IT ALSO PLAYS AN IMPORTANT ROLE IN TALENT DEVELOPMENT, BY CREATING IMPACTFUL MEETINGS BETWEEN THE FUTURE STEM WORKFORCE AND ACADEMIC, INDUSTRIAL AND GOVERNMENT LEADERS IN THEIR FIELDS. SINCE ITS INCEPTION IN 2003, BIRS HAS EXPANDED INTO FIVE CENTERS, WITH PRIMARY LOCATION IN BANFF, ALBERTA. IT MAINTAINS A PRESENCE IN FOUR DIFFERENT COUNTRIES (CANADA, MEXICO, CHINA AND SPAIN). A CHERISHED MEETING PLACE FOR SCIENTISTS, EDUCATORS AND SCHOLARS, BIRS'S INTENSE AND IMMERSIVE PROGRAMS HAVE ACQUIRED GLOBAL DISTINCTION AS INCUBATORS OF TRANSFORMATIVE IDEAS AND BREAKTHROUGHS AT THE FRONTIERS OF RESEARCH. EMERGING EVIDENCE SUGGESTS THAT THE COVID-19 PANDEMIC HAS RESULTED IN HEIGHTENED SOCIAL AND STRUCTURAL GENDER-BASED VIOLENCE AGAINST CIS AND TRANS WOMEN, WHO ARE SHOULDERING AN INEQUITABLE SHARE OF THE RESULTING IMPACT OF RESTRICTIONS, PHYSICAL DISTANCING AND ECONOMIC DOWNTOWN. IMPACTS FELT BY MARGINALIZED CIS AND TRANS WOMEN, INCLUDING WLWH, ARE EVEN MORE SEVERE AND THE SHORT- AND LONG-TERM CONSEQUENCES IN TERMS OF TRAUMA, VIOLENCE, SAFETY AND HIV CARE CONTINUUM OUTCOMES OF CIS AND TRANS WLWH WILL NEED TO BE ADDRESSED. EVEN PRIOR TO COVID-19 CRISIS, DESPITE SUBSTANTIAL PROGRAMMATIC ADVANCEMENTS, WLWH EXPERIENCED GAPS RELATIVE TO CISGENDER MEN IN ALL GLOBAL HIV TARGETS IN MANY SETTINGS. ALTHOUGH THE GLOBAL PREVALENCE OF SOCIAL AND STRUCTURAL VIOLENCE BY WLWH IS ALARMINGLY HIGH, THERE IS A LACK OF RESEARCH DELINEATING MECHANISMS THROUGH WHICH DIFFERENT TYPES OF VIOLENCE IMPACT HIV CARE CONTINUUM OUTCOMES, TO INFORM INTERVENTION TARGETS. MOREOVER, DESPITE CALLS TO APPLY A TRAUMA-AND VIOLENCE-INFORMED CARE APPROACH TO HIV CARE AND PRACTICE (TVI-HCP) AND THE UNIQUE NEEDS OF WLWH, SURPRISINGLY LITTLE IS KNOWN ABOUT HOW THIS APPROACH CAN BE TAILORED IN HIV CLINICAL PRACTICE ENVIRONMENTS. IN THE PROPOSED STUDY WE WILL LEVERAGE AN EXISTING COMMUNITY-BASED COHORT OF CIS AND TRANS WLWH IN A CANADIAN SETTING (SEXUAL HEALTH AND HIV/AIDS: WOMEN'S LONGITUDINAL NEEDS ASSESSMENT) AND RECAPTURE AND EXPAND THIS COHORT TO 300 WLWH IN THE CITY OF VANCOUVER. THIS QUANTITATIVE STUDY WILL INCLUDE SURVEY INTERVIEWS, INNOVATIVE MAPPING AND SPATIAL ANALYSES, HIV DISEASE MONITORING, AND CONFIDENTIAL ADMINISTRATIVE LINKAGES TO FILL CRITICAL RESEARCH GAPS AND INFORM COMPONENTS OF MULTI-LEVEL TVI-HCP PROGRAMMING TO OPTIMIZE HIV CARE CONTINUUM OUTCOMES BY: (1) PROSPECTIVELY CHARACTERIZING RELATIONSHIPS BETWEEN DIFFERENT TYPES OF VIOLENCE EXPERIENCES (INTERPERSONAL EMOTIONAL, PHYSICAL AND SEXUAL VIOLENCE; NEIGHBOURHOOD VIOLENCE) AND VIRAL LOAD SUPPRESSION FAILURE AND IDENTIFYING CRITICAL MEDIATORS IN THESE RELATIONSHIPS ON WHICH TO INTERVENE; (2) PROSPECTIVELY CHARACTERIZING THE KEY COMPONENTS OF DIFFERENTIATED HIV CARE MODELS THAT HAVE THE STRONGEST ASSOCIATIONS WITH VIRAL LOAD SUPPRESSION, EXPANDING ON OUR PRELIMINARY DEVELOPMENT AND VALIDATION OF A TVI-HCP SCALE FOR WLWH; AND (3) ESTABLISHING AN INNOVATIVE SOCIAL EPIDEMIOLOGICAL PLATFORM FOR INFORMING AND EVALUATING FUTURE MULTI-LEVEL TRAUMA- AND VIOLENCE-INFORMED HIV CARE INTERVENTIONS. DRUG CHECKING—THE ANALYSIS OF ILLICIT DRUG SAMPLES AND PROVISION OF DRUG COMPOSITION INFORMATION BACK TO PEOPLE WHO USE DRUGS (PWUD)—IS A HIGHLY INNOVATIVE PUBLIC HEALTH INTERVENTION THAT CAN EQUIP PWUD WITH CRUCIAL INFORMATION AND POTENTIALLY CONTRIBUTE TO LIFE-SAVING BEHAVIOR CHANGE. THIS IS PARTICULARLY RELEVANT IN LIGHT OF THE MOST SERIOUS DRUG OVERDOSE CRISIS IN NORTH AMERICAN HISTORY. EVIDENCE INDICATES THAT FENTANYL AND FENTANYL ANALOGUES ARE INCREASINGLY FOUND AS ADULTERANTS IN THE STREET OPIOID SUPPLY ACROSS MUCH OF THE CONTINENT, AND MAY ALSO BE FOUND IN THE STIMULANT AND PSYCHEDELIC STREET DRUG SUPPLY. WHILE VARIOUS DRUG CHECKING TECHNOLOGIES HAVE BEEN USED IN SOME EUROPEAN JURISDICTIONS TO REDUCE THE RISKS OF USING ILLICIT RECREATIONAL DRUGS (E.G., MDMA, ECSTASY) IN THE ELECTRONIC DANCE MUSIC SCENE, DRUG CHECKING OF OTHER STREET DRUGS IS STILL NASCENT IN NORTH AMERICA AND REMAINS LARGELY ILLEGAL IN THE U.S. IN THE CANADIAN PROVINCE OF BRITISH COLUMBIA, GOVERNMENTS