TRIBAL OPIOID TREATMENT AND PREVENTION PROJECT - THE STANDING ROCK SIOUX TRIBE (SRST) THROUGH TREATMENT PROGRAM PROPOSES THE "TRIBAL OPIOID TREATMENT AND PREVENTION PROJECT TO ADDRESS THE OPIOID CRISIS IN STANDING ROCK BY INCREASING ACCESS TO CULTURALLY APPROPRIATE AND EVIDENCE-BASED TREATMENT, INCLUDING MEDICATION-ASSISTED TREATMENT (MAT) USING FOOD AND DRUG ADMINISTRATION (FDA) -APPROVED MEDICATIONS FOR THE TREATMENT OF OPIOID USE DISORDER (OUD); DECREASE ILLICIT OPIOID USE AND PRESCRIPTION OPIOID MISUSE; AND REDUCE UNMET TREATMENT NEED AND OPIOID OVERDOSE-RELATED DEATHS THROUGH THE PROVISIONS OF PREVENTION, TREATMENT, AND RECOVERY SUPPORT SERVICES FOR OUD. THE GOALS OF THE OPIOID TREATMENT AND PREVENTION PROJECT INCLUDE: STRENGTHEN PROGRAM SERVICES CAPACITY BY HIRING STAFF; STRENGTHEN THE CAPACITY OF SRST TREATMENT PROGRAM, AND TRIBAL SYSTEMS TO INCREASE UTILIZATION OF CULTURALLY RELEVANT EVIDENCE-BASED AND EVIDENCE-INFORMED PROGRAMS FOR PROVIDING CLINICAL SERVICES, INCLUDING MAT AND SUPPORTS FOR OPIOID, PRESCRIPTION DRUG AND OTHER DRUG MISUSE; FACILITATE EDUCATION OF THE TRIBAL LABOR FORCE & COMMUNITY MEMBERS TO INCREASE KNOWLEDGE OF STRATEGIES FOR PREVENTION, TREATMENT & RECOVERY FROM OPIOID AND PRESCRIPTION DRUG MISUSE; STRENGTHEN THE CAPACITY OF SRST TRIBAL HEALTH AND TRIBAL SYSTEMS TO IMPROVE & IMPLEMENT SYSTEMATIC REFERRAL MECHANISMS FOR TRIBAL MEMBERS NEEDING OPIOID MISUSE TREATMENT; PREVENT THE INITIATION AND PROGRESSION OF YOUTH SUBSTANCE MISUSE (INCLUDING TOBACCO USE) IN THE COMMUNITY BY IMPLEMENTING PROMOTION AND PREVENTION STRATEGIES WITH SCHOOL AGE YOUTH; INCREASE AWARENESS OF AVAILABLE RESOURCES, SUPPORTS WRITTEN AND ELECTRONIC MATERIALS FOR COMMUNITY MEMBERS ON SUBSTANCE (INCLUDING OPIOIDS AND PRESCRIPTIONS) MISUSE, PREVENTION, TREATMENT AND RECOVERY; AND PROMOTE FAMILY AND COMMUNITY SUPPORT. TO ACHIEVE THESE GOALS THE PROJECT TEAM WILL INCREASE STAFF MEMBERS; PLAN AND ORGANIZE CULTURALLY RELEVANT AND CROSS-CULTURAL OPIOIDS, PRESCRIPTION DRUGS AND OTHER DRUG MISUSE TRAINING; IMPROVE PATIENT TRACKING SYSTEM AMONG PROVIDERS BOTH ON AND OUTSIDE THE RESERVATION; UTILIZE IHS BEHAVIORAL HEALTH TO PROVIDE MAT SERVICES; UTILIZE TELEHEALTH SERVICES TO REACH, ENGAGE AND RETAIN PATIENTS IN TREATMENT; PROVIDE HARM REDUCTION SERVICES, INCLUDING OPIOID MISUSE EDUCATION, NALOXONE DISTRIBUTION, SYRINGE SERVICES, AND OTHER SERVICES ACROSS THE TRIBE'S 8 DISTRICTS; COMPILE EXISTING/OR REVISE/OR DEVELOP POLICIES AND PROCEDURES FOR BEHAVIORAL HEALTH FOR SERVICE COORDINATION AMONG PARTNERING ORGANIZATIONS; UTILIZE SOCIAL MEDIA, SOCIAL MARKETING AND AWARENESS CAMPAIGNS TO PROVIDE ELECTRONIC INFORMATION AND ACCESS TO SUBSTANCE MISUSE RESOURCES; PROMOTE HEALTHY LIFESTYLE CHOICES, POSITIVE COMMUNICATION, PERSONAL SUCCESS AND CULTURAL INVOLVEMENT; AND REGULARLY ASSESS AND MONITOR COLLABORATION AMONG PARTICIPATING ORGANIZATIONS. THE PROJECT IS SLATED TO SERVE TWENTY-FIVE UNDUPLICATED INDIVIDUALS IN EACH YEAR FOR A TOTAL OF 125 OVER THE 5-YEAR PROJECT PERIOD. ADDITIONALLY, WE HOPE TO REACH THE RESERVATION POPULATION OF 8,612 THROUGH COMMUNITY EDUCATION AND MEDIA MESSAGES.

