PSA 3 AREA AGENCY ON AGING DBA THE AREA AGENCY ON AGING 3(AAA3) HAS BEEN ADMINISTERING STATE AND FEDERALLY FUNDED PROGRAMS SINCE 1974. THE AGENCY'S MISSION IS TO PROVIDE LIFE-SPAN RESOURCES THAT INSPIRE, EDUCATE, AND EMPOWER OLDER ADULTS, INDIVIDUALS WITH DISABILITIES, AND FAMILY CAREGIVERS. AAA3 WILL RECRUIT, TRAIN, AND DIRECT VOLUNTEER SERVICE FOR AN ESTIMATED 190 UNDUPLICATED VOLUNTEERS THROUGH THE RSVP EXPANSION GRANT. THE PROGRAM'S PRIMARY FOCUS IS HEALTHY FUTURES PROVIDING IMPROVING ACCESS TO CARE AND AGING IN PLACE. THESE COUNTIES ARE IN NORTHWEST OHIO AND ADJACENT TO THE RSVP PROGRAM OPERATED BY THE AREA OFFICE ON AGING OF NORTHWEST OHIO. AAA3 IS EXCITED TO COLLABORATE WITH OUR SISTER AGENCY IN TOLEDO WHO HAS OPERATED AN RSVP PROJECT FOR MANY YEARS. THE AREA OFFICE ON AGING OF NORTHWEST OHIO HAS AGREED TO MENTOR AAA3 THROUGH THE PROCESS OF REPLICATING THEIR SUCCESSFUL RSVP PROJECT. THE VOLUNTEERS WILL CONTRIBUTE SERVICE TO OLDER ADULTS, INDIVIDUALS WITH DISABILITIES, AND FAMILY CAREGIVERS. 190 UNDUPLICATED VOLUNTEERS, AND A NETWORK OF VOLUNTEER STATIONS WILL PROVIDE ALL SERVICES. THE FOLLOWING SERVICE ACTIVITIES ARE REPRESENTATIVE OF THE HEALTHY FUTURES WORK PLANS. THE VOLUNTEER SERVICE ACTIVITIES ADDRESS AGING IN PLACE WILL INCLUDE SERVICES SUCH AS WEEKLY ONE ON ONE VISITATION OF HOMEBOUND OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES TO PROVIDE COMPANIONSHIP, MONITOR THEIR ONGOING QUALITY OF LIFE, AND PROVIDE EDUCATION ON ELDER JUSTICE ISSUES. THIS SERVICE WILL BE PROVIDED THROUGH THE RETIRED SENIOR VOLUNTEER PATROL UNDER THE DIRECTION OF THE LIMA COMMUNITY ORIENTED POLICING PROGRAM. VOLUNTEERS ARE NEEDED TO PROVIDE RELIEF TO FAMILY CAREGIVERS ALONG WITH EDUCATION TO IMPROVE THE SELF-MANAGEMENT OF CAREGIVING. THROUGH THE COMPANIONSHIP RESPITE PROGRAM, VOLUNTEERS WILL ALSO PROVIDE WEEKLY NON-PROFESSIONAL RESPITE ASSISTANCE TO SUPPORT PRIMARY CAREGIVERS WITH THEIR CARE GIVING RESPONSIBILITIES. CAREGIVERS WILL BE GIVEN THE OPPORTUNITY TO ATTEND THE EVIDENCED BASED PROGRAM, POWERFUL TOOLS FOR CAREGIVERS. VOLUNTEERS WILL LEAD THE CLASSES AFTER EXTENSIVE TRAINING ON THE FIDELITY OF THE PROGRAM. AAA3 HAS 10 YEARS OF EXPERIENCE UTILIZING VOLUNTEERS TO LEAD EVIDENCED BASED CHRONIC DISEASE SELF-MANAGEMENT CLASSES. THE RSVP PROJECT WILL SUPPORT AAA3 IN EXPANDING OUR ABILITY TO RECRUIT VOLUNTEERS TO LEAD CLASSES AND IMPROVE THE HEALTH AND WELLNESS OF OUR COMMUNITIES. FINALLY, THE PROJECT WILL SUPPORT VOLUNTEER CAPACITY BUILDING FOR DISASTER PREPARATIONS, THE ELDER JUSTICE PROGRAM AWAKENINGS AND THE REGIONAL LONG TERM CARE OMBUDSMAN PROGRAM.

THE PROGRAM'S PRIMARY FOCUS AREA WILL BE ON HEALTHY FUTURES, IN CONTINUING TO IMPROVE ACCESS TO CARE AND THE ABILITY FOR INDIVIDUALS TO BE ABLE TO AGE IN PLACE IN A HOME AND COMMUNITY BASED SETTING OF THEIR CHOICE. OTHER FOCUS AREAS UNDER THE AGENCY'S WORK PLAN WILL INCLUDE EDUCATION WITH OBJECTIVES TARGETING K-12 SUCCESS; DISASTER SERVICES WITH EMPHASIS ON DISASTER PREPAREDNESS, CAPACITY BUILDING FOR OTHER COMMUNITY ORGANIZATIONS WHO ALSO BENEFIT FROM THE SUPPORT OF VOLUNTEERS, AND OTHER COMMUNITY IDENTIFIED PRIORITIES AS THEY MAY ARISE. THE CNCS FEDERAL INVESTMENT OF $153,980 WILL BE SUPPLEMENTED BY APPROXIMATELY $65,991 OF NON-FEDERAL MATCHING DOLLARS COMPRISED OF BOTH CASH AND OR APPROVED IN-KIND DONATION WITH A TOTAL PROJECT COST OF $219,971 FOR THE INITIAL PROJECT PERIOD.