AT ALL LEVELS HAVE SUPPORTED DRUG CHECKING PROGRAMS AS PART OF THEIR OPIOID OVERDOSE EMERGENCY RESPONSE AND HAVE INVESTED IN TECHNOLOGY AND OPERATIONAL FUNDING FOR DRUG CHECKING SERVICES, INCLUDING BOTH RAPID POINT-OF-CARE AND LABORATORY-BASED CONFIRMATORY TESTING. THIS REPRESENTS A SIGNIFICANT OPPORTUNITY TO STUDY THIS INTERVENTION AND ITS IMPACTS ON OPIOID OVERDOSE RISK. ACCORDINGLY, THIS APPLICATION SEEKS TO FUND A RIGOROUS SCIENTIFIC EVALUATION THAT AIMS TO EVALUATE AND DESCRIBE THE RATE AND PATTERNS OF UPTAKE OF POINT-OF-CARE DRUG CHECKING TECHNOLOGIES BY VARIOUS DRUG MARKET ACTORS (E.G., OPIOID USERS, STIMULANT USERS, DRUG DEALERS). IT FURTHER AIMS TO BEHAVIORAL IMPACTS AND OVERDOSE RISK REDUCTION PRACTICES (E.G., USING LESS) ASSOCIATED WITH RECEIPT OF QUANTITATIVE AND QUALITATIVE DRUG CHECKING RESULTS AMONG DIFFERENT DRUG MARKET ACTORS. LASTLY, IT AIMS TO EXAMINE THE IMPACT OF DRUG CHECKING SERVICE UTILIZATION ON NON-FATAL AND FATAL OVERDOSE EVENTS. THIS HIGHLY INNOVATIVE AND SIGNIFICANT PROGRAM OF RESEARCH INVOLVES THE USE OF THREE COMPLEMENTARY SOURCES OF DATA: 1) BASELINE QUESTIONNAIRE DATA, WHICH CAPTURES INFORMATION ON DEMOGRAPHICS, DRUG USE PATTERNS AND BEHAVIORS, AND SOCIAL-STRUCTURAL AND ENVIRONMENTAL EXPOSURES; 2) LONGITUDINAL DATA ON STREET DRUG CONTENTS USING POINT-OF-CARE AND ESTABLISHED LABORATORY TECHNIQUES AS WELL AS DATA ON INTENDED BEHAVIORS FOLLOWING RECEIPT OF DRUG CHECKING RESULTS; AND 3) DETAILED LONGITUDINAL ADMINISTRATIVE PUBLIC HEALTH DATA AVAILABLE ON OVERDOSE OUTCOMES THROUGH CANADA’S UNIVERSAL HEALTH CARE SYSTEM. COLLECTIVELY, THESE DATA WILL BE CONFIDENTIALLY LINKED AND USED TO EVALUATE HOW SUCH INFORMATION CAN BE INTEGRATED INTO ADDICTION TREATMENT AND SUBSTANCE USE FOCUSED PUBLIC HEALTH PROGRAMMING. EVIDENCE-BASED INTERVENTIONS TO PREVENT OVERDOSES EXIST BUT REMAIN POORLY ADAPTED TO THE DEVELOPMENTAL NEEDS OF YOUTH AND HAVE NOT BEEN IMPLEMENTED IN MOST PEDIATRIC HEALTHCARE SETTINGS. TO FILL THESE GAPS, THIS STUDY PROPOSES A THREE-YEAR COMMUNITY-ENGAGED APPROACH TO ADAPTING, PRODUCING, AND PILOT TESTING A WRITTEN YOUTH OVERDOSE PREVENTION “TOOLKIT” FOR USE BY CLINICIANS IN PEDIATRIC HEALTHCARE SETTINGS (E.G., HOSPITALS, COMMUNITY CLINICS). THE TOOLKIT WILL BE COMPRISED OF PRACTICAL AND EASY TO USE GUIDANCE FOR DELIVERING THE FOLLOWING EVIDENCE-BASED CORE ELEMENTS: NALOXONE AND TAKE-HOME DRUG TESTING STRIPS, HARM REDUCTION EDUCATION, AND MEDICATIONS FOR OPIOID USE DISORDER (MOUD; I.E., BUPRENORPHINE, METHADONE, OR NALTREXONE). OUR PROJECT WILL BE CONDUCTED IN BOSTON, MA, AND VANCOUVER, BC, TWO LOCALES THAT ARE HIGHLY IMPACTED BY THE OVERDOSE CRISIS AND HAVE BEEN INSTRUMENTAL IN DEVELOPING AND DISSEMINATING WORLD-LEADING YOUTH SUBSTANCE USE INTERVENTIONS. IN BOTH CITIES, THERE ARE CURRENTLY SIGNIFICANT EFFORTS UNDERWAY TO IMPROVE ACCESS TO OVERDOSE PREVENTION AND MOUD FOR YOUTH IN PEDIATRIC HEALTHCARE SETTINGS, ALLOWING US TO CAPITALIZE ON EXISTING INFRASTRUCTURES WHILE ALSO IDENTIFYING HOW HEALTH SYSTEMS DIFFERENCES AND OTHER IMPORTANT CONTEXTS MAY PRESENT UNIQUE BARRIERS AND FACILITATORS TO TOOLKIT IMPLEMENTATION. OUR PROJECT WILL BE GUIDED BY THE ADAPT-ITT IMPLEMENTATION SCIENCE FRAMEWORK, WITH A FOCUS ON THE ADAPTATION, PRODUCTION, AND TESTING PHASES. ACROSS EACH PHASE, WE WILL COLLABORATE CLOSELY WITH YOUTH WITH LIVED EXPERIENCE OF SUBSTANCE USE AND THEIR CAREGIVERS THROUGH THE CREATION OF COMMUNITY ADVISORY BOARDS (CABS) IN VANCOUVER AND BOSTON. DURING THE ADAPTATION PHASE (AIM 1), WE WILL CONDUCT SEMI-STRUCTURED QUALITATIVE INTERVIEWS WITH YOUTH, THEIR FAMILY MEMBERS AND OTHER CAREGIVERS, AND PEDIATRIC CLINICIANS TO IDENTIFY ADAPTATIONS TO THE TOOLKIT, RENDERING IT PRACTICAL AND EASY TO USE, DEVELOPMENTALLY APPROPRIATE AND YOUTH- AND CAREGIVER-CENTERED. DURING THE PRODUCTION PHASE (AIM 2), MEMBER CHECKING BY CLINICIANS AND CAB MEMBERS WILL BE USED TO ELICIT EXPERT FEEDBACK ON AND TO REFINE A WRITTEN TOOLKIT WITH LOCAL ADAPTATIONS TO THE BOSTON AND VANCOUVER SETTINGS. DURING THE TESTING PHASE (AIM 3), A SINGLE-ARM