FY 2024 BEHAVIORAL HEALTH SERVICE EXPANSION - PROJECT ABSTRACT THE LOS ANGELES FREE CLINIC (DBA SABAN COMMUNITY CLINIC) 8405 BEVERLY BOULEVARD, LOS ANGELES, CA 90048 PROJECT DIRECTOR: MURIEL NOUWEZEM (323) 337-1707 MNOUWEZEM@SABANCOMMUNITYCLINIC.ORG WWW.SABANCOMMUNITYCLINIC.ORG PROGRAM FUND REQUEST OF $600,000 IN YEAR ONE HEALTH CENTER PROGRAM GRANT NUMBER: H80C PROJECT TITLE: EXPANDING ACCESS TO BEHAVIORAL HEALTHCARE SERVICES FOR THE LOS ANGELES COMMUNITY FOUNDED AS THE LOS ANGELES FREE CLINIC IN 1967, SABAN COMMUNITY CLINIC (SCC) HAS BUILT A REPUTABLE HISTORY IN PROVIDING MEDICAL, DENTAL BEHAVIORAL HEALTH, AND SPECIALTY CARE SERVICES TO LOW-INCOME, MEDICALLY DEPRIVED PEOPLE IN LOS ANGELES COUNTY. OUR MISSION IS TO PROVIDE OUTSTANDING, COMPASSIONATE CARE TO ANYONE WHO NEEDS IT. SCC WORKS TO REDUCE HEALTHCARE DISPARITIES FOR HISTORICALLY DISADVANTAGED GROUPS, INCLUDING RECENT IMMIGRANTS, HIV POSITIVE PATIENTS, AND UNHOUSED PEOPLE. SCC IS A COMPREHENSIVE COMMUNITY HEALTH CENTER BASED ON THE PATIENT-CENTERED MEDICAL HOME MODEL, WITH A SERVICE DELIVERY PLAN DESIGNED TO ENSURE THE COMMUNITY’S ACCESS TO PROMPT, QUALITY, AFFORDABLE, CULTURALLY APPROPRIATE, AND COMPREHENSIVE HEALTHCARE REGARDLESS OF RACE, GROUP AFFILIATION, AGE, GENDER, OR THE ABILITY TO PAY. OVER THE GRANT PERIOD, SCC WILL: 1) INCREASE THE NUMBER OF PATIENTS RECEIVING MENTAL HEALTH SERVICES 2) INCREASE THE NUMBER OF PATIENTS RECEIVING SUD SERVICES, INCLUDING PATIENTS RECEIVING TREATMENT WITH MOUD. PATIENTS WILL BE DRAWN PRIMARILY FROM THE FOLLOWING AREA ZIP CODES: 90003, 90004, 90005, 90006, 90007, 90008, 90010, 90011, 90012, 90013, 90015, 90016, 90018, 90019, 90020, 90026, 90027, 90028, 90029. ADDITIONAL ZIP CODES INCLUDE: 90031, 90032, 90033, 90034, 90036, 90037, 90038, 90039, 90041, 90042, 90043, 90044, 90046, 90047, 90048, 90057, 90058, 90062, 90065, 90068, AND 90069. ALL ARE MAINLY LOCATED WITHIN THE LOS ANGELES COUNTY METRO SERVICE PLANNING AREA (SPA) 4. OVERALL, SCC IS WELL SITUATED TO PROVIDE INCLUSIVE, CULTURALLY COMPETENT, HIGH QUALITY BEHAVIORAL HEALTHCARE SERVICES AND SUD TREATMENT TO PATIENTS IN THE LOS ANGELES COMMUNITY.
