THE VISTA PROJECT WILL SEEK TO BUILD CAPACITY OF OUR COMMUNITY HEALTH CENTER TO EXPAND ACCESS TO HEALTH SERVICES VULNERABLE AREA RESIDENTS BY DEVELOPING AND COORDINATING FIVE COLLABORATIVES OR PARTNERSHIPS WITH NEW STAKEHOLDERS SERVING POTENTIAL PATIENTS, AND BY BUILDING AWARENESS AND SUPPORT FOR OUR COMMUNITY HEALTH CENTER BY DEVELOPING AND EXECUTING AN INTERNAL AND EXTERNAL COMMUNICATIONS PLAN. SALUD PROJECTS TO PROVIDE HEALTH CARE SERVICES TO 1,000 NEW, LOW-INCOME PATIENTS CURRENTLY IN SALUD'S SERVICE AREA THROUGH THESE CAPACITY BUILDING ACTIVITIES AND REACH 1,500 TARGETED POTENTIAL PATIENTS THROUGH OUTREACH AND COMMUNICATIONS ACTIVITIES. TWO VISTA MEMBERS WILL CONTRIBUTE TO THE GOALS OF THE OVERALL PROJECT BY IDENTIFYING AND DEVELOPING KEY STAKEHOLDER PARTNERSHIPS AND/OR COLLABORATIVES, DEVELOPING EFFECTIVE COLLABORATIVE MANAGEMENT SYSTEMS, AND CREATING AND IMPLEMENTING EXTERNAL AND INTERNAL COMMUNICATIONS PLANS OVER THE COURSE OF ONE YEAR.