ADAPTIVE PILOT STUDY WILL BE CONDUCTED WITH 40 YOUTH (20 IN BOSTON AND 20 IN VANCOUVER, FOLLOWED FOR 3 MONTHS). PLAN-DO-STUDY-ACT CYCLES DRIVEN BY QUANTITATIVE SURVEY MEASURES AND QUALITATIVE INTERVIEWS WITH YOUTH IN THE PILOT STUDY, AS WELL AS QUALITATIVE INTERVIEWS WITH CAREGIVERS AND TOOLKIT PROVIDERS (CLINICIANS) WILL BE USED TO FURTHER REFINE THE TOOLKIT. THE PRODUCT AT THE END OF THE R34 WILL BE AN OPTIMIZED OVERDOSE PREVENTION TOOLKIT DEVELOPED WITH EXTENSIVE YOUTH, CAREGIVER, AND CLINICIAN INPUT, READY FOR RIGOROUS R01 TRIAL. PROJECT SUMMARY THE LONG-TERM GOAL OF THE PROPOSED RESEARCH IS GAIN FUNDAMENTAL INSIGHTS INTO THE PROCESS OF TOOTH GENERATION AND LOSS USING THE INNOVATIVE LEOPARD GECKO MODEL. THE GECKO IS THE FIRST BIOMEDICAL RESEARCH MODEL WHERE THERE IS DIRECT ACCESS TO THE REPLACING DENTITION IN AN ADULT ANIMAL. OUR FIRST THEME IS TOOTH ERUPTION. THERE ARE VERY FEW ANIMALS IN WHICH WE CAN STUDY TOOTH ERUPTION WHICH IS KEY FOR CREATING A FUNCTIONING DENTITION. THERE IS A POOR UNDERSTANDING OF WHAT DRIVES TEETH TO MOVE UPWARDS TO THE ORAL CAVITY. WE PROPOSE PIONEERING EXPERIMENTS BASED IN PART ON DENTAL TECHNIQUES AND INNOVATIVE DEVELOPMENTAL BIOLOGY METHODS. OUR EXPERIMENTS WILL TEST WHETHER TEETH GAIN TRACTION FROM ATTACHING TO DENTAL EPITHELIUM, WHETHER SOFTENING OF THE MATRIX ABOVE THE TEETH HELPS TO MOVE TEETH UPWARDS. THIS WORK IS FUNDAMENTAL TO OUR UNDERSTANDING OF HUMAN DEFECTS IN TOOTH ERUPTION, PARTICULARLY TEETH THAT BECOME BURIED IN THE JAW AND ARE DIFFICULT TO TREAT. THE SECOND THEME IS TOOTH RESORPTION. THE GECKO REPEATEDLY LOSES TEETH BUT NOT RANDOMLY. INSTEAD THERE IS TIMED TOOTH SHEDDING THAT INVOLVES ODONTOCLASTS, A CELL TYPE THAT IS RARELY STUDIED. THE UNWANTED ACTIVITY OF ODONTOCLASTS IN HUMANS CAUSES ROOTS TO RESORB ULTIMATELY LEADING TO PREMATURE TOOTH LOSS. THE RATIONALE FOR CHOOSING THE LEOPARD GECKO IS AS FOLLOWS: 1) THE GECKO DISCOVERIES ARE LIKELY TO BE APPLICABLE TO HUMAN TOOTH REPLACEMENT DUE TO SHARED EVOLUTIONARY HISTORY, 2) ADULT GECKOS HAVE LARGE NUMBERS OF TEETH THAT ARE REPLACED THROUGHOUT LIFE, 3) GECKO TEETH ARE ONLY ATTACHED TO BONE ON THE CHEEK SIDE, LEAVING THE OTHER SIDE ACCESSIBLE TO EXPERIMENTAL MANIPULATION, 4) THE TOOTH REPLACEMENT CYCLE IS MUCH SHORTER THAN OTHER ADULT REPTILES SO A CHANGE IS VISIBLE SOONER, 5) GECKO TEETH ARE REGULARLY INVADED BY ODONTOCLASTS PROVIDING US WITH AN OPPORTUNITY TO STUDY ODONTOCLAST BEHAVIOUR DURING A NORMAL, PHYSIOLOGICAL PROCESS 6) THE RICHMAN LAB INITIATED STUDIES ON GECKO TOOTH REPLACEMENT. WE HAVE PROVEN THAT AFTER TOOTH REMOVAL THAT THE TEETH WILL REGROW. THE GECKO OFFERS UNIQUE ACCESS TO THE DEVELOPING ADULT DENTITION COMPARED TO MOUSE, FISH AND SHARK MODELS. THE ABILITY TO CONTINUALLY MONITOR THE EFFECTS OF DENTAL SURGERY ON TOOTH REGENERATION OR TO ODONTOCLAST RECRUITMENT IN LIVING ANIMALS IS NOT POSSIBLE IN OTHER MODELS. WE HAVE INCLUDED ADVANCED MATHEMATICAL ANALYSIS AND MODELLING TO DETERMINE THE MOST IMPORTANT LOCATIONS OF CELLS AND SIGNALS NEEDED TO START AND SLOW DOWN TOOTH SHEDDING. THE SYNERGISTIC RESEARCH ENVIRONMENT CONSISTS OF THE PI WHO IS A PEDIATRIC DENTIST AND A DEVELOPMENTAL BIOLOGIST, A MATHEMATICIAN SPECIALIZING IN MATHEMATICAL BIOLOGY, AN EXPERIENCED TECHNICIAN WHO SUPPORTS ALL MOLECULAR WORK, A BIOINFORMATITION WHO HAS WORKED ON THE GECKO GENOME WITH OUR LAB AND A NEW POSTDOC. A CRITICAL BARRIER TO DEVELOPING NEW TREATMENTS TO IMPROVE SYMPTOMS CAUSED BY URETERAL STENTS IS A LACK OF UNDERSTANDING OF HOW MOLECULES IN THE URINE CONTRIBUTE TO MECHANISMS AND PATIENT EXPERIENCES AFTER URETEROSCOPY. HERE, WE BUILD ON OUR RECENT DISCOVERIES THAT STENT-INDUCED URETERAL SMOOTH MUSCLE DYSFUNCTION, CESSATION OF PERISTALSIS, AND RESULTANT HYDRONEPHROSIS ARE DRIVEN BY STRETCH-INDUCED INFLAMMATION OF THE URETER. WE WILL LEVERAGE OUR ANALYTICAL PIPELINE THAT IDENTIFIED PROSTAGLANDIN E2 (PGE2) AND 1-METHYLNICOTINAMIDE (1-MNA) AS BIOMARKERS FOR STENT-INDUCED URETERAL INFLAMMATORY CHANGES AND DYSFUNCTION IN PIGS. WE HAVE AN INTERDISCIPLINARY TEAM THAT IS UNIQUELY