDING QI PROJECTS ACROSS THE STATE, CREATING AND ENHANCING CLINICAL AND COMMUNITY LINKAGES, AND CONNECTING PROJECTS THAT ENCOMPASS THE MATERNAL-INFANT DYAD, STARTING IN HIGH-NEED REGIONS IDENTIFIED AND SELECTED BASED ON PERINATAL HEALTH OUTCOMES DATA WITH AN EMPHASIS ON REGIONS WITH INEQUITABLE OUTCOMES FOR SUBPOPULATIONS. CPCQC WILL EMPHASIZE CLINICAL-COMMUNITY LINKAGES BY ESTABLISHING CARE NETWORKS THAT SPAN PRENATAL, HOSPITAL, AND POSTPARTUM NEEDS AND INTEGRATE HEALTH CARE DELIVERY WITH COMMUNITY STRATEGIES THAT ARE DEVELOPED AND GUIDED BY THOSE WITH LIVED EXPERIENCE. OUTCOMES: BY SEPTEMBER 29, 2023, CPCQC WILL: INCREASE OVERALL PARTICIPATION OF HEALTHCARE FACILITIES PARTICIPATING IN AIM SUD LC, CHOSEN, AND DEFINE BY 20%, SPECIFICALLY TARGETING FACILITIES THAT SERVE DISPROPORTIONATELY IMPACTED PREGNANT AND POSTPARTUM INDIVIDUALS, INFANTS, AND THEIR FAMILIES. SELECT ONE NEW PERINATAL QI INITIATIVE TO BEGIN IMPLEMENTATION PLANNING IN 2024. BY SEPTEMBER 29 2027, CPCQC WILL: INCREASE OVERALL PARTICIPATION OF HEALTHCARE FACILITIES PARTICIPATING IN ONE OR MORE QI INITIATIVES TO AT LEAST 75% EXPANDING ITS CAPACITY TO SERVE DISPROPORTIONATELY IMPACTED PREGNANT AND POSTPARTUM INDIVIDUALS, INFANTS, AND THEIR FAMILIES.

FROM THE PERSPECTIVES OF STUDENTS, INSTRUCTORS, AND INDUSTRY MENTORS. THIS INITIAL EVALUATION WILL UTILIZE A MIXED-METHODS APPROACH, COMBINING QUANTITATIVE DATA AND QUALITATIVE FEEDBACK. THE FINDINGS WILL SERVE AS A CRUCIAL BENCHMARK TO ASSESS THE ADDED VALUE OF THE AI COMPONENT ONCE INTEGRATED. THE PLATFORM ITSELF IS DESIGNED TO ADDRESS THE ISSUE OF LIMITED PRACTICAL SKILL DEVELOPMENT BY ENABLING INSTRUCTORS TO DESIGN CPBL ACTIVITIES WITHIN THEIR CURRICULUM, SEAMLESSLY GROUP STUDENTS BASED ON SKILL SETS, AND TRACK PROGRESS. IT FURTHER INTEGRATES INDUSTRY MENTORS FOR REAL-WORLD GUIDANCE AND UTILIZES EMBEDDED ASSESSMENTS TO EVALUATE STUDENT ACQUISITION OF CORE COMPETENCIES. DEVELOPMENT WILL LEVERAGE BOTH QUANTITATIVE AND QUALITATIVE ANALYSIS, WITH REAL-TIME USER FEEDBACK DRIVING ITERATIVE IMPROVEMENTS THROUGHOUT. YOUTH COLLABORATORY WILL LEVERAGE ITS EXPERIENCE, IN-DEPTH KNOWLEDGE OF THE PROBLEM, ABILITY TO WORK WITH A FOCUS OF EQUITY ACROSS COMMUNITIES AND SYSTEMS, AND WITH RELIABLE DATA COLLECTION AND ANALYTIC CAPABILITIES TO FULFIL THE GOALS OF CATEGORY 2 TRAINING AND TECHNICAL ASSISTANCE (TTA). YOUTH COLLABORATORY IS APPLYING UNDER PRIORITY AREAS 1A, 2, AND 3 WITH EXPERIENCE AND EXPERTISE IN WORKING TOWARDS RACIAL EQUITY AND REMOVING BARRIERS, SUPPORTING UNDERSERVED RURAL COMMUNITIES, AND ACHIEVING REPUTABLE OUTCOMES. THE PURPOSE/GOAL OF THE PROGRAM IS TO PROMOTE POSITIVE YOUTH OUTCOMES AND PUBLIC SAFETY FOR ADOLESCENTS EXPERIENCING RISK FACTORS AND YOUTH TRANSITIONING FROM FOSTER CARE THROUGHOUT THE U.S. (SERVICE AREA). TO MEET THIS GOAL, YOUTH COLLABORATORY’S PRIMARY ACTIVITIES INCLUDE PROVIDING TTA AND ACHIEVE THE SEVEN REQUIRED DELIVERABLES (D) THROUGH FOUR PHASES. PHASE I: DISCOVERY D1: DEVELOP TECHNICAL ASSISTANCE PLANS FOR EACH PROJECT SITE WITHIN THE FIRST 4 MONTHS. D2: PLAN AND COORDINATE, IN CONJUNCTION WITH OJJDP, ANNUAL CROSS-SITE MEETINGS OF PROJECT SITE GRANTEES. PHASE 2: LEARNING (MONTHS 6-12, ONGOING) & PHASE 3: INTEGRATION & SUPPORT (MONTHS 12-36, ONGOING) D3: PROVIDE TARGETED CONTENT TRAINING TO PROJECT SITE STAFF