QUESTION: PLEASE PROVIDE A ONE-PARAGRAPH SUMMARY (4-6 SENTENCES) OF YOUR PROPOSED VISTA PROJECT THAT INCLUDES THE FOLLOWING: - A DESCRIPTION OF YOUR ORGANIZATION'S MISSION - THE CNCS PROGRAMMING PRIORITY AREA THAT YOUR PROJECT ALIGNS WITH - A DESCRIPTION OF THE PROPOSED PROJECT GOAL(S), INCLUDING WHO WILL BENEFIT FROM THE PROJECT - THE NUMBER OF VISTAS AND LEADERS (IF APPLICABLE) YOU ARE REQUESTING AND AN OVERVIEW OF THE MAJOR ACTIVITIES THEY WILL PERFORM - THE ESTIMATED TIME REQUIRED TO COMPLETE THE PROJECT THE HEALTH TRUST (THT) IS A 501(C) (3) NON-PROFIT OPERATING FOUNDATION FOUNDED IN 1996 FROM THE SALE OF THREE LOCAL NON-PROFIT HOSPITALS. THE MISSION IS TO LEAD THE SILICON VALLEY COMMUNITY TO ADVANCE WELLNESS WITH A VISION TO MAKE SILICON VALLEY THE HEALTHIEST REGION IN AMERICA, WHERE EVERY RESIDENT CAN ACHIEVE OPTIMAL HEALTH THROUGHOUT HIS OR HER LIFETIME, IRRESPECTIVE OF BACKGROUND, INCOME, RACE, ETHNICITY, OR AGE. THT IS COLLABORATING WITH THREE COMMUNITY PARTNERS: SECOND HARVEST, VEGGIELUTION, AND LA MESA VERDE AT SACRED HEART COMMUNITY SERVICE. THESE SITES WILL CONTINUE TO IMPLEMENT THE HEALTHCORPS VISTA PROGRAM FOR A SECOND YEAR, WHICH WILL FOCUS ON HEALTHY FUTURES, SPECIFICALLY OBESITY AND FOOD ACCESS. THE HEALTHCORPS VISTA PROGRAM WILL SEEK TO CONTINUE TO INCREASE SEAMLESS ACCESS TO HEALTHY, AFFORDABLE FOOD IN LOW-INCOME, LOW-ACCESS COMMUNITIES AND BUILD/ STRENGTHEN THE CAPACITY OF PARTNER ORGANIZATIONS FOR THE THIRD YEAR. SPECIFICALLY, SEVEN VISTA MEMBERS WILL WORK AT 4 SEPARATE COMMUNITY-BASED ORGANIZATIONS IN SAN JOSE TO TRAIN STAFF AND COMMUNITY VOLUNTEERS, CREATE NEW OR ENHANCE SYSTEMS AND BUSINESS PROCESSES, SERVE NEW BENEFICIARIES, INCREASE THE NUMBER OF INDIVIDUALS RECEIVING FOOD AND INCREASE THE NUMBER OF INDIVIDUALS REPORTING INCREASED FOOD SECURITY OF THEMSELVES AND THEIR CHILDREN THROUGH CONDUCTING CLIENT SURVEYS TO INDIVIDUALS. THE VISTA MEMBERS WILL WORK SEPARATELY AT THEIR HOSTING SITES AND MEET ON A MONTHLY BASIS TO LEARN ABOUT THE WORK THEIR VISTA COLLEAGUES ARE COMPLETING DURING THEIR SERVICE YEAR. THE SEVEN VISTA MEMBERS WILL CONTRIBUTE TO THE GOALS OF THE PROJECT BY PERFORMING ACTIVITIES INCLUDING: CONDUCTING COMMUNITY ASSESSMENTS, INCREASING COMMUNITY AWARENESS AND ENGAGEMENT, EXPANDING OR STRENGTHENING PARTNERSHIPS AND NETWORKS, SUPPORTING EFFORTS TO INCREASE FINANCIAL RESOURCES, DEVELOPING EDUCATIONAL/ TRAINING/ OUTREACH MATERIALS, CONDUCTING COMMUNITY OUTREACH, MEASURING PROGRESS THROUGH PERFORMANCE MEASUREMENT, CONDUCTING PROGRAM EVALUATION/DEVELOPMENT/DELIVERY, AND RECRUITING AND MANAGING COMMUNITY AND ORGANIZATIONAL VOLUNTEERS.