POISED TO DISCOVER AND VALIDATE URINARY BIOMARKERS OF STENT SYMPTOMS BY COMBINING EXPERTISE IN USING ANALYTIC CHEMISTRY, URETERAL PHYSIOLOGY, AND CLINICAL EPIDEMIOLOGY. THE PROPOSED RESEARCH TESTS THE CENTRAL HYPOTHESIS THAT THAT INDWELLING URETERAL STENTS TRIGGER SIMILAR MECHANISMS IN HUMANS AND THAT AT LEAST PGE2 AND 1-MNA ARE URINARY BIOMARKERS FOR STENT-ASSOCIATED URETERAL DYSFUNCTION AND SYMPTOMS IN HUMANS. IN DOING SO, THE PROPOSED STUDIES WILL IDENTIFY PATHWAYS THAT COULD BE TARGETS FOR NOVEL THERAPEUTICS TO PREVENT STENT SYMPTOMS. IN THIS PROPOSAL, WE GENERATE KEY PRELIMINARY DATA USING DATA AND URINE SAMPLES FROM 50 OF THE 484 ADOLESCENT AND ADULT PARTICIPANTS WHO UNDERWENT URETEROSCOPY WITH URETERAL STENT PLACEMENT AND ENROLLED IN THE STUDY TO ENHANCE UNDERSTANDING OF STENT-ASSOCIATED SYMPTOMS (STENTS) CONDUCTED BY THE NIDDK-SUPPORTED URINARY STONE DISEASE RESEARCH NETWORK. IN AIM 1, WE DETERMINE THE ASSOCIATION BETWEEN PGE2 AND 1-MNA AND STENT SYMPTOMS. WE WILL EVALUATE THE ASSOCIATION BETWEEN THESE PUTATIVE BIOMARKERS IN PRE-OPERATIVE URINE, AND THE COURSE OF PATIENT-REPORTED PAIN INTENSITY, PAIN INTERFERENCE, AND URINARY SYMPTOMS AFTER URETEROSCOPY WITH STENT PLACEMENT. IN AIM 2, WE WILL EXPAND THE LIST OF URINE BIOMARKERS USING OUR UNTARGETED ANALYTICAL APPROACH TO QUANTIFY AND IDENTIFY UNKNOWN BIOMARKERS FOR URETERAL OBSTRUCTION AND STENT-ASSOCIATED SYMPTOMS. THE PROPOSED WORK PROVIDES KEY PRELIMINARY DATA VALIDATING KNOWN AND NEW URINARY BIOMARKERS FOR STENT-ASSOCIATED URETERAL INFLAMMATORY CHANGES AND DYSFUNCTION IN HUMANS. CONFERENCE: BANFF INTERNATIONAL RESEARCH STATION -THIS AWARD PROVIDES FUND TO SUPPORT PARTICIPATION OF US-BASED MATHEMATICAL SCIENTISTS IN ACTIVITIES CONDUCTED AT BIRS. THE BANFF INTERNATIONAL RESEARCH STATION (BIRS) IS A RESEARCH INFRASTRUCTURE DEDICATED TO ALL ASPECTS OF MATHEMATICAL SCIENCES, INCLUDING PURE AND APPLIED MATH, COMPUTER SCIENCE, STATISTICS, PHYSICS, BIOLOGY, DATA SCIENCE AND THEIR MYRIAD SYNERGIES. ITS MISSION IS TO FACILITATE MATHEMATICAL INNOVATION AND DISCOVERY AT THE HIGHEST INTERNATIONAL LEVEL THROUGH COLLABORATIVE RESEARCH AND CROSS-DISCIPLINARY SYNERGY. IT ALSO PLAYS AN IMPORTANT ROLE IN TALENT DEVELOPMENT, BY CREATING IMPACTFUL MEETINGS BETWEEN THE FUTURE STEM WORKFORCE AND ACADEMIC, INDUSTRIAL AND GOVERNMENT LEADERS IN THEIR FIELDS. SINCE ITS INCEPTION IN 2003, BIRS HAS EXPANDED INTO FIVE CENTERS, WITH PRIMARY LOCATION IN BANFF, ALBERTA. IT MAINTAINS A PRESENCE IN FOUR DIFFERENT COUNTRIES (CANADA, MEXICO, CHINA AND SPAIN). A CHERISHED MEETING PLACE FOR SCIENTISTS, EDUCATORS AND SCHOLARS, BIRS'S INTENSE AND IMMERSIVE PROGRAMS HAVE ACQUIRED GLOBAL DISTINCTION AS INCUBATORS OF TRANSFORMATIVE IDEAS AND BREAKTHROUGHS AT THE FRONTIERS OF RESEARCH. ITS UNIQUE ENVIRONMENT PROMOTES CREATIVITY, INNOVATION AND EXCHANGE OF KNOWLEDGE. BIRS PROGRAMMING REACHES FAR BEYOND THE TRADITIONAL MODEL OF WORKSHOPS AND CONFERENCES; IT BUILDS BRIDGES BETWEEN ACADEMIA, INDUSTRY AND GOVERNMENT, WITH LASTING IMPACT ON THE SCIENTIFIC LANDSCAPE. BEST PRACTICES IN EQUITY, DIVERSITY AND INCLUSION, FACILITATION OF TRAINING AND MENTORSHIP ARE INCORPORATED IN EVERY ASPECT OF BIRS OPERATIONS, FROM PROPOSAL SELECTION TO PROGRAM DELIVERY, ENSURING A BALANCED REPRESENTATION OF GROUPS, EXPERTISE AND CAREER STAGES IN EACH COHORT. BIRS'S POST-COVID PLANNING INVOLVES PROGRAMMING EXPANSION AND DEVELOPMENT IN A NUMBER OF NEW DIRECTIONS. TRANSITION TO THE HYBRID FORMAT HAS RESULTED IN GLOBAL ACCESS TO ALL BIRS PROGRAMS. THE NEW BIRS NOW!'' HAS GENERATED A NIMBLE WAY TO CREATE HIGH IMPACT COLLISION VENUES ON TOPICS THAT DEMAND RAPID RESPONSE FROM RESEARCH. SUCH TOPICS INCLUDE CLIMATE CHANGE, CLEAN ENERGY, INFECTIOUS DISEASE MODELLING, VACCINE DISTRIBUTION AND FAIRNESS, NATIONAL QUANTUM STRATEGY, RESPONSIBLE DEVELOPMENT OF ARTIFICIAL INTELLIGENCE, CYBERSECURITY, BLOCKCHAIN AND OTHERS. THE REGULARLY OFFERED CAREER AND INNOVATION HUB INTRODUCES STUDENTS AND EARLY CAREER RESEARCHERS TO EMERGING CAREER OPPORTUNITIES IN SCIENCE AND TECHNOLOGY