AND PARTNERS TO INCREASE THEIR CAPACITY TO IMPLEMENT STRATEGIES TO ADDRESS ISSUES RELATED TO THE TARGET POPULATION OF YOUTH. D4: ESTABLISH TOOLS AND TECHNIQUES FOR WORKING ACROSS DIFFERENT DISCIPLINES TO FACILITATE INFORMATION SHARING, COMMUNICATION, AND COORDINATION. FACILITATE PEER-TO-PEER INFORMATION SHARING AND LEARNING COMMUNITIES (CROSS-SITE MEETINGS) BETWEEN THE PROJECT SITES. PHASE 4: NATIONAL DISSEMINATION & ENGAGEMENT (MONTHS 1-36) D5: DEVELOP AND DISSEMINATE RESOURCES ON BEST PRACTICES AND LESSONS LEARNED FROM THE PROJECT SITES. D6: DEVELOP AND DELIVER GENERAL TRAINING ACTIVITIES ON RESIDENTIAL-BASED INNOVATIVE CARE, TREATMENT, AND SERVICES FOR COMMUNITIES NOT FUNDED THROUGH THIS PROJECT. D7: PRODUCE WRITTEN SYNOPSES OR SUMMARIES OF ALL MEETINGS, ONSITE VISITS, CONFERENCE CALLS, AND TRAININGS, AND WILL PROVIDE THEM TO OJJDP. THE INTENDED BENEFICIARIES ARE CATEGORY 1 PROJECT SITES (GOVERNMENTAL AND TRIBAL ENTITIES, INDEPENDENT SCHOOL DISTRICTS, AND NON-PROFIT AND FOR-PROFIT ORGANIZATIONS) AND OTHER STAKEHOLDERS SUPPORTING ADOLESCENTS EXPERIENCING RISK FACTORS AND YOUTH TRANSITIONING FROM FOSTER CARE. THE PROJECT PROPOSED BY COLLABORATIVE SOLUTIONS FOR COMMUNITIES OFFERS A COMPREHENSIVE RANGE OF SERVICES INCLUDING TRAINING FOR SCHOOL PERSONNEL AND STUDENTS, VIOLENCE INTERVENTION STRATEGIES, AND VARIOUS SCHOOL SAFETY SOLUTIONS. THE PROGRAM FOCUSES ON IMPLEMENTING EVIDENCE-BASED EDUCATIONAL CURRICULA, VIOLENCE INTERVENTION AND PREVENTION, MEDIATION, SOCIO-EMOTIONAL DEVELOPMENT, AND TRAINING FOR SCHOOL PERSONNEL. THE TRAINING MATERIALS THAT WILL EDUCATE THE STUDENTS ABOUT TRAUMA AND ITS NEGATIVE IMPACT ON BEHAVIOR AND WELL-BEING, AS WELL AS HEALTHY SOCIOEMOTIONAL SKILLS, INCLUDING THE TRAUMA, ADDICTIONS, MENTAL HEALTH, AND RECOVERY FOR YOUTH (TAMAR-Y) AND THE HIGH SCHOOL HEALTH CURRICULUM (HSHC). THE TAMAR PROGRAM WAS ORIGINALLY DESIGNED FOR THE MARYLAND DEPARTMENT OF JUVENILE SERVICES (DJS) IN PARTNERSHIP WITH YOUNG INDIVIDUALS, FAMILIES, AND DJS STAFF, LEADING TO THE CREATION OF TAMAR-Y. THIS THERAPEUTIC INITIATIVE COMBINES TRAUMA EDUCATION WITH PRACTICAL TECHNIQUES TO HELP PARTICIPANTS IDENTIFY TRIGGERS AND DEVELOP SKILLS TO MANAGE TRAUMA SYMPTOMS EFFECTIVELY. UTILIZING EXPRESSIVE ARTS THERAPIES, THE PROGRAM ENGAGES YOUTH IN A WAY THAT IS BOTH AGE-APPROPRIATE AND MEANINGFUL, ENCOURAGING EXPLORATION OF EMOTIONS AND EXPERIENCES TO DEEPEN SELF-AWARENESS AND RESPONSES TO TRAUMA. MSSA HAS CLOSE TO 150 SMALL SCHOOL MEMBER DISTRICTS AND 120 OF THEM ARE OPERATING WITHOUT A SINGLE CREDENTIALED SCHOOL COUNSELOR, LEAVING ONE OF THEIR TEACHERS TO TAKE A SINGLE THREE-HOUR SESSION (PROVIDED BY MSSA) AND THEN TAKE ON THE COUNSELING RESPONSIBILITIES FOR THEIR SCHOOL IN ADDITION TO THEIR TEACHING ROLE. SOME TEACHERS ALSO HAVE THE LIBRARY MEDIA SPECIALIST TASKS AS WELL. THESE RURAL AND TRIBAL SCHOOLS ARE EXCEPTIONALLY UNDER- RESOURCED. THE PURPOSE OF THIS PROJECT IS TO DELIVER TO UNDERSERVED AREAS COMPREHENSIVE SCHOOL COUNSELING, MENTAL HEALTH SERVICES, AND OTHER RESOURCES THAT ARE CRUCIAL FOR CREATING AND SUSTAINING SAFE SCHOOLS. INCREASED ACCESS TO RESOURCES, TRAUMA-INFORMED CARE, SCHOOL COUNSELORS, E-THERAPY ASSESSMENTS AND TREATMENT, AS PROPOSED IN THE PROJECT, IS VITAL TO IMPROVING THE PHYSICAL AND PSYCHOLOGICAL SAFETY OF THE SCHOOL COMMUNITY.