NATIONAL IMMUNOHISTOCHEMISTRY STANDARDIZATION PROGRAM - ESTABLISHMENT & LAUNCH - THE OVERARCHING GOAL OF THIS PROJECT IS TO CHANGE LABORATORY PRACTICE IN THE CLINICAL IMMUNOHISTOCHEMISTRY (IHC) LABORATORY. IHC IS THE ONLY CLINICAL LABORATORY DISCIPLINE WITHOUT REFERENCE STANDARDS AND TRACEABLE UNITS OF MEASURE. THIS IS DESPITE THE FACT IHC IS A MAJOR TESTING FORMAT IN SURGICAL PATHOLOGY; A TYPICAL HOSPITAL IHC LAB HAS A TEST MENU OF APPROXIMATELY 200 DIFFERENT TESTS. AS A RESULT, IHC ANALYTIC ERROR RATES RANGE FROM 10 – 30% AS COMPARED TO <1% FOR OTHER TYPES OF CLINICAL LABORATORY TESTING. THESE ERRORS AFFECT BOTH PREDICTIVE MARKERS (HER2, PD-L1) AS WELL AS DIAGNOSTIC MARKERS (E.G., TTF-1, P53). FROM THE 1950'S TO THE 1970'S, THE FIELD OF CLINICAL CHEMISTRY BROADLY ADOPTED THESE HIGHER STANDARDS OF PRACTICE AND ERROR RATES PLUMMETED. THIS PROJECT IS FOR THE PURPOSE OF DEMONSTRATING THE SAME BENEFIT IN CLINICAL IHC. THIS APPLICANT IS UNIQUELY QUALIFIED AFTER HAVING SURMOUNTED AN IMPORTANT TECHNICAL HURDLE. BOSTON CELL STANDARDS DEVELOPED THE FIRST IHC CALIBRATORS WITH ANALYTE CONCENTRATIONS TRACEABLE TO A RECOGNIZED STANDARD - NIST STANDARD REFERENCE MATERIAL 1934. THE TWO SPECIFIC AIMS OF THIS PROJECT ARE: (1) IDENTIFY QUANTITATIVE CALIBRATION RANGES FOR ACCURATE CLINICAL IHC TESTING, AND (2) MEASURE THE BENEFIT THAT CALIBRATORS OFFER TOWARDS REPORTING ACCURATE TEST RESULTS. THESE WILL BE THE FIRST STUDIES CORRELATING ANALYTIC SENSITIVITY WITH DIAGNOSTIC SENSITIVITY AND SPECIFICITY. NAMELY, HOW MANY MOLECULES PER CELL (OF THE TARGET ANALYTE) MUST THE ASSAY BE CAPABLE OF DETECTING IN ORDER TO PRODUCE A DIAGNOSTICALLY ACCURATE TEST RESULT? THIS IS IMPORTANT EVEN FOR QUALITATIVE TESTS THAT ARE REPORTED AS POSITIVE/NEGATIVE, WHICH ARE COMMON IN IHC. THIS ANALYSIS, WHILE COMMON IN OTHER FIELDS OF LABORATORY TESTING, IS NEW TO IHC. THE EXPERIMENTAL STUDY DESIGN USES A CLINICAL IHC LABORATORY SURVEY TOOL COMPRISING A TISSUE MICROARRAY (TMA) AND IHC CALIBRATORS. THE TMA GENERATES DIAGNOSTIC SENSITIVITY AND SPECIFICITY DATA WHILE CALIBRATORS GENERATE ANALYTIC SENSITIVITY DATA. THIS PROJECT INITIATES A NOVEL CONSORTIUM THAT INCLUDES BOSTON CELL STANDARDS, INTERNATIONAL IHC PROFICIENCY TESTING AGENCIES, AND ACADEMIA. ALTHOUGH THERE IS PRECEDENT IN CREATING HIGH-IMPACT STANDARDIZATION PROGRAMS FOR CLINICAL CHEMISTRY TESTING, SUCH AN ORGANIZATION IS UNPRECEDENTED FOR IHC. THE GOAL IS TO CREATE AND LAUNCH A NATIONAL IMMUNOHISTOCHEMISTRY STANDARDIZATION PROGRAM, TO OVERSEE AND EXECUTE DATA-DRIVEN STUDIES FOR INTEGRATING REFERENCE STANDARDS TO IHC. THE CONSORTIUM MEMBERS ARE ACTIVE PARTNERS IN EXPERIMENTAL DESIGN, EXECUTION, AND DATA INTERPRETATION. THE PROPOSED CLINICAL TRIAL FORMAT HAS ALREADY BEEN SUCCESSFULLY TESTED AND PUBLISHED WITH SOME OF THE CONSORTIUM PARTNERS.