SECTORS. ANNUAL SUMMER SCHOOLS, SOME IN PARTNERSHIP WITH OTHER INSTITUTES, TRAIN UNDERGRADUATE AND GRADUATE STUDENTS IN EMERGING AREAS FOLLOWING RECENT BREAKTHROUGHS THAT HAVE POTENTIAL FOR FUTURE GROWTH. PROGRAMS LIKE FOCUSED RESEARCH GROUPS, RESEARCH IN TEAMS AND GROUP FOLLOW-ONS PROVIDE OPPORTUNITIES OF DISTRACTION-FREE RESEARCH TO SMALL TEAMS OF COLLABORATORS, SOMETIMES FOR PROJECTS STARTED AT OTHER BIRS EVENTS. IN 2022-2027, AT LEAST 3943 US SCIENTISTS ARE EXPECTED TO VISIT BIRS. THIS AWARD, WHICH HAS BEEN DEEMED WORTHY OF SUPPORT THROUGH PEER-REVIEWED EVALUATION USING NSF'S INTELLECTUAL MERIT AND BROADER IMPACTS REVIEW CRITERIA, WILL BE USED EXCLUSIVELY FOR SUPPORTING PARTICIPATION OF US-BASED MATHEMATICIANS AT BIRS ACTIVITIES. SOCIAL AND STRUCTURAL VIOLENCE AND HIV CARE CONTINUUM OUTCOMES: DEVELOPING A TRAUMA-INFORMED HIV CARE INTERVENTION AMONG WLWH - PROJECT SUMMARY/ABSTRACT WE WILL USE INNOVATIVE SOCIAL EPIDEMIOLOGICAL METHODS TO LONGITUDINALLY CHARACTERIZE THE IMPACTS AND MECHANISMS OF GENDER-BASED VIOLENCE ON THE HIV CARE CONTINUUM, DEFINE KEY COMPONENTS OF DIFFERENTIATED HIV CARE MODELS THAT MITIGATE THESE RELATIONSHIPS AND DEVELOP A PLATFORM FOR TRAUMA- AND VIOLENCE-INFORMED HIV CARE AND PRACTICE (TVI-HCP) INTERVENTIONS, AMONG CISGENDER (CIS) AND TRANSGENDER (TRANS) WOMEN LIVING WITH HIV (WLWH). EMERGING EVIDENCE SUGGESTS THAT THE COVID-19 PANDEMIC HAS RESULTED IN HEIGHTENED SOCIAL AND STRUCTURAL GENDER-BASED VIOLENCE AGAINST CIS AND TRANS WOMEN, WHO ARE SHOULDERING AN INEQUITABLE SHARE OF THE RESULTING IMPACT OF RESTRICTIONS, PHYSICAL DISTANCING AND ECONOMIC DOWNTOWN. IMPACTS FELT BY MARGINALIZED CIS AND TRANS WOMEN, INCLUDING WLWH, ARE EVEN MORE SEVERE AND THE SHORT- AND LONG-TERM CONSEQUENCES IN TERMS OF TRAUMA, VIOLENCE, SAFETY AND HIV CARE CONTINUUM OUTCOMES OF CIS AND TRANS WLWH WILL NEED TO BE ADDRESSED. EVEN PRIOR TO COVID-19 CRISIS, DESPITE SUBSTANTIAL PROGRAMMATIC ADVANCEMENTS, WLWH EXPERIENCED GAPS RELATIVE TO CISGENDER MEN IN ALL GLOBAL HIV TARGETS IN MANY SETTINGS. ALTHOUGH THE GLOBAL PREVALENCE OF SOCIAL AND STRUCTURAL VIOLENCE BY WLWH IS ALARMINGLY HIGH, THERE IS A LACK OF RESEARCH DELINEATING MECHANISMS THROUGH WHICH DIFFERENT TYPES OF VIOLENCE IMPACT HIV CARE CONTINUUM OUTCOMES, TO INFORM INTERVENTION TARGETS. MOREOVER, DESPITE CALLS TO APPLY A TRAUMA-AND VIOLENCE-INFORMED CARE APPROACH TO HIV CARE AND PRACTICE (TVI-HCP) AND THE UNIQUE NEEDS OF WLWH, SURPRISINGLY LITTLE IS KNOWN ABOUT HOW THIS APPROACH CAN BE TAILORED IN HIV CLINICAL PRACTICE ENVIRONMENTS. IN THE PROPOSED STUDY WE WILL LEVERAGE AN EXISTING COMMUNITY-BASED COHORT OF CIS AND TRANS WLWH IN A CANADIAN SETTING (SEXUAL HEALTH AND HIV/AIDS: WOMEN'S LONGITUDINAL NEEDS ASSESSMENT) AND RECAPTURE AND EXPAND THIS COHORT TO 300 WLWH IN THE CITY OF VANCOUVER. THIS QUANTITATIVE STUDY WILL INCLUDE SURVEY INTERVIEWS, INNOVATIVE MAPPING AND SPATIAL ANALYSES, HIV DISEASE MONITORING, AND CONFIDENTIAL ADMINISTRATIVE LINKAGES TO FILL CRITICAL RESEARCH GAPS AND INFORM COMPONENTS OF MULTI-LEVEL TVI-HCP PROGRAMMING TO OPTIMIZE HIV CARE CONTINUUM OUTCOMES BY: (1) PROSPECTIVELY CHARACTERIZING RELATIONSHIPS BETWEEN DIFFERENT TYPES OF VIOLENCE EXPERIENCES (INTERPERSONAL EMOTIONAL, PHYSICAL AND SEXUAL VIOLENCE; NEIGHBOURHOOD VIOLENCE) AND VIRAL LOAD SUPPRESSION FAILURE AND IDENTIFYING CRITICAL MEDIATORS IN THESE RELATIONSHIPS ON WHICH TO INTERVENE; (2) PROSPECTIVELY CHARACTERIZING THE KEY COMPONENTS OF DIFFERENTIATED HIV CARE MODELS THAT HAVE THE STRONGEST ASSOCIATIONS WITH VIRAL LOAD SUPPRESSION, EXPANDING ON OUR PRELIMINARY DEVELOPMENT AND VALIDATION OF A TVI-HCP SCALE FOR WLWH; AND (3) ESTABLISHING AN INNOVATIVE SOCIAL EPIDEMIOLOGICAL PLATFORM FOR INFORMING AND EVALUATING FUTURE MULTI-LEVEL TRAUMA- AND VIOLENCE-INFORMED HIV CARE INTERVENTIONS. AMIDST THE ONGOING COVID-19 CRISIS, IN PARALLEL WITH A SYNDEMIC OF VIOLENCE, TRAUMA AND HIV IN NORTH AMERICA, OUR STUDY OFFERS AN UNPRECEDENTED OPPORTUNITY TO INFORM DIFFERENTIATED HIV CARE MODELS TO OPTIMIZE HIV CLINICAL OUTCOMES AMONG MARGINALIZED WOMEN.

EVALUATION OF DRUG CHECKING AS A NOVEL INTERVENTION TO ADDRESS THE OPIOID OVERDOSE CRISIS ATTRIBUTABLE TO FENTANYL AND ITS ANALOGUES - PROJECT SUMMARY THIS APPLICATION REQUESTS FUNDING FOR THE RIGOROUS SCIENTIFIC EVALUATION OF A PROVINCIAL DRUG CHECKING PROGRAM IN A CANADIAN SETTING WHERE DRUG CHECKING IS LEGAL. DRUG CHECKING—THE ANALYSIS OF ILLICIT DRUG SAMPLES AND PROVISION OF DRUG COMPOSITION INFORMATION BACK TO PEOPLE WHO USE DRUGS (PWUD)—IS A HIGHLY INNOVATIVE PUBLIC HEALTH INTERVENTION THAT CAN EQUIP PWUD WITH CRUCIAL INFORMATION AND POTENTIALLY CONTRIBUTE TO LIFE-SAVING BEHAVIOR CHANGE. THIS IS PARTICULARLY RELEVANT IN LIGHT OF THE MOST SERIOUS DRUG OVERDOSE CRISIS IN NORTH AMERICAN HISTORY. EVIDENCE INDICATES THAT FENTANYL AND FENTANYL ANALOGUES ARE INCREASINGLY FOUND AS ADULTERANTS IN THE STREET OPIOID SUPPLY ACROSS MUCH OF THE CONTINENT, AND MAY ALSO BE FOUND IN THE STIMULANT AND PSYCHEDELIC STREET DRUG SUPPLY. WHILE VARIOUS DRUG CHECKING TECHNOLOGIES HAVE BEEN USED IN SOME EUROPEAN JURISDICTIONS TO REDUCE THE RISKS OF USING ILLICIT RECREATIONAL DRUGS (E.G., MDMA, ECSTASY) IN THE ELECTRONIC DANCE MUSIC SCENE, DRUG CHECKING OF OTHER STREET DRUGS IS STILL NASCENT IN NORTH AMERICA AND REMAINS LARGELY ILLEGAL IN THE U.S. IN THE CANADIAN PROVINCE OF BRITISH COLUMBIA, GOVERNMENTS AT ALL LEVELS HAVE SUPPORTED DRUG CHECKING PROGRAMS AS PART OF THEIR OPIOID OVERDOSE EMERGENCY RESPONSE AND HAVE INVESTED IN TECHNOLOGY AND OPERATIONAL FUNDING FOR DRUG CHECKING SERVICES, INCLUDING BOTH RAPID POINT-OF-CARE AND LABORATORY-BASED CONFIRMATORY TESTING. THIS REPRESENTS A SIGNIFICANT OPPORTUNITY TO STUDY THIS INTERVENTION AND ITS IMPACTS ON OPIOID OVERDOSE RISK. ACCORDINGLY, THIS APPLICATION SEEKS TO FUND A RIGOROUS SCIENTIFIC EVALUATION THAT AIMS TO EVALUATE AND DESCRIBE THE RATE AND PATTERNS OF UPTAKE OF POINT-OF-CARE DRUG CHECKING TECHNOLOGIES BY VARIOUS DRUG MARKET ACTORS (E.G., OPIOID USERS, STIMULANT USERS, DRUG DEALERS). IT FURTHER AIMS TO BEHAVIORAL IMPACTS AND OVERDOSE RISK REDUCTION PRACTICES (E.G., USING LESS) ASSOCIATED WITH RECEIPT OF QUANTITATIVE AND QUALITATIVE DRUG CHECKING RESULTS AMONG DIFFERENT DRUG MARKET ACTORS. LASTLY, IT AIMS TO EXAMINE THE IMPACT OF DRUG CHECKING SERVICE UTILIZATION ON NON-FATAL AND FATAL OVERDOSE EVENTS. THIS HIGHLY INNOVATIVE AND SIGNIFICANT PROGRAM OF RESEARCH INVOLVES THE USE OF THREE COMPLEMENTARY SOURCES OF DATA: 1) BASELINE QUESTIONNAIRE DATA, WHICH CAPTURES INFORMATION ON DEMOGRAPHICS, DRUG USE PATTERNS AND BEHAVIORS, AND SOCIAL-STRUCTURAL AND ENVIRONMENTAL EXPOSURES; 2) LONGITUDINAL DATA ON STREET DRUG CONTENTS USING POINT-OF-CARE AND ESTABLISHED LABORATORY TECHNIQUES AS WELL AS DATA ON INTENDED BEHAVIORS FOLLOWING RECEIPT OF DRUG CHECKING RESULTS; AND 3) DETAILED LONGITUDINAL ADMINISTRATIVE PUBLIC HEALTH DATA AVAILABLE ON OVERDOSE OUTCOMES THROUGH CANADA’S UNIVERSAL HEALTH CARE SYSTEM. COLLECTIVELY, THESE DATA WILL BE CONFIDENTIALLY LINKED AND USED TO EVALUATE HOW SUCH INFORMATION CAN BE INTEGRATED INTO ADDICTION TREATMENT AND SUBSTANCE USE FOCUSED PUBLIC HEALTH PROGRAMMING. INVESTIGATING INNOVATIVE SOLUTIONS TO THE OPIOID CRISIS IS PERFECTLY ALIGNED WITH NIDA’S PLANS TO HIGHLIGHT RESEARCH ON SYNTHETIC PSYCHOACTIVE DRUGS, AS WELL AS WITH THE BROAD BEHAVIORAL INTERVENTION GOALS IDENTIFIED BY NIH’S HEAL AND OPIOID INITIATIVES, WHICH INCLUDE RESEARCH DEDICATED TO PREVENTING AND REVERSING OVERDOSES.

AT THE IAF, WE ARE INVESTING IN A REGIONAL EFFORT THAT WILL AMPLIFY THE VOICES OF FARMERS AND DIRECTLY SUPPORT THEIR COLLECTION, ANALYSIS, AND COMPILATION OF DATA THAT WILL IMPROVE THE MANAGEMENT AND EFFICIENCY OF THEIR FARMS. THE UNIVERSITY OF BRITISH COLUMBIA (UBC) IS A GLOBAL LEADER IN RESEARCH ON FOOD SYSTEMS AND SUSTAINABLE AGRICULTURAL DEVELOPMENT. UBC IS PROVIDING RESOURCE-POOR FARMERS AND THEIR NETWORKS ACCESS TO LITEFARM, A FREE AND OPEN-SOURCE WEB-BASED APPLICATION, TO USE EVIDENCE TO MANAGE THEIR FARMS SUSTAINABLY. THIS WILL STRENGTHEN FARMERS’ ABILITY TO ADVOCATE FOR BETTER PRACTICES AND POLICIES AND ENGAGE WITH OTHER FUNDING PARTNERS. IT WILL ALSO DEMONSTRATE HOW COMMUNITY-LED ACTIONS LEAD TO MORE ENVIRONMENTALLY, SOCIALLY, AND ECONOMICALLY-SUSTAINABLE OUTCOMES, SUCH AS FOOD SECURITY, CLEAN WATER, GREATER BIODIVERSITY, AND RESILIENCE TO NATURAL DISASTERS. ADAPTING AND TESTING A YOUTH OVERDOSE PREVENTION TOOLKIT - PROJECT SUMMARY/ABSTRACT OVERDOSE IS NOW A LEADING CAUSE OF DEATH AMONG ADOLESCENTS AND YOUNG ADULTS AGES 12 TO 24 (HEREAFTER “YOUTH”) IN THE US AND CANADA AS A RESULT OF AN INCREASINGLY TOXIC ILLICIT DRUG