INTERVENTIONS TARGETING TRADITIONAL HIV RISK BEHAVIORS WITHOUT ADDRESSING CONTEXTUAL ISSUES LIKE ACCESS TO CARE, MENTAL HEALTH, AND STIGMA ARE LIKELY TO EXACERBATE INEQUITIES IN HIV INCIDENCE AND PERPETUATE ONGOING HEALTH DISPARITIES. DIGITAL HEALTH INTERVENTIONS (DHIS) ARE WELL-SUITED TO ADDRESS THESE CHALLENGES GIVEN THE UBIQUITY OF HIGHLY PERSONAL MOBILE TECHNOLOGY, HOWEVER, DHI CREATORS FACE MAJOR BARRIERS WHEN CREATING, MANAGING, DISSEMINATING, AND SCALING EVIDENCE-BASED TAILORED INTERVENTIONS. RECENT ADVANCES IN DATA SCIENCE PRESENT A SALIENT OPPORTUNITY TO CREATE RESOURCES THAT ACCELERATE AND IMPROVE THE PROCESS OF DEVELOPING DHIS AND EMPOWER INTERVENTION DEVELOPERS TO BETTER ADDRESS DIVERSE REAL-WORLD NEEDS AND DELIVER HEALTH-PROMOTING ACTIVITIES ACROSS AT-RISK POPULATIONS. OUR TEAM HAS BEEN AT THE FOREFRONT OF DESIGNING STAND-ALONE DHIS THAT HAVE SHOWN PROMISE FOR IMPACTING DISTINCT HIV CONTINUUM OUTCOMES. OUR DHI PLATFORM, HEALTHMPOWERMENT (HMP), WAS BUILT UPON YEARS OF PRIOR HUMAN- CENTERED DESIGN AND RIGOROUS CLINICAL TRIALS RESEARCH AND HAS DEMONSTRATED SIGNIFICANT REDUCTIONS IN HIV TRANSMISSION RISK BEHAVIORS, INCREASED ENGAGEMENT IN HIV CARE AND DECREASES IN PERCEIVED HIV STIGMA. HOWEVER, TO BE MOST IMPACTFUL, HMP REQUIRES OPTIMIZATION AND MODERNIZATION TO FURTHER IMPROVE ITS REACH AND TO BETTER MEET THE NEEDS OF THE RESEARCH COMMUNITY AND POTENTIAL END-USERS. THIS PHASE II PROJECT PROPOSES TO BUILD ON THE PRIOR FUNDED WORK AND DEVELOP A UNIFIED STUDY MANAGEMENT PLATFORM TO AID RESEARCHERS TO EFFICIENTLY DEVELOP AND DEPLOY BEHAVIOR CHANGE INTERVENTIONS AND EFFECTIVELY MANAGE PARTICIPANTS THROUGHOUT THE DURATION OF THEIR TRIAL ENGAGEMENT. WE HAVE ASSEMBLED A TEAM WITH EXPERTISE IN DIGITAL USER EXPERIENCE, SOFTWARE DEVELOPMENT, AND BEHAVIORAL SCIENCE WITH A STRONG FOCUS ON INTERVENTION RESEARCH RELATED TO HIV PREVENTION. LEVERAGING THIS EXPERTISE, OUR PROPOSAL AIMS TO REFINE AND EVALUATE HMP NEXT GEN, A COMPREHENSIVE, EVIDENCE-BASED DIGITAL HEALTH SOLUTION POISED TO EXPEDITE CULTURALLY MEANINGFUL RESEARCH TO PROMOTE HEALTH EQUITY. IN AIM 1, USING HUMAN CENTERED DESIGN, AND WORKING ITERATIVELY WITH EXPERT STAKEHOLDERS, WE WILL STREAMLINE AND EXPEDITE PLATFORM DEPLOYMENT BY AUTOMATING THE PROCESS THROUGH THE DEVELOPMENT OF RESEARCHER FACING TOOLS. THIS INCLUDES TRANSITION TO A CLOUD-BASED SERVER, AND DEVELOPMENT OF A ROBUST CONTENT MANAGEMENT SYSTEM AND SELF-SERVICE PORTAL FOR RESEARCH PROJECT DEPLOYMENT.