SUPPLY. EVIDENCE-BASED INTERVENTIONS TO PREVENT OVERDOSES EXIST BUT REMAIN POORLY ADAPTED TO THE DEVELOPMENTAL NEEDS OF YOUTH AND HAVE NOT BEEN IMPLEMENTED IN MOST PEDIATRIC HEALTHCARE SETTINGS. TO FILL THESE GAPS, THIS STUDY PROPOSES A THREE-YEAR COMMUNITY-ENGAGED APPROACH TO ADAPTING, PRODUCING, AND PILOT TESTING A WRITTEN YOUTH OVERDOSE PREVENTION “TOOLKIT” FOR USE BY CLINICIANS IN PEDIATRIC HEALTHCARE SETTINGS (E.G., HOSPITALS, COMMUNITY CLINICS). THE TOOLKIT WILL BE COMPRISED OF PRACTICAL AND EASY TO USE GUIDANCE FOR DELIVERING THE FOLLOWING EVIDENCE-BASED CORE ELEMENTS: NALOXONE AND TAKE-HOME DRUG TESTING STRIPS, HARM REDUCTION EDUCATION, AND MEDICATIONS FOR OPIOID USE DISORDER (MOUD; I.E., BUPRENORPHINE, METHADONE, OR NALTREXONE). OUR PROJECT WILL BE CONDUCTED IN BOSTON, MA, AND VANCOUVER, BC, TWO LOCALES THAT ARE HIGHLY IMPACTED BY THE OVERDOSE CRISIS AND HAVE BEEN INSTRUMENTAL IN DEVELOPING AND DISSEMINATING WORLD-LEADING YOUTH SUBSTANCE USE INTERVENTIONS. IN BOTH CITIES, THERE ARE CURRENTLY SIGNIFICANT EFFORTS UNDERWAY TO IMPROVE ACCESS TO OVERDOSE PREVENTION AND MOUD FOR YOUTH IN PEDIATRIC HEALTHCARE SETTINGS, ALLOWING US TO CAPITALIZE ON EXISTING INFRASTRUCTURES WHILE ALSO IDENTIFYING HOW HEALTH SYSTEMS DIFFERENCES AND OTHER IMPORTANT CONTEXTS MAY PRESENT UNIQUE BARRIERS AND FACILITATORS TO TOOLKIT IMPLEMENTATION. OUR PROJECT WILL BE GUIDED BY THE ADAPT-ITT IMPLEMENTATION SCIENCE FRAMEWORK, WITH A FOCUS ON THE ADAPTATION, PRODUCTION, AND TESTING PHASES. ACROSS EACH PHASE, WE WILL COLLABORATE CLOSELY WITH YOUTH WITH LIVED EXPERIENCE OF SUBSTANCE USE AND THEIR CAREGIVERS THROUGH THE CREATION OF COMMUNITY ADVISORY BOARDS (CABS) IN VANCOUVER AND BOSTON. DURING THE ADAPTATION PHASE (AIM 1), WE WILL CONDUCT SEMI-STRUCTURED QUALITATIVE INTERVIEWS WITH YOUTH, THEIR FAMILY MEMBERS AND OTHER CAREGIVERS, AND PEDIATRIC CLINICIANS TO IDENTIFY ADAPTATIONS TO THE TOOLKIT, RENDERING IT PRACTICAL AND EASY TO USE, DEVELOPMENTALLY APPROPRIATE AND YOUTH- AND CAREGIVER-CENTERED. DURING THE PRODUCTION PHASE (AIM 2), MEMBER CHECKING BY CLINICIANS AND CAB MEMBERS WILL BE USED TO ELICIT EXPERT FEEDBACK ON AND TO REFINE A WRITTEN TOOLKIT WITH LOCAL ADAPTATIONS TO THE BOSTON AND VANCOUVER SETTINGS. DURING THE TESTING PHASE (AIM 3), A SINGLE-ARM ADAPTIVE PILOT STUDY WILL BE CONDUCTED WITH 40 YOUTH (20 IN BOSTON AND 20 IN VANCOUVER, FOLLOWED FOR 3 MONTHS). PLAN-DO-STUDY-ACT CYCLES DRIVEN BY QUANTITATIVE SURVEY MEASURES AND QUALITATIVE INTERVIEWS WITH YOUTH IN THE PILOT STUDY, AS WELL AS QUALITATIVE INTERVIEWS WITH CAREGIVERS AND TOOLKIT PROVIDERS (CLINICIANS) WILL BE USED TO FURTHER REFINE THE TOOLKIT. THE PRODUCT AT THE END OF THE R34 WILL BE AN OPTIMIZED OVERDOSE PREVENTION TOOLKIT DEVELOPED WITH EXTENSIVE YOUTH, CAREGIVER, AND CLINICIAN INPUT, READY FOR RIGOROUS R01 TRIAL. THIS WORK SQUARELY ALIGNS WITH NIDA’S STRATEGIC PRIORITIES TO IMPROVE THE IMPLEMENTATION OF EVIDENCE-BASED STRATEGIES IN REAL-WORLD SETTINGS, AND FOSTER INTERNATIONAL COLLABORATIONS TO ADDRESS HEALTH DISPARITIES AMONG DRUG-USING PRIORITY POPULATIONS.

THE ADULT LEOPARD GECKO, AN EMERGING, UNCONVENTIONAL EXPERIMENTAL MODEL FOR DENTAL RESEARCH - 7. PROJECT SUMMARY THE LONG-TERM GOAL OF THE PROPOSED RESEARCH IS GAIN FUNDAMENTAL INSIGHTS INTO THE PROCESS OF TOOTH GENERATION AND LOSS USING THE INNOVATIVE LEOPARD GECKO MODEL. THE GECKO IS THE FIRST BIOMEDICAL RESEARCH MODEL WHERE THERE IS DIRECT ACCESS TO THE REPLACING DENTITION IN AN ADULT ANIMAL. OUR FIRST THEME IS TOOTH ERUPTION. THERE ARE VERY FEW ANIMALS IN WHICH WE CAN STUDY TOOTH ERUPTION WHICH IS KEY FOR CREATING A FUNCTIONING DENTITION. THERE IS A POOR UNDERSTANDING OF WHAT DRIVES TEETH TO MOVE UPWARDS TO THE ORAL CAVITY. WE PROPOSE PIONEERING EXPERIMENTS BASED IN PART ON DENTAL TECHNIQUES AND INNOVATIVE DEVELOPMENTAL BIOLOGY METHODS. OUR EXPERIMENTS WILL TEST WHETHER TEETH GAIN TRACTION FROM ATTACHING TO DENTAL EPITHELIUM, WHETHER SOFTENING OF THE MATRIX ABOVE THE TEETH HELPS TO MOVE TEETH UPWARDS. THIS WORK IS FUNDAMENTAL TO OUR UNDERSTANDING OF HUMAN DEFECTS IN TOOTH ERUPTION, PARTICULARLY TEETH THAT BECOME BURIED IN THE JAW AND ARE DIFFICULT TO TREAT. THE SECOND THEME IS TOOTH RESORPTION. THE GECKO REPEATEDLY LOSES TEETH BUT NOT RANDOMLY. INSTEAD THERE IS TIMED TOOTH SHEDDING THAT INVOLVES ODONTOCLASTS, A CELL TYPE THAT IS RARELY STUDIED. THE UNWANTED ACTIVITY OF ODONTOCLASTS IN HUMANS CAUSES ROOTS TO RESORB ULTIMATELY LEADING TO PREMATURE TOOTH LOSS. THE RATIONALE FOR CHOOSING THE LEOPARD GECKO IS AS FOLLOWS: 1) THE GECKO DISCOVERIES ARE LIKELY TO BE APPLICABLE TO HUMAN TOOTH REPLACEMENT DUE TO SHARED EVOLUTIONARY HISTORY, 2) ADULT GECKOS HAVE LARGE NUMBERS OF TEETH THAT ARE REPLACED THROUGHOUT LIFE, 3) GECKO TEETH ARE ONLY ATTACHED TO BONE ON THE CHEEK SIDE, LEAVING THE OTHER SIDE ACCESSIBLE TO EXPERIMENTAL MANIPULATION, 4) THE TOOTH REPLACEMENT CYCLE IS MUCH SHORTER THAN OTHER ADULT REPTILES SO A CHANGE IS VISIBLE SOONER, 5) GECKO TEETH ARE REGULARLY INVADED BY ODONTOCLASTS PROVIDING US WITH AN OPPORTUNITY TO STUDY ODONTOCLAST BEHAVIOUR DURING A NORMAL, PHYSIOLOGICAL PROCESS 6) THE RICHMAN LAB INITIATED STUDIES ON GECKO TOOTH REPLACEMENT. WE HAVE PROVEN THAT AFTER TOOTH REMOVAL THAT THE TEETH WILL REGROW. THE GECKO OFFERS UNIQUE ACCESS TO THE DEVELOPING ADULT DENTITION COMPARED TO MOUSE, FISH AND SHARK MODELS. THE ABILITY TO CONTINUALLY MONITOR THE EFFECTS OF DENTAL SURGERY ON TOOTH REGENERATION OR TO ODONTOCLAST RECRUITMENT IN LIVING ANIMALS IS NOT POSSIBLE IN OTHER MODELS. WE HAVE INCLUDED ADVANCED MATHEMATICAL ANALYSIS AND MODELLING TO DETERMINE THE MOST IMPORTANT LOCATIONS OF CELLS AND SIGNALS NEEDED TO START AND SLOW DOWN TOOTH SHEDDING. THE SYNERGISTIC RESEARCH ENVIRONMENT CONSISTS OF THE PI WHO IS A PEDIATRIC DENTIST AND A DEVELOPMENTAL BIOLOGIST, A MATHEMATICIAN SPECIALIZING IN MATHEMATICAL BIOLOGY, AN EXPERIENCED TECHNICIAN WHO SUPPORTS ALL MOLECULAR WORK, A BIOINFORMATITION WHO HAS WORKED ON THE GECKO GENOME WITH OUR LAB AND A NEW POSTDOC. THE TEAM IS POISED TO MAKE SIGNIFICANT DISCOVERIES THAT CAN BE TRANSLATED TO TREATMENTS FOR FAILURE OF ERUPTION AND TOOTH LOSS IN HUMANS AS WELL AS OTHER DISEASES SUCH AS OSTEOPOROSIS AND OSTEOARTHRITIS.

IDENTIFICATION OF URINARY BIOMARKERS FOR STENT ASSOCIATED SYMPTOMS IN HUMANS - (PLEASE KEEP IN WORD, DO NOT PDF) URETERAL STENTS PLACED AFTER URETEROSCOPY TO REMOVE KIDNEY STONES CAUSE SIGNIFICANT MORBIDITY, WITH MOST PATIENTS WITH AN INDWELLING STENT REPORTING IMPAIRED QUALITY OF LIFE, PAIN, AND URINARY SYMPTOMS AFFECTING THEIR EVERYDAY LIFE. A CRITICAL BARRIER TO DEVELOPING NEW TREATMENTS TO IMPROVE SYMPTOMS CAUSED BY URETERAL STENTS IS A LACK OF UNDERSTANDING OF HOW MOLECULES IN THE URINE CONTRIBUTE TO MECHANISMS AND PATIENT EXPERIENCES AFTER URETEROSCOPY. HERE, WE BUILD ON OUR RECENT DISCOVERIES THAT STENT-INDUCED URETERAL SMOOTH MUSCLE DYSFUNCTION, CESSATION OF PERISTALSIS, AND RESULTANT HYDRONEPHROSIS ARE DRIVEN BY STRETCH-INDUCED INFLAMMATION OF THE URETER. WE WILL LEVERAGE OUR ANALYTICAL PIPELINE THAT IDENTIFIED PROSTAGLANDIN E2 (PGE2) AND 1-METHYLNICOTINAMIDE (1-MNA) AS BIOMARKERS FOR STENT-INDUCED URETERAL INFLAMMATORY CHANGES AND DYSFUNCTION IN PIGS. WE HAVE AN INTERDISCIPLINARY TEAM THAT IS UNIQUELY POISED TO DISCOVER AND VALIDATE URINARY BIOMARKERS OF STENT SYMPTOMS BY COMBINING EXPERTISE IN USING ANALYTIC CHEMISTRY, URETERAL PHYSIOLOGY, AND CLINICAL EPIDEMIOLOGY. THE PROPOSED RESEARCH TESTS THE CENTRAL HYPOTHESIS THAT THAT INDWELLING URETERAL STENTS TRIGGER SIMILAR MECHANISMS IN HUMANS AND THAT AT LEAST PGE2 AND 1-MNA ARE URINARY BIOMARKERS FOR STENT-ASSOCIATED URETERAL DYSFUNCTION AND SYMPTOMS IN HUMANS. IN DOING SO, THE PROPOSED STUDIES WILL IDENTIFY PATHWAYS THAT COULD BE TARGETS FOR NOVEL THERAPEUTICS TO PREVENT STENT SYMPTOMS. IN THIS PROPOSAL, WE GENERATE KEY PRELIMINARY DATA USING DATA AND URINE SAMPLES FROM 50 OF THE 484 ADOLESCENT AND ADULT PARTICIPANTS WHO UNDERWENT URETEROSCOPY WITH URETERAL STENT PLACEMENT AND ENROLLED IN THE STUDY TO ENHANCE UNDERSTANDING OF STENT-ASSOCIATED SYMPTOMS (STENTS) CONDUCTED BY THE NIDDK-SUPPORTED URINARY STONE DISEASE RESEARCH NETWORK. IN AIM 1, WE DETERMINE THE ASSOCIATION BETWEEN PGE2 AND 1-MNA AND STENT SYMPTOMS. WE WILL EVALUATE THE ASSOCIATION BETWEEN THESE PUTATIVE BIOMARKERS IN PRE-OPERATIVE URINE, AND THE COURSE OF PATIENT-REPORTED PAIN INTENSITY, PAIN INTERFERENCE, AND URINARY SYMPTOMS AFTER URETEROSCOPY WITH STENT PLACEMENT. IN AIM 2, WE WILL EXPAND THE LIST OF URINE BIOMARKERS USING OUR UNTARGETED ANALYTICAL APPROACH TO QUANTIFY AND IDENTIFY UNKNOWN BIOMARKERS FOR URETERAL OBSTRUCTION AND STENT-ASSOCIATED SYMPTOMS. THE PROPOSED WORK PROVIDES KEY PRELIMINARY DATA VALIDATING KNOWN AND NEW URINARY BIOMARKERS FOR STENT-ASSOCIATED URETERAL INFLAMMATORY CHANGES AND DYSFUNCTION IN HUMANS. ONCE VALIDATED IN A SMALLER COHORT, THEIR ASSOCIATION WITH STENT-ASSOCIATED PAIN AND DISCOMFORT CAN BE FURTHER VALIDATED IN A LARGER PATIENT COHORT AND INCORPORATED INTO EXISTING PREDICTIVE MODELS FOR STENT-COMPLICATIONS TO IDENTIFY PATIENTS MORE LIKELY TO DEVELOP SEVERE SYMPTOMS. THIS WILL FORM THE BASIS OF A SUBSEQUENT R01 APPLICATION.
